RESUMO

OBJECTIVE: Multilocular cystic renal cell carcinoma has been considered as a distinct subtype of clear cell renal cell carcinoma according to 2004 WHO classification. CA9 has proven to be a diagnostic and prognostic marker for clear cell renal cell carcinoma, but the study has been limited to solid tumors. The aim of this article was to analyse the clinical features of multilocular cystic renal cell carcinoma with focus on CA9 expression. PATIENTS AND METHODS: Nine multilocular cystic renal cell carcinomas were found. Their age was from 33 to 74 years old with a median of 54 years. There were five men and four women. The computerized tomography was analyzed. There were three total nephrectomies and six partial nephrectomies. The follow-up ranged from five to 102 months with a median 56 months. Immunohistochemical staining was performed on surgical samples to detect CA9 expression. RESULTS: There were seven pT1a, one pT1b and one pT2; four grade 1 and five grade 2. The Bosniak classification was: one B2F, six B3 and two B4. In CT examination, all tumors showed a hypodense mass. Seven out of nine showed multiple intern septa, more or less thick and vascularized with an enhancement after injection of contrast. No metastasis or relapse was found during follow-up. Eight out of nine multilocular cystic renal cell carcinomas expressed strongly CA9. CONCLUSION: Multilocular cystic renal cell carcinoma was a low malignant tumor with a good prognosis. The diagnostic criteria based on the WHO classification 2004 should be adopted in routine. CA9 could be a new diagnostic marker for this tumor.

PURPOSE: To evaluate the effects of carbomer sodium hyaluronate trehalose (CHT) and sodium hyaluronate eye drops on tear film stability and ocular discomfort after cataract surgery. SETTING: Santa Maria della Misericordia Hospital, Rovigo, Italy. DESIGN: Prospective randomized case-control study. PATIENTS AND METHODS: This study enrolled sixty patients scheduled for unilateral cataract surgery. After phacoemulsification, subjects received carbomer sodium hyaluronate trehalose (trehalose group) or sodium hyaluronate tears (HG group) substitute and were assessed through objective (break up time, corneal and conjunctival staining) and subjective (OSDI questionnaire) clinical evaluations after a two times a day topical administration. Outcome measures were collected preoperatively (baseline), one week (day 7) and 1 month (day 30) after surgery. Finally, each patient was asked to give his personal treatment satisfaction score. RESULTS: Trehalose group showed a steeper break up time (BUT) increase compared to patient treated with hyaluronic acid (P<0.001). OSDI questionnaire presented a opposite trend, trehalose patients evidenced a significantly major improvement (P<0.001), and in seven days mean values reduced by more than three times. Fluorescein staining reduction was documented with both treatments, although there was no statistically significant difference between groups. Finally CHT resulted in a significantly greater global satisfaction score (P<0.001). CONCLUSIONS: CHT was effective and well tolerated in reducing dry eye disease symptoms and improving the clinical outcome after cataract surgery. On some parameters (BUT, OSDI), this new formulation was more effective than commonly used sodium hyaluronate in treating ocular irritation and tear film alterations.

Se analizaron 47 pacientes operados por carcinoma mamarioo que concurrían a control a la Unidad de Patología Mamaria del HZGA San Roque de Gonnet. Se evaluó la calidad de vida, por medio de instrumentos estandarizados: MMS, SCL-90 y PS Kamolsky index. Las enfermas se separaron en grupos considerados cinco variables: edad, tipo de cirugía, tiempo desde la cirugía, estado civil y estado menopáusico. No se encontraron diferencias al nivel de significación de p= 0,05. Analizada la muestra en conjunto y comparada a una muestra de población de mujeres no pacientes, el 44,7 por ciento de las enfermas fueron consideradas casos o diagnósticos positivos. Se podría concluir que estas pacientes presentan un elevado nivel de distress comparadas a una muestra estándar, pese a no haberse identificado diferencias significativas en las variables analizadas
