RESUMO

Nitrile compounds containing ester and amide groups are important functionalized chemicals in synthetic and medicinal chemistry. In this article, an efficient and convenient palladium-catalyzed carbonylative procedure toward 2-cyano-N-acetamide and 2-cyanoacetate compounds has been developed. The reaction proceeds under mild conditions via radical intermediate which is suitable for late-stage functionalization. Gram-scale experiment was performed successfully under low catalyst loading and gave the target product in excellent yield. Additionally, this transformation can be performed under atmospheric pressure and provide alternative routes to 7 drug precursors.

AIM: This study aimed to synthesize qualitative evidence on experiences of patients with atrial fibrillation (AF) during the course of diagnosis and treatment. We addressed three main questions: (a) What were the experiences of patients with AF during the course of diagnosis and treatment? (b) How did they respond to and cope with the disease? (c) What were the requirements during disease management? DESIGN: In this study, qualitative evidence synthesis was performed using the Thomas and Harden method. DATA SOURCES: Electronic databases, including PubMed, the Cochrane Library, Embase, Web of Science, Cumulative Index to Nursing and Allied Health Literature, the China Biomedical Database, the WanFang Database, Chinese National Knowledge Infrastructure and VIP, were searched. The databases were searched from inception to August 2021. REVIEW METHODS: Two researchers independently selected studies using qualitative assessment and review instruments for quality evaluation and thematic synthesis for the data analysis. RESULTS: A total of 2627 studies were identified in the initial search and 15 studies were included. Five analytical themes were generated: 'Diagnosing AF'; 'The impact of AF on the patients'; 'Self-reorientation in the therapeutic process'; 'Living with AF and QoL'; and 'External support to facilitate coping strategies.' CONCLUSIONS: Our findings point out unique experiences of patients across the trajectory of AF related to delayed diagnosis, feelings of nonsupport, disappointment of repeated treatment failure and multiple distress associated with unpredictable symptoms. Future research and clinical practice are expected to improve the quality of medical diagnosis and treatment, optimize administrative strategy and provide diverse health support for patients with AF. IMPACT: Understanding the experiences and needs of patients with AF in the entire disease process will inform future clinical practice in AF integrated management, which would be helpful in improving the professionalism and confidence of healthcare providers. In addition, our findings have implications for improving the effectiveness of AF diagnostic and treatment services. OBJECTIVE: To assess the effectiveness of tele-monitored cardiac rehabilitation in patients who have undergone ablation for atrial fibrillation. DESIGN: Single-centre, prospective, assessment-blinded, randomised controlled trial. SETTING: Domiciliary rehabilitation with support from a tertiary care hospital. SUBJECTS: One hundred patients who underwent ablation for atrial fibrillation were recruited. INTERVENTIONS: Participants were randomly allocated to a 12-week standard rehabilitation treatment (control group) or a comprehensive, domiciliary, mobile application-guided and tele-monitored cardiac rehabilitation program (intervention group) in a 1:1 fashion. MAIN OUTCOME MEASURES: The primary endpoint was the improvement in VO2peak. The secondary outcomes included adherence, physical activity, beliefs related to cardiovascular disease and exercise self-efficacy. RESULTS: Ninety-seven patients completed follow-up. The mean VO2peak increased significantly in both the intervention group (n = 49) (baseline vs 12 weeks: 19.1 ± 4.7 vs 27.3 ± 5.6 ml/(min kg), P < 0.01) and the control group (n = 48) (baseline vs 12 weeks: 18.7 ± 4.9 vs 22.9 ± 6.3 ml/(min kg), P < 0.01). The results of the between-group analysis of aerobic capacity were significantly in favour of the intervention group. During the 12-week program, patients in the intervention group exhibited better adherence than those in the control group. Moreover, self-reported physical activity improved more in the intervention group than in the control group, as did the beliefs related to cardiovascular disease and exercise self-efficacy (all P < 0.01). CONCLUSIONS: Our domiciliary, mobile application-guided and tele-monitored cardiac rehabilitation program could lead to a more significant improvements in physical fitness, adherence and health beliefs than standard cardiac rehabilitation in patients who have undergone ablation for atrial fibrillation.

Visual simultaneous localization and mapping (VSLAM) plays a vital role in the field of positioning and navigation. At the heart of VSLAM is visual odometry (VO), which uses continuous images to estimate the camera's ego-motion. However, due to many assumptions of the classical VO system, robots can hardly operate in challenging environments. To solve this challenge, we combine the multiview geometry constraints of the classical stereo VO system with the robustness of deep learning to present an unsupervised pose correction network for the classical stereo VO system. The pose correction network regresses a pose correction that results in positioning error due to violation of modeling assumptions to make the classical stereo VO positioning more accurate. The pose correction network does not rely on the dataset with ground truth poses for training. The pose correction network also simultaneously generates a depth map and an explainability mask. Extensive experiments on the KITTI dataset show the pose correction network can significantly improve the positioning accuracy of the classical stereo VO system. Notably, the corrected classical stereo VO system's average absolute trajectory error, average translational relative pose error, and average translational root-mean-square drift on a length of 100-800 m in the KITTI dataset is 13.77 cm, 0.038 m, and 1.08%, respectively. Therefore, the improved stereo VO system has almost reached the state of the art.

MOTIVATION: Electron tomography (ET) is a widely used technology for 3D macro-molecular structure reconstruction. To obtain a satisfiable tomogram reconstruction, several key processes are involved, one of which is the calibration of projection parameters of the tilt series. Although fiducial marker-based alignment for tilt series has been well studied, marker-free alignment remains a challenge, which requires identifying and tracking the identical objects (landmarks) through different projections. However, the tracking of these landmarks is usually affected by the pixel density (intensity) change caused by the geometry difference in different views. The tracked landmarks will be used to determine the projection parameters. Meanwhile, different projection parameters will also affect the localization of landmarks. Currently, there is no alignment method that takes interrelationship between the projection parameters and the landmarks. RESULTS: Here, we propose a novel, joint method for marker-free alignment of tilt series in ET, by utilizing the information underlying the interrelationship between the projection model and the landmarks. The proposed method is the first joint solution that combines the extrinsic (track-based) alignment and the intrinsic (intensity-based) alignment, in which the localization of landmarks and projection parameters keep refining each other until convergence. This iterative approach makes our solution robust to different initial parameters and extreme geometric changes, which ensures a better reconstruction for marker-free ET. Comprehensive experimental results on three real datasets show that our new method achieved a significant improvement in alignment accuracy and reconstruction quality, compared to the state-of-the-art methods. RESUMO

A new strategy to obtain ß,γ-unsaturated ketones via the cross-coupling of 1,3-butadiene, alkyl bromides, and arylboronic acids under 1 bar of CO with nickel as the catalyst has been developed. This newly developed four-component carbonylation procedure features advantages including using a cheap catalytic system, high step economy, mild reaction conditions, and excellent 1,4-regioselectivity, thereby providing a sustainable and alternative tool for ß,γ-unsaturated ketones production compared to the present tactics. To elucidate the application potential of this method, olefin synthons are derived from the representative coupling product.

Ketones exist widely in naturally occurring products and are indispensable building blocks in organic synthesis. Carbonylation represents one of the most straightforward methods for ketone preparation and has become an attractive field in modern organic chemistry as well. Among the strategies, photocatalytic carbonylation is also worthy of further exploration. Herein, we developed a three-component carbonylation that provides a new method for the synthesis of ketones from Hantzsch esters, CO and styrenes. The reaction was performed under a blue light environment and yields a series of ketones with moderate to good yields.

RESUMO

Herein, a new reaction for the site-selective carbonylation of arenes via C(sp2)-H thianthrenation under mild conditions has been developed. With low loadings of palladium catalysts, various desired 1,2-diarylethanones are produced in good yields. This strategy also enables the late-stage modification of complex molecules, which was previously challenging with similar carbonylative Negishi-type reactions.

Carbenes are highly active reaction intermediates, which can be used as reaction precursors to modify organisms, drugs, and material molecules. In this work, we realized a new cheap metal-catalyzed carbonylation of carbene to give propanedioic acid derivatives. With copper salt as the catalyst, synthetically important malonates and related compounds were produced in good yields under mild reaction conditions.

BACKGROUND: Radiofrequency catheter ablation (RFCA) for patients with atrial fibrillation (AF) can generate considerable physical and psychological discomfort under conscious sedation. App-based mindfulness meditation combined with an electroencephalography (EEG)-based brain-computer interface (BCI) shows promise as effective and accessible adjuncts in medical practice. OBJECTIVE: This study aimed to investigate the effectiveness of a BCI-based mindfulness meditation app in improving the experience of patients with AF during RFCA. METHODS: This single-center pilot randomized controlled trial involved 84 eligible patients with AF scheduled for RFCA, who were randomized 1:1 to the intervention and control groups. Both groups received a standardized RFCA procedure and a conscious sedative regimen. Patients in the control group were administered conventional care, while those in the intervention group received BCI-based app-delivered mindfulness meditation from a research nurse. The primary outcomes were the changes in the numeric rating scale, State Anxiety Inventory, and Brief Fatigue Inventory scores. Secondary outcomes were the differences in hemodynamic parameters (heart rate, blood pressure, and peripheral oxygen saturation), adverse events, patient-reported pain, and the doses of sedative drugs used in ablation. RESULTS: BCI-based app-delivered mindfulness meditation, compared to conventional care, resulted in a significantly lower mean numeric rating scale (mean 4.6, SD 1.7 [app-based mindfulness meditation] vs mean 5.7, SD 2.1 [conventional care]; P=.008), State Anxiety Inventory (mean 36.7, SD 5.5 vs mean 42.3, SD 7.2; P<.001), and Brief Fatigue Inventory (mean 3.4, SD 2.3 vs mean 4.7, SD 2.2; P=.01) scores. No significant differences were observed in hemodynamic parameters or the amounts of parecoxib and dexmedetomidine used in RFCA between the 2 groups. The intervention group exhibited a significant decrease in fentanyl use compared to the control group, with a mean dose of 3.96 (SD 1.37) mcg/kg versus 4.85 (SD 1.25) mcg/kg in the control group (P=.003).The incidence of adverse events was lower in the intervention group (5/40) than in the control group (10/40), though this difference was not significant (P=.15). CONCLUSIONS: BCI-based app-delivered mindfulness meditation effectively relieved physical and psychological discomfort and may reduce the doses of sedative medication used in RFCA for patients with AF. Objectives: This study aimed to compare effects of different hand exercises on improving limb function in patients after transradial cardiac catheterization. Methods: This single-center, randomized clinical trial was conducted from August 20, 2020, to July 20, 2021, at an academic medical center. A total of 102 participants were selected from a Class A tertiary hospital in Nanjing, China and randomized into three groups: finger exercise group (n = 34), acupoint massage group (n = 34), and handgrip exercise group (n = 34). Symptoms of edema and pain were defined as primary outcomes, while skin temperature and degree of bleeding as secondary outcomes, which were monitored and measured 1, 2, and 4 h, after the intervention. Results: Among the 99 participants who completed the process, the palm edema was gradually relieved in the handgrip exercise group at 2 h (H = 6.710, P = 0.035) and 4 h (H = 10.060, P < 0.001) following the intervention. The edema of fingers in the handgrip exercise group was obviously relieved at 2 h (H = 9.353, P < 0.01) and 4 h (H = 10.699, P < 0.001) following the intervention compared with the other two groups. In addition, the pain score at 4 h (H = 7.048, P = 0.029) was clearly decreased in the handgrip exercise group. However, there was no significant difference in the punctured limb's skin temperature (H = 0.922, P = 0.631) and the degree of bleeding (H = 0.123, P = 0.940) among the three groups. Conclusions: We found that handgrip exercise is more effective in reducing the edema of the limbs than finger exercise and acupoint massage.

Transforming carbon monoxide (C1) and ethylene (C2) into high value-added chemicals is of great significance from an economic perspective, especially to multifunctionalized C3 compounds. Herein, we developed a palladium-catalyzed thiocarbonylative 1,2-difunctionalization of ethylene. Employing NiXantPhos as the ligand and DCE as the solvent, a series of organic disulfides can be successfully transformed into ß-thiopropionate thioesters in good yields.

The introduction of fluorine atoms into organic molecules is an attractive but challenging topic. In this work, an interesting palladium-catalyzed difluoroalkylative carbonylation of aryl olefins has been developed. A wide range of aryl olefins were transformed into the corresponding difluoropentanedioate compounds with good functional-group tolerance and excellent regioselectivity. Inexpensive ethyl bromodifluoroacetate acts both as a difluoroalkyl precursor and a nucleophile here. Additionally, a scale-up reaction was also performed successfully, and further transformations of the obtained product were shown as well.

A palladium-catalyzed perfluoroalkylative carbonylation of 2-allylaryl trifluoromethanesulfonates has been developed. A range of 2-allyl trifluoromethanesulfonates, perfluoroalkyl halides, and amines were applied in this tandem procedure to provide the corresponding ß-perfluoroalkyl amides with good functional group tolerance and high chemoselectivity. The final products were controlled by the base applied.

In this communication, an efficient procedure using DMC as both a C1 building block and solvent in the aminocarbonylation reaction with nitro compounds has been developed. A nickel-catalyzed one-pot carbonylation reaction of 2-bromobenzenesulfonyl chlorides with alkynes for the synthesis of thiochromenones has been established. Both terminal and internal alkynes were suitable substrates in this carbonylative transformation, and a broad range of 2-mono- and 2,3-disubstituted thiochromenone products were obtained in moderate to good yields with quite high functional group compatibility. Notably, this procedure presents the first example of nickel-catalyzed carbonylative synthesis of thiochromenones with 2-bromobenzenesulfonyl chlorides as a promising sulfur precursor.

Objective: This study aims to explore the influence of patient activation (PA) and relational aspects on the quality of life (QoL) in patients with Atrial Fibrillation (AF) for developing measures to improve PA and QoL. Methods: A cross-sectional study was undertaken in 2021 among 190 AF patients in Nanjing, China. Research instruments included a self-designed social-demographic characteristics scale, the Patient Activation Measure (PAM), the Atrial Fibrillation Effect on Quality of Life (AFEQT). The data analysis was performed using IBM SPSS 25.0. Spearman correlation analysis, multiple linear regression analysis, and Wilcoxon rank-sum tests were used to assess the association accordingly. Results: The average AFEQT score for the 190 AF patients was 69.32 ± 14.52. The distribution of activation Levels 1, 2, 3, and 4, were where 4.7, 34.2, 47.4, and 13.7%, respectively. The multiple linear regression analysis revealed that patient activation, work status, and cardiac rehabilitation of AF patients predicted AF-related QoL (ß = 0.270, -0.205, and 0.183, respectively; all P < 0.05). The influences of PA level on subdimensions of AF-related QoL were as follows: symptoms, daily activities and treatment concern. Conclusion: The level of QoL of patients with AF was moderate. Higher levels of patient activation in those with AF were associated with milder symptoms, more positive daily activities and fewer treatment concern. Based on our findings, we suggest that healthcare personnel should encourage AF patients to take active participation in cardiac rehabilitation, disease self-management and foster progression of PA level. Future research is warranted to develop tailor-made interventions aimed at the activation level.

RESUMO

An interesting procedure for thioester synthesis via nickel-catalyzed thiocarbonylation of arylboronic acid with sulfonyl chlorides as the sulfur source has been explored. Using Mo(CO)6 as a solid CO surrogate and reductant, a broad range of thioesters were obtained in moderate to good yields with good functional group tolerance.
