RESUMO

An outbreak of staphylococcal bullous impetigo occurred over a period of five months in a maternity ward involving seven infected and two colonised neonates. The skin lesions were due to epidermolytic toxin A-producing Staphylococcus aureus. Infection control measures were implemented and a retrospective case-control study performed. Contact with an auxiliary nurse was the only risk factor for cases of bullous impetigo (P<0.01). The nurse cared for all seven cases and was an asymptomatic nasal carrier of the epidemic strain. Repeated courses of decontamination treatment failed to eradicate carriage. Nine months after the last case, another neonate developed a more severe form of bullous impetigo and the auxiliary nurse was reassigned to an adult ward.

UNLABELLED: Extrapulmonary tuberculosis is usually more difficult to diagnose than pulmonary tuberculosis. It often involves inaccessible sites and it is paucibacillary. OBJECTIVE AND METHOD: In this study, we tried to analyze the performance of various bacteriological methods used to diagnose 51 cases of extrapulmonary tuberculosis in an infectious diseases ward. RESULTS AND COMMENTS: The culture was positive for 55% of patients. The new amplification methods used were very disappointing for the testing of nonrespiratory samples. The sensitivity of PCR was 32% compared to that of diagnostic culture which remains the most sensitive reference method. Liquid media allow for rapid growth and limit the contaminations. The combined inoculation of liquid and solid medium increases the sensitivity and helps to identify the pathogen.

RESUMO

The purpose was to evaluate the interest of stereotaxic fine-needle aspiration for round opacities when the ultrasound and echoguided punctures are inefficient; especially when women are under menopausal hormonal replacement therapy. Sixty stereotactic guided fine-needle aspirations detected by mammography have been performed between january 1990 and august 1996. The stereotaxic procedure is performed with a DMR unit (GE with Stereotix II). Stereotaxic views are done to verify needle position. After aspiration, cytologic examination is realised. Cystic fluid was always obtained and cytologic examination proved benign cysts in all cases. In 50 cases, cysts completely disappeared. There were 2 relapses that received after a second. This method is reliable for evaluation of non palpable mammographically detected opacities. The use of this technique spares the patient a surgical biopsy. This procedure enables women under menopausal hormone replacement therapy to continue the treatment.
