RESUMO

The authors describe a case of gastric tuberculosis in a 23 years old African patient from Abidjan. Symptomatology included epigastralgia, post prandial vomiting, severe impoverishment of health state and dehydration. Endoscopy showed a pseudopolypoid diffuse gastritis with an infiltrated like mucosa. Endoscopic biopsy was carried out, but, because of clinical aggravation (epigastric muscular defense), the patient underwent a laparotomy before knowing the results of the biopsy. Gastrotomy showed an hypertrophied granulomatous hemorrhagic mucosa. By histological examination it was concluded in favour of a granulomatous gastritis with pan-parietal tuberculoid lesions without acid-alcohol-fast bacilli. The different causes of granulomatous gastritis were reviewed and a tuberculous etiology suspected. An antituberculous therapy was prescribed with excellent results on clinical and endoscopic standpoint one year after the treatment started.

RESUMO

Staphyococcal septicemia is rare (0.19 p. 100 of admittances). The origin of this kind of infection could be located in all cases out of the sanitary units, and in 12 out of the 14 cases, patients under 30 years old were concerned. The main disorders were in pulmonary tract, kidneys and heart. A fatal issue occured 6 times due in most cases to a delayed admittance. A good study of sensitivity to antibiotics and a treatment associating several antibiotics are necessary.

RESUMO

The authors report a cause of acute cor pulmonale caused by Schistosoma mansoni bilharziosis. The clinical feature was a collapse with right ventricular failure following a delivery. The hemodynamic exploration showed a pre-capillary and supra-systemic pulmonary hypertension. The histopathologic study in post-mortem showed pulmonary lesions of fibrosis endarteritis with granulomatosis reaction round many eggs of Schistosoma mansoni. The authors discuss the frequency of various anatomic lesions (20, 30%), hemodynamic (20%) and clinical (2,5%) of the pulmonary bilharziosis. They recall the anatomical and histological mechanisms of the pulmonary artery hypertension in this disease. They emphasize the originality of this observation which is due to its super acute character and its Schistosoma mansoni bilharziosis etiology whereas pulmonary demonstrations are usually due to Schistosoma haematobium.

RESUMO

Seven cases of L.I.H. are noted simultaneously in a military group in the suburbs of Algier. It is an anademic by contamination at the same point, without interhuman infection. Only two of these cases have clinical symptoms to permit diagnosis of the others by research of specific antibodies (Immunodiagnosis). Anicteric forms are very frequent. This explains probably failure of diagnosis when epidemiological conditions are realized. This unawareness of leptospiroses is unfortunately a general phenomenon in the world.
