RESUMO

In this paper we demonstrate strain-dependent photoacoustic (PA) characteristics of free-standing nanocomposite transmitters that are made of carbon nanotubes (CNT) and candle soot nanoparticles (CSNP) with an elastomeric polymer matrix. We analyzed and compared PA output performances of these transmitters which are prepared first on glass substrates and then in a delaminated free-standing form for strain-dependent characterization. This confirms that the nanocomposite transmitters with lower concentration of nanoparticles exhibit more flexible and stretchable property in terms of Young's modulus in a range of 4.08-10.57 kPa. Then, a dynamic endurance test was performed revealing that both types of transmitters are reliable with pressure amplitude variation as low as 8-15% over 100-800 stretching cycles for a strain level of 5-28% with dynamic endurance in range of 0.28-2.8%. Then, after 2000 cycles, the transmitters showed pressure amplitude variation of 6-29% (dynamic endurance range of 0.21-1.03%) at a fixed strain level of 28%. This suggests that the free-standing nanocomposite transmitters can be used as a strain sensor under a variety of environments providing robustness under repeated stretching cycles.

Osteoarthritis sufferers commonly have first metatarsophalangeal joint (MTPJ) problems in which articular surfaces are changed permanently due to fatigue. Therefore, medical devices for early diagnosis would increase the opportunity for prevention of disease progression. In previous studies on stiffness of the first MTPJ many details, although functionally of great importance, have not been fully considered including: design and size of the device, tribology consideration, and errors from device. Therefore, the motivation of our research was to enhance the device design by reducing the size of the device, and device design was enhanced by minimizing measurement errors through development of a new ergonomic left and right foot instrument located medial to the first MTPJ (instead of beneath the foot). The first MTPJ stiffness (N mm/kg radian) measurement was taken on 28 subjects with two replicates per subject by the same tester. The first MTPJ stiffness ranged from 3.49 to 14.42 N mm/kg radian with the mean (SD) value of 8.28 (3.15) N mm/kg radian for the left feet and 3.91 to 11.90 N mm/kg radian with the mean (SD) value of 7.65 (2.07) N mm/kg radian for the right feet. Reliability evaluation was measured using intraclass correlation coefficient and described an excellent reliability between two tests.

Modern machining requires reduction in energy usage, surface roughness, and burr width to produce finished or near-finished parts. To ensure high surface quality in machining processes, it is crucial to minimize surface finish and minimize burr width, which are considered as significant parameters as specific cutting energy. The objective of this study was to identify the optimal machining parameters for milling in order to minimize surface roughness, burr width, and specific cutting energy. To achieve this, the research investigated the impact of feed per tooth, cutting speed, depth of cut, and number of inserts on the responses across three intervals using Taguchi L9 array. Observing the responses by varying these parameters, underlined the need for multi objective optimisation. Machining conditions of 0.14 mm/tooth f z , 350 m/min V c and 2 mm ap using 1 cutting insert (exp no 9) was identified as the best machining run using grey relational analysis owing to its highest grey relational grade of 0.936. ANOVA examination identified cutting speed as the leading factor impacting the grey relational grade with 31.07 % contribution ratio, with the number of inserts, depth of cut, and feed per tooth also making notable contributions. Conclusively, machining parameters identified through response surface optimisation resulted in 21.69 % improvement in surface finish, 11.39 % reduction in specific energy consumption, and 6.2 % decrease in burr width on the down milling side albeit with an increase of 9 % in burr width on the up-milling side.

The gut-brain axis is a communication pathway that allows a two-way exchange of information between the microbiota of the gastrointestinal tract and the nervous system of humans. The vagus nerve, which is responsible for facilitating communication, provides support for this axis. The gut-brain axis is currently the subject of research, but studies into the diversity and stratification of the gut microbiota are just getting started. Researchers have discovered several positive trends by analyzing numerous studies examining the gut microbiota's impact on the effectiveness of SSRIs. It is common knowledge that a specific group of measurable, microbial markers has been recognized as being present in the feces of individuals suffering from depression. Specific bacterial species are a common denominator among therapeutic bacteria used to treat depression. It can also play a role in determining the severity of disease progression. Evidence that selective serotonin reuptake inhibitors (SSRIs) rely on the vagus nerve to exert their therapeutic effects has provided further support for the importance of the vagus nerve in the gut-brain axis, which is necessary for beneficial changes in the gut microbiota. This review will analyze the research linking gut microbiota to depression.

BACKGROUND & OBJECTIVE: Rape is a severe kind of sexual violence that represents a significant violation of human rights. The objective was to investigate the demographic and clinical features of female survivors of sexual assault in Lahore, Pakistan. METHOD: A retrospective study was conducted at the Department of Forensic Medicine of Lahore General Hospital, analysing records of female sexual assault survivors investigated by affiliated police stations from November 2020 to November 2022. Data were collected from medicolegal certificates and medical notes and analysed using SPSS software. RESULT: 282 female survivors reported to Lahore General Hospital's Forensic Medicine Department, aged 5-55 years, with a mean age of 22.74 ± 8.30. A significant proportion (50.4%) were aged 16-25 years. Most survivors were examined between 6 and 24 h (34%) and 2-7 days (37.9%) after the assault, with the majority (95%) exhibiting well-developed secondary sexual characteristics. Sexual assault and physical abuse were both reported in 56.4% of cases. In 77% of cases, the perpetrator was known, and in 36.5% of cases, a weapon was used. No external body injury was observed in 74.8% of cases, and 2.8% showed signs of fresh hymen rupture. The study identified 92.2% of cases lost to follow-up. CONCLUSION: This study highlights the pressing need to implement effective measures to prevent and address rape in Pakistan. The findings emphasise the need for education, improved forensic infrastructure and policy reforms to address sexual assault effectively.

Polymer matrix wave transparent composites are used in a variety of high-speed communication applications. One of the applications of these involves making protective enclosures for antennas of microwave towers, air vehicles, weather radars, and underwater communication devices. Material performance, structural, thermal, and mechanical degradation are matters of concern as advanced wireless communication needs robust materials for radomes that can withstand mechanical and thermal stresses. These polymer composite radomes are installed externally on antennas and are exposed directly to ambient as well as severe conditions. In this research, epoxy resin was reinforced with a small amount of quartz fibers to yield an improved composite radome material compared to a pure epoxy composite with better thermal and mechanical properties. FTIR spectra, SEM morphology, dielectric constant (Ɛr) and dielectric loss (δ), thermal degradation (weight loss), and mechanical properties were determined. Compared to pure epoxy, the lowest values of Ɛr and δ were 3.26 and 0.021 with 30 wt.% quartz fibers in the composite, while 40% less weight loss was observed which shows its better thermal stability. The mechanical characteristics encompassing tensile and bending strength were improved by 42.8% and 48.3%. In high-speed communication applications, compared to a pure epoxy composite, adding only a small quantity of quartz fiber can improve the composite material's dielectric performance, durability, and thermal and mechanical strength.

Autoimmune hemolytic anemia (AIHA) is a type of hemolytic anemia in which autoantibodies attack the membrane antigens of red blood cells, causing cell rupture (lysis). Hemolysis stimulates compensatory RBC production by boosting erythropoietin levels; however, this response is often insufficient to restore normal hemoglobin blood levels, resulting in anemia. It is a rare disease, with an annual incidence of one case in every 80 000 live births. Infants of any age can be affected, though neonatal incidence is unusual. Here, the authors report a rare case of AIHA in the neonatal period with concomitant atrial septal defect, ventricular septal defect, and patent ductus arteriosus. Case presentation: A one-hour-old male neonate weighing 3 kg who was born at 38 weeks of gestation presented to the pediatric department with the complaint of respiratory distress. Examination revealed obvious respiratory distress with subcostal and intercostal recessions and a continuous grade 2 murmur at the left upper chest; the liver was palpable 1 cm below the right subcostal margin with a palpable splenic tip. Laboratory investigations were ordered, which showed hemoglobin was decreasing continuously and bilirubin was raised, suspecting AIHA. A positive blood culture, tachycardia, tachypnea, and a raised leukocyte count showed that the baby was in sepsis. The baby improved clinically, and the complete blood count showed improved Hb. Cardiac examination findings and a second-grade continuous murmur at the left upper chest were further investigated through echocardiography, which showed a grade 2 atrial septal defect, a muscular ventricular septal defect, and a patent ductus arteriosus. Clinical discussion: Childhood AIHA is a rare and underrated disease that differs from the adult form. The disease's initial manifestation and subsequent course are both poorly understood. It affects mostly young children, and a high prevalence (21%) is found in infants. In some patients, there is a genetic predisposition to the development of this disease, and there is underlying immune deregulation in more than half of the cases, necessitating long-term homogeneous multidisciplinary follow-up. It is of two types, primary and secondary, and according to the study conducted in France, AIHA is associated not only with other autoimmune diseases but with some systemic diseases as well, like neurological, digestive, chromosomal abnormalities, and cardiac diseases, as in our case. Conclusion: There is a scarcity of data on clinical management and treatment strategies. More research should be done to know the environmental factors that can trigger the immune response against red blood cells. Moreover, a therapeutic trial is essential for a better outcome and helps prevent serious complications.

Domperidone is an antagonist of the peripheral dopamine (D2) receptor. It works as an antiemetic by blocking D2-receptors at the chemoreceptor trigger zone and as a gastroprokinetic drug by blocking GI tract D2-receptors. According to research, using domperidone significantly raises the risk of cardiac arrhythmia and sudden cardiac death by 70%, most likely through prolonging the QT interval. Blockade of hERG voltage-gated potassium channels is thought to be the reason. Here in Pakistan, this drug is being prescribed by every other physician and even patients frequently self-medicate themselves with it. Due to the serious side effects of this medication, extreme caution should be exercised when prescribing it, especially to the elderly, those who have underlying QT prolongation, those taking medications known to prolong QT, and even more so in pregnant women as there is some evidence that domperidone crosses into breast milk in small amounts and causes an irregular heartbeat in the baby. At least we, on our part, can limit the usage of the drug only with a prescription and, where necessary, if not completely, stop it.

Acute coronary syndrome (ACS) is a complex condition characterized by myocardial ischemia or infarction which can lead to significant morbidity and death. Antiplatelet drugs play a crucial role in the management of ACS and have been shown to minimize the incidence of significant adverse cardiovascular events and recurrent myocardial infarction (MI). This comprehensive literature review is intended to summarize current information on the effectiveness, safety, and function of frequently used antiplatelet medications in treating ACS. Aspirin, clopidogrel, prasugrel, ticagrelor, abciximab, tirofiban, dipyridamole, cilostazol, and novel antiplatelets are included in the review. Aspirin's effectiveness as a first-line antiplatelet medication in ACS is well established. It has significantly lowered the risk of serious adverse cardiovascular events. Clopidogrel, prasugrel, and ticagrelor are P2Y12 receptor inhibitors found to lower the incidence of recurrent ischemia episodes in ACS patients. Using glycoprotein IIb/IIIa inhibitors such as abciximab, tirofiban, and eptifibatide is effective in managing ACS, especially in high-risk patients. Dipyridamole effectively reduces the risk of recurrent ischemic events in patients with ACS, particularly when used with aspirin. Cilostazol, a phosphodiesterase III inhibitor, has also been shown to reduce the risk of major adverse cardiovascular events (MACE) in patients with ACS. Antiplatelet drugs' safety in managing ACS has also been well established. Aspirin is generally well-tolerated with a low risk of adverse effects, although the risk of bleeding events, particularly gastrointestinal bleeding, cannot be eliminated. The P2Y12 receptor inhibitors have been associated with a small increase in the risk of bleeding events, particularly in patients with a high risk of bleeding. The glycoprotein IIb/IIIa inhibitors are associated with a higher risk of bleeding than other antiplatelet drugs, especially in high-risk patients. To summarize, antiplatelet drugs play a crucial role in the management of ACS, and the efficacy and safety of these drugs have been well-established in the literature. The choice of antiplatelet drugs will depend on the patient's risk factors, including age, comorbidities, and bleeding risk. The novel antiplatelets may offer new therapeutic options for managing ACS, and further studies are needed to determine their role in managing this complex condition.

In recent years, the study of interleukins (ILs), crucial cytokines involved in immune response and inflammation, has garnered significant attention within the sphere of cardiovascular diseases (CVDs). The research has provided insights into the involvement of ILs in diverse CVDs, including arrhythmias, myocardial infarction, atherosclerosis, and heart failure (HF). ILs have emerged as promising therapeutic targets for drug interventions through their involvement in disease development and progression. This comprehensive review provides a detailed overview of ILs, elucidating their functions within the immune system and offering insights into their specific contributions to various CVDs. Moreover, the article delves into the examination of current and potential drug therapies that selectively target ILs in the management of CVDs, presenting a comprehensive analysis of the advantages and disadvantages associated with these therapeutic approaches. A comprehensive literature review was conducted to investigate the involvement of ILs in CVDs. The relevant articles were searched on PubMed, PubMed Central, Medline, Cochrane, Google Scholar, and ScienceDirect databases. The search encompassed articles published from these databases' inception until July 12, 2023. We first examine generalized aspects of ILs, particularly CVDs. Then, we shift focus towards examining the direct impact of ILs on cardiac cells and tissue; on the immune system and inflammation; endothelial cells and vascular function; and finally, their interactions with other signaling pathways and molecules. Then, we discuss the molecular mechanisms of various ILs. Sequentially, we delve into a comprehensive analysis of the individualized role of each distinct IL in diverse CVDs, examining their specific contributions. Finally, we explore the potential for targeted drug therapy to modulate IL activity, aiming to enhance outcomes for patients burdened with CVD. The objective is the identification of gaps in current knowledge and highlight areas that require further investigation within the context of cardiovascular medicine. Through deepening our comprehension of the intricate involvement of ILs in CVDs and harnessing their potential for targeted drug therapy, novel treatment strategies can be devised, leading to improved patient outcomes in cardiovascular medicine.

Percutaneous coronary intervention (PCI) is a widely used therapy for coronary artery disease (CAD), but it carries risks and complications. Adhering to evidence-based practice guidelines is crucial for optimal outcomes. This review compares the recommendations of the 2021 American College of Cardiology/American Heart Association/Society for Cardiovascular Angiography and Interventions (ACC/AHA/SCAI) and 2018 European Society of Cardiology (ESC) guidelines for coronary artery revascularization and discusses emerging trends and novel devices in PCI. A comprehensive literature review of mixed studies, clinical trials, and guidelines was conducted. Intravascular imaging, including intravascular ultrasound and optical coherence tomography, for stent optimization, is also recommended when feasible. However, differences reflecting variations in evidence quality interpretation and applicability were identified. Furthermore, novel devices and technologies with the potential for improving outcomes were highlighted, but their safety and efficacy compared to standard-of-care techniques require further evaluation through extensive randomized trials. Clinicians should stay updated on advancements and personalize treatment decisions based on individual patient factors. Future research should address evidence gaps and barriers to adopting innovative devices and techniques. This review provides recommendations for clinical practice, emphasizing the need to remain current with the evolving landscape of PCI to optimize patient outcomes. The discoveries provide valuable counsel for the deliberation of clinical interventions and prospective inquiries within the realm of interventional cardiology. Overall, the review underscores the importance of evidence-based practice and ongoing advancements in PCI for CAD management.

Cardiovascular diseases (CVDs) pose a significant burden on global health. Developing effective diagnostic, therapeutic, and prognostic indicators for CVDs is critical. This narrative review explores the role of select non-coding RNAs (ncRNAs) and provides an in-depth exploration of the roles of miRNAs, lncRNAs, and circRNAs in different aspects of CVDs, offering insights into their mechanisms and potential clinical implications. The review also sheds light on the diverse functions of ncRNAs, including their modulation of gene expression, epigenetic modifications, and signaling pathways. It comprehensively analyzes the interplay between ncRNAs and cardiovascular health, paving the way for potential novel interventions. Finally, the review provides insights into the methodologies used to investigate ncRNA-mediated gene regulation in CVDs, as well as the implications and challenges associated with translating ncRNA research into clinical applications. Considering the broader implications, this research opens avenues for interdisciplinary collaborations, enhancing our understanding of CVDs across scientific disciplines.

A new threat to global health re-emerged with monkeypox's advent in early 2022. As of November 10, 2022, nearly 80,000 confirmed cases had been reported worldwide, with most of them coming from places where the disease is not common. There were 53 fatalities, with 40 occurring in areas that had never before recorded monkeypox and the remaining 13 appearing in the regions that had previously reported the disease. Preliminary genetic data suggest that the 2022 monkeypox virus is part of the West African clade; the virus can be transmitted from person to person through direct interaction with lesions during sexual activity. It is still unknown if monkeypox can be transmitted via sexual contact or, more particularly, through infected body fluids. This most recent epidemic's reservoir host, or principal carrier, is still a mystery. Rodents found in Africa can be the possible intermediate host. Instead, the CDC has confirmed that there are currently no particular treatments for monkeypox virus infection in 2022; however, antivirals already in the market that are successful against smallpox may mitigate the spread of monkeypox. To protect against the disease, the JYNNEOS (Imvamune or Imvanex) smallpox vaccine can be given. The spread of monkeypox can be slowed through measures such as post-exposure immunization, contact tracing, and improved case diagnosis and isolation. Final Thoughts: The latest monkeypox epidemic is a new hazard during the COVID-19 epidemic. The prevailing condition of the monkeypox epidemic along with coinfection with COVID-19 could pose a serious condition for clinicians that could lead to the global epidemic community in the form of coinfection.

Revolutionary advancements in regenerative medicine have brought stem cell therapy to the forefront, offering promising prospects for the regeneration of ischemic cardiac tissue. Yet, its full efficacy, safety, and role in treating ischemic heart disease (IHD) remain limited. This literature review explores the intricate mechanisms underlying stem cell therapy. Furthermore, we unravel the innovative approaches employed to bolster stem cell survival, enhance differentiation, and seamlessly integrate them within the ischemic cardiac tissue microenvironment. Our comprehensive analysis uncovers how stem cells enhance cell survival, promote angiogenesis, and modulate the immune response. Stem cell therapy harnesses a multifaceted mode of action, encompassing paracrine effects and direct cell replacement. As our review progresses, we underscore the imperative for standardized protocols, comprehensive preclinical and clinical studies, and careful regulatory considerations. Lastly, we explore the integration of tissue engineering and genetic modifications, envisioning a future where stem cell therapy reigns supreme in regenerative medicine.

Cement and concrete are among the major contributors to CO2 emissions in modern society. Researchers have been investigating the possibility of replacing cement with industrial waste in concrete production to reduce its environmental impact. Therefore, the focus of this paper is on the effective use of wheat straw ash (WSA) together with silica fume (SF) as a cement substitute to produce high-performance and sustainable concrete. Different binary and ternary mixes containing WSA and SF were investigated for their mechanical and microstructural properties and global warming potential (GWP). The current results indicated that the binary and ternary mixes containing, respectively, 20% WSA (WSA20) and 33% WSA together with 7% SF (WSA33SF7) exhibited higher strengths than that of control mix and other binary and ternary mixes. The comparative lower apparent porosity and water absorption values of WSA20 and WSA33SF7 among all mixes also validated the findings of their higher strength results. Moreover, SEM-EDS and FTIR analyses has revealed the presence of dense and compact microstructure, which are mostly caused by formation of high-density calcium silicate hydrate (C-S-H) and calcium hydroxide (C-H) phases in both blends. FTIR and TGA analyses also revealed a reduction in the portlandite phase in these mixes, causing densification of microstructures and pores. Additionally, N2 adsorption isotherm analysis demonstrates that the pore structure of these mixes has been densified as evidenced by a reduction in intruded volume and a rise in BET surface area. Furthermore, both mixes had lower CO2-eq intensity per MPa as compared to control, which indicates their significant impact on producing green concretes through their reduced GWPs. Thus, this research shows that WSA alone or its blend with SF can be considered as a source of revenue for the concrete industry for developing high-performance and sustainable concretes.

The emission of greenhouse gases and natural-resource depletion caused by the production of ordinary Portland cement (OPC) have a detrimental effect on the environment. Thus, an alternative means is required to produce eco-friendly concrete such as geopolymer concrete (GPC). However, GPC has a complex cementitious matrix and an ambiguous mix design. Aside from that, the composition and proportions of materials utilized may have an impact on the compressive strength. Similarly, the use of robust and efficient machine-learning (ML) approaches is now required to forecast the strength of such a composite cementitious matrix. As a result, this study anticipated the compressive strength of GPC with waste resources using ensemble and non-ensemble ML algorithms. This was accomplished through the use of Anaconda (Python). To build a strong ensemble learner by integrating weak learners, adaptive boosting, random forest (RF), and ensemble learner bagging were employed. Furthermore, ensemble learners were utilized on non-ensemble or weak learners, such as decision trees (DT) and support vector machines (SVM) via regression. The data encompassed 156 statistical samples in which nine variables, namely superplasticizer (kg/m3), fly ash (kg/m3), ground granulated blast-furnace slag (GGBS), temperature (°C), coarse and fine aggregate (kg/m3), sodium silicate (Na2SiO3), and sodium hydroxide (NaOH), were chosen to anticipate the results. Exploring it in depth, twenty sub-models with ensemble boosting and bagging approaches were trained, and tuning was performed to achieve the highest possible coefficient of determination (R2). Moreover, cross K-Fold validation analysis and statistical checks were performed via indicators for the evaluation of the models. The result revealed that ensemble approaches yielded robust performance compared to non-ensemble algorithms. Generally, an ensemble learner with the RF and bagging approach on a DT yielded robust performance by achieving a better R2 as 0.93, and with the lowest statistical errors. The communal model in artificial-intelligence analysis, on average, improved the accuracy of the model.

RESUMO

An intrusion detection system (IDS) is designed to detect and analyze network traffic for suspicious activity. Several methods have been introduced in the literature for IDSs; however, due to a large amount of data, these models have failed to achieve high accuracy. A statistical approach is proposed in this research due to the unsatisfactory results of traditional intrusion detection methods. The features are extracted and selected using a multilayer convolutional neural network, and a softmax classifier is employed to classify the network intrusions. To perform further analysis, a multilayer deep neural network is also applied to classify network intrusions. Furthermore, the experiments are performed using two commonly used benchmark intrusion detection datasets: NSL-KDD and KDDCUP'99. The performance of the proposed model is evaluated using four performance metrics: accuracy, recall, F1-score, and precision. The experimental results show that the proposed approach achieved better accuracy (99%) compared with other IDSs.

Rapid industrialization is leading to the pollution of underground natural soil by alkali concentration which may cause problems for the existing expansive soil in the form of producing expanding lattices. This research investigates the effect of stabilizing alkali-contaminated soil by using fly ash. The influence of alkali concentration (2 N and 4 N) and curing period (up to 28 days) on the unconfined compressive strength (UCS) of fly ash (FA)-treated (10%, 15%, and 20%) alkali-contaminated kaolin and black cotton (BC) soils was investigated. The effect of incorporating different dosages of FA (10%, 15%, and 20%) on the UCSkaolin and UCSBC soils was also studied. Sufficient laboratory test data comprising 384 data points were collected, and multi expression programming (MEP) was used to create tree-based models for yielding simple prediction equations to compute the UCSkaolin and UCSBC soils. The experimental results reflected that alkali contamination resulted in reduced UCS (36% and 46%, respectively) for the kaolin and BC soil, whereas the addition of FA resulted in a linear rise in the UCS. The optimal dosage was found to be 20%, and the increase in UCS may be attributed to the alkali-induced pozzolanic reaction and subsequent gain of the UCS due to the formation of calcium-based hydration compounds (with FA addition). Furthermore, the developed models showed reliable performance in the training and validation stages in terms of regression slopes, R, MAE, RMSE, and RSE indices. Models were also validated using parametric and sensitivity analysis which yielded comparable variation while the contribution of each input was consistent with the available literature.
