RESUMO

BACKGROUND: The prevalence of daily cigarette smoking has dropped to 10% in Hong Kong (HK) in 2017, however, smoking still kills 5700 persons per year. Studies suggest that abstinence rates are higher with combined NRT than single NRT, although local data on safety and benefits of combined NRT are lacking. The aim of this study is to compare the effectiveness of combined NRT with single NRT among HK Chinese. METHODS: This is a one-year, two-arm, parallel randomised trial. Five hundred sixty smokers, who smoked ≥10 cigarettes/day for ≥1 year, were randomized to combined and single NRT. Combined NRT group received counseling and nicotine patch & gum. Single NRT group received counselling and nicotine patch. Primary outcome was abstinence rate measured as self-reported 7-day point prevalence with CO validated at 52 weeks. Secondary outcomes included smoking abstinence rates at 4, 12, & 26 weeks. Crude odds ratio and p-value were reported from logistic regression without adjustment; for trend analysis, adjusted odds ratio (AOR) and p-value were reported from Generalized Estimating Equation (GEE) (controlling for time). All AORs were adjusted for age, sex, baseline CO and clusters. RESULTS: Abstinence rates at 4, 12, 26 and 52 weeks were all higher in the combined NRT group (35.8, 21.9, 16.8, 20.1%) compared with the single NRT group (28, 16.8, 11.2, 14.3%). At 4 weeks, combined NRT group was more likely to quit smoking (OR 1.43, 95% CI, 1.00 to 2.05) than the single NRT group. From GEE analysis, combined NRT group had a significantly higher abstinence rate (23.6%) than the single NRT group (17.6%) across repeated measures at all-time points. Combined NRT group was more likely to quit smoking (OR 1.43, 95% CI, 1.15 to 1.77). No significant difference in the side effect profile was detected between groups. CONCLUSIONS: Smokers given 8 weeks of combined NRT were more likely to quit smoking at 4, 12, 26 and 52 weeks compared with single NRT. Combined NRT was as well tolerated as single NRT and it should be further promoted in our community. TRIAL REGISTRATION: NCT03836560 from ClinicalTrial.gov , 9 Feb 2019.

RESUMO

INTRODUCTION: Coagulopathy-associated intracerebral haemorrhage has become increasingly common because of the rising demand in the ageing population for anticoagulation for atrial fibrillation. This study compared the clinical features and neurological outcomes of intracerebral haemorrhage in patients with atrial fibrillation who were prescribed warfarin with those who were not. METHODS: This was a retrospective matched case series of patients with intracerebral haemorrhage from three tertiary hospitals in Hong Kong from 1 January 2006 to 31 December 2011. Patients who developed intracerebral haemorrhage and who were prescribed warfarin for atrial fibrillation (ICH-W group) were compared with those with intracerebral haemorrhage and not prescribed warfarin (ICH-C group); they were matched for age and gender in 1:1 ratio. Clinical features and neurological outcomes were compared, and the impact of coagulopathy on haematoma size was also studied. RESULTS: We identified 114 patients in the ICH-W group with a mean age of 75 years. Both ICH-W and ICH-C groups had a median intracerebral haemorrhage score of 2. There was a non-statistically significant trend of higher intracerebral haemorrhage volume in the ICH-W group (12.9 mL vs 10.5 mL). The median modified Rankin Scale and the proportion with good recovery (modified Rankin Scale score ≤3) at 6 months were comparable. Nonetheless, ICH-W patients had higher hospital mortality (51.8% vs 36.0%; P=0.02) and 6-month mortality (60.5% vs 43.0%; P=0.01) than ICH-C patients. Overall, 60% of ICH-W patients had their admission international normalised ratio within the therapeutic range during intracerebral haemorrhage, and 14% had a subtherapeutic admission international normalised ratio. International normalised ratio at admission was not associated with intracerebral haemorrhage volume or neurological outcome. CONCLUSION: Warfarin-associated intracerebral haemorrhage in patients with atrial fibrillation carried a higher stroke mortality than the non-warfarinised patients.

RESUMO

BACKGROUND: In Singapore, employees aged 62-65 can continue to be employed if they meet the re-employment eligibility criteria. This policy, coupled with an ageing workforce, calls for age-friendly initiatives, specific to work-related challenges faced by older nurses. AIM: To determine work-related challenges faced by older nurses. METHODS: A mixed method sequential explanatory study was conducted with nurses, aged 50 and above, working in a healthcare cluster in Singapore. In the quantitative phase, a questionnaire was administered to 534 nurses to elicit work-related challenges, then in-depth interviews with 30 nurses were carried out to help explain why certain tasks and work circumstances became harder. RESULTS: Results of the survey indicated that the top three challenges were coping with changes, working with computers and reading labels. Place of work, salary range, gender and race were significantly associated with different work-related challenges. Five themes emerged from the qualitative data: physical demands of work and workload, new technology, need for further education, working with younger nurses and in intercultural teams, and changing public expectations and professional image. DISCUSSION AND CONCLUSION: The study supports the current literature on the challenges older nurses face with technological advancement. However, older nurses in our study reported less aches and pain as compared to that reported elsewhere. There is a need for specific strategies that will address changes in work processes and environment in order to retain older nurses. IMPLICATIONS FOR NURSING AND NURSING POLICY: When devising age-friendly work improvement initiatives, it is important for nurse leaders to factor in the needs of nurses working in different care environments, who are of different ranks, or are from different ethnic backgrounds.

RESUMO

BACKGROUND: High blood pressure (BP) is a leading contributor to cardiovascular mortality globally. There is scarcity of information on effective health systems interventions to lower BP and reduce cardiovascular risk in Southeast Asian countries. We conducted a pilot exploratory trial on 100 adults aged 40 years or older with uncontrolled hypertension to optimize the design for a structured multi-component intervention in primary care clinics for management of hypertension. METHODS: Two clinics were involved, each enrolling 50 participants, with one as the intervention clinic and the other as the control (usual care). The intervention comprised the following four components: 1) an algorithm-driven intervention using a fixed-dose combination (FDC) antihypertensive treatment and lipid lowering medication for high risk individuals, 2) subsidized FDC antihypertensive medication; 3) motivational conversation (MC) for high risk individuals; and 4) telephone follow-ups of all individuals. The process outcomes were intervention fidelity measures. The outcomes of change in parameters of interest were healthy lifestyle index (composite score of body mass index, physical activity, dietary habit, dietary quality and smoking), adherence to antihypertensive medications, and systolic and diastolic BP from baseline to follow-up at 3 months. RESULTS: Greater than 90 % fidelity was achieved for 3 of the 4 intervention components. Although not designed for conclusive results, the healthy lifestyle score increased by 0.16 (±0.68) with the intervention and decreased by 0.18 (±0.75) with usual care (p = 0.02). Adherence to anti-hypertensive medications at follow-up was 95.3 % in the intervention group compared to 83.8 % for usual care (p = 0.01). Systolic and diastolic BP decreased in both intervention and control groups, although statistical significance between groups was not achieved. Hypertensive individuals rated all intervention components 'highly favorable' on a Likert scale. CONCLUSIONS: Our findings indicate that the proposed, structured multi-component approach for management of hypertension is feasible for implementation in primary care clinics in Singapore, with some changes to the protocol. The observed improvement in the healthy lifestyle index and adherence to anti-hypertensive medications is promising. A large scale, adequately powered trial would be informative to assess intervention effectiveness on BP and cardiovascular risk reduction. RESUMO

Three human test series were conducted to evaluate the uniformity of the thermal environments in a stratum-ventilated chamber with dimensions of 8.8 m (L) × 5.1 m (W) × 2.4 m (H). In all, nineteen conditions were generated by adjusting the room temperature, supply airflow rate, and supply terminal type. An air diffuser performance index (ADPI) of at least 80% was achieved for most cases. This result shows that the air velocity and temperature in the occupied zone are reasonably uniform. Subjective assessments using the ASHRAE 7-point scale indicate that the thermal sensations of the subjects in stratum ventilation are also uniform. This study examines the applicability of the predicted mean vote (PMV) model for evaluating stratum ventilation. When compared to the actual mean thermal sensation votes (ATS), the PMV values are acceptable. The PMV results at a height of 1.1 m above the floor show better agreement with the ATS than at a height of 0.1 m.

RESUMO

Streptococcus suis is a bacterium of clinical importance in diverse animal hosts including companion animals and humans. Companion animals are closely associated in the living environment of humans and are potential reservoirs for zoonotic pathogens. Given the zoonotic potential of S. suis, it is crucial to determine whether this bacterium is present among the companion animal population. This study aimed to detect Streptococcus suis in companion animals namely cats and dogs of the central west coast of Peninsular Malaysia and further characterize the positive isolates via molecular and genomic approach. The detection of S. suis was done via bacterial isolation and polymerase chain reaction assay of gdh and recN gene from oral swabs. Characterization was done by multiplex PCR serotyping, as well as muti-locus sequence typing, AMR gene prediction, MGE identification and phylogenomic analysis on whole genome sequence acquired from Illumina and Oxford Nanopore sequencing. Among the 115 samples, PCR assay detected 2/59 of the cats were positive for S. suis serotype 8 while all screened dog samples were negative. This study further described the first complete whole genome of S. suis strain SS/UPM/MY/F001 isolated from the oral cavity of a companion cat. Genomic analysis revealed a novel strain of S. suis having a unique MLST profile and antimicrobial resistance genes of mefA, msrD, patA, patB and vanY. Mobile genetic elements were described, and pathogenic determinants matched to human and swine strains were identified. Phylogenetic tree analysis on the core genome alignment revealed strain SS/UPM/MY/F001 was distinct from other S. suis strains. This study provided insight into the detection and genomic features of the S. suis isolate of a companion cat and highlighted its potential for antimicrobial resistance and pathogenicity.

RESUMO

This study is a direct assessment of blood heavy metal concentrations of frequent users of Chinese medicines (CM), who had been taking prescribed CM at least 6 days per week for not less than 3 months, to determine whether their intake of CM could cause an increased load of toxic heavy metals in the body. From November 2009 to June 2010, 85 subjects were recruited with informed consent, and their blood samples were collected for measurement of arsenic, cadmium, lead and mercury concentrations. Results showed that blood concentrations of four heavy metals of nearly all 85 subjects were within reference ranges. Only one subject who had consumed plentiful seafood was found to have transiently increased blood arsenic concentration (29% higher than the upper limit of the reference range). However, after refraining from eating seafood for 1 month, his blood arsenic concentration returned to normal. Eighty commonly prescribed CM in both raw medicine and powder concentrate supplied by local distributors were also tested for the four heavy metals. Twelve out of the 80 raw medicines were found to contain one or more of the heavy metals that exceeded the respective maximum permitted content. Cadmium was most frequently found in the contaminated samples. None of the powder concentrates had heavy metal content exceeding their respective maximum permitted level.

RESUMO

Circumsporozoite protein (CSP) central repeat region is one of the main target regions of the RTS,S/AS01 vaccine for falciparum infection as it consists of immunodominant B cell epitopes. However, there is a lack of study for P. knowlesi CSP central repeat region. This study aims to characterise the CSP repeat motifs of P. knowlesi isolates in Peninsular Malaysia. CSP repeat motifs of 64 P. knowlesi isolates were identified using Rapid Automatic Detection and Alignment of Repeats (RADAR). Antigenicity of the repeat motifs and linear B cell epitopes were predicted using VaxiJen 2.0, BepiPred-2.0 and BCPred, respectively. A total of 35 dominant repeat motifs were identified. The repeat motif "AGQPQAQGDGANAGQPQAQGDGAN" has the highest repeat frequency (n=15) and antigenicity index of 1.7986. All the repeat regions were predicted as B cell epitopes. In silico approaches revealed that all repeat motifs were antigenic and consisted of B cell epitopes which could be designed as knowlesi malaria vaccine.

RESUMO

In Malaysia presently, the main cause of human malaria is by the zoonotic monkey parasite Plasmodium knowlesi. A previous study has suggested that the P. knowlesi merozoite surface protein 1 (Pkmsp-1) block IV to be a suitable multiplicity of infection (MOI) genotyping marker for knowlesimalaria. This study therefore aimed to investigate the usefulness of Pkmsp-1 block IV in assessing the MOI of P. knowlesi in clinical isolates from Malaysia. Two allele-specific PCR primer pairs targeting the two allelic families of block IV (T1 and T2) were designed, and used to genotype P. knowlesi in 200 blood samples (100 from Peninsular Malaysia and 100 from Malaysian Borneo). Results showed that the mean MOI in Malaysian Borneo was slightly higher as compared to Peninsular Malaysia (1.58 and 1.40, respectively). Almost half of the total blood samples from Malaysian Borneo (52%) had polyclonal infections (i.e., more than one allele of any family type) as compared to Peninsular Malaysia (33%) samples. The T1 allelic family was more prevalent in Peninsular Malaysia (n=75) than in Malaysian Borneo (n=60). The T2 allelic family, however, was more prevalent in the Malaysian Borneo (n=87 vs n=53 respectively). This study shows that the single locus Pkmsp-1 block IV can serve as a simple alternative genetic marker for estimating knowlesi malaria MOI in a population. Future MOI studies should focus on macaque populations as macaques are the natural host of P. knowlesi.

RESUMO

BACKGROUND: Understanding factors associated with SARS-CoV-2 exposure risk in the hospital setting may help improve infection control measures for prevention. AIM: To monitor SARS-CoV-2 exposure risk among healthcare workers and to identify risk factors associated with SARS-CoV-2 detection. METHODS: Surface and air samples were collected longitudinally over 14 months spanning 2020-2022 at the Emergency Department (ED) of a teaching hospital in Hong Kong. SARS-CoV-2 viral RNA was detected by real-time reverse-transcription polymerase chain reaction. Ecological factors associated with SARS-CoV-2 detection were analysed by logistic regression. A sero-epidemiological study was conducted in January-April 2021 to monitor SARS-CoV-2 seroprevalence. A questionnaire was used to collect information on job nature and use of personal protective equipment (PPE) of the participants. FINDINGS: SARS-CoV-2 RNA was detected at low frequencies from surfaces (0.7%, N = 2562) and air samples (1.6%, N = 128). Crowding was identified as the main risk factor, as weekly ED attendance (OR = 1.002, P=0.04) and sampling after peak-hours of ED attendance (OR = 5.216, P=0.03) were associated with the detection of SARS-CoV-2 viral RNA from surfaces. The low exposure risk was corroborated by the zero seropositive rate among 281 participants by April 2021. CONCLUSION: Crowding may introduce SARS-CoV-2 into the ED through increased attendances. Multiple factors may have contributed to the low contamination of SARS-CoV-2 in the ED, including hospital infection control measures for screening ED attendees, high PPE compliance among healthcare workers, and various public health and social measures implemented to reduce community transmission in Hong Kong where a dynamic zero COVID-19 policy was adopted.

RESUMO

INTRODUCTION: Laparoscopic surgery is technically challenging and assessment of competency is necessary to ensure patient safety and guide training. We report on the development of LapPass®, an accessible objective simulation assessment tool with credentialing potential. We provide a preliminary evaluation of its usability and aspects of validity. METHODS: The domains of LapPass® were defined through a consensus process by the executive council of the Association of Laparoscopic Surgeons of Great Britain and Ireland (ALSGBI). A survey of both assessors and trainees was used to test for usability, face and content validity of LapPass®. Internal consistency was tested with Cronbach's alpha, and a composite marker of validity and usability was obtained. RESULTS: LapPass® was developed to consist of four tasks: (1) grasping and manipulation, (2) simulated appendicectomy, (3) cutting a disk and (4) intracorporeal suturing. A total of 76 participants contributed to the evaluation of LapPass®: 13 assessors and 63 trainees. For assessors, Cronbach's alpha for usability of tasks 1-4 was 0.84, 0.84, 0.76 and 0.86, whereas validity was 0.80, 0.85, 0.88, 0.95, respectively. For trainees, Cronbach's alpha was 0.75, 0.77, 0.80 and 0.85 for usability, and 0.79, 093, 0.87 and 0.91 for validity. Consensus was that each task was usable and had face and content validity, with median scores of 4.0 or higher (interquartile range 0.0-1.0). CONCLUSION: LapPass® has potential for the objective assessment of basic laparoscopic skills but further research is required to explore its predictive capabilities in a clinical setting.

RESUMO

Recent reports of natural human infection by Plasmodium cynomolgi indicate the increased risk of zoonotic transmission by this simian parasite. The P. cynomolgi Duffy binding protein 2 (PcDBP2) has a potential role in the invasion pathway of host erythrocytes, and it is a possible vaccine candidate against cynomolgi malaria. This study investigates the genetic diversity, haplotypes, and natural selection of PcDBP2 region II from isolates collected from wild macaques in Peninsular Malaysia. Blood samples from 50 P. cynomolgi -infected wild macaques were used in the study. Genomic DNA extracted from the blood samples was used as template for PCR amplification of the PcDBP2 region II. The amplicons were cloned into a plasmid vector and sequenced. MEGA X and DnaSP ver.6.12.03 programmes were used to analyse the DNA sequences. A genealogical relationship of PcDBP2 region II were determined using haplotype network tree on NETWORK ver.10.2. Result showed high genetic diversity (ð = 0.017 ± 0.002; Hd = 1.000 ± 0.001) of the PcDBP2 region II. The Z-test indicates a purifying selection, with population expansion as shown in Tajima's D analysis. A total of 146 haplotypes of PcDBP2 region II were observed. Phylogenetic tree analysis showed that these haplotypes were grouped into three allelic types (136 for Strain B type, 9 for Berok type, and 1 recombinant type). In the haplotype network, PcDBP2 region II revealed no geographical groupings but was divided into two distinct clusters.

RESUMO

The Plasmodium knowlesi secreted protein with an altered thrombospondin repeat (PkSPATR) is an important protein that helps in the parasite's invasion into the host cell. This protein has been regarded as one of the potential vaccine candidates against P. knowlesi infection. This study investigates the genetic diversity and natural selection of PkSPATR gene of P. knowlesi clinical isolates from Malaysia. PCR amplification of the full length PkSPATR gene was performed on 60 blood samples of infected P. knowlesi patients from Peninsular Malaysia and Malaysian Borneo. The amplified PCR products were cloned and sequenced. Sequence analysis of PkSPATR from Malaysia showed higher nucleotide diversity (CDS p: 0.01462) than previously reported Plasmodium vivax PvSPATR (p = 0.0003). PkSPATR from Peninsular Malaysia was observed to have slightly higher diversity (CDS p: 0.01307) than those from Malaysian Borneo (CDS p: 0.01212). Natural selection analysis on PkSPATR indicated significant purifying selection. Multiple amino acid sequence alignment revealed 69 polymorphic sites. The phylogenetic tree and haplotype network did not show any distinct clustering of PkSPATR. The low genetic diversity level, natural selection and absence of clustering implied functional constrains of the PkSPATR protein.

RESUMO

UNLABELLED: Thermal sensation is studied experimentally under mixing ventilation, displacement ventilation, and stratum ventilation in an environmental chamber. Forty-eight subjects participated in all tests under the same boundary conditions but different ventilation methods in the classroom. Thermal comfort analysis was carried out according to the designated supply airflow rate, room temperature, and relative humidity for the three ventilation methods. The thermal neutral temperature under stratum ventilation is approximately 2.5 °C higher than that under mixing ventilation and 2.0 °C higher than that under displacement ventilation. This result indicates that stratum ventilation could provide satisfactory thermal comfort level to rooms of temperature up to 27 °C. The energy saving attributable to less ventilation load alone is around 12% compared with mixing ventilation and 9% compared with displacement ventilation. PRACTICAL IMPLICATIONS: The confirmation of the significantly elevated thermal neutral temperature can have a number of implications for both thermal comfort in an air-conditioned room and energy consumption of the associate air-conditioning system. With respect to the former, it provides scientific basis for the feasibility of elevated room temperatures, and with respect to the latter, it reveals considerable potentials for energy saving.

RESUMO

Between June 2008 and March 2009, a cross-sectional study of human malaria was carried out in four governorates of Yemen, two (Taiz and Hodiedah) representing the country's highlands and the others (Dhamar and Raymah) the country's coastal plains/foothills. The main aims were to determine the prevalences of Plasmodium infection among 455 febrile patients presenting for care at participating health facilities and to investigate the potential risk factors for such infection. Malarial infection was detected in 78 (17·1%) of the investigated patients and was more likely to be detected among the febrile patients from the highlands than among those presenting in the coastal plains/foothills (22·6% v.13·9%; χ(2)=10·102; P=0·018). Binary logistic-regression models identified low household income [odds ratio (OR)=13·52; 95% confidence interval (CI)=2·62-69·67; P=0·002], living in a household with access to a water pump (OR=4·18; CI=1·60-10·96; P=0·004) and living in a household near a stream (OR=4·43; CI=1·35-14·56; P=0·014) as significant risk factors for malarial infection in the highlands. Low household income was the only significant risk factor identified for such infection in the coastal plains and foothills (OR = 8·20; CI=1·80-37·45; P=0·007). It is unclear why febrile patients in the highlands of Yemen are much more likely to be found to have malarial infection than their counterparts from the coastal plains and foothills. Although it is possible that malarial transmission is relatively intense in the highlands, it seems more likely that, compared with those who live at lower altitudes, those who live in the highlands are less immune to malaria, and therefore more likely to develop febrile illness following malarial infection. Whatever the cause of the symptomatic malarial infection commonly found in the highlands of Yemen, it is a matter of serious concern that should be addressed in the national strategy to control malaria.</p> <br><h5 class="title2">Assuntos</h5> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Malária/epidemiologia"'>Malária/epidemiologia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Adolescente"'>Adolescente</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Adulto"'>Adulto</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Altitude"'>Altitude</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Criança"'>Criança</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Estudos Transversais"'>Estudos Transversais</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Feminino"'>Feminino</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Humanos"'>Humanos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Malária/etiologia"'>Malária/etiologia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Malária/transmissão"'>Malária/transmissão</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Masculino"'>Masculino</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Fatores de Risco"'>Fatores de Risco</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Fatores Socioeconômicos"'>Fatores Socioeconômicos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Iêmen/epidemiologia"'>Iêmen/epidemiologia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Adulto Jovem"'>Adulto Jovem</a> </div> </div> </div> <div class="box1"> <div class="inputCheck1 d-print-none"> <input type="checkbox" class="my_selection" value="mdl-34172705" id="select_mdl-34172705"> <small>17.</small> </div> <div class="textArt"> <div class="titleArt"> <a href="https://pesquisa.bvsalud.org/bvsdip/resource/pt/mdl-34172705" title="Genetic diversity of the full length apical membrane antigen-1 of Plasmodium knowlesi clinical isolates from Peninsular Malaysia." onclick="gtag('event', 'Article', {'event_category': 'Show detail', 'event_label': this.href});"> Genetic diversity of the full length apical membrane antigen-1 of Plasmodium knowlesi clinical isolates from Peninsular Malaysia. </a> </div> <div class="author"> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Ng, Y L"'>Ng, Y L</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Fong, M Y"'>Fong, M Y</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Lau, Y L"'>Lau, Y L</a>. </div> <!-- display source fields --> <!-- source --> <div class="reference"> <em> <a href="http://portal.revistas.bvs.br/transf.php?xsl=xsl/titles.xsl&xml=http://catserver.bireme.br/cgi-bin/wxis1660.exe/?IsisScript=../cgi-bin/catrevistas/catrevistas.xis|database_name=TITLES|list_type=title|cat_name=ALL|from=1|count=50&lang=pt&comefrom=home&home=false&task=show_magazines&request_made_adv_search=false&lang=pt&show_adv_search=false&help_file=/help_pt.htm&connector=ET&search_exp=Trop Biomed" target="_blank"> <span>Trop Biomed</span> </a>; 38(2): 159-164, 2021 Jun 01. </em> </div> <div class="dataArticle"> Artigo em Inglês <!-- database --> | MEDLINE | ID: <span class="doc_id">mdl-34172705</span> </div> <!-- More details div --> <div class="reference-detail collapse" > <br><h5 class="title2">RESUMO</h4> <p>The Plasmodium knowlesi apical membrane antigen-1 (PkAMA-1) plays an important role in the invasion of the parasite into its host erythrocyte, and it has been regarded as a potential vaccine candidate against human knowlesi malaria. This study investigates genetic diversity and natural selection of the full length PkAMA-1 of P. knowlesi clinical isolates from Peninsular Malaysia. Blood samples were collected from P. knowlesi malaria patients from Peninsular Malaysia. The PkAMA-1 gene was amplified from DNA samples using PCR, cloned into a plasmid vector and sequenced. Results showed that nucleotide diversity of the full length PkAMA-1 from Peninsular Malaysia isolates (π: 0.006) was almost similar to that of Sarawak (π: 0.005) and Sabah (π: 0.004) isolates reported in other studies. Deeper analysis revealed Domain I (π: 0.007) in the PkAMA-1 had the highest diversity as compared to Domain II (π: 0.004) and Domain III (π: 0.003). Z-test indicated negative (purifying) selection of the gene. Combined alignment analysis at the amino acid level for the Peninsular Malaysia and Sarawak PkAMA-1 sequences revealed 34 polymorphic sites. Thirty-one of these sites were dimorphic, and 3 were trimorphic. The amino acid sequences could be categorised into 31 haplotypes. In the haplotype network, PkAMA-1 from Peninsular Malaysia and Sarawak were separated into two groups.</p> <br><h5 class="title2">Assuntos</h5> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Antígenos de Protozoários/genética"'>Antígenos de Protozoários/genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Variação Genética"'>Variação Genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Membrana/genética"'>Proteínas de Membrana/genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Plasmodium knowlesi"'>Plasmodium knowlesi</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Protozoários/genética"'>Proteínas de Protozoários/genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Haplótipos"'>Haplótipos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Humanos"'>Humanos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Malária/parasitologia"'>Malária/parasitologia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Malásia"'>Malásia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Plasmodium knowlesi/genética"'>Plasmodium knowlesi/genética</a> </div> </div> </div> <div class="box1"> <div class="inputCheck1 d-print-none"> <input type="checkbox" class="my_selection" value="mdl-31993966" id="select_mdl-31993966"> <small>18.</small> </div> <div class="textArt"> <div class="titleArt"> <a href="https://pesquisa.bvsalud.org/bvsdip/resource/pt/mdl-31993966" title="Are Type and Screen Samples Routinely Necessary Before Laparoscopic Cholecystectomy?" onclick="gtag('event', 'Article', {'event_category': 'Show detail', 'event_label': this.href});"> Are Type and Screen Samples Routinely Necessary Before Laparoscopic Cholecystectomy? </a> </div> <div class="author"> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Fong, M L"'>Fong, M L</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Urriza Rodriguez, D"'>Urriza Rodriguez, D</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Elberm, H"'>Elberm, H</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Berry, D P"'>Berry, D P</a>. </div> <!-- display source fields --> <!-- source --> <div class="reference"> <em> <a href="http://portal.revistas.bvs.br/transf.php?xsl=xsl/titles.xsl&xml=http://catserver.bireme.br/cgi-bin/wxis1660.exe/?IsisScript=../cgi-bin/catrevistas/catrevistas.xis|database_name=TITLES|list_type=title|cat_name=ALL|from=1|count=50&lang=pt&comefrom=home&home=false&task=show_magazines&request_made_adv_search=false&lang=pt&show_adv_search=false&help_file=/help_pt.htm&connector=ET&search_exp=J Gastrointest Surg" target="_blank"> <span>J Gastrointest Surg</span> </a>; 25(2): 447-451, 2021 02. </em> </div> <div class="dataArticle"> Artigo em Inglês <!-- database --> | MEDLINE | ID: <span class="doc_id">mdl-31993966</span> </div> <!-- More details div --> <div class="reference-detail collapse" > <br><h5 class="title2">RESUMO</h4> <p>AIMS: Type and screen (T&S) samples are routinely requested before all laparoscopic cholecystectomies (LCs) at our centre despite the low reported risk of major vascular injury and peri-operative transfusion. Our retrospective case series aimed to identify local transfusion need to inform policy. METHODS: Emergency and elective LC performed at a single tertiary centre between March 2014 and October 2016 (30 months) were analysed. This included all patients aged ≥ 16, and procedures converted to open where LC was the primary procedure. Peri-operative complications and transfusion data were obtained from electronic records. RESULTS: In total, 1002 consecutive patients met inclusion criteria; 12 patients were transfused during index admission (1.20%). No patients required emergency transfusion or had major vascular injuries. Despite local policy, 106 patients (10.6%) did not have a valid T&S sample prior to their procedure. Transfused patients were more likely to be emergency admissions (n = 10/12). The most common indications for transfusion were pre-operative anaemia (n = 7/12) and septic coagulopathy (n = 2/12). CONCLUSIONS: Peri-operative transfusions at our centre were low. No patients required intra-operative blood transfusions dependent on a pre-operative T&S sample. Patients requiring transfusion were predictable from their pre-operative clinical status. We propose that a highly selective T&S policy is safe and can reduce costs.</p> <br><h5 class="title2">Assuntos</h5> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Anemia"'>Anemia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Colecistectomia Laparoscópica"'>Colecistectomia Laparoscópica</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Idoso"'>Idoso</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Transfusão de Sangue"'>Transfusão de Sangue</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Colecistectomia Laparoscópica/efeitos adversos"'>Colecistectomia Laparoscópica/efeitos adversos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Procedimentos Cirúrgicos Eletivos"'>Procedimentos Cirúrgicos Eletivos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Humanos"'>Humanos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Estudos Retrospectivos"'>Estudos Retrospectivos</a> </div> </div> </div> <div class="box1"> <div class="inputCheck1 d-print-none"> <input type="checkbox" class="my_selection" value="mdl-34172703" id="select_mdl-34172703"> <small>19.</small> </div> <div class="textArt"> <div class="titleArt"> <a href="https://pesquisa.bvsalud.org/bvsdip/resource/pt/mdl-34172703" title="Experimental Study on Plasmodium knowlesi Normocyte Binding Protein Xa Region II (PkNBPXaII) for Erythrocyte Binding." onclick="gtag('event', 'Article', {'event_category': 'Show detail', 'event_label': this.href});"> Experimental Study on Plasmodium knowlesi Normocyte Binding Protein Xa Region II (PkNBPXaII) for Erythrocyte Binding. </a> </div> <div class="author"> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Wong, K C"'>Wong, K C</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Lai, M Y"'>Lai, M Y</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"De Silva, J R"'>De Silva, J R</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Cheong, F W"'>Cheong, F W</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Fong, M Y"'>Fong, M Y</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Lau, Y L"'>Lau, Y L</a>. </div> <!-- display source fields --> <!-- source --> <div class="reference"> <em> <a href="http://portal.revistas.bvs.br/transf.php?xsl=xsl/titles.xsl&xml=http://catserver.bireme.br/cgi-bin/wxis1660.exe/?IsisScript=../cgi-bin/catrevistas/catrevistas.xis|database_name=TITLES|list_type=title|cat_name=ALL|from=1|count=50&lang=pt&comefrom=home&home=false&task=show_magazines&request_made_adv_search=false&lang=pt&show_adv_search=false&help_file=/help_pt.htm&connector=ET&search_exp=Trop Biomed" target="_blank"> <span>Trop Biomed</span> </a>; 38(2): 143-148, 2021 Jun 01. </em> </div> <div class="dataArticle"> Artigo em Inglês <!-- database --> | MEDLINE | ID: <span class="doc_id">mdl-34172703</span> </div> <!-- More details div --> <div class="reference-detail collapse" > <br><h5 class="title2">RESUMO</h4> <p>Normocyte binding protein Xa (NBPXa) has been implied to play a significant role in parasite invasion of human erythrocytes. Previous phylogenetic studies have reported the existence of three types of NBPXa for Plasmodium knowlesi (PkNBPXa). PkNBPXa region II (PkNBPXaII) of type 1, type 2 and type 3 were expressed on mammalian cell surface and interacted with human and macaque (Macaca fascicularis) erythrocytes. The binding activities of PkNBPXaII towards human and macaque erythrocytes were evaluated using erythrocyte-binding assay (EBA). Three parameters were evaluated to achieve the optimal protein expression of PkNBPXaII and erythrocyte binding activity in EBA: types of mammalian cells, post transfection time and erythrocyte incubation time. COS-7, HEK-293, and CHO-K1 cells showed successful expression of PkNBPXaII, despite the protein expression is weak compared to the positive control. COS-7 was used in EBA. All three types of PkNBPXaII showed rosette formation with macaque erythrocytes but not with human erythrocytes. Future studies to enhance the PkNBPXaII expression on surface of mammalian cells is indeed needed in order to elucidate the specific role of PkNBPXaII in erythrocytes invasion.</p> <br><h5 class="title2">Assuntos</h5> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Eritrócitos/parasitologia"'>Eritrócitos/parasitologia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Membrana/metabolismo"'>Proteínas de Membrana/metabolismo</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Plasmodium knowlesi"'>Plasmodium knowlesi</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Protozoários/metabolismo"'>Proteínas de Protozoários/metabolismo</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Animais"'>Animais</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Antígenos de Protozoários"'>Antígenos de Protozoários</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Células CHO"'>Células CHO</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Células COS"'>Células COS</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Chlorocebus aethiops"'>Chlorocebus aethiops</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Cricetulus"'>Cricetulus</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Eritrócitos/metabolismo"'>Eritrócitos/metabolismo</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Células HEK293"'>Células HEK293</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Humanos"'>Humanos</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Membrana/genética"'>Proteínas de Membrana/genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Filogenia"'>Filogenia</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Plasmodium knowlesi/genética"'>Plasmodium knowlesi/genética</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Plasmodium knowlesi/metabolismo"'>Plasmodium knowlesi/metabolismo</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Ligação Proteica"'>Ligação Proteica</a> , <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=mh:"Proteínas de Protozoários/genética"'>Proteínas de Protozoários/genética</a> </div> </div> </div> <div class="box1"> <div class="inputCheck1 d-print-none"> <input type="checkbox" class="my_selection" value="mdl-7525853" id="select_mdl-7525853"> <small>20.</small> </div> <div class="textArt"> <div class="titleArt"> <a href="https://pesquisa.bvsalud.org/bvsdip/resource/pt/mdl-7525853" title="Evidence for the involvement of interleukin 10 in the differential deactivation of murine peritoneal macrophages by prostaglandin E2." onclick="gtag('event', 'Article', {'event_category': 'Show detail', 'event_label': this.href});"> Evidence for the involvement of interleukin 10 in the differential deactivation of murine peritoneal macrophages by prostaglandin E2. </a> </div> <div class="author"> <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Strassmann, G"'>Strassmann, G</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Patil-Koota, V"'>Patil-Koota, V</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Finkelman, F"'>Finkelman, F</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Fong, M"'>Fong, M</a>; <a href='https://pesquisa.bvsalud.org/bvsdip/?lang=pt&q=au:"Kambayashi, T"'>Kambayashi, T</a>. </div> <!-- display source fields --> <!-- source --> <div class="reference"> <em> <a href="http://portal.revistas.bvs.br/transf.php?xsl=xsl/titles.xsl&xml=http://catserver.bireme.br/cgi-bin/wxis1660.exe/?IsisScript=../cgi-bin/catrevistas/catrevistas.xis|database_name=TITLES|list_type=title|cat_name=ALL|from=1|count=50&lang=pt&comefrom=home&home=false&task=show_magazines&request_made_adv_search=false&lang=pt&show_adv_search=false&help_file=/help_pt.htm&connector=ET&search_exp=J Exp Med" target="_blank"> <span>J Exp Med</span> </a>; 180(6): 2365-70, 1994 Dec 01. </em> </div> <div class="dataArticle"> Artigo em Inglês <!-- database --> | MEDLINE | ID: <span class="doc_id">mdl-7525853</span> </div> <!-- More details div --> <div class="reference-detail collapse" > <br><h5 class="title2">RESUMO</h4> <p>Among other effects, prostaglandins (PG) of the E series are known to inhibit several acute and chronic inflammatory conditions in vivo and proinflammatory cytokine production by activated macrophages in culture. The research presented here demonstrates that the inhibitory effect of PGE2 on tumor necrosis factor alpha (TNF-alpha) and interleukin 6 (IL-6) production by lipopolysaccharide (LPS)-stimulated murine peritoneal macrophages involves IL-10. In a dose-dependent manner, PGE2 inhibits LPS-induced release of TNF-alpha and IL-6, but not of lactate or nitric oxide. The decrease in the level of these cytokines is inversely proportional to the increase in immunoreactive IL-10. This differential inhibitory effect of PGE2 is mimicked by agents that elevate intracellular levels of cAMP, but not cGMP. Neutralizing anti IL-10 antibody but not neutralizing antibodies against other macrophage secretory products (IL-6, leukemia inhibitory factor, and transforming growth factor beta [TGF-beta]), significantly reverse the potent inhibitory effect of PGE2. In vivo, the administration of PGE2 before LPS challenge significantly reduces circulating TNF-alpha and IL-6 levels. Anti-IL-10 antibody substantially enhanced the LPS-induced TNF-alpha and IL-6 levels in mice that received either LPS alone or LPS plus PGE2. These results suggest that the anti-inflammatory effect of PGE2 on mononuclear phagocytes is mediated in part by an autocrine feedback mechanism involving IL-10.</p> <br><h5
