RESUMO

Lung perfusion scintigraphy is a common nuclear medicine exam performed for the evaluation of pulmonary emboli, often in the emergency setting. There can be confusion when a radiotracer is located outside of the normal physiologic distribution. This can occur due to improper radionuclide tagging or may be due to anatomic variations. We present a case where a patient presented with bilateral lower extremity deep vein thrombosis and a nuclear medicine lung perfusion scintigraphy showing a complete right-to-left shunt related to a rare anatomical variant of a duplicated superior vena cava (SVC) with the right SVC draining directly into the systemic circulation via the left atrium.

Chronic functional gallbladder disorder, characterized by biliary pain in the absence of structural pathology, poses a diagnostic challenge necessitating reliable cholecystagogues for accurate evaluation. However, recurrent shortages of synthetic cholecystokinin analogs have prompted the exploration of alternative agents. This paper describes the efficacy of Ensure Plus as a viable fatty meal substitute for hepatobiliary scintigraphy in assessing chronic functional gallbladder disorder. Through comparative studies, Ensure Plus demonstrates comparable diagnostic accuracy to cholecystokinin in similar patient populations. Furthermore, Ensure Plus demonstrates significant symptom improvement after cholecystectomy in patients with anomalous gallbladder ejection fractions. This paper offers a detailed protocol for the seamless integration of Ensure Plus into hepatobiliary scintigraphy, providing clinicians with a valuable tool to navigate cholecystokinin shortages while maintaining diagnostic precision in cases of chronic functional gallbladder disorder. The use of Ensure Plus not only addresses practical supply challenges but also underscores its potential as a cost-effective and clinically sound alternative in biliary diagnostics.

Meningiomas are common, benign tumors found in the dural layers of the central nervous system (CNS) that are often found incidentally. 18F-fluciclovine is an amino acid radiotracer that is used to monitor the recurrence of prostate cancer due to its high uptake in prostate cancer cells. This case report outlines a patient with a retinoblastoma tumor suppressor gene (RB1) mutation and prostate cancer metastasis to the bone incidentally noted to have an enhancing, extra-axial mass on a screening MRI. On prior scans, the mass displayed increased 18F-fluciclovine uptake. Because prostate cancer can metastasize to the meninges, especially in older patients with advanced disease, consideration was given to the progression of his oncological disease. However, additional imaging validated the existence and size of the mass, making a meningioma the final diagnosis. Meningeal metastases can be virtually indistinguishable from other CNS tumors, including meningiomas appearing as single or multiple dural-based, enhancing masses, and without prior imaging, further investigation is warranted.
