RESUMO

Seroma, a fluid collection that can develop after surgery, can be a challenging complication to manage. Conventional treatment options, such as quilting suture and drainage tubes, may not be effective in resolving refractory seromas. This article presents two cases of refractory seroma after axillary osmidrosis surgery that were successfully treated with silver nitrate. Silver nitrate, a topical agent with antiseptic, anti-inflammatory, and wound-healing properties, has been shown to be effective in treating perianal fistulas and persistent tracheocutaneous fistulas. In both cases presented here, silver nitrate resulted in complete seroma resolution within 7 and 14 days, respectively. This study suggests that silver nitrate may be a promising treatment option for refractory seroma after axillary osmidrosis surgery. Further research is warranted to validate these findings and establish optimal dosage and treatment protocols.

BACKGROUND: Pincer nail deformity (PND) is a dystrophy characterized by transverse overcurvature of the nail plate that may cause intractable pain and decrease the quality of life of patients. OBJECTIVES: To evaluated the efficacy of a superelastic nickel-titanium (SE NiTi) wire for the treatment of PND using transverse curvature improvement and subjective assessment of pain relief during and after the treatment. METHODS: SE NiTi wire was implanted over the distal tip of the nail for the treatment of PND in 43 patients (28 female, 15 male), with a total of 73 digits receiving treatment. Evaluations of improvement included measuring changes in transverse curvature of the nail and subjective assessment of pain relief throughout the follow-up period. RESULTS: In all patients, treatment of the pincer nail with implantation of SE NiTi wire achieved satisfactory results. Significant improvement (p < .05) of the transverse overcurvature of the nail was seen in all patients at 2 months, and relief of pain was determined in 100% of cases throughout our follow-up period. CONCLUSION: This simple SE NiTi wire insertion method is noninvasive and inexpensive, leaves no cosmetic disfigurement, and leads to excellent therapeutic results. Patients achieved great satisfaction. Thus, this technique should be considered the first line of treatment in the correction of mild to moderate PND.

RESUMO

A case series study describes a minimally invasive surgical technique for removing axillary apocrine glands using blunt scissors. The technique involved making two small incisions, removing the glands using three maneuvers, and evaluating patient satisfaction and post-surgery complications. Of 100 patients, 92% reported being satisfied with the results, with no reported complications. The study suggests this technique is safe and effective, offering a minimally invasive alternative to traditional surgery with fewer negative cosmetic outcomes. However, further research is needed to assess its long-term efficacy and safety.

The safety of antiperspirants has been a subject of intense debate, with conflicting perspectives and research findings. This study aimed to contribute to the discussion by investigating the distribution of aluminum chloride (AlCl3), a common ingredient in antiperspirants, within the sweat glands. We enrolled a total of 10 patients seeking surgery for bromhidrosis in our study. Before surgery, we instructed the study participants to apply a specially formulated AlCl3 staining antiperspirant to their underarms using a surgical pen with ink that resists easy removal. We excised the apocrine glands, fragmented them finely, and examined them microscopically. We did not detect any stained aluminum compounds. These results are consistent with previous research, which suggests that aluminum compounds do not enter the apocrine glands. Consequently, concerns regarding the potential absorption of aluminum into the human body appear to be unfounded, particularly regarding cancer risks such as breast cancer.

Purpose: Many patients with axillary osmidrosis (AO) cannot tolerate the local irritation of strong antiperspirants and discontinue AO use within a short time. This study evaluates the effect of long-term antiperspirant use on postoperative complications after osmidrosis surgery. Patients and Methods: A total of 116 females (66 antiperspirant and 50 non-antiperspirant cases) who underwent osmidrosis surgery were retrospectively reviewed. Postoperative complications were compared between the 2 groups. Results: Patients with long-term antiperspirant use had a lower risk of full-thickness skin necrosis compared with those who did not use antiperspirants (odds ratio [OR] = 0.048, 95% confidence Interval [CI]: 0.006-0.392, p = 0.005). Patients with antiperspirants use also had a lower risk of moderate-to-severe erythema compared to those without antiperspirants use (moderate vs mild erythema: OR = 0.351, 95% CI: 0.129-0.959, p = 0.041; severe vs mild erythema: OR = 0.161, 95% CI: 0.047-0.550, p = 0.004). Patients who used antiperspirants also had a lower risk of severe skin erosion compared to those who did not use antiperspirants (severe vs mild skin erosion: OR = 0.164, 95% CI: 0.037-0.725, p = 0.017). There was a trend of lower risk in moderate skin erosion in patients with antiperspirant use compared to those without antiperspirant use, but it was not statistically significant (moderate vs mild epidermal damage and peeling: OR = 0.406, 95% CI: 0.158-1.043, p = 0.061). Conclusion: Postoperative complications in patients with AO who undergo osmidrosis surgery are lower in those with a long-term antiperspirant use compared to patients who did not use antiperspirants.

Cholesterol embolism (CE) is characterized by emboli containing cholesterol crystal in the arterioles, most commonly found in the skin and the kidney. The skin presentations of CE include livedo reticularis, blue toe syndrome, ulceration and gangrene. Cutaneous reactive angiomatosis (CRA) is a recently proposed term to describe a group of reactive vascular proliferation in skin caused by various diseases. Its occurrence in association with CE is extremely uncommon. An 82-year-old man with a history of cerebral infarction and on long-term warfarin therapy developed progressive, multiple violaceous papules and nodules on the dorsal feet, soles and toes, simulating Kaposi's sarcoma. Skin biopsy showed marked vascular endothelial cell proliferations characteristic of CRA affecting the full thickness of dermis. In addition, cholesterol crystal emboli were found in dermal arterioles. The skin lesions improved after the warfarin dose was reduced. We emphasize the possible presence of CE in a patient presented with CRA, especially in those with a pre-existing atherosclerotic disease, on anticoagulation therapy, or having a prior history of invasive vascular procedure.

RESUMO

Higher risk of rapid progression in alopecia or male pattern baldness was observed in men who had family history. This could result from accumulation of DHT in hair follicles. Hair follicles on frontal region are more vulnerable to DHT. With development of minimal invasive hair transplantation surgery, hair follicles transplantation could be performed from frontal or occipital region to frontal region. However, limited hair follicles remained a problem. With development of technology of vitrification, we suggested extracting hair follicles from frontal region without affecting the appearance and preserving them with vitrification when the patient was young. When alopecia progressively developed, these extracted hair follicles would increase the donor number of hair follicles used for transplantation, which could extend longer dense hair appearance.
