RESUMO

Intensive rice (Oryza sativa)-based cropping systems in south Asia provide much of the calorie and protein requirements of low to middle-income rural and urban populations. Intensive tillage practices demand more resources, damage soil quality, and reduce crop yields and profit margins. Crop diversification along with conservation agriculture (CA)-based management practices may reduce external input use, improve resource-use efficiency, and increase the productivity and profitability of intensive cropping systems. A field study was conducted on loamy soil in a sub-tropical climate in northern Bangladesh to evaluate the effects of three tillage options and six rice-based cropping sequences on grain, calorie, and protein yields and gross margins (GM) for different crops and cropping sequences. The three tillage options were: (1) conservation agriculture (CA) with all crops in sequences untilled, (2) alternating tillage (AT) with the monsoon season rice crop tilled but winter season crops untilled, and (3) conventional tillage (CT) with all crops in sequences tilled. The six cropping sequences were: rice-rice (R-R), rice-mung bean (Vigna radiata) (R-MB), rice-wheat (Triticum aestivum) (R-W), rice-maize (Zea mays) (R-M), rice-wheat-mung bean (R-W-MB), and rice-maize-mung bean (R-M-MB). Over three years of experimentation, the average monsoon rice yield was 8% lower for CA than CT, but the average winter crops yield was 13% higher for CA than CT. Systems rice equivalent yield (SREY) and systems calorie and protein yields were about 5%, 3% and 6%, respectively, higher under CA than CT; additionally, AT added approximately 1% more to these benefits. The systems productivity gain under CA and AT resulted in higher GM by 16% while reducing the labor and total production cost under CA than CT. The R-M rotation had higher SREY, calorie, protein yields, and GM by 24%, 26%, 66%, and 148%, respectively, than the predominantly practiced R-R rotation. The R-W-MB rotation had the highest SREY (30%) and second highest (118%) GM. Considering the combined effect of tillage and cropping system, CA with R-M rotation showed superior performance in terms of SREY, protein yield, and GM. The distribution of labor use and GM across rotations was grouped into four categories: R-W in low-low (low labor use and low GM), R-M in low-high (low labor use and high GM), R-W-MB and R-M-MB in high-high (high labor use and high GM) and R-R and R-MB in high-low (high labor use and low GM). In conclusion, CA performed better than CT in different winter crops and cropping systems but not in monsoon rice. Our results demonstrate the multiple benefits of partial and full CA-based tillage practices employed with appropriate crop diversification to achieve sustainable food security with greater calorie and protein intake while maximizing farm profitability of intensive rice-based rotational systems.

OBJECTIVE: Anterior approach cervical surgery is widely used for accessing C3 lesions. When operating with an anterior approach, the surgical field is obstructed by mandible. Neck extension is popular method to secure better surgical field but risk devastating neurological damage. To overcome this limited surgical field without neck extension, we adopted nasotracheal intubation and evaluated its efficiency. METHODS: We retrospectively analyzed 16 patients who underwent anterior cervical discectomy or corpectomy of C3 lesions via nasotracheal intubation. We enrolled an additional 29 patients who underwent anterior cervical discectomy or corpectomy of C3 lesions via orotracheal intubation as a control group. All patients had been diagnosed with cervical spondylotic myelopathy or ossification of the posterior longitudinal ligament. We measured the mandibular-cervical angle, which is the angle between the lower mandibular line and anterior vertebral line. RESULTS: The mandibular-cervical angle was increased by 7.3 with nasotracheal intubation compared to orotracheal intubation. CONCLUSIONS: Nasotracheal intubation is an effective surgical option for securing the surgical field without neck extension in anterior cervical surgery including C3 lesions.

We conducted a yearlong prospective study of febrile patients admitted to a tertiary referral hospital in Chittagong, Bangladesh, to assess the proportion of patients with rickettsial illnesses and identify the causative pathogens, strain genotypes, and associated seasonality patterns. We diagnosed scrub typhus in 16.8% (70/416) and murine typhus in 5.8% (24/416) of patients; 2 patients had infections attributable to undifferentiated Rickettsia spp. and 2 had DNA sequence-confirmed R. felis infection. Orientia tsutsugamushi genotypes included Karp, Gilliam, Kato, and TA763-like strains, with a prominence of Karp-like strains. Scrub typhus admissions peaked in a biphasic pattern before and after the rainy season, whereas murine typhus more frequently occurred before the rainy season. Death occurred in 4% (18/416) of cases; case-fatality rates were 4% each for scrub typhus (3/70) and murine typhus (1/28). Overall, 23.1% (96/416) of patients had evidence of treatable rickettsial illnesses, providing important evidence toward optimizing empirical treatment strategies.

BACKGROUND: Antibiotics prescribing by physicians have gained due importance across the globe, mainly because of an increase in antibiotic usage, prevalence of infections and drug resistances. The present study is aimed to evaluate the physicians prescribing pattern of antibiotics, their usages by outpatients and disease conditions for which the antibiotics are prescribed in three cities of Bangladesh. METHODS: This cross sectional health survey was carried out with a self designed standard questionnaire by manual data collection over a three months period (20.03.2013 to 20.06.2013) at three adjacent cities Jessore Sadar, Monirampur and Keshabpur upazila respectively. The data were collected from the patient's prescription and by directly interviewing the patients who were prescribed at least one antibiotic during the study period. WHO Anatomical Therapeutic Chemical (ATC) classifications for antibiotics was used and descriptive statistics were applied to the collected data and analyzed using Microsoft Excel software. Modified Wald method was applied to calculate 95% CI. RESULTS: A total of 900 prescriptions were analyzed during the study period. It was found that the prescriber prescribed antibiotics to the patients who were suffering mainly from cold and fever, infections, diarrhea and gonorrhea. The highest prescribed antibiotic groups were cephalosporins (31.78%), macrolides (27.33%), quinolones (16.33%), penicillins (7.11%), and metronidazoles (6.78%) respectively. Two or more antibiotics were prescribed in 25.44% of prescriptions. A total of 66.89% prescriptions had complete information on dosage form, 57% had complete direction for antibiotics use and 64.22% patients completed full course of antibiotics. Although 83% prescriptions have no clinical test for using antibiotics, even though the percentages of patients' disease recovery were 61.78% and in compliance were 38.22%. CONCLUSION: From this research, it is observed that physicians prescribed antibiotics rationally in some cases but needs to ensure in all cases of prescription. Because irrational use leads to the spread of bacterial resistance to antibiotics and related health problems, our findings have important implications for public education and the enforcement of regulations regarding the prescription of antibiotics in Bangladesh.

We investigated the effectiveness and safety of a new composite-based biodegradable anterior cervical plate/screw (ACPS) system for the anterior cervical discectomy and fusion (ACDF) fixation. A biocomposite in combination with 30 wt% ß-tricalcium phosphate (ß-TCP; a biodegradable ceramic having osteoconductive ability) and 70 wt% poly-l/d-lactide copolymer (PLDLA; a biodegradable polymer) was developed and used in the ACPS device, comprising one plate and four screws for fixation. Based on a literature review, a clinically required period of performance maintenance was set as 16 weeks, and to verify the performance for a period of 16 weeks or more, the test was conducted for 26 weeks. Following ISO 13781:2017 testing protocols, an in vitro degradation test was performed to verify the performance and evaluate the decomposition characteristics of the biodegradable ACPS system. Using an animal model as a preclinical investigation, the prepared ACPS device was implanted into five mongrel dogs weighing over 30 kg to evaluate the detachment prevention effect of the ACPS system on polyether ether ketone (PEEK) cage after ACDF. By week 26, the molecular weight was decreased by 45.35% for the plate and 52.56% for the screw; the bending strength of the plate was decreased by approximately 26.2% when compared with the initial stage. The torsional yield strength and pullout strength of the screw was increased by 52.31% and 5.3%, respectively by week 2 and then subsequently decreased until week 26. No detachment or dislocation of the inserted PEEK cage was observed for 26 weeks in vivo study. These findings recommend that the ACPS system might be a promising biodegradable tool for the fixation of interbody implants and supporting the fusion in an ACDF model. Furthermore, additional clinical trials are planned for the future.

OBJECTIVE: Transforaminal lumbar interbody fusion (TLIF) is performed worldwide with polyetheretherketone (PEEK) and titanium (Ti) cages for the treatment of degenerative lumbar diseases. The aim of this study was to compare radiologic outcomes between a PEEK and three-dimensional-printed titanium (3DP-Ti) cage after TLIF with >1 year of follow-up. METHODS: A total of 140 patients with degenerative lumbar diseases who underwent TLIF operation were included in this study. Intervertebral disc height and whole lumbar lordosis were measured and evaluated from the preoperative stage to the final follow-up. Subsidence of the cage was indicated if the cage sunk into the adjacent vertebral body or if there was a reduction in height of the fused segment by ≥3 mm during the postoperative follow-up. Migration of the cage was determined as the displacement of the interbody cage by ≥2 mm during the postoperative period. Fusion status was assessed at the 1 year and final follow-up using standard methods. RESULTS: Both disc height and lumbar lordosis were well maintained throughout the study period, and no significant differences were observed between PEEK and 3DP-Ti groups. Both PEEK and 3DP-Ti cages demonstrated low rates of cage subsidence, with no significant difference was noted. A significant cage migration rate was observed in the PEEK group and the revision operation was required for 2 patients. The fusion rate of this study was not found to be statistically significant, although the 3DP-Ti cage was known to have an improved fusion rate than PEEK cage after lumbar interbody fusion. CONCLUSIONS: Radiologic results suggest that the 3DP-Ti cage may be a better interbody cage for TLIF than is the PEEK cage.

3D-printed titanium (Ti) cages present an attractive alternative for addressing issues related to osteoporosis-induced fractures, accidental fractures, and spinal fusion surgery due to disc herniation. These Ti-based bone implants possess superior strength compared to other metals, allowing for versatile applications in orthopedic scenarios. However, when used as standalone solutions, certain considerations may arise, such as interaction with soft tissues. Therefore, to overcome these issues, the combination with hydrogel has been considered. In this study, to impart Ti with regenerative abilities a 3D-printed Ti cage was loaded with gelatin and hyaluronic acid (G-H) to improve the cell attachment ability of the Ti-based bone implants. The void spaces within the mesh structure of the 3D Ti cage were filled with G-H, creating a network of micro-sized pores. The filled G-H acted as the bridge for the cells to migrate toward the large inner pores of the 3D Ti cage. Due to the microporous surface and slow release of gelatin and hyaluronic acid, the biocompatibility of the coated Ti cage was increased with an elevation in osteoconduction as depicted by the up-regulation of bone-related gene expressions. The in vivo implantation in the rabbit femur model showed enhanced bone regeneration due to the coated G-H on the Ti cage compared to the pristine hollow Ti cage. The G-H filled the large holes of the 3D Ti cage that acted as a bridge for the cells to travel inside the implant and aided in the fast regeneration of bone.

Three-dimensional (3D) printed titanium (Ti-6Al-4V alloy) cages are widely used for spinal fusion applications. However, the structural design and shape of the cages are a major determinant of the optimal clinical outcome. In this study, we constructed a newly designed 3D-printed helical-shaped titanium cage (HTC) with a flexible body, and compared its healing and fusion efficacy in cervical vertebral defects after corpectomy in rabbits to that of a 3D-printed traditional titanium cage (TTC). We performed radiological examinations 1 and 16 weeks after TTC and HTC implantation. We assessed bone ingrowth in TTC and HTC using micro-computed tomography (micro-CT) and histological staining of tissue sections at 16 weeks. The radiographic data showed that the HTC-implanted group had better restoration of vertebral height than the TTC group, indicating a lower risk of cage subsidence. The micro-CT and histological observations showed that HTC promoted bone regeneration and osseointegration more effectively than TTC. Histomorphometry further revealed significant new bone formation in the HTC group compared to the TTC group. These findings demonstrate that HTC has better healing and bone fusion effects than TTC in cervical vertebral defects in rabbits, indicating its potential clinical value.

Bronchogenic cyst results from abnormal bronchial budding. Thin-stalked mobile bronchogenic cysts are rare and sometimes radiologically mimic mass lesion, making preoperative diagnosis difficult. We present a 12-year-old boy with a preoperative diagnosis of intraparenchymal cystic lung lesion misled by radiology. We performed a mini-thoracotomy revealing a thin stalked mobile elongated cyst that arose from the right inferior pulmonary ligament, confirmed as a bronchogenic cyst in histopathology.

The purpose of this study was to test several modifications of the polymethylmethacrylate (PMMA) bone cement by incorporating osteoconductive and biodegradable materials for enhancing bone regeneration capacity in an osteoporotic rat model. Three bio-composites (PHT-1 [80% PMMA, 16% HA, 4% ß-TCP], PHT-2 [70% PMMA, 24% HA, 6% ß-TCP], and PHT-3 [30% PMMA, 56% HA, 14% ß-TCP]) were prepared using different concentrations of PMMA, hydroxyapatite (HA), and ß-tricalcium phosphate (ß-TCP). Their morphological structure was then examined using a scanning electron microscope (SEM) and mechanical properties were determined using a MTS 858 Bionics test machine (MTS, Minneapolis, MN, USA). For in vivo studies, 35 female Wister rats (250 g, 12 weeks of age) were prepared and divided into five groups including a sham group (control), an ovariectomy-induced osteoporosis group (OVX), an OVX with pure PMMA group (PMMA), an OVX with PHT-2 group (PHT-2), and an OVX with PHT-3 group (PHT-3). In vivo bone regeneration efficacy was assessed using micro-CT and histological analysis after injecting the prepared bone cement into the tibial defects of osteoporotic rats. SEM investigation showed that the PHT-3 sample had the highest porosity and roughness among all samples. In comparison to other samples, the PHT-3 exhibited favorable mechanical properties for use in vertebroplasty procedures. Micro-CT and histological analysis of OVX-induced osteoporotic rats revealed that PHT-3 was more effective in regenerating bone and restoring bone density than other samples. This study suggests that the PHT-3 bio-composite can be a promising candidate for treating osteoporosis-related vertebral fractures.

RESUMO

Pulmonary agenesis is a rare disorder, and the right-sided one is much rarer. Most of the cases are diagnosed during early life. Because of rarity, it can be misdiagnosed and even more challenging to diagnose when presented during adult life. However, we report a rare late manifestation of right-sided unilateral lung agenesis in a 22-year-old female patient who was treated for pneumonia several times, the first reported case from Bangladesh. We also highlighted the diagnostic approach of the case in low-resource settings.

An inventive, cellulose nano crystal (CNC) and isolated soya flour (SPI) laden brushite-based injectable bone substitute (IBS) material has been developed in the present research. The purpose of the study was to discover the physical, mechanical, in-vitro biological, and in-vivo bone forming ability of the prepared IBS. The incomparable abilities of CNC together with SPI resulted in enhanced biocompatibility, mechanical strength, and biodegradability, which together with its exclusive properties, sort it ideal for bone restoration. The CNC/SPI laden composites showed suitable mechanical strength of ~10.5 MPa for BM23 composite. The in- vitro cytocompatibility of the prepared samples were evaluated by osteoblast type MC3T3-E1 cells via MTT assay. Protein absorption and mineralization behavior of BRCNC2.0 was around (1.7 and 2.3)-fold higher than that of BR, respectively. In vivo performance was also found appreciable with ~(31.33 ± 2.04) % BV/TV. Incorporation of SPI resulted in enhanced bone formation at the central zone of the defect, while unmodified samples resulted in bone formation only at the peripheral zone. The findings of the current study proposed that CNC/SPI laden, brushite based injectable bone substitute might be proficient for bone regeneration ability.

Objective: To examine the response of a calorie-restricted Dietary Approaches to Stop Hypertension diet on indicators of cardiometabolic health in a cohort of sedentary obese older adults. Design: This was a controlled-feeding trial with a parallel design. Each participant consumed either 3 oz (85 g; n = 15) or 6 oz (170.1 g; n = 13) of lean fresh beef within a standardized calorie-restricted DASH-like diet for 12-weeks. Fasted blood samples were collected and used to measure conventional biomarkers of cardiovascular, metabolic and inflammatory health. Participants: Caucasian older (70.8 years), obese (BMI: 32 ± 6.9 kg/m2; WC: 101 ± 16.4 cm) females (n = 17) and males (n = 11) from the rural community of Brookings, South Dakota. Results: 28 participants completed the 12-week feeding trial, with no differences (p > 0.05) among the biomarkers of cardiometabolic health between the 3 and 6 oz beef intake groups. However, when the beef intake groups were combined, all biomarkers changed concentration in response to the intervention diet. Total cholesterol (p < 0.001), LDL-C (p = 0.004), HDL-C (p < 0.0001), insulin (p = 0.014), glucose (p = 0.008), HOMA-IR (p < 0.05), IL-12 (p < 0.001), and CRP (p = 0.006) all decreased in response to the study diet. IGF-1 (p < 0.001) and IL-8 (p = 0.005) increased in response to the intervention. Correlations among cardiometabolic biomarkers and body composition measures were observed. By study end, the decrease in insulin (R 2 = 0.22; P = 0.012) and HOMA-IR (R 2 = 0.22; P = 0.01) was positively correlated with the decrease in waist circumference. The increase in IGF-1 was significantly correlated with the decrease in waist circumference (R 2 = 0.21; p = 0.014). The increase in IGF-1 was significantly correlated with the increase in sit-to-stand (R 2 = 0.21; p = 0.016). The increase in IL-8 was significantly correlated with decreases in total cholesterol (R 2 = 0.24; P = 0.008), LDL-C (R 2 = 0.17; P = 0.031) and glucose (R 2 = 0.44; P = 0.0001). Conclusions: These findings suggest that a DASH-like diet with restricted calories may potentially improve biomarkers of cardiometabolic health in sedentary obese older adults. These results also point to interrelationships between body composition changes and changes in cardiometabolic biomarkers. Lastly, regardless of meat intake amount, positive impacts on cardiometabolic biomarkers were observed in this cohort of older adults with an obese phenotype.

Tuberculosis (TB) usually involves the lungs and hilar lymph nodes. Musculoskeletal TB occurs in 1%-3% of patients with TB, while chest wall TB constitutes 1%-5% of all cases of musculoskeletal TB. The present case reports a twenty six year old immunocompetent patient with multiple chest wall tuberculous abscesses along with multiple ribs destruction, in absence of active pulmonary TB or intrathoracic lymphadenopathy. Chest wall TB though rare, should have a high degree of suspicion in multiple destructive bone lesions, especially in patients from regions where TB is endemic. Plain chest x-ray postero-anterior (CXR-PA) view revealed osteolytic rib lesions with overlying opacities; fine needle aspiration cytology (FNAC) was not helpful. Computerized tomography (CT) of thorax and histopathology of the biopsied unhealthy tissue from the cavity were consistent with the diagnosis of TB. Oral antitubercular therapy (ATI) was given for 12 months, following which the chest wall abscesses and rib lesions healed.

This study examined the effect of the Dietary Approaches to Stop Hypertension (DASH) diet containing lean red meat on measures of body composition and muscle strength in a cohort of obese adults 65 and older; 36 males (n = 15) and females (n = 21) consumed 1800 kcal/day for 12 weeks under controlled feeding conditions. The study diet included daily intakes of 126 g of meat. Measures of body composition and muscle strength were obtained at weeks 0, 3, 6, 9, and 12. Breakfast, lunch, and dinner were provided every day for 12 weeks, and equal portions of meat were distributed at each meal. Significant effects of the study diet were detected across time for total body weight, body mass index (BMI), waist circumference, hip circumference, body fat percentage, absolute fat mass (AFM), and blood pressure such that a decrease (p < 0.001) was observed over 12 weeks. Significant effects of the study diet were detected across time for sit/stand (p < 0.001) such that an increase was observed. From baseline to study end, total body weight decreased by 6.3% (p < 0.001), body fat percentage decreased by 2.5% (p < 0.001), and absolute fat mass (AFM) decreased by 4.4 kg (p < 0.001). By the study end, skeletal muscle mass (SMM) was positively correlated with handgrip strength (R2 = 0.75; p = 0.001) and resting energy expenditure (REE) (R2 = 0.29; p = 0.001). Handgrip strength, gait, balance, and resting energy expenditure (REE) were well maintained (p > 0.05) throughout the study. These findings suggest that the DASH diet has the potential to be a tool to preserve muscle strength while reducing fat mass in obese older adults.

Here we estimated the accuracy of the InBios Scrub Typhus Detect™ immunoglobulin M (IgM) ELISA to determine the optimal optical density (OD) cut-off values for the diagnosis of scrub typhus. Patients with undifferentiated febrile illness from Chittagong, Bangladesh, provided samples for reference testing using (i) qPCR using the Orientia spp. 47-kDa htra gene, (ii) IFA ≥1:3200 on admission, (iii) immunofluorescence assay (IFA) ≥1:3200 on admission or 4-fold rise to ≥3200, and (iv) combination of PCR and IFA positivity. For sero-epidemiological purposes (ELISA vs. IFA ≥1:3200 on admission or 4-fold rise to ≥3200), the OD cut-off for admission samples was ≥1.25, resulting in a sensitivity (Sn) of 91.5 (95% confidence interval (95% CI: 96.8⁻82.5) and a specificity (Sp) of 92.4 (95% CI: 95.0⁻89.0), while for convalescent samples the OD cut-off was ≥1.50 with Sn of 66.0 (95% CI: 78.5⁻51.7) and Sp of 96.0 (95% CI: 98.3⁻92.3). Comparisons against comparator reference tests (ELISA vs. all tests including PCR) indicated the most appropriate cut-off OD to be within the range of 0.75⁻1.25. For admission samples, the best Sn/Sp compromise was at 1.25 OD (Sn 91.5%, Sp 92.4%) and for convalescent samples at 0.75 OD (Sn 69.8%, Sp 89.5%). A relatively high (stringent) diagnostic cut-off value provides increased diagnostic accuracy with high sensitivity and specificity in the majority of cases, while lowering the cut-off runs the risk of false positivity. This study underlines the need for regional assessment of new diagnostic tests according to the level of endemicity of the disease given the high levels of residual or cross-reacting antibodies in the general population.
