Two-step liberation of labelled calcium from beta-cells was shown to correlate with biphase secretion of insulin. Data on redistribution of calcium in the cells, its binding with calmodulin and participation of glucose metabolites in the insulin secretion are discussed.

Functions of the anticoagulation system were restored in patients with insulin-dependent diabetes after neutralization of the diabetogenic factor in blood of patients by repeated administration of heparin at low doses. Development of thrombosis was also prevented in these patients.

Insulinotropic action of glucose after a glucose tolerance test and fructose after a fructose tolerance test as well as insulinotropic and hypoglycemic effects of diamicron were examined in 21 patients with type II diabetes mellitus. The data obtained indicate that insulinotropic and hyperglycemic activity of fructose yields to that of glucose. Apart from increasing insulin concentration in the blood, diamicron enhances glucose utilization at the periphery.

Glycemia level and diurnal glucosuria were studied in the morning, afternoon and evening in a group of 27 patients with insulin-dependent diabetes mellitus before and after diamicron therapy for 1.5 to 3 months. A similar investigation was performed in a group of 20 patients, suffering from diabetes mellitus of the second type before and following maninil treatment during 1.5 to 2 months. It was shown that diamicron exerts a more pronounced sugar-lowering effect in comparison with that of maninil. It promotes stable glycemia development and should take an appropriate place among hypoglycemics used now in diabetology.

The authors analyzed the results of investigation of insulin residual secretion determined by the concentration of C-peptide in response to the stimulation of 1 mg of glucagon. The blood level of the diabetogenic factor (DGF) and activity of the anticoagulative system (ACS) were studied in parallel in patients with insulin-dependent type of diabetes mellitus before and after heparin therapy. The blood DGF disappeared, ACS function was restored, and the patient's body resistance to blood hypercoagulation developed against a background of heparin therapy. A decrease in insulin residual secretion was shown to be related to a duration of diabetes mellitus.
