RESUMO

[Purpose] The present study was performed to evaluate the changes in the scapular alignment, pressure pain threshold and pain in subjects with scapular downward rotation after 4 weeks of wall slide exercise or sling slide exercise. [Subjects and Methods] Twenty-two subjects with scapular downward rotation participated in this study. The alignment of the scapula was measured using radiographic analysis (X-ray). Pain and pressure pain threshold were assessed using visual analogue scale and digital algometer. Patients were assessed before and after a 4 weeks of exercise. [Results] In the within-group comparison, the wall slide exercise group showed significant differences in the resting scapular alignment, pressure pain threshold, and pain after four weeks. The between-group comparison showed that there were significant differences between the wall slide group and the sling slide group after four weeks. [Conclusion] The results of this study found that the wall slide exercise may be effective at reducing pain and improving scapular alignment in subjects with scapular downward rotation.

[Purpose] The aim of this study was to analyze the joint space width of the humeral head and glenoid fossa during traction under 2 grade conditions (grade 2/grade 3). [Subjects and Methods] The subjects were 20 healthy male adults who had not experienced any shoulder injury. Three radiographs were obtained with the subjects in the supine position (resting, grades 2 and 3). The glenohumeral joint space was examined on radiography. Joint space width was measured by a radiologist at the points described by Petersson and Redlund-Johnell. A radiologist blinded to the variable "resting" or "traction" performed all radiographic measurements. The joint space widths were compared by using one-way repeated-measures analysis of variance. [Results] The results of this study indicated significant differences in the changes in joint space width according to traction grade. Compared to resting, grades 2 and 3 traction significantly increased joint space width. However, no significant difference in joint space width was found between grades 2 and 3 traction. [Conclusion] Although no significant differences were found between grades 2 and 3 traction during glenohumeral joint traction, the increase in joint space width between the glenoid fossa and humeral head was highest during grade 3 traction.

[Purpose] The objective of this study was to examine the effect of ankle strategy exercises on unstable surfaces on balance and walking ability in stroke patients. [Subjects and Methods] Among hospitalized stroke patients, 30 were selected based on the study criteria and were randomly divided into three groups: an ankle strategy group (n=10), balance exercise group (n=10), and control group (n=10). Patients in two groups (ankle strategy, balance exercise group) performed 15-minute exercise sessions three times a week for six weeks. To analyze the effect of the exercise, center of pressure, Berg balance Scale, Timed Up and Go test, and Functional Reach Test were assessed before and after the exercise. [Results] The ankle strategy exercise group showed more improvement in mediolateral center of pressure and Berg Balance Scale and Timed Up and Go test scores than the balance exercise group. [Conclusion] The results of this study suggest that ankle strategy exercises on unstable surfaces is feasible and efficacious for stroke patients.

[Purpose] The purpose of the present study is to examine the effects of abductor hallucis and gluteus maximus strengthening exercises on pronated feet. [Subjects and Methods] The present study was conducted with 18 adults without no history of surgery on the foot or ankle. One group performed both gluteus maximus strengthening exercises and abductor hallucis strengthening exercises, while the other group performed only abductor hallucis strengthening exercises five times per week for four weeks. [Results] The group that performed both gluteus maximus and abductor hallucis strengthening exercises showed smaller values in the height of navicular drop than the group that performed only abductor hallucis strengthening exercises. The muscle activity of the gluteus maximus and the vastus medialis increased during heel-strike in the group that added gluteus maximus exercises, and the muscle activity of the abductor hallucis significantly increased in both groups. [Conclusion] Given the results of the present study, it can be suggested that strengthening the gluteus maximus while also performing exercises to correct the pronated foot is an effective method for achieving normal gait.

[Purpose] This study aimed to determine the effects of inspiration- and expiration-oriented breathing on pulmonary function and chest expansion. [Subjects and Methods] Twenty healthy male university students were divided randomly into inspiration-oriented and expiration-oriented breathing groups. Their pulmonary function and chest size during inspiration or expiration were evaluated and then re-evaluated after 15 minutes of breathing exercise five times a week for four weeks. [Results] The breathing with mainly inspiration group (BMIG) showed significant differences in chest size during inspiration (CSI), chest expansion values (CEVs), forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and peak expiratory flow (PEF) after four weeks. The breathing with mainly expiration group (BMEG) showed significant differences in all measured variables except CSI. Comparison of the groups after exercise showed that the BMEG demonstrated differences from the BMIG in chest size during expiration (CSE), CEV, and PEF. Comparison of the changes in variables after exercise showed that the BMEG demonstrated significantly different changes in CSE, CEV, FEV1/FVC, and PEF. The BMIG showed a significantly different change in FVC. [Conclusion] Although both groups demonstrated improvements in pulmonary function and chest expansion, inter-group differences were observed. Therefore, inspiration- or expiration-oriented breathing may be recommended differently according to the desired outcome.

[Purpose] The purpose of this study was to compare the reliability of the measurement of the passive range of motion (PROM) of shoulder horizontal adduction (SHA) measurements using a smartphone for the assessment posterior shoulder tightness (PST) between the side-lying and supine test positions. [Subjects and Methods] Forty-seven subjects (mean ± age, 24.9 ± 3.5 years) without shoulder pathology were included in this study. Intra-rater and inter-rater reliabilities were determined using intraclass correlation coefficients. The SHA PROM of each subject's dominant shoulder was measured using a smartphone by two investigators in two positions: the standard supine position, and a side-lying position on the tested side. [Results] The intra-rater reliability of the supine measurements was fair to good (ICC3,1 = 0.72-0.89), and for the side-lying measurements was excellent (ICC3,1 = 0.95-0.97). The inter-rater reliability of the supine measurements was fair (ICC2,2 = 0.79) and for the side-lying measurements was excellent (ICC2,2 = 0.94). [Conclusion] These results suggest that for healthy subjects, measurements of SHA using smartphones in the side-lying position has superior intra-rater and inter-rater reliabilities compared to the standard supine position.

[Purpose] This study examined the differences in the activities of three parts of the trapezius muscle-the upper trapezius (UT), middle trapezius (MT), and lower trapezius (LT)-among three different rotation conditions of the shoulders, while subjects performed prone horizontal abduction (PHA) at 30°, 60°, 90°, and 120° of abduction. [Subjects and Methods] The subjects of this experimental study were 16 healthy male adults. Surface electromyography was used to collect data on the activity of each part of the trapezius. A two-way analysis of variance was used to compare the activities of each area of the trapezius-the UT, MT, and LT-among internal rotation (IR), the neutral position (NP), and external rotation (ER) of the shoulders during PHA with shoulder abduction of 30°, 60°, 90°, and 120°. [Results] Activity of the UT, MT, and LT significantly increased as the shoulder abduction angle increased during PHA. There was a significant difference only in the activity of the LT, with change in shoulder rotation. In addition, the muscle activity of the LT was highest during shoulder IR at 120°. [Conclusion] Although activity of the LT was the highest during IR at 120 abduction, PHA accompanied by ER at an abduction angle of 120° would be effective at eliciting high activity in the LT when PHA is performed. Nonetheless, at an early stage of rehabilitation, PHA accompanied by ER at low abduction angles of 30° and 60° would be desirable to elicit low activity of the UT and high activity of the LT.

[Purpose] This study compared the use of the Maitland mobilization and Kaltenborn mobilization techniques for improving pain and range of motion in patients with frozen shoulders. [Subjects and Methods] The subjects were 20 patients with frozen shoulder who visited Hospital H, Ulsan, Korea. The subjects were divided randomly into two groups to receive Maitland or Kaltenborn mobilization to the affected shoulder. Grade III anteroposterior oscillation and posterior translation were used for the Maitland and Kaltenborn mobilization groups, respectively. Pain and range of motion of external and internal rotation were evaluated pre- and post-intervention in both groups. Paired t-tests were used to compare the pre- and post-intervention results in both groups, and independent t-tests were used to compare groups. [Results] Both groups exhibited significant decreases in pain post-intervention. Moreover, the range of motion of internal and external rotation increased significantly post-intervention in both groups. However, there was no significant difference between groups with respect to pain improvement or range of motion. [Conclusion] The posterior Maitland and Kaltenborn mobilization techniques are effective for improving pain and range of motion in frozen shoulder patients. Therefore, we recommend both techniques for such patients.

[Purpose] The purpose of this study was to examine the effects of selective exercise for the deep abdominal muscles (SEDA) and lumbar stabilization exercise (LSE) on the thickness of the transversus abdominis and postural maintenance on an unstable base of support. [Subjects and Methods] The subjects of this study were 20 male and 10 female adults in their 20s without lumbar pain. They were equally and randomly assigned to a SEDA group and a LSE group. The thickness of the transversus abdominis was measured using ultrasound imaging during rest and drawing-in. The thickness of the transversus abdominis was measured when subjects raised their right and left legs while lying on a Swiss ball. [Results] Initially, there were no differences between the two groups. After the intervention, significant differences were observed in all parameters. A significant interaction between group and period was not found for any parameters. [Conclusion] In conclusion, both SEDA and LSE thickened the transversus abdominis, which is a deep abdominal muscle, thereby adjusting posture, and stabilizing the trunk. These exercises increased the thickness of the deep abdominal muscles. They are important exercises for improving the stability of athletes or patients who need postural adjustment.
