RESUMO

There are very few conditions that present with dopa-responsive juvenile parkinsonism. We present two such children with neuronal intranuclear inclusion disease (NIID) who had an initial good levodopa response that was soon complicated by disabling dopa-induced dyskinesia. One child was diagnosed by rectal biopsy in life, and the other diagnosis was confirmed at postmortem. In this patient, dopamine transporter imaging showed severely decreased binding of the radiotracer in the striatum on both sides. Bilateral subthalamic deep brain stimulation in this patient produced initial improvement, but this was not sustained. Both patients died within 10 years of symptom onset. As well as levodopa responsiveness with rapid onset of dyskinesia, clues to the diagnosis of NIID in patients presenting with parkinsonism include the presence of gaze-evoked nystagmus, early onset dysarthria and dysphagia and oculogyric crises. Differential diagnosis of clinical symptoms and neuropathological findings are discussed including the approach to rectal biopsy for early diagnosis.

To investigate the DYT1 gene mutation in Chinese ethnic, we examined a series of 200 patients with primary dystonias (11 familial and 189 sporadic), 53 of their asymptomatic relatives, 97 patients with familial or early-onset parkinsonism, and 200 healthy subjects. The GAG deletion at codon 946 was only found in three sporadic dystonia patients and seven of their asymptomatic familial members. The frequency of GAG deletion was 1.5% in dystonia patients, and was 6.7% in early-onset dystonias (< or = 26 years). We conclude that DYT1 mutation is a minor cause of primary dystonias in a cohort of Taiwanese population.

Sesquiterpenes, arecoic acids A-F and arecolactone, were isolated from the ethyl acetate extracts of the fermented broth of Arecophila saccharicola YMJ96022401 along with two known analogues 1,7α,10α-trihydroxyeremophil-11(13)-en-12,8-olide and 1,10α,13-trihydroxyeremophil-7(11)-en-12,8-olide. Their structures were elucidated on the basis of spectroscopic data analyses. The inhibitory effects of all of these compounds on nitric oxide (NO) production in lipopolysaccharide (LPS, 200 µg/mL)-activated murine macrophage RAW264.7 cells were also evaluated. Among these compounds, 1,7α,10α-trihydroxyeremophil-11(13)-en-12,8-olide significantly inhibited NO production without any cytotoxicity, and its average maximum inhibition (E(max)) at 100 µM and median inhibitory concentration (IC50) were 85.7%±0.8% and 16.5±1.0 µM, respectively. Arecolactone was the most potent, with the E(max) at 12.5 µM and IC50 being 94.7%±0.8% and 1.32±0.1 µM, respectively, but displayed cytotoxicity at considerable higher concentrations than 25 µM. Analyses of Western blotting indicated that arecolactone (0.8-12.5 µM) inhibited induction of inducible NO synthase (iNOS) by LPS, which involved suppression of NF-κB activation and the phosphorylation of extracellular signal-regulated kinase (ERK), c-Jun NH2-terminal kinase (JNK) and p38 mitogen-activated protein kinases (MAPKs) in activated RAW 264.7 cells. In addition, arecolactone concentration-dependently prevented the vascular hyporeactivity to phenylephrine induced by LPS (300 ng/mL) through iNOS pathway in isolated rat thoracic aortic rings. These results indicated that both of these naturally occurring iNOS inhibitors may provide a rationale for the potential anti-inflammatory effect of A. saccharicola YMJ96022401.

Femtosecond laser was employed to fabricate nanostructured Ag surface for surface-enhanced Raman scattering (SERS) application. The prepared nanostructured Ag surface was characterized by field emission scanning electron microscopy (FESEM), atomic force microscopy (AFM), X-ray diffraction (XRD), and X-ray photoelectron spectroscopy (XPS). The FESEM images demonstrate the formation of nanostructure-covered femtosecond laser-induced periodic surface structure, also termed as ripples, on the Ag surface. The AFM images indicate that the surface roughness of the produced nanostructured Ag substrate is larger than the untreated Ag substrate. The XRD and XPS of the nanostructured Ag surface fabricated by femtosecond laser show a face centered cubic phase of metallic Ag and no impurities of Ag oxide species. The application of the produced nanostructured Ag surface in SERS was investigated by using rhodamine 6G (R6G) as a reference chemical. The SERS intensity of R6G in aqueous solution at the prepared nanostructured Ag surface is 15 times greater than that of an untreated Ag substrate. The Raman intensities vary linearly with the concentrations of R6G in the range of 10(-8)-10(-4)M. The present methodology demonstrates that the nanostructured Ag surface fabricated by femtosecond laser is potential for qualification and quantification of low concentration molecules.

RESUMO

Trigeminal neuralgia is rare in patients with paranasal sinusitis. The authors describe a case of paranasal sinusitis that appeared as an insidious onset of trigeminal neuralgia along the distribution of ophthalmic and maxillary branches, followed by rapid intracranial invasion with temporal meningoencephalitis and subdural empyema, and finally a fatal outcome.
