RESUMO

The spread of antibiotic resistance genes (ARGs), particularly those carried on plasmids, poses a major risk to global health. However, the extent and frequency of ARGs transfer in microbial communities among human, animal, and environmental sectors is not well understood due to a lack of effective tracking tools. We have developed a novel fluorescent tracing tool, CRISPR-AMRtracker, to study ARG transfer. It combines CRISPR/Cas9 fluorescence tagging, fluorescence-activated cell sorting, 16S rRNA gene sequencing, and microbial community analysis. CRISPR-AMRtracker integrates a fluorescent tag immediately downstream of ARGs, enabling the tracking of ARG transfer without compromising the host cell's antibiotic susceptibility, fitness, conjugation, and transposition. Notably, our experiments demonstrate that sfGFP-tagged plasmid-borne mcr-1 can transfer across diverse bacterial species within fecal samples. This innovative approach holds the potential to illuminate the dynamics of ARG dissemination and provide valuable insights to shape effective strategies in mitigating the escalating threat of antibiotic resistance.

The perovskite nanocrystals (PeNCs) are emerging as a promising emitter for light-emitting diodes (LEDs) due to their excellent optical and electrical properties. However, the ultrafast growth of PeNCs often results in large sizes exceeding the Bohr diameter, leading to low exciton binding energy and susceptibility to nonradiative recombination, while small-sized PeNCs exhibit a large specific surface area, contributing to an increased defect density. Herein, Zn2+ ions as a negative catalyst to realize quantum-confined FAPbBr3 PeNCs with high photoluminescence quantum yields (PL QY) over 90%. Zn2+ ions exhibit robust coordination with Br- ions is introduced, effectively retarding the participation of Br- ions in the perovskite crystallization process and thus facilitating PeNCs size control. Notably, Zn2+ ions neither incorporate into the perovskite lattice nor are absorbed on the surface of PeNCs. And the reduced growth rate also promotes sufficient octahedral coordination of PeNC that reduces defect density. The LEDs based on these optimized PeNCs exhibits an external quantum efficiency (EQE) of 21.7%, significantly surpassing that of the pristine PeNCs (15.2%). Furthermore, the device lifetime is also extended by twofold. This research presents a novel approach to achieving high-performance optoelectronic devices.

Companion animals have the potential to greatly enhance the physical and mental health of humans, thus leading to an increased focus on the interactions between humans and pets. Currently, the inappropriate and excessive utilization of antimicrobial agents has become prevalent in veterinary clinical practice for pets. This antibiotic contamination phenomenon has a profound impact on the enrichment of antibiotic resistance bacteria (ARB) and antibiotic resistance genes (ARGs) in pets. However, the pet-associated resistome, especially the novel ARGs in pets, represents a relatively neglected area. In this study, we successfully constructed a total of 12 libraries using the functional metagenomics approach to assess the diversity of ARGs in pet cats and dogs from four pet hospitals. Through the integration of functional screening and high-throughput sequencing, a total of 122 antibiotic resistance determinants were identified, of which 15 were classified as putative novel ARGs originating from five classes. Functional assessment demonstrated that 6 novel ARGs including one ß-lactam, two macrolides, two aminoglycosides, and one rifamycin (RIF), namely blaPF, ermPF, msrPF, aac(6')PF, aph(3')PF, and arrPF, exhibited functionally activity in conferring bacterial phenotypic resistance by increasing the minimum inhibitory concentrations (MICs) with a 4- to 128-fold. Genetic context analysis demonstrated that, with the exception of aac(6')PF and arrPF, the remaining four novel ARGs were found adjacent to mobile genetic elements (MGEs) including IS elements or transposases, which provided a prerequisite for horizontal transfer of these novel ARGs, thereby offering an explanation for their detection in diverse samples collected from various sampling sites. The current study has unveiled the significant role of cat and dog feces as one source of reservoirs of diverse novel ARGs, while also highlighting the potential adverse consequences of their further spread to medically significant pathogens and human commensal organisms.

BACKGROUND: WNT4 gene polymorphism are common in endometriosis and may functionally link estrogen and estrogen receptor signaling. Previous study confirmed estrogen and estrogen receptor signaling recruit macrophage to promote the pathogenesis of endometriosis. To investigate the effect of WNT4 in endometriosis involved in macrophage polarization and whether WNT4 could reduce the apoptosis of granulosa cells. METHODS: An observational study consisting of 8 cases of women with endometriosis (diagnosed by surgery and histology) and 22 mice of endometriosis animal model was conducted. Granulosa cells were isolated from 16 patients with endometriosis and co-cultured with macrophage under WNT4 treatment using TUNEL assay, quantitative reverse transcription PCR, flow cytometry and ELISA analysis. 22 mice of endometriosis animal model confirmed the WNT4 treatment effects using histology and immunohistochemistry, Western blot and flow cytometry. RESULTS: We observed that the apoptotic proportion of granulosa cells was significantly decreased and M2 macrophage was significantly increased after WNT4 treatment during the granulosa cell and macrophage co-culture system. To reveal the underlying mechanism for this, we conducted a series of experiments and found that high expression of granulosa cell M-CSF led to the M2 polarization of macrophages. The animal model also suggested that the anti-apoptotic effect of WNT4 on granulosa cells were conducted by the M2 polarized macrophage. CONCLUSIONS: WNT4 could reduce granulosa cell apoptosis and improve ovarian reserve by promoting macrophage polarization in endometriosis. M-CSF secreted by granulosa cell after WNT4 treatment was the main mediator of macrophage polarization.

Salmonellosis is a globally extensive food-borne disease, which threatens public health and results in huge economic losses in the world annually. The rising prevalence of antibiotic resistance in Salmonella poses a significant global concern, emphasizing an imperative to identify novel therapeutic agents or methodologies to effectively combat this predicament. In this study, self-assembly hydrogen sulfide (H2S)-responsive nanoprodrugs were fabricated with poly(α-lipoic acid)-polyethylene glycol grafted rhein and geraniol (PPRG), self-assembled into core-shell nanoparticles via electrostatic, hydrophilic and hydrophobic interactions, with hydrophilic exterior and hydrophobic interior. The rhein and geraniol are released from self-assembly nanoprodrugs PPRG in response to Salmonella infection, which is known to produce hydrogen sulfide (H2S). PPRG demonstrated stronger antibacterial activity against Salmonella compared with rhein or geraniol alone in vitro and in vivo. Additionally, PPRG was also able to suppress the inflammation and modulate gut microbiota homeostasis. In conclusion, the as-prepared self-assembly nanoprodrug sheds new light on the design of natural product active ingredients and provides new ideas for exploring targeted therapies for specific Enteropathogens. Graphical  illustration for construction of self-assembly nanoprodrugs PPRG and its antibacterial and anti-inflammatory activities on experimental Salmonella infection in mice.

3,4-Difluorobenzyl(1-ethyl-5-(4-((4-hydroxypiperidin-1-yl)-methyl)thiazol-2-yl)-1H-indol-3-yl)carbamate (NAI59), a small molecule with outstanding therapeutic effectiveness to anti-pulmonary fibrosis, was developed as an autotaxin inhibitor candidate compound. To evaluate the pharmacokinetics and plasma protein binding of NAI59, a UPLC-MS/MS method was developed to quantify NAI59 in plasma and phosphate-buffered saline. The calibration curve linearity ranged from 9.95 to 1990.00 ng/mL in plasma. The accuracy was -6.8 to 5.9%, and the intra- and inter-day precision was within 15%. The matrix effect and recovery, as well as dilution integrity, were within the criteria. The chromatographic and mass spectrometric conditions were also feasible to determine phosphate-buffered saline samples, and it has been proved that this method exhibits good precision and accuracy in the range of 9.95-497.50 ng/mL in phosphate-buffered saline. This study is the first to determine the pharmacokinetics, absolute bioavailability, and plasma protein binding of NAI59 in rats using this established method. Therefore, the pharmacokinetic profiles of NAI59 showed a dose-dependent relationship after oral administration, and the absolute bioavailability in rats was 6.3%. In addition, the results of protein binding showed that the combining capacity of NAI59 with plasma protein attained 90% and increased with the increase in drug concentration.

As common contaminants, metals are non-negligible risk factors for diabetes and chronic kidney disease. However, whether there is an association between multiple metals exposure and incident chronic kidney disease (CKD) risk in patients with diabetes is unclear. We conducted a prospective study to evaluate these associations. In total, 3071 diabetics with baseline estimated glomerular filtration rate (eGFR) ≥ 60 mL/min/1.73 m2 from the Dongfeng-Tongji cohort were included. We measured baseline plasma concentrations of 23 metals and investigated the associations between plasma metal concentrations and CKD in diabetics using logistic regression, the least absolute shrinkage and selection operator (LASSO), and the Bayesian Kernel Machine Regression (BKMR) models. During average 4.6 years of follow-up, 457 diabetics developed CKD (14.9 %). The three models consistently found plasma levels of zinc, arsenic, and rubidium had a positive association with incident CKD risk in patients with diabetes, while titanium, cadmium, and lead had an inverse correlation. The results of BKMR showed a significant and positive overall effect of 23 metals on the risk of CKD, when all of the metals were above the 50th percentile as compared to the median value. In addition, potential interactions of zinc and arsenic, zinc and cadmium, zinc and lead, titanium and arsenic, and cadmium and lead on CKD risk were observed. In summary, we found significant associations of plasma titanium, zinc, arsenic, rubidium, cadmium, and lead with CKD in diabetes and interactions between these metals except for rubidium. Co-exposure to multiple metals was associated with increased CKD risk in diabetics.

Pixel pitch calibration is an essential step to make the fundus structures in the fundus image quantitatively measurable, which is important for the diagnosis and treatment of many diseases, e.g., diabetes, arteriosclerosis, hereditary optic atrophy, etc. The conventional calibration approaches require the specific parameters of the fundus camera or several specially shot images of the chess board, but these are generally not accessible, and the calibration results cannot be generalized to other cameras. Based on automated ROI (region of interest) and optic disc detection, the diameter ratio of ROI and optic disc (ROI-disc ratio) is quantitatively analyzed for a large number of fundus images. With the prior knowledge of the average diameter of an optic disc in fundus, the pixel pitch can be statistically estimated from a large number of fundus images captured by a specific camera without the availability of chess board images or detailed specifics of the fundus camera. Furthermore, for fundus cameras of FOV (fixed field-of-view), the pixel pitch of a fundus image of 45° FOV can be directly estimated according to the automatically measured diameter of ROI in the pixel. The average ROI-disc ratio is approximately constant, i.e., 6.404 ± 0.619 in the pixel, according to 40,600 fundus images, captured by different cameras, of 45° FOV. In consequence, the pixel pitch of a fundus image of 45° FOV can be directly estimated according to the automatically measured diameter of ROI in the pixel, and results show the pixel pitches of Canon CR2, Topcon NW400, Zeiss Visucam 200, and Newvision RetiCam 3100 cameras are 6.825 ± 0.666 µm, 6.625 ± 0.647 µm, 5.793 ± 0.565 µm, and 5.884 ± 0.574 µm, respectively. Compared with the manually measured pixel pitches, based on the method of ISO 10940:2009, i.e., 6.897 µm, 6.807 µm, 5.693 µm, and 6.050 µm, respectively, the bias of the proposed method is less than 5%. Since our method doesn't require chess board images or detailed specifics, the fundus structures on the fundus image can be measured accurately, according to the pixel pitch obtained by this method, without knowing the type and parameters of the camera.

The global spread of antimicrobial-resistant bacteria has been one of the most severe threats to public health. The emergence of the mcr-1 gene has posed a considerable threat to antimicrobial medication since it deactivates one last-resort antibiotic, colistin. There have been reports regarding the mobilization of the mcr-1 gene facilitated by ISApl1-formed transposon Tn6330 and mediated rapid dispersion among Enterobacteriaceae species. Here, we developed a CRISPR/Cas9 system flanked by ISApl1 in a suicide plasmid capable of exerting sequence-specific curing against the mcr-1-bearing plasmid and killing the strain with chromosome-borne mcr-1. The constructed ISApl1-carried CRISPR/Cas9 system either restored sensitivity to colistin in strains with plasmid-borne mcr-1 or directly eradicated the bacteria harboring chromosome-borne mcr-1 by introducing an exogenous CRISPR/Cas9 targeting the mcr-1 gene. This method is highly efficient in removing the mcr-1 gene from Escherichia coli, thereby resensitizing these strains to colistin. The further results demonstrated that it conferred the recipient bacteria with immunity against the acquisition of the exogenous mcr-1 containing the plasmid. The data from the current study highlighted the potential of the transposon-associated CRISPR/Cas9 system to serve as a therapeutic approach to control the dissemination of mcr-1 resistance among clinical pathogens.

OBJECTIVES: In this study, we developed an IS26-based CRISPR/Cas9 system as a proof-of-concept study to explore the potential of a re-engineered bacterial translocatable unit (TU) for curing and immunizing against the replication genes and antimicrobial resistance genes. METHODS: A series of pIS26-CRISPR/Cas9 suicide plasmids were constructed, and specific guide RNAs were designed to target the replication gene of IncX4, IncI2 and IncHI2 plasmids, and the antibiotic resistance genes mcr-1, blaKPC-2 and blaNDM-5. Through conjugation and induction, the transposition efficiency and plasmid-curing efficiency in each recipient were tested. In addition, we examined the efficiency of the IS26-CRISPR/Cas9 system of cell immunity against the acquisition of the exogenous resistant plasmids by introducing this system into antimicrobial-susceptible hosts. RESULTS: This study aimed to eliminate the replication genes and antimicrobial resistance genes using pIS26-CRISPR/Cas9. Three plasmids with different replicon types, including IncX4, IncI2 and IncHI2 in three isolates, two pUC19-derived plasmids, pUC19-mcr-1 and pUC19-IS26mcr-1, in two lab strains, and two plasmids bearing blaKPC-2 and blaNDM-5 in two isolates were all successfully eliminated. Moreover, the IS26-based CRISPR/Cas9 system that remained in the plasmid-cured strains could efficiently serve as an immune system against the acquisition of the exogenous resistant plasmids. CONCLUSIONS: The IS26-based CRISPR/Cas9 system can be used to efficiently sensitize clinical Escherichia coli isolates to antibiotics in vitro. The single-guide RNAs targeted resistance genes or replication genes of specific incompatible plasmids that harboured resistance genes, providing a novel means to naturally select bacteria that cannot uptake and disseminate such genes.

PURPOSE: Inflammation has been reported as a facilitator in cervical oncogenesis, but the correlation between inflammation and cytological abnormality remains uncertain. The aim of this study was to investigate the correlation between inflammation and cytological abnormality. METHODS: ThinPrep cytological test (TCT) was used to detect cervical cytological abnormalities and inflammation degrees of 46,255 women in this prospective cross-sectional study. Histopathological examination was used to define the cervical intraepithelial neoplasia (CIN) in patients with cervical cytological abnormalities. RESULTS: The study revealed that 8.87% (4102/46,255) of TCT results had cytological abnormalities. The 4102 included cases were classified as the case group, including atypical squamous cells (ASC), low-grade squamous intraepithelial lesions (LSIL) and high-grade squamous intraepithelial lesions (HSIL). Women with negative intraepithelial lesion or malignancy (NILM) were classified as the control group. About 88.83% (3644/4102) of women with cytological abnormalities showed inflammations. The rate of severe inflammation was significantly higher in the case group than the control group (23.86% vs. 2.0%, P = 0.000). Our results also showed that patients with severe inflammation had a significantly increasing incidence of cytological abnormality by 12.598 times and elevated the risk of HSIL by 756.47 times, compared to the inflammation negative group. CONCLUSION: Severe inflammation was positively related to HSIL. Patients with severe cervical inflammation should be given more follow-ups and regular examinations and treated more carefully than those with mild or no inflammation.

The mobile colistin resistance gene mcr-3 has globally disseminated since it was first reported in 2017 in Escherichia coliIn vitro mobilization assays in this study demonstrate the functionality of the composite transposon structure ISKpn40-mcr-3.11-dgkA-ISKpn40 in wild-type and recA-E. coli strains. These transpositions generated 4-bp duplications at the target sites. This is the first report demonstrating the mobility of the mcr-3.11 gene by transposition.

Building extraction from high spatial resolution remote sensing images is a hot spot in the field of remote sensing applications and computer vision. This paper presents a semantic segmentation model, which is a supervised method, named Pyramid Self-Attention Network (PISANet). Its structure is simple, because it contains only two parts: one is the backbone of the network, which is used to learn the local features (short distance context information around the pixel) of buildings from the image; the other part is the pyramid self-attention module, which is used to obtain the global features (long distance context information with other pixels in the image) and the comprehensive features (includes color, texture, geometric and high-level semantic feature) of the building. The network is an end-to-end approach. In the training stage, the input is the remote sensing image and corresponding label, and the output is probability map (the probability that each pixel is or is not building). In the prediction stage, the input is the remote sensing image, and the output is the extraction result of the building. The complexity of the network structure was reduced so that it is easy to implement. The proposed PISANet was tested on two datasets. The result shows that the overall accuracy reached 94.50 and 96.15%, the intersection-over-union reached 77.45 and 87.97%, and F1 index reached 87.27 and 93.55%, respectively. In experiments on different datasets, PISANet obtained high overall accuracy, low error rate and improved integrity of individual buildings.

In the remote sensing community, accurate image registration is the prerequisite of the subsequent application of remote sensing images. Phase correlation based image registration has drawn extensive attention due to its high accuracy and high efficiency. However, when the Discrete Fourier Transform (DFT) of an image is computed, the image is implicitly assumed to be periodic. In practical application, it is impossible to meet the periodic condition that opposite borders of an image are alike, and image always shows strong discontinuities across the frame border. The discontinuities cause a severe artifact in the Fourier Transform, namely the known cross structure composed of high energy coefficients along the axes. Here, this phenomenon was referred to as effect of image border. Even worse, the effect of image border corrupted its registration accuracy and success rate. Currently, the main solution is blurring out the border of the image by weighting window function on the reference and sensed image. However, the approach also inevitably filters out non-border information of an image. The existing understanding is that the design of window function should filter as little information as possible, which can improve the registration success rate and accuracy of methods based on phase correlation. In this paper, another approach of eliminating the effect of image border is proposed, namely decomposing the image into two images: one being the periodic image and the other the smooth image. Replacing the original image by the periodic one does not suffer from the effect on the image border when applying Fourier Transform. The smooth image is analogous to an error image, which has little information except at the border. Extensive experiments were carried out and showed that the novel algorithm of eliminating the image border can improve the success rate and accuracy of phase correlation based image registration in some certain cases. Additionally, we obtained a new understanding of the role of window function in eliminating the effect of image border, which is helpful for researchers to select the optimal method of eliminating the effect of image border to improve the registration success rate and accuracy.

Deformable image registration (DIR) is the key process for contour propagation and dose accumulation in adaptive radiation therapy (ART). However, currently, ART suffers from a lack of understanding of "robustness" of the process involving the image contour based on DIR and subsequent dose variations caused by algorithm itself and the presetting parameters. The purpose of this research is to evaluate the DIR caused variations for contour propagation and dose accumulation during ART using the RayStation treatment planning system. Ten head and neck cancer patients were selected for retrospective studies. Contours were performed by a single radiation oncologist and new treatment plans were generated on the weekly CT scans for all patients. For each DIR process, four deformation vector fields (DVFs) were generated to propagate contours and accumulate weekly dose by the following algorithms: (a) ANACONDA with simple presetting parameters, (b) ANACONDA with detailed presetting parameters, (c) MORFEUS with simple presetting parameters, and (d) MORFEUS with detailed presetting parameters. The geometric evaluation considered DICE coefficient and Hausdorff distance. The dosimetric evaluation included D95 , Dmax , Dmean , Dmin , and Homogeneity Index. For geometric evaluation, the DICE coefficient variations of the GTV were found to be 0.78 ± 0.11, 0.96 ± 0.02, 0.64 ± 0.15, and 0.91 ± 0.03 for simple ANACONDA, detailed ANACONDA, simple MORFEUS, and detailed MORFEUS, respectively. For dosimetric evaluation, the corresponding Homogeneity Index variations were found to be 0.137 ± 0.115, 0.006 ± 0.032, 0.197 ± 0.096, and 0.006 ± 0.033, respectively. The coherent geometric and dosimetric variations also consisted in large organs and small organs. Overall, the results demonstrated that the contour propagation and dose accumulation in clinical ART were influenced by the DIR algorithm, and to a greater extent by the presetting parameters. A quality assurance procedure should be established for the proper use of a commercial DIR for adaptive radiation therapy.

The successful launch of Luojia 1-01 complements the existing nighttime light data with a high spatial resolution of 130 m. This paper is the first study to assess the potential of using Luojia 1-01 nighttime light imagery for investigating artificial light pollution. Eight Luojia 1-01 images were selected to conduct geometric correction. Then, the ability of Luojia 1-01 to detect artificial light pollution was assessed from three aspects, including the comparison between Luojia 1-01 and the Suomi National Polar-Orbiting Partnership Visible Infrared Imaging Radiometer Suite (NPP-VIIRS), the source of artificial light pollution and the patterns of urban light pollution. Moreover, the advantages and limitations of Luojia 1-01 were discussed. The results showed the following: (1) Luojia 1-01 can detect a higher dynamic range and capture the finer spatial details of artificial nighttime light. (2) The averages of the artificial light brightness were different between various land use types. The brightness of the artificial light pollution of airports, streets, and commercial services is high, while dark areas include farmland and rivers. (3) The light pollution patterns of four cities decreased away from the urban core and the total light pollution is highly related to the economic development. Our findings confirm that Luojia 1-01 can be effectively used to investigate artificial light pollution. Some limitations of Luojia 1-01, including its spectral range, radiometric calibration and the effects of clouds and moonlight, should be researched in future studies.

Despite a significant surge in the number of investigations into chirality at the nanoscale, especially thiolated chiral molecules capping gold clusters, only limited knowledge is currently available to elaborate the alloying effect on chiroptical behavior of bimetallic nanoparticles (NPs). Also, few successful cases as to the efforts toward the development of chirality-dependent applications on the optically active nanomaterial have been made. Herein, as a positive test case for chiral alloy nanoparticle synthesis, the stable and large chiroptical ultrafine Au-Ag alloy NPs were prepared by reduction of different molar fractions of HAuCl4 and AgNO3 with NaBH4 in the presence of d/l-penicillamine (d/l-Pen). Compared with those of monometallic Au and Ag counterparts with comparable size, the Au-Ag alloy NPs (Ag mole fraction, 70%) obviously displayed the largest optical activities with the maximum g-factors of ∼1.6 × 10-3. Impressively, the Pen-mediated synthesis of chiral Au-Ag alloy NPs possesses a colorimetric self-recognition function and can be used as an incisive circular dichroism (CD) probe toward d- and l-Pen enantiomers. The plasmonic CD signal amplification (ΔICD) shows good linearity with the amount of Pen over the range of 5.0-80.0 µM with a detection limit (3σ) of 1.7 µM for l-Pen and 1.5 µM for d-Pen, respectively. In addition, the sensing system exhibits good selectivity toward d- and l-Pen in the presence of other enantiomers; therefore, it is highly expected that the approach described here would open new opportunities for design of more novel enantioselective analyses of important species related to biological processes.

The interaction of planar aromatic molecules with the graphene oxide (GO) sheets is often marked by the fluorescence quenching of the former. Here, the α,ß,γ,δ-tetrakis[4-(trimethylammoniumyl)phenyl]porphyrin (TAPP) molecules and the GO, corresponding to the energy donor and the acceptor respectively, are initially separated by encapsulating the TAPP molecules within the liposomes, to obstruct the formation of the self-assembled energy transfer-based quenching system. Upon disruption of the liposome membranes by the PLA2 or the α-toxin, the encapsulated TAPP molecules are released and subsequently result in significant fluorescence changes. Thus, a platform based on the fluorescence signal for monitoring the activity of the membrane pore-forming protein with advantages of high sensitivity and commonality was established. Using this strategy, we can detect the PLA2 and the α-toxin concentrations as low as 200 pM and 9.0 nM, respectively. Furthermore, by taking chlorpromazine and baicalin as the examples, we use the assay to evaluate the prohibition effects on the PLA2 and the α-toxin, and the IC50 values of chlorpromazine toward the PLA2 (9.6 nM) and that of baicalin toward the α-toxin (289.2 nM) were found to be 12.0 ± 0.62 µM and 26.9 ± 2.6 µM, respectively.

AIM: The aim of this study was to compare the feasibility, reliability, safety and pregnancy outcomes following transabdominal myomectomy (TAM) and laparoscopic myomectomy (LM) at Beijing Obstetrics and Gynecology Hospital. MATERIAL AND METHODS: The study included two parts: between January 2005 and December 2010, data on 157 patients were retrospectively collected; and between January 2011 and January 2013, 111 patients were prospectively collected. All of them had fertility requirements following myomectomy. Patients' demographics, leiomyomas' characteristics, perioperative data regarding surgical complications, relapses, subsequent pregnancy outcomes and obstetric characteristics were collected. RESULTS: The patients' demographics and leiomyoma characteristics were comparable in the TAM and LM groups (P > 0.05). There was no significant difference in the average drop in hemoglobin between the two groups (P = 0.887), while the postoperative ileus duration, postoperative ambulation duration and dose of analgesia were significantly higher in the TAM group (P < 0.001). There was no significant difference in the overall relapse and subsequent cumulative pregnancy rates and obstetric complications between the two groups. The contraception interval after myomectomy was significantly longer (P = 0.038) after TAM, however the cesarean section rate only due to myomectomy history was higher (P = 0.034) after TAM than after LM. Four patients in the LM group were identified as having uterine scar defective repair on the site of the previous myomectomy scar during elective cesarean section, while this was not identified in any patient in the TAM group. CONCLUSION: LM is a feasible treatment for women who have fertility requirements but suffer from leiomyoma. Although LM does not increase the rate of uterine rupture in the subsequent pregnancy, it is advisable for surgeons to limit the use of electrosurgery.

Trichilemmal carcinoma (TC) is a rare, malignant cutaneous adnexal tumor. TC often has nonspecific clinical manifestations and its aggressive nature is frequently overlooked. Metastasis of TC is rarely reported and there is no standard treatment for recurrent or metastatic TC. We report a complicated case of TC arising from the parotid gland with metastasis to cervical lymph nodes. The tumor progressed after multiple surgeries, radiation and chemotherapy. Finally, the patient achieved good response and disease control with pembrolizumab, an immune checkpoint inhibitor targeting programmed cell death protein-1. Currently, the patient has received 19 cycles of pembrolizumab and the disease remains well controlled. This represents the first reported use of immune checkpoint blockade to treat TC.


This paper discusses a rare form of skin cancer called trichilemmal carcinoma (TC) and presents a distant metastasis TC case. The patient was treated with an immunotherapy called pembrolizumab and after 19 courses of treatment, the tumor was significantly reduced and the symptoms were relieved. This case report is the first recorded case study of pembrolizumab for the treatment of TC and provides a new approach to the treatment of challenging malignancies.
