RESUMO

The objective of the present study was the enhancement of the effectiveness of the treatment of acute inflammatory diseases of the paranasal sinuses with the unilateral application of a medication mixture containing the vasoconstricting antiseptic agents, antibiotics, and other medications introduced into all paranasal sinuses by means of the procedure performed with the use of the Yamik nasal catheter.

RESUMO

The authors describe a patient presenting with initially multiple metachronous cancer spreading over the right-handed side of the mouth floor and the right-handed part of the lower jaw. The patient who underwent cross-plastic surgery for the correction of the extensive soft tissue defect on the neck with the use of the musculocutaneous pectoral flap with the axial blood supply is described. The observation during the 14 year follow-up period confirmed the good functional outcome of the treatment.

RESUMO

The authors report their experience with the application of the local antiseptic preparation Ambipor for the treatment of operative wound infection in the patients presenting with malignant neoplasms. Ambipor was used in 23 patients receiving surgical treatment of locally advanced intranasal and paranasal tumours. It was shown that the application of ambipor allows decreasing the risk of development of wound infection in the postoperative period.

RESUMO

The author emphasizes the importance of early diagnosis of Wegener's granulomatosis based on clinical manifestations of the disease with the participation of an otorhinolaringologist. An algorithm of its treatment is proposed that allowed life expectancy to be increased from 8 to 25 years in 10 of the eleven patients.

RESUMO

The objective of this study was to compare results of combined and surgical treatment in 114 patients with stage III-IV T3-4N0M0 laryngeal cancer. Fifty eight patients were treated by laryngectomy without preceding tracheostomy; in the remaining 56 laryngectomy followed tracheostomy (control group). The two groups were matched for all major characteristics, such as age and sex of the patients, histological structure of the tumour, its growth rate and localization, dose and schedule of radiotherapy, extent of surgical intervention. The data obtained were used to evaluate oncological and functional outcomes of the treatment.

RESUMO

We analysed medical histories of 68 patients with T2-3N0M0 laryngeal cancer who had received combined or surgical treatment. All the patients given the organ preservation treatment were divided into two groups one of which included 36 patients after laryngeal resection without tracheostomy, the other consisted of 32 patients who had undergone laryngectomy with tracheostomy (control). The two groups were matched for age and sex of the patients, the degree of advancement and histological structure of the tumour, its growth rate and localization, dose and schedule of radiotherapy, extent of surgical intervention. The data obtained were used to analyse oncological and functional outcomes of the treatment. Oncological result was estimated based on the survival rate expressed in percentage terms.

RESUMO

The authors present the results of treatment of 62 patients with locally advanced laryngeal cancer (LALC) stage III-IV (T3-4NOMO) treated in 1986-2000. The study was made of tracheostomy effects in laryngostenosis following radical operation (not tracheostomy as a stage of laryngectomy) on regional and distant metastasizing as well as on further growth, tumor recurrence and survival in treated patients with LALC. The authors conclude that tracheostomy conducted before radical surgery has a negative effect on heeling the postoperative wound, promotes regional metastasizing and tumor recurrence in patients with LALC, lowers 5-year survival.

RESUMO

Multifunctional sinus-catheter [symbol: see text] is a helpful modality in investigation of affected nasal cavity and paranasal sinuses. It can be used to obtain wash-offs from nasal and paranasal mucosa, contrast image of unilateral paranasal sinuses, to prepare for operations, to treat acute and chronic nasal and paranasal conditions, to stop nasal hemorrhage, to take expertise and prognosis of nasal outlet openings patency.

RESUMO

The efficacy of "YAMIK" sinus catheter versus conventional sinus punctures has been tried in acute purulent polysinusitis. "YAMIK" provides atraumatic evacuation of the discharge from all the sinuses on one side as well as introduction of the drugs into the sinuses via natural shunts by means of controlled intermittent pressure produced in the nasal cavity and paranasal sinuses. The methods was tested in 101 patients with acute purulent polysinusitis versus punctures of the maxillary and frontal sinuses in 30 controls. The differences in the outcomes were insignificant, through the catheter has the advantage of more rapid sinus cleansing. The authors recommend the above catheterization as pathogenetic, usable in inpatient and outpatient settings.

RESUMO

To improve the outcomes of surgical or combined treatment of patients with laryngeal cancer stage III-IV in terms of preventing the recurrences and metastases, the authors placed biocompatible polymer material saturated with cytostatic in the operative wound in the end of the surgical intervention. The response assessed in 107 patients with laryngeal cancer treated with prospidin on polymer film (20 cases) versus 87 prospidin-untreated controls was more positive in the test group (5-year survival was higher).

RESUMO

A new method for the diagnosis and treatment of PNS has been designed. The devices are named sinus-catheters YAMIK and consist of elongated flexible body with two inflatable vessels and three passages. Two of the passages have separate outlets into the vessel cavities and serve for their inflation. The third, working passage, opens on the body surface between the vessels and has an adaptor on the other end. Also a glass model of nasal sinuses has been developed. On the model YAMIK the procedure performs as follows: sinus catheter is inserted into the nasal cavity, the nostril and rhinopharynx are sealed off by inflating the vessels, a syringe is connected to the adaptor of the working passage. It creates controlled negative pressure in the nasal cavity, and this makes pathological secretion come out of the paranasal sinuses through their natural openings. After the evacuation of the pathological contents, medicine is introduced into the sinuses. This glass model is helpful in understanding the principle of YAMIK procedure which provides a good opportunity to evacuate pathological secretion from PNS through natural ostia and introduce medicine into the PNS.

RESUMO

The [symbol: see text] sinus-catheter was used to treat purulent sinusitis with septal perforation or defective septum of the sinuses. The procedure described implies simultaneous introduction of two sinus-catheters the functioning channels of which unite into one output by means of T-joint. The method has been successfully tried in the treatment of exudative and aggravated chronic purulent hemisinusitis (6 cases). The authors hold that communication between nasal sinuses is not a contraindication for sinus-catheter application. This fact widely extends potentialities of its use.

RESUMO

Sinus catheters were inserted in 44 children (mean age 8.5 years) suffering from acute and chronic purulent polysinusitis. In 33 outpatients sinus catheterization was combined with nasal irrigation providing a persistent therapeutic effect on the inflammation focus. In view of good treatment results both methods are offered for inpatient and outpatient pediatric practice of treating sinusitis.

RESUMO

The paper outlines 15-year experience in anesthesia perfection in laryngeal resections with polymeric endoprosthesis (tube endoprosthesis EG 1-4, TU 42-2-467-85) made in 115 patients with laryngeal cancer stage III; presents the analysis of complications in the above resection depending on the time of postoperative and decanulation and anesthesiological procedure. The authors propose methods of shortening the time to decanulation and even to avoid tracheostomy in transcutaneous jet high-frequency artificial lung ventilation.

RESUMO

The analysis of effectiveness of combined vertical and horizontal laryngeal resections with application of endoprostheses for cancer stage III-IV has shown that the biocompatible polymeric endoprostheses based on vinylpyrrolidone and alkyl methacrylate are superior to endoprostheses made of medical silicone by frequency of operative wound healing by first intention, by feasibility of tracheostoma one-stage permanent suturing at operation and early decannulation, by the degree of correction of external respiration dysfunction and blood gas composition. Long-term therapeutic results of application of biocompatible polymeric endoprosthesis are similar to those in using endoprosthesis made of biologically intact material--medical silicone.
