OBJECTIVES: International growth charts have been used in the past decades to identify atypical growth and diagnose the nutritional status of individuals. The aim of this study was to construct and compare growth patterns of normo-nourished children between 6-59 months from Afghanistan, Haiti, and the Democratic Republic of the Congo, to assess if it would be worth developing growth charts at a national level. METHODS: We used an international sample of 46 466 subjects (53.7% female; 46.3% male) from the aforementioned regions. To create the growth charts, we used different statistical methodologies: the Lambda-Mu-Sigma (LMS), LMSP, and LMST models, and regression models based on fractional polynomials. The LMSP models were the ones that fitted our data best and were therefore the ones used to make comparisons between countries using percentiles (3rd, 50th, and 97th). RESULTS: We found that Haitian children were both, taller and heavier than their Afghan and Congolese equals of the same ages. Moreover, differences were bigger in the highest percentiles (i.e., 97th percentile). These differences might be the result of the influence that genetics and diverse social and environmental contexts have on growth rates. CONCLUSIONS: Using the same international reference standards for all populations could result in the overestimation or underestimation of the proportion of malnourished children. In light of our results, we recommend the future development of national and regional growth charts to provide health workers with more precise tools to evaluate the nutritional status in the child population.

OBJECTIVE: Studying eating habits can aid in the design of specific measures that reduce the negative effects of an unhealthy diet on health. In this context, the aim of the present study was to examine the eating habits and food preferences of students and their level of satisfaction with the catering services of the university. DESIGN: Survey conducted during 2011 using a questionnaire that asked participants abut their sex, age and frequency of use of catering services placed on campus. Participants were also asked about their level of satisfaction with five aspects (hygiene, quality, taste of food, price and convenience of facilities) of the university catering services, what their preferred dishes were and whether they followed a special diet. SETTING: Complutense University of Madrid, Spain. SUBJECTS: Nine hundred and sixty-four students (381 males; 583 females). RESULTS: The students used the university food service 2·3 (sd 1·3) times/week. With respect to satisfaction level, 44·1 % gave an average score (3) to the combination of surveyed aspects (hygiene, quality, taste of food, price and comfort of the dining rooms). Regarding food choices, 61·0 % of students preferred pasta dishes, followed by meat (59·1 %) and salads (32·5 %). The least popular dishes were vegetables (16·8 %), fruits (13·6 %), milk products (12·2 %) and legumes (9·8 %). Of the students, 20·1 % followed special diets. CONCLUSIONS: The degree of satisfaction with the university meal service was low and the most common choices of dishes and foods among students were far from the guidelines of the Mediterranean diet. It is necessary to extend policies related to diet to this sector of the population and also to the management and food offer of university canteens.

Introduction: Introduction: eating behaviors may modulate or influence diet quality and the degree of adherence to the Mediterranean diet in children and adolescents. Aims: to investigate the association between diet quality and eating behavior in a group of Spanish schoolchildren. Methods: descriptive cross-sectional study of 283 Spain schoolchildren aged six to 16. Each subject was assessed using the KIDMED questionnaire for adherence to the Mediterranean diet and the Children's Eating Behaviour Questionnaire (CEBQ) questionnaire to assess eating behavior. Results: of the sample, 12.80 % had low adherence to the Mediterranean diet, 59.80 % had poor adherence, 59.80 % could be improved, and 27.40 % had high adherence. When relating eating behavior to dietary quality, it was found that, generally, schoolchildren with greater enjoyment of food and less demand for food have double the adherence to the Mediterranean diet due to almost twice the consumption of vegetables, nuts, and fish, and a lower intake of low-quality processed foods (sweets). The response to satiety and the speed of ingestion also played an important role in food choices. Conclusions: eating behaviors may play an essential role in the quality of school children's diets highlighting the enjoyment and acceptance or rejection of food. Given the bidirectional nature of the effects between eating behaviors and diet quality, the joint analysis may be the basis for future research with the aim of a better nutritional approach from the earliest ages.


Introducción: Introducción: las conductas alimentarias pueden modular o influir en la calidad de la dieta y el grado de adherencia a la dieta mediterránea de niños y adolescentes. Objetivos: investigar la asociación entre la calidad de la dieta y el comportamiento alimentario en un grupo de escolares españoles. Métodos: se trata de un estudio transversal descriptivo en una muestra de 283 escolares españoles de seis a 16 años. Cada sujeto fue evaluado mediante el cuestionario KIDMED de adherencia a la dieta mediterránea y el cuestionario Children's Eating Behaviour Questionnaire (CEBQ) para evaluar el comportamiento alimentario. Resultados: un 12,80 % de la muestra presentó baja adherencia a la dieta mediterránea; un 59,80 %, adherencia mejorable; y un 27,40 %, adherencia alta. Al relacionar las conductas alimentarias con la calidad dietética, los escolares con mayor disfrute por los alimentos y menos exigencia con la comida presentan el doble de adherencia a la dieta mediterránea, por un consumo de casi el doble de vegetales, frutos secos o pescado, así como menor ingesta de procesados de baja calidad (golosinas y dulces). La respuesta a la saciedad y la velocidad de ingesta también tuvieron un papel importante en las elecciones alimentarias. Conclusiones: las conductas alimentarias pueden tener un importante papel en la calidad de la dieta de los escolares, destacando el disfrute y la aceptación o rechazo por los alimentos. Dada la naturaleza bidireccional de los efectos entre las conductas alimentarias y la calidad de la dieta, el análisis conjunto puede ser la base de futuras investigaciones con el objetivo de un mejor abordaje nutricional desde las edades más tempranas.

Olive oil and lycopene are foods that have potent antioxidant activity. The objective was to determine the effects of consumption of olive oil enriched with lycopene on oxidative stress biomarkers in hypercholesterolemic subjects. We examined the effects of oil enriched with lycopene extract daily intake during 1 month on plasma antioxidant capacity, lipids profile (triacylgycerols, total cholesterol, cHDL; cLDL, ox-LDL), biomarkers of oxidative stress, and inflammatory markers related with atherosclerosis risk (C-reactive protein (CRP), IL-6; sDC4L) in subjects hypercholesteremics (cholesterol > 220 mg/dL). In the group consuming olive oil-lycopene, significant increases (p < 0.05) in the levels of plasma lycopene concentration (0.146 ± 0.03 versus 0.202 ± 0.04 (µmol/L)), α-carotene (0.166 ± 0.064 versus 0.238 ± 0.07) and in ß-carotene (0.493 ± 0.187 versus 0.713 ± 0.221) were observed. These results are linked with the increases of plasma antioxidants and decreases biomarkers of oxidative stress (carbonyl groups, malondialdehyde and 8-hydroxy-deoxiguanosine) observed in hypercholesterolemic group. In relation to lipid profile, a significant decrease was observed in the levels of ox-LDL (781 ± 302 versus 494 ± 200), remaining unchanged the levels of TG, cholesterol, HDL and LDL-c. Regarding inflammatory biomarkers, the levels of CRP and IL-6 decreased significantly. The positive results obtained in this study support the use of olive oil enriched with lycopene to reduce the risk of coronary disease.

From the point of view of prevention, it is convenient to explore the association between eating behavior and the obese phenotype during school and adolescent age. The aim of the present study was to identify eating behavior patterns associated with nutritional status in Spanish schoolchildren. A cross-sectional study of 283 boys and girls (aged 6 to 16 years) was carried out. The sample was evaluated anthropometrically by Body Mass Index (BMI), waist-to-height ratio (WHtR) and body fat percentage (%BF). Eating behavior was analyzed using the CEBQ "Children's Eating Behavior Questionnaire". The subscales of the CEBQ were significantly associated with BMI, WHtR and %BF. Pro-intake subscales (enjoyment of food, food responsiveness, emotional overeating, desire for drinks) were positively related to excess weight by BMI (ß = 0.812 to 0.869; p = 0.002 to <0.001), abdominal obesity (ß = 0.543-0.640; p = 0.02 to <0.009) and high adiposity (ß = 0.508 to 0.595; p = 0.037 to 0.01). Anti-intake subscales (satiety responsiveness, slowness in eating, food fussiness) were negatively related to BMI (ß = -0.661 to -0.719; p = 0.009 to 0.006) and % BF (ß = -0.17 to -0.46; p = 0.042 to p = 0.016).

Commercial aviation pilots are an occupational group that work in particular conditions, with frequent schedule changes, shift work, unfavorable environmental conditions, etc. These circumstances can lead to fatigue, work overload (WO), and daytime sleepiness, factors that can affect their health and safety. This study aimed to assess the prevalence and the association between these parameters in a sample of Spanish commercial airline pilots. The Raw TLX, Fatigue Severity Scale, and the Epworth Sleepiness Scale questionnaires were administered in a sample of 283 participants. The relationships of the total scores between all the questionnaires were studied by the chi-square test and the risk scores (odds ratio) were calculated. Different models using multiple linear regression were carried out to evaluate the effects of WO, fatigue, and daytime sleepiness, among the total scores, age, and flight hours. Additionally, the internal consistency of each questionnaire was estimated. A total of 28.2% presented WO above the 75th percentile, with mental and temporal demand the dimensions with the greatest weight. A total of 18% of pilots presented fatigue, 15.8% moderate sleepiness, and 3.9% severe sleepiness. We observed an association among WO, fatigue, and daytime sleepiness, important factors related to pilot health and aviation safety.

IntroductionCommon or non-syndromic obesity is a complex polygenic trait conditioned by biallelic or single-base polymorphisms called SNPs (Single-Nucleotide Polymorphisms) that present an additive effect and act synergistically. Most genotype-obese phenotype association studies include body mass index (BMI) or waist-to-height ratio (WtHR), and very few introduce a broad anthropometric profile. ObjectiveTo verify whether a genetic risk score (GRS) developed from 10 SNPs is associated with the obesity phenotype assessed from anthropometric measures indicative of excess weight, adiposity and fat distribution. Material and methodsA series of 438 Spanish schoolchildren (6-16 years old) were evaluated anthropometrically (weight, height, waist circumference, skinfold thickness, BMI, WtHR, body fat percentage [%BF]). Ten SNPs were genotyped from saliva samples, generating a GRS for obesity, establishing genotype-phenotype association. ResultsSchoolchildren categorised as obese by BMI, ICT and %BF had higher GRS than their non-obese peers. The prevalence of overweight and adiposity was higher in subjects with a GRS above the median. Similarly, between 11 and 16 years of age, all anthropometric variables presented higher averages. ConclusionsGRS estimated from the 10 SNPs can be a diagnostic tool for the potential risk of obesity in Spanish schoolchildren and could be useful from the preventive perspective.

Cardiovascular accidents are the most disabling event for pilots, causing complicated situations during flight and the withdrawal of license. The study aims to assess the modifiable risk factors and the atherogenic index of plasma (AIP) associated with anthropometric, physiometabolic and lifestyle profiles in a sample of Spanish aviation pilots. Data from pilots' clinical and professional history, anthropometric and bioelectrical impedance assessments of nutritional status, and diet and physical activity questionnaires. The sample comprised 304 men pilots. Up to 53.6% showed excess weight, of which 6.4% were obese, 64.3% presented high relative adiposity and 64.6% showed abdominal obesity. Regarding the physiometabolic profile, 10.0% had hypertension, 42.6% hypercholesterolemia, 9.4% high LDL and 10.6% low HDL, 9.4% hyperglycemia and 8.1% hypertriglyceridemia. The adherence to the Mediterranean diet (MedDiet) was high in 29.7% and low in 14.7%. Most of the sample showed a good physical activity level. The AIP risk increased with higher obesity indicators and LDL cholesterol levels. There was an inverse relationship between the MedDiet adherence and vigorous physical activity and the risk of atherogenicity. Elevated rates of overweight, abdominal obesity and hypercholesterolemia were found, contributing to the atherogenic risk of plasma (AIP). This parameter was significantly associated with all anthropometric indicators and LDL cholesterol. Prevention plans on reducing excess fat and blood cholesterol levels are recommended to reduce cardiovascular risk in Spanish aviation pilots and ensure flight safety.

OBJECTIVE: We assessed the influence of alcohol-free beer on factors implicated in atherosclerosis, such as lipid profile, oxidative stress parameters, and proinflammatory cytokines, in postmenopausal women, a population particularly at risk for atherosclerotic disease. METHODS: The study was carried out in 29 nuns, 58 to 73 y old, who live in a convent with a disciplined, regular, and homogeneous lifestyle. The nuns maintained their habits and diet routine, but their meals were supplemented with 500 mL/d of alcohol-free beer (0.0%) divided into two doses over a 45-d period. Lipid profile, inflammatory markers such as C-reactive protein, interleukins 1 and 6, and tumor necrosis factor-alpha, and parameters of oxidative metabolism were determined before and after the study period. RESULTS: There were no differences in the levels of C-reactive protein and proinflammatory cytokines after diet supplementation. The antibody titers to oxidized low-density lipoprotein were significant lower (P < 0.05), and thiobarbituric acid-reactive substances (-18%, P < 0.001) and plasma carbonyl group content (-21%, P < 0.001) were decreased when compared with initial values. Increases in alpha-tocopherol levels (+9%, P < 0.05) and erythrocytic glutathione levels (+29%, P < 0.001) were also noted. With respect to lipid profile, only subjects with cholesterol levels higher than 240 mg/dL showed lower levels after supplementation. CONCLUSION: Consumption of non-alcoholic beer produces a decrease in oxidative stress that can have a beneficial impact on cardiovascular risk; however, the circulating concentrations of inflammatory mediators involved in its pathophysiology remained unchanged.

INTRODUCTION: Background: the prevalence of overweight and obesity in childhood and adolescence in Spain is alarming. The high consumption of processed products of low nutritional quality but high caloric density, together with sedentary lifestyle, are two of the main causes. Objective: to know the nutritional status of a population of children and adolescents, and some of their eating habits, tastes and preferences, and physical activity practice. Methods: a cross-sectional descriptive study based on an anthropometric assessment and a survey on food habits and lifestyle in 1,939 schoolchildren between seven and 16 years of school age in the Community of Madrid. Updated indexes such as the body mass index (BMI) adapted according to the Cole cut-off points and the waist-height index (CTI), which have a high correlation with infant adiposity, are used. Results: the rates of overweight and obesity are high in both children and adolescents. It should be noted that abdominal obesity according to ICT represents a greater percentage than global obesity diagnosed with BMI (overweight + total obesity according to BMI: 27.23%, and according to ICT: 35.17%). The sedentariness is high (25.12%), as well as the consumption of highly caloric products, but of low nutritional value mainly at breakfast, mid-morning and snack. Conclusions: the prevalence of overweight and obesity is high, especially at the abdominal level, probably associated with the increasing consumption of ultra-processed products and the high rates of sedentary lifestyle. The conclusion is the need to develop programs to prevent obesity and nutritional education.


INTRODUCCIÓN: Introducción: la prevalencia de sobrepeso y obesidad en la infancia y adolescencia en España es alarmante. El alto consumo de productos procesados de baja calidad nutricional pero alta densidad calórica, junto al sedentarismo, son dos de las principales causas. Objetivos: conocer la condición nutricional de una población de niños y adolescentes, y algunos de sus hábitos alimentarios, gustos y preferencias y práctica de actividad física. Métodos: estudio descriptivo transversal basado en una valoración antropométrica y encuesta sobre hábitos alimentarios y estilo de vida a 1.939 escolares de entre siete y 16 años de centros escolares de la Comunidad de Madrid. Se emplean índices actualizados como el índice de masa corporal (IMC) adaptado según los puntos de corte de cole y el índice cintura-talla (ICT), que presentan alta correlación con la adiposidad infantil. Resultados: las tasas de sobrepeso y obesidad son elevadas tanto en población infantil como adolescente. Cabe destacar que la obesidad abdominal según ICT representa un mayor porcentaje que la obesidad global diagnosticada con el IMC (sobrepeso + obesidad total según IMC: 27,23%; y según ICT: 35,17%). El sedentarismo es elevado (25,12%), así como el consumo de productos altamente calóricos pero de bajo valor nutricional tanto en desayuno como en media mañana y merienda. Conclusión: la prevalencia de sobrepeso y obesidad es elevada, sobre todo a nivel abdominal, probablemente asociada al creciente consumo de productos ultraprocesados y a las altas tasas de sedentarismo. Se concluye la necesidad de desarrollar programas de prevención de obesidad y educación nutricional.

In Spain, recently, the public institutions have given information to the population in relation to fish consumption and the risk that it poses to health from the ingestion of mercury supposedly contained in the fish. At the same time, several scientific societies have published various works in this direction. All this without there being, up to now, any study on the evaluation of a probabilistic risk from mercury due to fish and seafood intake in Spain, which is the objective of this present work. For that purpose, we took individual data from a survey of the total diet of 3000 people, whose consumption of the principal fish and seafood species (49) was estimated. We compiled individualized data (2000) on the total mercury content of those species, which were completed and validated with bibliographic statistical data. After estimating the distributions of each fish and seafood species, both of their consumption and their mercury content, a simulation was made of the distribution of mercury ingestion from fish and seafood offered by 2.6% of the Spanish population at risk of exceeding total mercury recommendations, and between 12.2% and 21.2% of those exceeding methylmercury ones. The main species responsible were tuna fish, swordfish and hake, and significant differences were identified in fish consumption between sexes and ages, although, in the risk percentage, what stands out is an increase in the latter with an increase in age.

The beneficial effects of fish consumption in both children and adults are well known. However, the intake of methylmercury, mainly from contaminated fish and shellfish, can have adverse health effects. The study group on the prevention of exposure to methylmercury (GEPREM-Hg), made up of representatives from different Spanish scientific societies, has prepared a consensus document in a question and answer format, containing the group's main conclusions, recommendations and proposals. The objective of the document is to provide broader knowledge of factors associated with methylmercury exposure, its possible effects on health amongst the Spanish population, methods of analysis, interpretation of the results and economic costs, and to then set recommendations for fish and shellfish consumption. The group sees the merit of all initiatives aimed at reducing or prohibiting the use of mercury as well as the need to be aware of the results of contaminant analyses performed on fish and shellfish marketed in Spain. In addition, the group believes that biomonitoring systems should be set up in order to follow the evolution of methylmercury exposure in children and adults and perform studies designed to learn more about the possible health effects of concentrations found in the Spanish population, taking into account the lifestyle, eating patterns and the Mediterranean diet.

The beneficial effects of fish consumption are well- known. Nevertheless, there is worldwide concern regard methylmercury concentrations in fish, which is why many countries such as the United States, Australia, New Zealand, Canada and numerous European countries have made fish consumption recommendations for their populations, particularly vulnerable groups, in order to México methylmercury intake. Blood and hair are the best biological samples for measuring methylmercury. The most widely-used method to analyse methylmercury is cold vapor atomic absorption spectrometry, although there are also direct methods based on the thermal decomposition of the sample. In recent years, the number of laboratories that measure mercury by inductively coupled plasma mass spectrometry has increased. In addition, the different kinds of mercury can be distinguished by coupling chromatography methods of separation. Laboratories that analyse mercury in biological samples need to participate in external quality control programmes. Even if mercury emissions are reduced, mercury may remain in the environment for many years, so dietary recommendations are fundamental in order to reduce exposure. It is necessary to propose public health measures aimed at decreasing mercury exposure and to evaluate the benefits of such measures from the economic and social standpoints.


Los efectos beneficiosos del consumo de pescado son bien reconocidos. Sin embargo, existe preocupación a nivel mundial sobre los niveles de metilmercurio en el pescado, por lo que muchos países como Estados Unidos, Australia, Nueza Zelanda, Canadá y muchos países europeos han realizado recomendaciones de consumo de pescado a la población , especialmente a los grupos vulnerables con el fin de reducir la ingesta de metilmercurio. La sangre y el pelo son las mejores muestras biológicas para medir el metilmercurio. El método de análisis más empleado ha sido la espectroscopia de absorción atómica con la técnica del vapor frío, aunque existen también métodos directos que se basan en la descomposición térmica de la muestra. En los últimos años han aumentado los laboratorios que miden el mercurio por espectrometría de masas con plasma acoplado por inducción. Además, se puede diferenciar las distintas especies de mercurio acoplando métodos de separación cromatográficos. Es necesario que los laboratorios que analizan mercurio en muestras biológicas participen en programas de garantía externa de la calidad. Aunque se logre reducir las emisiones de mercurio, el mercurio del medio ambiente todavía puede permanecer muchos años, por lo que es fundamental el consejo dietético para disminuir la exposición. No es aconsejable el uso de terapia quelante con fines diagnósticos o en pacientes asintomáticos. Es preciso proponer medidas de salud pública encaminadas a la disminución de la exposición al mercurio y que se evalúe también los beneficios de las mismas desde el punto de vista económico y social.

The beneficial effects of fish consumption in both children and adults are well known. However, the intake of methylmercury, mainly from contaminated fish and shellfish, can have adverse health effects. The study group on the prevention of exposure to methylmercury (GEPREM-Hg), made up of representatives from different Spanish scientific societies, has prepared a consensus document in a question and answer format, containing the group's main conclusions, recommendations and proposals. The objective of the document is to provide broader knowledge of factors associated with methylmercury exposure, its possible effects on health among the Spanish population, methods of analysis, interpretation of the results and economic costs, and to then set recommendations for fish and shellfish consumption. The group sees the merit of all initiatives aimed at reducing or prohibiting the use of mercury as well as the need to be aware of the results of contaminant analyses performed on fish and shellfish marketed in Spain. In addition, the group believes that biomonitoring systems should be set up in order to follow the evolution of methylmercury exposure in children and adults and perform studies designed to learn more about the possible health effects of concentrations found in the Spanish population, ta king into account the lifestyle, eating patterns and the Mediterranean diet.


Los efectos beneficiosos del consumo de pescado tanto en niños como en adultos han sido bien reconocidos. Sin embargo, la ingesta de metilmercurio principalmente a través del pescado y marisco contaminado puede producir efectos adversos en la salud. El grupo de estudio para la prevención de la exposición al metilmercurio (GEPREM-Hg), constituido por representantes de diferentes sociedades científicas españolas, ha elaborado un documento de consenso donde se recogen en forma de preguntas y respuestas las principales conclusiones, recomendaciones y propuestas planteadas en el grupo. El objetivo del documento es profundizar en el conocimiento de los factores asociados a la exposición al metilmercurio, los posibles efectos sobre la salud en la población española, los métodos de análisis, la interpretación de los resultados, el coste económico y establecer finalmente recomendaciones de consumo de pescados y mariscos. El grupo considera acertadas todas las iniciativas encaminadas a reducir o prohibir el uso del mercurio y la necesidad de conocer los resultados de los análisis de contaminantes que se realizan en los pescados y mariscos que se comercializan en España. Además, opina que se deberían establecer sistemas de biomonitorización para conocer la evolución de la exposición al metilmercurio en niños y adultos y realizar estudios diseñados para conocer los posibles efectos sobre la salud de las concentraciones halladas en la población española, teniendo en cuenta el estilo de vida, los patrones de consumo alimentarios y la dieta mediterránea.

Mercury is an environmental toxicant that causes numerous adverse effects on human health and natural ecosystems. The factors that determine the existance of adverse effects, as well as their severity are, among others: the chemical form of mercury (elemental, inorganic, organic), dosis, age, period of exposure, pathways of exposure and environmental, nutritional and genetic factors. In the aquatic cycle of mercury, once it has been deposited, it is transformed into methylmercury due to the action of certain sulphate-reducing bacteria, which bioaccumulates in the aquatic organisms and moves into the food chain. The methylmercury content of large, long-lived fish such as swordfish, shark, tuna or marlin, is higher. Methylmercury binds to protein in fish and is therefore not eliminated by cleaning or cooking the fish. Fetuses and small children are more vulnerable to the neurotoxic effects of methylmercury from the consumption of contaminated fish. Methylmercury is absorbed in the gastrointestinal tract and crosses the blood-brain barrier and the placenta. The intake of certain dietary components such as polyunsaturated fatty acids, selenium, fiber, thiol compounds, certain phytochemicals and other nutrients can modify methylmercury bioaccesibility and its toxicity. Apart from environmental factors, genetic factors can influence mercury toxicity and explain part of the individual vulnerability.


El mercurio es un tóxico ambiental que causa numerosos efectos adversos en la salud humana y en los ecosistemas naturales. Los factores que determinan la aparición de efectos adversos y su severidad son entre otros: la forma química del mercurio (elemental, inorgánico, orgánico), la dosis, la edad, la duración de la exposición, la vía de exposición y los factores ambientales, nutricionales y genéticos. En el ciclo acuático del mercurio, una vez que se ha depositado, se transforma en metilmercurio por la acción de determinadas bacterias sulfato reductoras y se bioacumula en los organismos acuáticos incorporándose a la cadena trófica de alimentos. El contenido de metilmercurio es mayor en las especies depredadoras de mayor tamaño y que viven más años como el emperador, pez espada, tiburón, atún o marlín. El metilmercurio se halla unido a las proteínas del pescado por lo que no se elimina mediante la limpieza ni el cocinado del mismo. El feto en desarrollo y los niños pequeños son los más vulnerables a los efectos neurotóxicos del metilmercurio procedente de la ingesta de pescado contaminado. El metilmercurio se absorbe en el tracto gastrointestinal y atraviesa la barrera hematoencefálica y la placenta. Algunos componentes de la dieta como los ácidos grasos poliinsaturados, el selenio, la fibra, los compuestos tiol, algunos fitoquímicos y otros nutrientes pueden modificar la bioaccesibilidad del mercurio y su toxicidad. Además de los factores ambientales, los factores genéticos pueden influir en la toxicidad del mercurio y explicar parte de la vulnerabilidad individual.

The benefit of fish consumption in children and adults is well-known. However, it has been pointed out that excessive methylmercury intake due to consumption of contaminated fish leads to neurological toxicity in children, affecting cognitive function, memory, visual-motor function and language. After the intoxications in Minamata and Iraq, wide-ranging epidemiological studies were carried out in New Zealand, the Faroe Islands and the Seychelles and international recommendations were established for fish consumption in pregnant women and small children. In Spain, the Childhood and Environmental project (INMA, its Spanish acronym) has studied the effects of diet and the environment on fetal and childhood development in different geographic areas of Spain. National and international sudies have demonstrated that mercury concentrations are mainly dependent on fish consumption, although there are variations among countries which can be explained not only by the levels of fish consumption, but also by the type or species of fish that is consumed, as well as other factors. Although the best documented adverse effects of methylmercury are the effects on nervous sytem development in fetuses and newborns, an increasing number of studies indicate that cognitive function, reproduction and, especially, cardiovascular risk in the adult population can also be affected. However, more studies are necessary in order to confirm this and establish the existance of a causal relationship.


Los efectos beneficiosos del consumo de pescado tanto en niños como en adultos han sido bien reconocidos. Sin embargo, se ha referido que la ingesta excesiva de metilmercurio procedente del pescado contaminado produce toxicidad neurológica en los niños afectando a la función cognitiva, la memoria, la función visual-motora y al lenguaje. Después de las intoxicaciones de Minamata e Iraq, se realizaron grandes estudios epidemiológicos en Nueva Zelanda, las islas Féroe y las islas Seychelles y se establecieron recomendaciones internacionales sobre el consumo de pescado y marisco en las mujeres embarazadas y niños pequeños. En España, el proyecto Infancia y Medio Ambiente (INMA) ha estudiado los efectos del medio ambiente y de la dieta sobre el desarrollo fetal e infantil en diversas zonas geográficas de España. Los estudios realizados nacionales e internacionales muestran que la concentración de mercurio depende principalmente del consumo de pescado, aunque existe una variabilidad entre los países que podría explicarse no solo por la cantidad de pescado consumida, sino también por el tipo o especies de pescados que se consumen, así como por otro tipo de factores. Aunque los efectos perjudiciales del metilmercurio mejor documentados son los que se producen sobre el desarrollo del sistema nervioso en el feto y en el recién nacido, cada vez hay más estudios que indican que también puede afectar a la función cognitiva, reproducción y especialmente al riesgo cardiovascular en la población adulta. Sin embargo, son necesarios más estudios para confirmarlo y establecer la existencia de una relación causal.

Sugar content in foods cannot be distinguished from added sugar, although it is true that this added sugar brings mainly energy and no other essential nutrients. On the other hand, in the context of diet, sugar helps make it more varied and palatable allowing including foods that may otherwise not be would consume, thus indirectly contributing to the intake of other nutrients. Having interest in knowing the possible relationship between a high intake of sugars and the decrease in micronutrients intake, we noted that the nutrient density of the diet might be influenced by factors such as the high presence of sugar added to food. It seems that this nutritional dilution produced by adding sugar to food is, in general, not very significant and, often, offset by the fortification in micronutrients that we usually can find in many sugary products. After a detailed analysis of the published studies on the subject, it has been found that there is no a clear evidence of the hypothetical micronutrient dilution that would occur by adding sugar to the diet. On the other hand, given that the addition of sugar to the diet doesn't seem to report any remarkable advantages from the point of view of the intake of micronutrients; It seems reasonable to promote a moderate consumption of foods and sugary drinks, so in that way, they become an important extra energy source. It should also be borne in mind that the addition of sugar to the diet does not seem remarkable report any advantage in terms of intake of micronutrients. For this reason, it seems logical that the consumption of sugary food and sweet drinks will be moderate given the ease of consumption and the likelihood of their becoming a major source of energy. Finally, it is concluded on the need for further research on the mechanisms underlying that, up to now showed no, possible displacement of micro-nutrients and other food components of the diet that could occur in cases of consumption of foods with a high sugar content, as well as their impact from a clinical point of view.


El azúcar contenido de forma natural en los alimentos no se puede distinguir del aªadido, aunque lo cierto es que este azúcar de adición aporta fundamentalmente energía y no otros nutrientes esenciales. Por otro lado, en el contexto de la dieta, el azúcar contribuye a hacerla más variada y apetecible permitiendo incluir alimentos que quizá de otra manera no se consumirían, contribuyendo pues indirectamente a la ingesta de otros nutrientes. Habiendo interés en conocer la posible relación entre una elevada ingesta de azúcares y la disminución de la de micronutrientes, se ha observado que la densidad nutritiva de la dieta podría estar influida por factores como la presencia elevada de azúcar aªadido en los alimentos. Parece ser que esta dilución nutricional producida al aªadir azúcar a los alimentos es, en general, poco significativa y, a menudo, contrarrestada por la fortificación en micronutrientes que frecuentemente encontramos en numerosos productos azucarados. Tras el análisis en detalle de los estudios publicados al respecto, se ha comprobado que no existe una evidencia clara de la hipotética dilución de micronutrientes que se produciría al aªadir azúcares a la dieta. Por otra parte, dado que la adición de azúcar a la dieta no parece reportar ninguna ventaja destacable desde el punto de vista de la ingesta de micronutrientes; parece razonable que se promueva un consumo moderado de alimentos y bebidas azucaradas, para de ese modo, evitar que se conviertan en una importante fuente energética extra. Finalmente, se concluye sobre la necesidad de profundizar en la investigación acerca de los mecanismos subyacentes a ese, hasta ahora no demostrado, posible desplazamiento de micronutrientes y otros alimentos componentes de la dieta que podría ocurrir en casos de consumo de alimentos con un elevado contenido en azúcares, así como su repercusión desde un punto de vista clínico.

BACKGROUND AND OBJECTIVE: To explore the sensitivity and specificity of waist to height index as indicator of overweight and obesity in pediatric age and to obtain cut-off points to simplify the diagnosis. SUBJECTS AND METHODS: Two thousand and three hundred and nineteen schoolchildren between 6 and 14 years were analyzed. Anthropometric measures were taken (height, weight, waist circumference and skinfolds thickness), and waist to height ratio (WHR), BMI and fat percentage were calculated. ANOVA test was used to evaluate the performance of anthropometric variables during the growth period. ROC curve analysis (Receiver Operating Characteristics) was applied using WHR as test variable and overweight and obesity status as criterion variable. Overweight and obesity were defined by BMI standards and adiposity references. The sensitivity and specificity values, areas under the curve (AUC), confidence intervals 95%, and cut-offs points were obtained. The statistical and graphical procedure was performed using SPSS(®) 18.0. RESULTS: WHR does not vary with age. AUC ranged from 0.786 to 0.953 indicating that the WHR has a high predictive power to identify the subjects previously classified as overweight or obese using both considered criteria. CONCLUSION: WHR proved to be an appropriate and effective predictor of overweight and obesity in children between 6 and 14 years. Cut-offs points of WHR that identify obesity are: 0.51 in males and 0.50 in girls. For the overweight, cut-offs range between 0.47 and 0.48 depending on sex and the criterion variable.

INTRODUCTION: Epidemiological studies frequently use weight and height collected by questionnaires, but the inaccuracy of self-reported data may bias the evaluation result. The aim of this study is to validate the self-report in Spanish adult population emphasizing the effect of age and nutritional status of the subjects. METHODS: The sample consist of 9,294 adults (8,072 women and 1,222 men) recruited from dietetic counselling centers dependents of Arkopharma laboratories in 46 Spanish provinces. Weight and height were asked and subsequently measured, calculating differences between self-reported and real parameters. Error in the estimation of body mass index (BMI) was evaluated considering the effect of age and nutritional status (T-test and multiple linear regression). Correlation between the classification based on self-reported BMI and anthropometric was analyzed using the Kappa test. RESULTS AND DISCUSSION: Using the self-reported data, BMI was underestimated (2.62% in men, 3.10% in women). The error increases with age and extreme nutritional categories. The agreement between self-reported and real BMI in the nutritional assessment was good and correctly classified 74.71% of the males and 89.5% of women (Kappa: 0.695 and 0.782 respectively). CONCLUSIONS: Considering the effect of age and nutritional status on the self-awareness of body size, we recommend caution in the use of questionnaires for epidemiological assessment.


Introducción: En epidemiología es frecuente recabar el peso y talla mediante cuestionario, pero la inexactitud de los datos auto-referidos puede sesgar el resultado de la evaluación. El objetivo es validar el auto-reporte en población adulta española enfatizando el efecto de la edad y la condición nutricional de los sujetos. Material y métodos: La muestra consta de 9.294 adultos (8.072 mujeres y 1.222 varones) reclutados en centros de orientación dietética dependientes de los laboratorios Arkopharma en 46 provincias españolas. Se preguntaron peso y talla midiéndose posteriormente y calculando las diferencias entre parámetros auto-referidos y reales. Se evaluó el error en la estima del Índice de Masa Corporal (IMC) considerando el efecto de la edad y la condición nutricional (T de Student y regresión lineal múltiple). Se analizó la concordancia entre la clasificación realizada a partir del IMC auto-referido y antropométrico mediante el test de Kappa. Resultados y discusión: Utilizando datos auto-referidos, el IMC se infravalora (2,62% en varones; 3,10% en mujeres). El error aumenta con la edad y en las categorías nutricionales extremas. El acuerdo en la evaluación nutricional a partir del IMC auto-referido y real es bueno, clasificándose de manera correcta el 74,71% de los varones y el 89,5 % de las mujeres (Kappa: 0,695 y 0,782 respectivamente). Conclusiones: Teniendo en cuenta el efecto de la edad y de la condición nutricional sobre el auto-conocimiento del tamaño corporal, se recomienda cautela en el empleo de cuestionarios encaminados a la valoración epidemiológica.
