BACKGROUND: Delirium is one of the most common adverse events in older people during hospitalization, especially in the emergency department. Reliable, easy-to-use instruments are necessary to properly manage delirium in this setting. This study aims to evaluate the diagnostic validity of the Spanish version of the 4 'A's Test (4AT) in the ED. METHODS: A diagnostic accuracy study was conducted in patients over 65 years old admitted to the Emergency Department who did not have a formal diagnosis of dementia or a severe mental health disorder. Face and content validity were evaluated by an expert panel. Emergency nurses performed the evaluation with 4AT, whilst blinded and trained researchers assessed patients with the Revised Delirium Rating Scale as the gold standard. The content validity index, sensitivity, specificity, positive and negative predictive values, likelihood ratios, Youden's Index and ROC curves were calculated to evaluate the diagnostic accuracy of the instrument. RESULTS: Of 393 eligible patients, 380 were finally analyzed. Content validity yielded a median content validity index of 4 (interquartile range: 0). The Spanish 4AT sensitivity (95.83%; 95% ECI: 78.9-99.9%), specificity (92.98%; 95% CI: 89.8-95.4%), positive predictive value (47.92%) and negative predictive value (99.7%) were satisfactory. Youden's index was 0.89. Positive likelihood ratio was 13.65, and negative likelihood ratio 0.045. The area under the curve was 0.97. CONCLUSIONS: The Spanish version of the 4AT for use in the Emergency Departments is easy-to-use and applicable. The validation results indicate that it is a valid instrument with sufficient predictive validity to identify patients at risk of delirium in the Emergency Departments. Moreover, it is a tool that facilitates the management of an adverse event that is associated with increased mortality and morbidity.

(1) Background: There is currently a global consensus that the quality of comprehensive care for acutely hospitalised elderly people should include addressing functionality and mobility, cognitive status, prevention of pressure ulcers, urinary incontinence, falls and delirium, as well as pain control and medication-related problems. The aim of this study is to develop and validate a clinical prediction rule for multimorbid patients admitted to an acute care hospital unit for any of the five adverse events included in our vulnerability pentad: falls, pressure ulcers, urinary incontinence, pain and delirium. (2) Methods: Longitudinal analytical clinimetric study, with two cohorts. The study population will consist of multimorbid patients hospitalised for acute care, referred from the Emergency Room. A clinical prediction rule will be proposed, incorporating predictive factors of these five adverse outcomes described. This study has received funding, awarded in November 2020 (PI-0107-2020), and was approved in October 2019 by the Research Ethics Committee ″Costa del Sol″. (3) Conclusions: Preventing adverse events in hospitalised patients is particularly important for those with multimorbidity. By applying a clinical prediction rule to detect specific risks, an estimate can be obtained of their probability of occurrence.

BACKGROUND AND PURPOSE: The kynurenine pathway has been proposed as a target for modulating drug abuse. We previously demonstrated that inhibition of kynurenine 3-monooxygenase (KMO), using Ro 61-8048, reduces ethanol consumption in a binge drinking model. Here, we investigate the effect of the kynurenine pathway modulation in ethanol-dependent mice. EXPERIMENTAL APPROACH: Adult male and female mice were subjected to a Chronic Intermittent Ethanol (CIE) paradigm. On the last day of CIE, mice were treated with Ro 61-8048, Ro 61-8048 + PNU-120596, a positive allosteric modulator of α7nAChR, and Ro 61-8048 + L-leucine or probenecid, which blocks the influx or efflux of kynurenine from the brain, respectively. Ethanol, water consumption and preference were measured and kynurenine levels in plasma and limbic forebrain were determined. KEY RESULTS: Ro 61-8048 decreases consumption and preference for ethanol in both sexes exposed to the CIE model, an effect that was prevented by PNU-120596. The Ro 61-8048-induced decrease in ethanol consumption depends on the influx of kynurenine into the brain. CONCLUSION AND IMPLICATIONS: Inhibition of KMO reduces ethanol consumption and preference in both male and female mice subjected to CIE model by a mechanism involving α7nAChR. Moreover, this centrally-mediated effect depends on the influx of peripheral kynurenine to the brain and can be prolonged by blocking the efflux of kynurenine from the brain. Here, for the first time, we demonstrate that the modulation of the kynurenine pathway is an effective strategy for the treatment of ethanol dependence in both sexes.

3,4-Methylenedioxymethamphetamine (MDMA) is an amphetamine derivative that has been shown to produce serotonergic damage in the brains of primates, including humans, and of rats. Tryptophan, the precursor of serotonin, is primarily degraded through the kynurenine (KYN) pathway, producing among others KYN, the main metabolite of this route. KYN has been reported as an endogenous agonist of the aryl hydrocarbon receptor (AhR), a transcription factor involved in several neurological functions. This study aims to determine the effect of MDMA on the KYN pathway and on AhR activity and to establish their role in the long-term serotonergic neurotoxicity induced by the drug in rats. Our results show that MDMA induces the activation of the KYN pathway, mediated by hepatic tryptophan 2,3-dioxygenase (TDO). MDMA also activated AhR as evidenced by increased AhR nuclear translocation and CYP1B1 mRNA expression. Autoradiographic quantification of serotonin transporters showed that both the TDO inhibitor 680C91 and the AhR antagonist CH-223191 potentiated the neurotoxicity induced by MDMA, while administration of exogenous l-kynurenine or of the AhR positive modulator 3,3'-diindolylmethane (DIM) partially prevented the serotonergic damage induced by the drug. The results demonstrate for the first time that MDMA increases KYN levels and AhR activity, and these changes appear to play a role in limiting the neurotoxicity induced by the drug. This work provides a better understanding of the physiological mechanisms that attenuate the brain damage induced by MDMA and identify modulation of the KYN pathway and of AhR as potential therapeutic strategies to limit the negative effects of MDMA.

RESUMO

Drug use poses a serious threat to health systems throughout the world and the number of consumers rises relentlessly every year. The kynurenine pathway, main pathway of tryptophan degradation, has drawn interest in this field due to its relationship with addictive behaviour. Recently it has been confirmed that modulation of kynurenine metabolism at certain stages of the pathway can reduce, prevent or abolish drug seeking-like behaviours in studies with several different drugs. In this review, we present an up-to-date summary of the evidences of a relationship between drug use and the kynurenine pathway, both the alterations of the pathway due to drug use as well as modulation of the pathway as a potential approach to treat drug addiction. The review discusses ethanol, nicotine, cannabis, amphetamines, cocaine and opioids and new prospects in the drug research field are proposed.

BACKGROUND: informal caregivers have a high risk of suffering from diseases derived from the chronic stress to which they are subjected for their dedication to the care of their relatives. Such stress has a direct influence on the person cared for, mainly affecting the quality of their care. Therefore, the aim of the present study is to assess the association of caregiving on physical and mental perceived health in family caregivers of dependent adults with complex chronic diseases. METHODS: a prospective longitudinal cohort study, with a follow-up period of 36 months (HUELLA cohort). The exposed cohort will be formed by family caregivers of dependent patients with complex chronic pathologies. The unexposed cohort will be taken from the general population adjusted for age, sex and health. Outcome variables will include attendance to health services, consumption of psychoactive drugs, dedication to care (only in exposed cohort), concession of the Act on Promotion of Personal Autonomy and Care for dependent persons (exposed only), perceived physical and mental health, depression level, burden level and new diagnosis of chronic pathology of the caregiver during the study. RESULTS: the expected results will be applicable and will incorporate improvements to the usual health system clinical practice, providing feedback to professionals dedicated to the provision, planning and design of services to family caregivers, as well as to groups and organizations of caregivers. CONCLUSIONS: investments in preventing low-quality informal care are key, mainly through early identification and interventions to support caregivers who suffer from stress, anxiety or depression.

Aqueous dispersions of magnetic nanocomposites have been proposed as draw electrolytes in forward osmosis. One possible approach for the production of nanocomposites based on magnetite nanoparticles and sodium polyacrylate is the synthesis of the magnetic iron oxide by coprecipitation or oxidative precipitation in the presence of an excess of the polymer. In this work, we explored the effect of the polymer proportion on the nanomaterials produced by these procedures. The materials obtained were compared with those obtained by the coating of magnetite nanocrystals produced beforehand with the same polymer. The samples were characterized by chemical analysis, photon correlation spectroscopy, thermogravimetry, X-ray diffraction, infrared spectroscopy, transmission electron microscopy, and magnetometry. The general trend observed is that the polymers heavily modify the texture of the magnetic material during the synthesis, with a drastic reduction of the particle size and magnetic response. The polycrystalline texture that is generated permits the incorporation of the polymer both on the external surface and in the intergranular space. The aqueous dispersions of the nanocomposites were highly stable, with a hydrodynamic size that was roughly independent of the polymer/magnetite ratio. Such dispersions show an osmotic pressure that is proportional to the concentration of the polymer. Interestingly, the proportionality constant was similar to that of the free polymer only in the case of the samples prepared by oxidative precipitation, being lower in the case of the samples prepared by coprecipitation. Finally, the possibilities of using these materials as draw electrolytes in forward osmosis will be briefly discussed.

Introducción: La enfermedad periodontal es una infección inmunoinflamatoria crónica de origen multifactorial. Puede avanzar a nivel sistémico por el paso de bacterias y sus productos al torrente sanguíneo, lo cual constituye un factor de riesgo para alteraciones sistémicas. La revisión bibliográfica se realizó de julio 2022 hasta febrero 2023. Se utilizaron las bases de datos PubMed, SciELO y Elsevier y el motor de búsqueda Google Académico. Objetivos: Describir la relación de la enfermedad periodontal inflamatoria crónica con enfermedades sistémicas. Desarrollo: La medicina periodontal estudia la relación que existe entre las periodontopatías y enfermedades sistémicas, como las cardiovasculares, cerebrovasculares, pulmonares, la renal crónica, la artritis reumatoide y el Alzheimer. Las bacterias provenientes de las bolsas periodontales pasan hacia la circulación sanguínea, producen infección metastásica y daño metastásico, mediante la producción de endotoxinas, lipopolisacáridos e inflamación metastásica. Conclusiones: La enfermedad periodontal crónica constituye un factor de riesgo para el desarrollo de enfermedades cardiovasculares, pulmonares, renales, trastornos cerebrovasculares, artritis reumatoide y el Alzheimer debido a reacciones inflamatorias producidas por microorganismos patogénicos; se establece una relación bidireccional entre estas enfermedades y las periodontopatías(AU)


Introduction: Periodontal disease is a chronic immunoinflammatory infection of multifactorial origin. It can advance at a systemic level due to the passage of bacteria and their products into the bloodstream, which constitutes a risk factor for systemic alterations. The bibliographic review was carried out from July 2022 to February 2023. The PubMed, SciELO and Elsevier databases and the Google Scholar search engine were used. Objectives: Describe the relationship of chronic inflammatory periodontal disease with systemic diseases. Development: Periodontal medicine studies the relationship between periodontopathies and systemic diseases, such as cardiovascular, cerebrovascular, pulmonary, chronic kidney disease, rheumatoid arthritis and Alzheimer's. Bacteria from periodontal pockets pass into the blood circulation, producing metastatic infection and metastatic damage, through the production of endotoxins, lipopolysaccharides and metastatic inflammation. Conclusions: Chronic periodontal disease constitutes a risk factor for the development of cardiovascular, pulmonary, renal diseases, cerebrovascular disorders, rheumatoid arthritis and Alzheimer's due to inflammatory reactions produced by pathogenic microorganisms; A bidirectional relationship is established between these diseases and periodontopathies. The analysis of this relationship and the mechanisms by which it occurs guarantees the development of a more integrative care practice(AU)

In the frame of this work, the aim was to create a superparamagnetic nanocomposite system with a maximized magnetic moment when magnetized by an external field and a blocking temperature far below room temperature. For this purpose, iron oxide nanoparticles of 3.8-, 5- and 8-nm size have been infiltrated into the pores of porous silicon. To fabricate tailored magnetic properties of the system, the particle size and the magnetic interactions among the particles play a crucial role. Different concentrations of the particles dispersed in hexane have been used for the infiltration to vary the blocking temperature TB, which indicates the transition between the superparamagnetic behavior and blocked state. TB is not only dependent on the particle size but also on the magnetic interactions between them, which can be varied by the particle-particle distance. Thus, a modification of the pore loading on the one hand and of the porous silicon morphology on the other hand results in a composite material with a desired blocking temperature. Because both materials, the mesoporous silicon matrices as well as the Fe3O4 nanoparticles, offer low toxicity, the system is a promising candidate for biomedical applications.
