RESUMO

Growing interest in quantum computing for practical applications has led to a surge in the availability of programmable machines for executing quantum algorithms1,2. Present-day photonic quantum computers3-7 have been limited either to non-deterministic operation, low photon numbers and rates, or fixed random gate sequences. Here we introduce a full-stack hardware-software system for executing many-photon quantum circuit operations using integrated nanophotonics: a programmable chip, operating at room temperature and interfaced with a fully automated control system. The system enables remote users to execute quantum algorithms that require up to eight modes of strongly squeezed vacuum initialized as two-mode squeezed states in single temporal modes, a fully general and programmable four-mode interferometer, and photon number-resolving readout on all outputs. Detection of multi-photon events with photon numbers and rates exceeding any previous programmable quantum optical demonstration is made possible by strong squeezing and high sampling rates. We verify the non-classicality of the device output, and use the platform to carry out proof-of-principle demonstrations of three quantum algorithms: Gaussian boson sampling, molecular vibronic spectra and graph similarity8. These demonstrations validate the platform as a launchpad for scaling photonic technologies for quantum information processing.

The objective of this prospective cohort study was to evaluate the association between the prepartum days in the close-up group (DINCU) and milk yield, milk components, reproductive performance, and culling risk in the subsequent lactation for Holstein dairy cows. Dry cow feeding management of 20 farms was evaluated during 2 farm visits. All farms were feeding an acidogenic diet in the close-up group. Data from 14,843 cows were collected for 365 d following the second farm visit. Data sets of 13,314 cows were available for final statistical analysis after exclusion of cows with missing information about gestation length, cows with a gestation length shorter than 262 d or longer than 292, cows with 0 DINCU, and cows with >42 DINCU. At enrollment, 3,871 and 9,443 of those animals were nulliparous and parous cows, respectively. Continuous data such as energy corrected milk (ECM), the ratio of fat and protein, and somatic cell score (SCS) at first test day were analyzed using linear mixed models. Binary data such as stillbirth, culling within 60 DIM, and pregnancy within 150 DIM were analyzed using logistic regression models. Based on their different physiology, separate models were built for nulliparous and parous cows. All results displayed are the predicted least squares means from the multivariable analyses. A significant association between DINCU and milk yield at first test day was observed for nulliparous and parous cows. Nulliparous cows with 7, 21, or 35 DINCU had a first test day ECM of 31.8, 33.3, and 35.5 kg, respectively. Parous cows with 7, 21, or 35 DINCU had a first test day ECM of 42.8, 45.6, and 44.6 kg of ECM, respectively. In nulliparous cows, there was a tendency for an association between DINCU and the ratio of fat and protein at first test day. In parous cows, however, a significant association was observed. Parous cows with 7, 21, or 35 DINCU had a ratio of fat and protein of 1.31, 1.35, and 1.37, respectively. There was a significant association between DINCU and SCS at first test day in nulliparous and parous cows. In nulliparous cows with 7, 21, or 35 DINCU, SCS was 2.39, 2.49, and 2.85, respectively. In parous cows with 7, 21, or 35 DINCU, SCS was 2.46, 2.53, and 2.78, respectively. No associations were observed between DINCU and occurrence of stillbirth and DINCU and the risk of pregnancy within 150 DIM. The multivariable model predicted a tendency for an association between DINCU and the risk of being culled within 60 DIM in parous cows. Particularly, 0 to 6 DINCU were associated with a substantially increased risk of being culled. In conclusion, a short stay in the close-up group should be avoided to improve milk yield at first test day and to minimize culling risk for parous cows. A long stay in the close-up group (>30 d) was associated with reduced milk production and an increased ratio of fat and protein in milk of parous cows and increased SCS of nulliparous and parous cows.

UNLABELLED: : Thoracic hyperkyphosis is a frequent problem and can impact greatly on patient's quality of life during adolescence. This condition can be idiopathic or secondary to Scheuermann disease, a disease disturbing vertebral growth. To date, there is no sound scientific data available on the management of this condition. Some studies discuss the effects of bracing, however no guidelines, protocols or indication's of treatment for this condition were found. The aim of this paper was to develop and verify the consensus on managing thoracic hyperkyphosis patients treated with braces and/or physiotherapy. METHODS: The Delphi process was utilised in four steps gradually modified according to the results of a set of recommendations: we involved the SOSORT Board twice, then all SOSORT members twice, with a Pre-Meeting Questionnaire (PMQ), and during a Consensus Session at the SOSORT Lyon Meeting with a Meeting Questionnaire (MQ). RESULTS: There was an unanimous agreement on the general efficacy of bracing and physiotherapy for this condition. Most experts suggested the use of 4-5 point bracing systems, however there was some controversy with regards to physiotherapeutic aims and modalities. CONCLUSION: The SOSORT panel of experts suggest the use of rigid braces and physiotherapy to correct thoracic hyperkyphosis during adolescence. The evaluation of specific braces and physiotherapy techniques has been recommended.

INTRODUCTION: In late 1997, the German Cardiac Society set up a multicenter registry to evaluate the acute and mid-term course of all patients (pts.) treated with septal ablation for symptomatic hypertrophic obstructive cardiomyopathy (HOCM). An analysis of the acute results has already been published. We now report on the mid-term course (3-6 months) of 242 pts. registered through September 1999. RESULTS: Follow-up was 92% complete (n=222). During follow-up (mean: 4.9+/-2.3 months), an additional 3 pts. died (in-hospital mortality: 3 pts.). A satisfactory clinical effect was reported by 195 pts. (88%); 27 pts. (12%) remained in NYHA classes III and IV. Overall symptomatic improvement (NYHA class: from 2.8+/-0.7 to 1.7+/-0.7) paralleled the outflow gradient (LVOTG) reduction which was further accentuated as compared with the acute result (Doppler measurement at rest: from 57+/-31 to 25+/-25 mmHg to 20+/-21 mmHg; with provocation: from 107+/-53 to 49+/-40, to 44+/-40 mmHg, p<0.001, resp.). Left atrial (LA) diameter (from 46+/-8 to 44+/-7 mm) and septal thickness (from 20+/-5 to 15+/-5 mm; p<0.001, resp.) were also reduced. Comparing the methods for target vessel selection (i.e., with contrast echo monitoring vs pressurefluoroscopy guidance), at followup clinical improvement and hemodynamic measurements were comparable. CONCLUSION: Clinical success can be achieved by septal ablation, both with the echocontrast guided and gradient-fluoroscopy guided method, in 88% of highly symptomatic HOCM pts. At mid-term follow-up, symptoms, left atrial size and septal thickness are reduced, and outflow gradients are further improved as compared to the acute result.

In 20 open-chest dogs intramyocardial pressure was measured simultaneously in two regions: a) in the normal myocardium (IMPsp) b) in a controlled perfused area of the myocardium, supplied by a cannulated coronary artery (IMPp). The preparation permitted the study of the influence of contractility, ventricular pressure and coronary blood flow on IMP. 1. A 47% change in coronary flow with a 26% change in coronary perfusion pressure results in changes of systolic and diastolic IMPp of 14% and 29% respectively. These effects can be explained by variations of intramural fluid content and, hence, of diastolic fiber length. 2. Changes of systolic ventricular pressure (mean 27%) by preload variations result in a mean change of systolic IMPsp of 19% and of systolic IMPp of 11%. 3. Changes of systolic ventricular pressure (mean 33%) by afterload variations augment IMPsp by 30% and IMPp by 13%. 4. The increases of IMPsp and IMPp induced by instantaneous changes of afterload are equal. The most important determinants of IMP are myocardial contractility, coronary perfusion and intraventricular pressure. The influence of coronary perfusion and ventricular pressure on IMP is of the same order of magnitude.

RESUMO

A 56-year-old male patient became symptomatic with angina pectoris. Selective coronary angiography revealed an aneurysm of the stem of the circumflex artery and a subtotal stenosis of the left anterior descending coronary artery (LAD). At cardiac surgery the anterior wall of the aneurysm was resected and the resulting defect closed with a venous patch. Bypass grafts to the LAD and the first marginal branch were also implanted. Three months later both grafts were shown to be patent, and flow through the reconstructed stem of the circumflex artery was undisturbed.

BACKGROUND: Registry results of the new catheter-based method in the treatment for HOCM are missing so far. In 1997, the Transcoronary Ablation of Septal Hypertrophy Registry (TASH Registry) was established by the German Cardiac Society (GCS) as a multicenter, national registry of patients with HOCM undergoing the new catheter interventional therapy. This is the report of the in-hospital outcome of patients who underwent the procedure during the first two years of data collection in the registry. METHODS AND RESULTS: Information was based on three standard forms for each patient, with a total of 86 variables. Information was collected on an "intention to treat" basis. The TASH Registry includes the establishment of a data base in the data collecting center. Ten centers participated. Enrollment forms were received for 264 patients out of 279 patients registered up to January 2000. There was a history of medical treatment of 3.6+/-3.9 years. The vast majority of patients (91%) were treated in three centers. The Vasalva maneuver and the exercise Doppler echocardiography were used for noninvasive stress testing. Exercise Doppler echocardiography induced a significantly higher augmentation of the baseline gradient (70.1% vs 133.4%; p<0.01). The echo-contrast guided technique was used for the intervention in 50.8% and the pressure angiography guided technique in 49.2%. On the average 2.8+/-1.3 ml of alcohol were injected. Before the procedure, the gradient measured by catheterization was 60.4+/-38.6 mmHg at baseline and 142.7+/-46.2 mmHg following the extrasystolic beat. At the end of the session it was reduced significantly by 75% and 67%. The peak phosphocreatine kinase activity was 482.5+/-246.4 U/L. Major complications occurred in 15.6% including a mortality rate of 1.2% and a permanent pacemaker implantation rate because of total heart block in 9.6%. There was an early in-hospital improvement of dyspnoe corresponding to a significant decrease of NYHA functional class from 2.8+/-0.7 to 1.8+/-0.6 (p<0.001). Similar hemodynamic and clinical benefit was found in patients with and without resting gradient at baseline. CONCLUSION: This analysis for the first time gives a comprehensive overview of clinical characteristics, technique, procedural data, in-hospital outcome and complications in a large number of patients with HOCM who were treated by the new catheter-based method and prospectively enrolled in a registry. The results contribute considerably to critical evaluation and validation of the new technique. This analysis supports the catheter-based method to constitute a new therapeutic option for very symptomatic patients, to be effective both in patients with and without intraventricular pressure gradient at rest and to be an alternative to surgical treatment, as has been stated recently.
