RESUMO

Advances in industrial and technological innovations have led to significant socio-economic benefits, but with overwhelming negative impacts on the environment. These impacts include the infiltration of organic contaminants into soil, water, and air, posing a threat to the environment and public health. Polybrominated diphenyl ethers (PBDEs), heavy metals, and polycyclic aromatic hydrocarbons (PAHs) are increasingly released as waste, endangering the environment. In countries like Ghana, where regulations are weakly enforced, industrial waste is released uncontrollably, posing threats to public health, environmental integrity, and food systems. This study systematically evaluated existing literature on PBDEs, heavy metals, PAHs, and organic contaminant exposure in Ghana and proposes a roadmap for achieving food safety and protecting the environment and human health. The research identified high mobility of specific heavy metals and risks associated with PBDEs and PAHs in sediments, dumpsites, and various food items. Unregulated dumping of electronic waste with PBDEs raised environmental concerns. An integrated approach is needed to address the multifaceted impact of organic pollutants on public health and ecosystems. Urgent implementation of effective environmental management strategies and regulatory measures is crucial. The study proposed short- to mid-term priorities emphasising the need to foster collaboration and implementing global measures. The mid- to long-term strategy includes a national information surveillance system, local monitoring capacity development, and integrating land contamination controls with food safety legislation. These measures would mitigate risks, ensure sustainable practices, and improve overall food safety management in Ghana, serving as a model for regions facing similar challenges with diverse pollutants.

Arsenic is a well-known carcinogen with emerging reports showing a range of health outcomes even for low to moderate levels of exposure. This study deals with arsenic exposure and associated increased lifetime cancer risk for populations in arsenic-endemic regions of rural Bengal, where arsenic-safe drinking water is being supplied at present. We found a median total exposure of inorganic arsenic to be 2. 9 µg/Kg BW/day (5th and 95th percentiles were 1.1 µg/Kg BW/day and 7.9 µg/Kg BW/day); with major contribution from cooked rice intake (2.4 µg/Kg BW/day). A significant number of households drank arsenic safe water but used arsenic-rich water for rice cooking. As a result, 67% participants had inorganic arsenic intake above the JEFCA threshold value of 3 µg/Kg BW/day for cancer risk from only rice consumption when arsenic contaminated water was used for cooking (median: 3.5 µg/Kg BW/day) compared to 29% participants that relied on arsenic-free cooking water (median: 1.0 µg/kg BW/day). Arsenic in urine samples of study participants ranged from 31.7 to 520 µg/L and was significantly associated with the arsenic intake (r = 0.76); confirming the preponderance of arsenic exposure from cooked rice. The median arsenic attributable cancer risks from drinking water and cooked rice were estimated to be 2.4 × 10-5 and 2.7 × 10-4 respectively, which further emphasized the importance of arsenic exposure from staple diet. Our results show that any mitigation strategy should include both drinking water and local staple foods in order to minimize the potential health risks of arsenic exposure.

BACKGROUND: The ger ("tent city") areas in Mongolia are a product of rapid urbanization and transitional economic development combine with lack of institutional, administrative and financial capacity of governments to cope with the pace. These areas have become traps for inequities in social and environmental services and the associated effects on human health. Disparities in childhood lead exposure in such communities are largely unexplored. METHODS: We measured the concentrations of lead in blood of children, aged 4-7 years, in Erdenet (Orkhon Province) and Darkhan (Darkhan-Uul Province), the second and third largest cities in Mongolia. A survey instrument was used to gather information on influencing factors on lead exposure and the Strengths and Difficulties Questionnaire (SDQ) was used to assess a spectrum of behavioral problems among the children. RESULTS: The mean blood lead level (BLL) of children in the two cities was found to be 3.8 ± 2.6 µg/dL (range: < 1.5-17.2 µg/dL) and 27.8% of the children had BLLs ≥5 µg/dL. Average BLL of children in Erdenet (a mining center) was significantly higher than that for children in Darkhan, and there was statistically significant difference between average BLL of children who live in ger district (4.2 ± 2.8 µg/dL) compared to those of children in housing units within the city (3.2 ± 2.4 µg/dL). In spite of the low values, BLLs was significantly associated with a number of effects on the spectrum of behavioral disorders, specifically with the scores for hyperactivity, conduct disorder and pro-social behavior. CONCLUSIONS: This study shows that childhood lead poisoning is common especially in ger communities of the urban areas of Mongolia. It contributes evidence showing that BLL low as 3.8 µg/dL can selectively activate some effects from a spectrum of likely behavioral disorders in children.

BACKGROUND: Poor malaria knowledge can negatively impact malaria control programmes. This study evaluates knowledge distribution in the domains of causation, transmission, vulnerability, symptoms, and treatment of malaria. It assesses the association between a caregiver's knowledge about malaria and ownership and use of insecticide-treated nets (ITNs) by children. METHODS: Some 1939 caregivers of young children were recruited through a school-based survey in two Nigerian states. A 20-item, multi-dimensional survey instrument was developed and used to rank each caregiver's knowledge in five dimensions (cause, transmission, vulnerability, symptoms, treatment of malaria). Scores for each domain were used to create an aggregate knowledge score for each caregiver. The outcome measures were ITN ownership, and ITN use the night and week before the study. Regression models were used to evaluate the relationship between caregiver's knowledge (individual domains and aggregate score) and ownership and use of ITN after controlling for likely confounders. RESULTS: The main predictor of ITN use was ITN ownership (r = 0.653; p < 0.001); however, ownership only explains 43 % of variance in net use. Total knowledge index for the study population was significantly associated with both ITN ownership (r = 0.122; p = 0.001) and use (r = 0.095; p = 0.014). The spectrum of caregiver's knowledge of malaria and its causes captured in the various domains was, however, found to be poor. Fifty percent of the respondents knew that malaria is transmitted by female mosquitoes and 65 % still believe that too much exposure to the sun is a risk factor for malaria. Knowledge of populations most vulnerable to malaria (83 %) and knowledge of malaria transmission (32 %) were the domains with the highest and lowest average correct answers. CONCLUSIONS: There is a need to improve ITN coverage in Nigeria as ITN ownership was associated with ITN use. Additionally, treating knowledge as a multi-dimensional phenomenon revealed that a lot of misperceptions about malaria still exist. Distribution of ITNs through the public/private sector may need to be augmented with tailored behavioural change communication to dispel myths and improve the multi-dimensional knowledge of malaria in the local population.

This review contends that "healthy" water in terms of electrolyte balance is as important as "pure" water in promoting public health. It considers the growing use of desalination (demineralization) technologies in drinking water treatment which often results in tap water with very low concentrations of sodium, potassium, magnesium and calcium. Ingestion of such water can lead to electrolyte abnormalities marked by hyponatremia, hypokalemia, hypomagnesemia and hypocalcemia which are among the most common and recognizable features in cancer patients. The causal relationships between exposure to demineralized water and malignancies are poorly understood. This review highlights some of the epidemiological and in vivo evidence that link dysregulated electrolyte metabolism with carcinogenesis and the development of cancer hallmarks. It discusses how ingestion of demineralized water can have a procarcinogenic effect through mediating some of the critical pathways and processes in the cancer microenvironment such as angiogenesis, genomic instability, resistance to programmed cell death, sustained proliferative signaling, cell immortalization and tumorigenic inflammation. Evidence that hypoosmotic stress-response processes can upregulate a number of potential oncogenes is well supported by a number studies. In view of the rising production and consumption of demineralized water in most parts of the world, there is a strong need for further research on the biological importance and protean roles of electrolyte abnormalities in promoting, antagonizing or otherwise enabling the development of cancer. The countries of the Gulf Cooperative Council (GCC) where most people consume desalinated water would be a logical place to start this research.

Three analytical methods using automatic mercury analyzer (AMA), direct mercury analyzer (DMA) and inductively coupled plasma-mass spectrometry (ICP-MS) were applied to determine mercury in fish and sediment samples from the Volta Lake in Ghana with the aim of comparing their accuracy, precision, and limit of quantifications. There was statistically no significant difference (p < 0.05) between the concentrations recorded by the methods. This indicates their suitability for the accurate determination of mercury. Limit of quantification was found to be in the order; ICP-MS (0.053 ng/g) < DMA (0.527 ng/g) < AMA (2.193 ng/g). Though each of the three methods has a suitable ability in determining accurately the concentrations of mercury in fish and sediment, for the determination of very low concentrations of mercury ICP-MS should be preferred considering the order of the detection limit which follows the trend ICP-MS (0.016 ng/g) < DMA (0.158 ng/g) < AMA (0.509 ng/g).

Communities in many parts of the world are unintentionally exposed to arsenic (As) and other toxic metals through ingestion of local drinking water and foods. The concentrations of individual toxic metals often exceed their guidelines in drinking water but the health risks associated with such multiple-metal exposures have yet to receive much attention. This study examines the co-occurrence of toxic metals in groundwater samples collected from As-rich areas of Nadia district, West Bengal, India. Arsenic in groundwater (range: 12-1064 µg L(-1); mean ± S.D: 329±294 µg L(-1)) was the most important contaminant with concentrations well above the WHO guideline of 10 µg L(-1). Another important toxic metal in the study area was manganese (Mn) with average concentration of 202±153 µg L(-1), range of 18-604 µg L(-1). The average concentrations (µg L(-1)) of other elements in groundwater were: Cr (5.6±5.9), Mo (3.5±2.1), Ni (8.3±8.7), Pb (2.9±1.3), Ba (119±43), Zn (56±40), Se (0.60±0.33), U (0.50±0.74). Saliva collected from the male participants of the area had mean concentrations of 6.3±7.0 µg As L(-1) (0.70-29 µg L(-1)), 5.4±5.5 µg Mn L(-1) (0.69-22 µg L(-1)), 2.6±3.1 µg Ni L(-1) (0.15-13 µg L(-1)), 0.78±1.0µg Cr L(-1) (

Saliva, an easily accessible biofluid, is validated as biomarker of arsenic (As) exposure in several villages of West Bengal, India. Pentavalent arsenic [As(V)] was found to be the predominant species in saliva, with the amount of inorganic As [As(V) and trivalent form, As(III)] being more than half of the total As in the samples. Significant association was found between total daily ingestion of As and As(V) (r = 0.59; p = 0.000), As(III) (r = 0.60; p = 0.000), dimethylarsinous acid (DMA(V)) (r = 0.40; p = 0.000), and monomethylarsonous acid (MMA(V)) (r = 0.44; p = 0.000), implying that these species have mainly been derived from the methylation of the inorganic As in the water that study participants drank and the food they ate. Analysis of confounding effects of age, sex, smoking, body mass index and the prevalence of skin lesion suggests that women and controls with no skin lesion had a higher capacity to methylate the ingested As compared to the rest of the population. Thus, our study demonstrates that As species in saliva can be an useful tool to predict the individual susceptibility where higher As exposure and a lower methylation capacity are implicated in the development of As-induced health effects.

Qatar׳s farm workers provide a unique population for exposure study: they are young, healthy males. This study combined trace element profiles in urine and toenail with survey information from 239 farm workers to assess the extent to which the biomarkers provide complementary exposure information. Urinary Mo levels (average=114 µg/L) were elevated; average urinary values (µg/L) for all other elements were: V (1.02), Cr (0.55), Mn (2.15), Fe (34.1), Co (0.47), Ni (2.95), Cu (15.0), As (47.8), Se (25.7), Cd (1.09), Ba (22.5), Pb (2.50) and U (0.15). Average toenail concentrations (mg/kg) were: Mn (2.48), Cu (4.43), As (0.26), Se (0.58), Mo (0.07), Cd (0.03), Ba (1.00), Pb (0.51) and U (0.02). No significant association was found between corresponding elements in urine and toenails. Elemental profiles suggest groundwater (with the exception of Mo) and soil-dust-crop exposure pathways cannot account for elemental variations. The main factors moderating trace element contents are related to depuration processes involving participants׳ trace element body burden prior to work in Qatar, and interactions of trace element metabolic cycles which over-ride the exposure footprint. Toenail and urine need to be carefully validated before reliable use as biomarkers of exposure in general populations for most elements in the study.

We used a combination of subjective (questionnaire) and objective (urinary metabolites) measurements to evaluate factors that can predict the exposure of farm workers in Qatar to organophosphate pesticides and to assess whether the levels of exposure are associated with any self-reported health outcomes. The results show that pesticides were being extensively mishandled in the farms. Very few (<2%) of the farm workers knew the names of the pesticide they were using, and about one-third of the participants did not know the amount of pesticides to be applied to the crops. Nearly all (96%) of the participants had participated in mixing pesticides together before use and few (29%) used protective clothing while engaged in this operation. A significant number of participants (18%) had no knowledge that pesticides are a health hazard. At least one dialkyllphosphate (DAP) metabolite was detected in every worker. The geometric mean (GM) concentration of the dimethylalkylphosphates (DMAP) was 108 nM (range, from below the limit of detection (LOD) to 351 nM), and the GM for the diethylalkylphosphates (DEAP) was 43 nM (range, LOD-180 nM). The GM for total concentration of the metabolites (DAP) of 146 nM (maximum value estimated to be 531 nM) is below the values that have been reported for farmers in some countries, but higher than the levels in the general populations of many countries. We explored the influence of metal exposure and found consistent and negative relationships between the DAP metabolites and the concentrations of most of the trace elements in the urine of the farm workers; the negative associations were statistically significant for Cr, Mn, Fe, Ni, As, and Pb. We suspect that the negative associations are not source-dependent but may be reflective of antagonistic relationships in human metabolism of OPPs and trace metals; hence we recommend that metals should be included as co-factors in assessing the health effects of OPP exposure.

The purpose of this study is to measure maternal blood lead level (BLL) and cord BLL in Nigeria and to compare Nigerian data with other data. We investigated the association among maternal and cord BLLs, and some anthropometric parameters of their babies. BLL was measured in the umbilical and maternal blood samples (using inductively coupled plasma / mass spectrometry (ICP-MS)) of 119 women who delivered at three different hospitals in Nnewi, South Eastern Nigeria. Anthropometric variables of the babies (head circumference, abdominal circumference, birth weight, birth length, crown rump length) were measured. Lead was detected at >10 µg/l in 10.9 percent of the maternal and 3.4 percent of the cord blood samples. The maternal BLL was 6.19 ± 2.77(mean ± SD) µg/dl while cord BLL was 4.75 ± 2.59(mean ± SD) µg/dl. With the exception of cord BLL and crown rump length positive correlation (R=0.204, P=0.026), neither the maternal nor the cord BLL showed any significant association with any of the children's anthropometric parameters.

Saliva is a biological fluid that has not been used extensively as a biomonitoring tool in epidemiological studies. This study presents the arsenic (As) concentrations in saliva and urine samples collected from populations of West Bengal, India who had been previously exposed to high As levels in their drinking water. We found a significant (p < 0.05) association between the Log transformed Daily Ingestion of As (µg day(-1)) and the As concentration in saliva (r = 0.68). Additionally, As concentration of saliva and urine also had a significant positive correlation (r = 0.60, p < 0.05). Male participants, smokers, and cases of skin lesion were independently and significantly associated with an increase in salivary As. Thus our findings show that saliva is a useful biomarker of As exposure in the study population. The study also advocates that measurement of the forms of As in saliva may additionally provide insight into the internal dose and any individual differences in susceptibility to As exposure.

This study investigates the risk of arsenic (As) exposure to the communities in rural Bengal, even when they have been supplied with As safe drinking water. The estimates of exposure via dietary and drinking water routes show that, when people are consuming water with an As concentration of less than 10 µg L(-1), the total daily intake of inorganic As (TDI-iAs) exceeds the previous provisional tolerable daily intake (PTDI) value of 2.1 µg day(-1) kg(-1) BW, recommended by the World Health Organization (WHO) in 35% of the cases due to consumption of rice. When the level of As concentration in drinking water is above 10 µg L(-1), the TDI-iAs exceeds the previous PTDI for all the participants. These results imply that, when rice consumption is a significant contributor to the TDI-iAs, supplying water with an As concentration at the current national drinking water standard for India and Bangladesh would place many people above the safety threshold of PTDI. We also found that the consumption of vegetables in rural Bengal does not pose a significant health threat to the population independently. This study suggests that any effort to mitigate the As exposure of the villagers in Bengal must consider the risk of As exposure from rice consumption together with drinking water.

Arsenic and cadmium in rice grain are of growing concern in the global food supply chain. Paradoxically, the two elements have contrasting behaviors in soils, making it difficult to develop a strategy that can concurrently reduce their uptake and accumulation by rice plant. This study examined the combined impacts of watering (irrigation) schemes, different fertilizers and microbial populations on the bioaccumulation of arsenic and cadmium by rice as well as on rice grain yield. Compared to drain-flood and flood-drain treatments, continuously flooded condition significantly reduced the accumulation of cadmium in rice plant but the level of arsenic in rice grain remained above 0.2 mg/kg, which exceeded the China national food safety standard. Application of different fertilizers under continuously flooded condition showed that compared to inorganic fertilizer and biochar, manure addition effectively reduced the accumulation of arsenic over three to four times in rice grain and both elements were below the food safety standard (0.2 mg/kg) while significantly increasing the rice yield. Soil Eh was the critical factor in the bioavailability of cadmium, while the behavior of arsenic in rhizosphere was associated with the iron cycle. The results of the multi-parametric experiments can be used as a roadmap for low-cost and in-situ approach for producing safe rice without compromising the yield.

BACKGROUND: Prostate cancer (PCa) may result from an interplay between many factors including exposure to trace elements. This study examined the association between cadmium exposure and PCa among Nigerian men and evaluated possible modification of this effect by zinc status. METHODS: This case-control study involved men with histologically confirmed PCa (n = 82), benign prostatic hyperplasia (BPH; n = 93) and controls (n = 98), aged between 40 and 80 years. Study participants were recruited from the main teaching hospital that draws clients from the entire Anambra State in South-Eastern Nigeria. Blood and urine samples were collected from these participants and were analyzed for trace elements using ICP-MS. Statistical models were used to assess the exposure risk for cadmium exposure as well as the mediating effect of zinc status. RESULTS: Among men with prostatic disorders (case-case analysis), every 10-fold increase in urinary cadmium was associated with increased risk of PCa (adjusted odds ratios: 2.526; 95% CI: 1.096-5.821). Men in the highest blood zinc quartile had lower odds of PCa compared to the lowest quartile (AOR: 0.19; 95% CI, 0.06-0.54; p-for trend = 0.001). Zinc-specific effect was observed in this group: every 10-fold increase in urinary cadmium was associated with increased risk of PCa among men with creatinine-adjusted urinary zinc levels below the median value (AOR: 8.46; 95% CI: 1.97 -36.39) but not in those above the median value (AOR: 1.55; 95% CI: 0.45 - 5.39). CONCLUSION: Higher exposure to cadmium may be associated with increased risk of PCa in Nigeria and probably other countries with high prevalence of Zn deficiency. These results point to the need to consider co-occurring trace metals in any effort to mitigate the toxicity of Cd in the environment.

This study assesses the arsenic (As) accumulation in different varieties of rice grain, that people in rural Bengal mostly prefer for daily consumption, to estimate the potential risk of dietary As exposure through rice intake. The rice samples have been classified according to their average length (L) and L to breadth (B) ratio into four categories, such as short-bold (SB), medium-slender (MS), long-slender (LS), and extra-long slender (ELS). The brown colored rice samples fall into the SB, MS, or LS categories; while all Indian Basmati (white colored) are classified as ELS. The study indicates that the average accumulation of As in rice grain increases with a decrease of grain size (ELS: 0.04; LS: 0.10; MS: 0.16; and SB: 0.33 mg kg(-1)), however people living in the rural villages mostly prefer brown colored SB type of rice because of its lower cost. For the participants consuming SB type of brown rice, the total daily intake of inorganic As (TDI-iAs) in 29% of the cases exceeds the previous WHO recommended provisional tolerable daily intake value (2.1 µg day(-1) kg(-1) BW), and in more than 90% of cases, the As content in the drinking water equivalent to the inorganic As intake from rice consumption (C(W,eqv)) exceeds the WHO drinking water guideline of 10 µg L(-1). This study further demonstrates that participants in age groups 18-30 and 51-65 yrs are the most vulnerable to the potential health threat of dietary As exposure compared to participants of age group 31-50 yrs, because of higher amounts of brown rice consumption patterns and lower BMI.

BACKGROUND: Ingestion of groundwater with high concentrations of inorganic arsenic has been linked to adverse health outcomes, including bladder cancer, however studies have not consistently observed any elevation in risk at lower concentrations. Genetic variability in the metabolism and clearance of arsenic is an important consideration in any investigation of its potential health risks. Therefore, we examined the association between genes thought to play a role in the metabolism of arsenic and bladder cancer. METHODS: Single nucleotide polymorphisms (SNPs) in GSTO-1, As3MT and MTHFR were genotyped using DNA from 219 bladder cancer cases and 273 controls participating in a case-control study in Southeastern Michigan and exposed to low to moderate (<50 µg/L) levels of arsenic in their drinking water. A time-weighted measure of arsenic exposure was constructed using measures from household water samples combined with past residential history, geocoded and merged with archived arsenic data predicted from multiple resources. RESULTS: While no single SNP in As3MT was significantly associated with bladder cancer overall, several SNPs were associated with bladder cancer among those exposed to higher arsenic levels. Individuals with one or more copies of the C allele in rs11191439 (the Met287Thr polymorphism) had an elevated risk of bladder cancer (OR = 1.17; 95% CI = 1.04-1.32 per 1 µg/L increase in average exposure). However, no association was observed between average arsenic exposure and bladder cancer among TT homozygotes in the same SNP. Bladder cancer cases were also 60% less likely to be homozygotes for the A allele in rs1476413 in MTHFR compared to controls (OR = 0.40; 95% CI = 0.18-0.88). CONCLUSIONS: Variation in As3MT and MTHFR is associated with bladder cancer among those exposed to relatively low concentrations of inorganic arsenic. Further investigation is warranted to confirm these findings.

Arsenic is a notorious element with the potential to harm exposed individuals in ways that include cancerous and non-cancerous health complications. Millions of people across the globe (especially in South and Southeast Asian countries including China, Vietnam, India and Bangladesh) are currently being unknowingly exposed to precarious levels of arsenic. Among the diverse effects associated with such arsenic levels of exposure is the propensity to alter the epigenome. Although a large volume of literature exists on arsenic-induced genotoxicity, cytotoxicity, and inter-individual susceptibility due to active research on these subject areas from the last millennial, it is only recently that attention has turned on the ramifications and mechanisms of arsenic-induced epigenetic changes. The present review summarizes the possible mechanisms involved in arsenic induced epigenetic alterations. It focuses on the mechanisms underlying epigenome reprogramming from arsenic exposure that result in improper cell signaling and dysfunction of various epigenetic components. The mechanistic information articulated from the review is used to propose a number of novel therapeutic strategies with a potential for ameliorating the burden of worldwide arsenic poisoning.

A key problem facing epidemiologists who wish to account for residential mobility in their analyses is the cost and difficulty of obtaining residential histories. Commercial residential history data of acceptable accuracy, cost, and coverage would be of great value. The present research evaluated the accuracy of residential histories from LexisNexis, Inc. The authors chose LexisNexis because the Michigan Cancer Registry has considered using their data, they have excellent procedures for privacy protection, and they make available residential histories at 25 cents per person. Only first and last name and address at last-known residence are required to access the residential history. The authors compared lifetime residential histories collected through the use of written surveys in a case-control study of bladder cancer in Michigan to the 3 residential addresses routinely available in the address history from LexisNexis. The LexisNexis address matches, as a whole, accounted for 71.5% of participants' lifetime addresses. These results provided a level of accuracy that indicates routine use of residential histories from commercial vendors is feasible. More detailed residential histories are available at a higher cost but were not analyzed in this study. Although higher accuracy is desirable, LexisNexis data are a vast improvement over the assumption of immobile individuals currently used in many spatial and spatiotemporal studies.
