RESUMO

PIP: The article presents the case of a 38 year old patient, on OC (oral contraception) for 10 years. She had had recent administration of triacetyloleandomycin for a sinusitis. Pruritus and jaundice appeared suddenly and hospitalization was advised. Laparoscopy showed a cholestatic liver, and lobectomy diagnosed the presence of an hemangioma. If it is not possible to associate with certainty OC administration with the presence of benign hepatic tumors, it is possible to state that OC increases their volume and favors their rupture.^ieng

RESUMO

The study concerns 100 patients who are divided in two dissimilar groups. The first group is composed of 39 patients who have signs of possible Yersiniosis infection. In second group composed of 61 cirrhotics patients, the serodiagnosis was systematic. The rate of positive serology, comparing with healthy patients, is significantly high in both groups: 61,5 p. 100 (first group) 64 p. 100 (second group). Six cases in the first group were very likely caused by Yersinia infection and confirm the utility of the serodiagnosis in front of evocative symptomatology. But many positive serodiagnosis remain unexplained and the greatest care must be taken in appreciation of these results. Among cirrhotic patients, the positive serology is not correlated with evocative symptoms and would be the result of asymptomatic bacteriemias.

RESUMO

Pulmonary complications are non very frequent in drepanocytemia (3 to 7 p. 100) but they are often severe and sometimes dramatic. They happen in the homozygotic forms or in the double heterozygotic ones (SC and Sbeta Tahal). Infections, most often due to pneumococcus, are specially frequent in children, though infractions are generally observed in youth and grown-ups. All these patients suffer of an impairement of their respiratory function and of their immunological capacity.

RESUMO

Report of a case of P. falciparum malaria observed in Rwanda. Relapse and positive blood thick smear has been observed 8 days after the beginning of the treatment of the previous fit of fever by chloroquine. Authors recall the resistance criteria fixed by O.M.S. for antimalarial drugs and question wether drug-resistant malaria is not spreading in East-Africa, considering some similar cases reported in the region.

RESUMO

161 patients affected by schistosomiasis due in most cases to S. haematobium, and less frequently to S. mansoni, received one day treatment with 4,5 g a day of oltipraz in a first group and, later on, 3 g a day in another group. A control after one year was possible for patients having received 4,5 g one day dosis. Urin control, rectal mucosa biopsy (R.M.B.) and indirect immunofluorescence test (I.I.F.) were performed in patients who did not return to infested areas during that period of time; in almost all cases, the urin control was negative; in 52 p. 100 of the cases the R.M.B. shown no live egg and I.I.F. test was significantly decreased with individual variations. -- These results are compared to those of a previous and similar study on the effects of niridazole. -- Side-effects, mostly digestive or nervous, are frequent with a 4,5 g dose and less frequent with 3 g. They rarely compel to discard the treatment and they never persist after the morning following the treatment is stopped. There is no electrocardiographic change. -- Doses below 3 g in one-day intake are presently studied in order to fix the minimal acting dosis.

RESUMO

A study has been carried out in 690 patients admitted in a Paris hospital and from Mali, Senegal and Mauretania origin, in order to compare the three available techniques for the diagnosis of infection with S. haematobium: urine centrifugation deposit control, biopsy of rectal mucosa and indirect immunofluorescence test. The prognostic value of each of these three techniques has been reevaluated. (Formula: see text), Living eggs have been detected in 55 p. 100 of the patients by direct examinations and a prevalence of 75 p. 100 has been computaned for S. haematobium.

RESUMO

A 52-year old woman with systemic scleroderma had been treated since 1978 with factor XIII. The skin lesions had improved, and respiratory lesions had become stabilized. In 1984, she developed a bronchiolo-alveolar carcinoma which was treated by surgery. The authors emphasize that factor XIII is well tolerated and has beneficial effects on the skin and probably also on the lungs, without tachyphylaxis; however, the possibility of a bronchioloalveolar carcinoma occurring cannot be ruled out.

RESUMO

The authors report ten cases of multiple malignant tumours noted on a general medical unit : 4 cases of chronic lymphatic leukemia associated with one visceral carcinoma. One case of polycythemia associated with a bifocal colonic carcinoma and a skin carcinoma. 5 double carcinomas mainly bronchial and digestive. They recall the general characteristics of these multiple carcinomas and discuss their pathogenesis with special reference to factors of immunodepression and genetic factors.

RESUMO

It is well-known that cirrhosis is a predisposing factor to Yersinia septicemia. This study includes 73 cirrhotics and shows a high number of positive serologic tests (47/73 : 64.4%). However, there is no correlation with clinical features or bacteriological findings. The most frequent serotypes, i.e. pseudotuberculosis IV and enterocolitica 0:9, differ from those which are usually found in Yersinia septicemias. Iron overload in cirrhosis, increased intestinal load of gram-negative bacilli and possible latent bacteremia may partly explain these results. However, the role probably played by as yet poorly known cross-reactions between Yersinia and other pathogens (Shigella, E. Coli...) must be underscored. The authors conclude that slightly positive, stable, serodiagnostic tests have little meaning in cirrhotics.

RESUMO

A case of primary reticulum-cell sarcoma of the rib is reported. This rare tumor arose in an unusual site and was revealed by an exceptional clinical picture. Extensive hemothorax was the initial manifestation. Follow-up exceeds five years. Prognosis is not as poor for reticulum-cell sarcoma as for Ewing sarcoma. After radical surgery, radiation therapy and chemotherapy, the five-year total remission rate approximates 50%.

RESUMO

Authors compare results shown by Abrams needle and a new needle in pleural biopsies. With the new needle, Abrams-derived, out of 102 biopsies, there are 30 percent of unquestionable diagnosis and 10 percent of failure, though handlers were not in training. These results are statistically significant better than those of 244 biopsies with Abrams needle and recorded by the same pathologist (M.J.). They are comparable with those of handlers well trained.

RESUMO

The distribution of HLA genes and haplotype frequencies was studied in a population of 119 Sarakole West Africans. The absence of HLA-B15, the rarity of HLA-A10, B12 and B17 and the relatively high frequency of HLA-A2 and B5 were noted. The presence of a Da 6 gene in the African populations is confirmed. The main gametic associations were Aw19-Bw35 and Aw23-B5.

RESUMO

The authors report two cases of hemoglobin C trait and hemoglobin C disease with bone tuberculosis and, in the first case, bone infarction of one condyle two years after the tuberculosis of the other knee. This suggests the possibility of osseous manifestations in the course of hemoglobin C trait or hemoglobin C disease apart from hemoglobin SC disease.

RESUMO

The authors report the cases of six immigrated Black Africans with clinical and radiological (4 cases with condensing osteitis ; 2 cases with condensing forms and multiple lacunae) bone manifestations ; these manifestations, the serological tests and the osseous biopsy (three cases out of four) are consistent with the diagnosis of bone syphilis. These facts are discussed and ascribed to tertiary syphilis rather than to congenital or secondary syphilis, to yaws or endemic syphilis.

RESUMO

This study of 152 cases of tuberculosis in African Negro immigrants seen between 1972 and 1976 showed the predominance of especially pulmonary respiratory lesions and/or hilar ganglio-mediastinal lesions. The other lesions are frequent, i.e. the lesions of lymph nodes, bones, peritoneum, liver, pericardium ; the multiple lesions are likewise frequent. Smears and culture of sputum and/or biopsy of organs or tissues such as pleura, lymph node, liver and peritoneum are the diagnostic procedures. In spite of frequent resistance, treatment with isoniazid, rifampicin and ethambutol gave good results in high risk subjects, who are almost always allergic on arrival in France.
