RESUMO

The authors describe the principal difficulties and intra and post-operative complications frequently encountered in IOL implantation surgery in P.C. in patients with pseudoexfoliation syndrome (SPE); particular attention is given to incomplete mydriasis, the facilitation of expulsion by sphyncterotomy or radial iridotomy, zonule rupture, uveal retraction, endothelial involvement, hyphema and lens dislocation. These complications were more numerous in patients with SPE plus glaucoma. The authors conclude that, with adequate pre-operative investigation and appropriate measures during surgery, SPE with or without glaucoma is not to be considered an absolute contraindication to IOL implantation in P.C.

RESUMO

Extracapsular extraction of cataract followed by I.O.L., implantation in P.C. was performed in 10 patients affected by Primary Ocular Hypertension and cataract. The aim of this study was to evaluate if implantation could support a modification of I.O.P. and an evolution from hypertension to glaucoma. The postsurgical stability of I.O.P., the absence of alterations in visual field and optic disc during the 6 months after surgery demonstrate that ocular hypertension is not contraindicative to I.O.L. implantation in P.C.

RESUMO

Excessive iris trauma during cataract surgery with or without IOL implantation and intermittent contact between the lens implant and the iris in the post-operative period may give rise to a pigment dispersion syndrome that can, in some cases, lead to a pigmentary glaucoma. This communication examines possible causes of the syndrome and the precautions to be undertaken in order to reduce its incidence.

RESUMO

Focal dermal hypoplasia is a syndrome characterized by anomalies of cutaneous, osseous, dental and ocular structures. Because of the ocular anomalies, this syndrome should be regarded as a fifth type of phakomatosis. The differences between the fibroblasts obtained from skin lesions and fibroblasts obtained from normal skin and controls could be the demonstration of mosaicism and the consequence of lyonization. The authors present the case of a baby with typical anomalies of the Gorlin-Goltz syndrome and abnormal growth characteristics of skin fibroblasts.

The authors describe a family showing coloboma of the optic nerve associated with chorioretinal coloboma and coloboma of the iris. The absence of its occurrence in association with extraocular malformations points to an autosomal dominant mode of transmission, with reduced penetration. The authors describe the probable pathogenetic mechanism of the disorder and discuss the differential diagnosis both for chorioretinal coloboma (inflammatory chorioretinitis, especially by toxoplasma) and for colobomas of the optic disc (papillary pits and morning glory syndrome). The molecular basis of the malformation is however still unknown and thus a prenatal diagnosis is impossible.

The authors describe a case of congenital retinal macrovessel with decreased visual acuity. Ophthalmoscopy and fluorescein angiography showed the malformation to be congenital and benign.

RESUMO

Cefamandole concentrations were determined in the aqueous humor of the anterior chamber of the eye in 10 rabbits and in 15 patients undergoing cataract extraction. The rabbits received 50 mg of cefamandole per kilogram of body weight in the ear vein and humans received 2 g of cefamandole by intravenous injection before operation. The levels recovered were several times higher than the minimum inhibitory concentrations (MICs) against the bacteria that are most often responsible for ocular infections.
