BACKGROUND: Preoperative intravenous iron administration is a frequently used patient blood management procedure. If the timeframe of intravenous iron administration before surgery is short, (1) the concentration of the intravenous iron compound might still be high in patients' plasma when undergoing surgery and (2) this iron in patients' plasma is at risk to be lost due to blood loss. The aim of the current study was, therefore, to track the iron compound ferric carboxymaltose (FCM) before, during, and after cardiac surgery requiring cardiopulmonary bypass, with an emphasis on intraoperative iron losses in shed blood and potential recovery through autologous cell salvage. METHODS: Concentrations of FCM were analyzed in patients' blood using a hyphenation of liquid chromatography and inductively coupled plasma-mass spectrometry to distinguish between pharmaceutical compound FCM and serum iron. In this prospective, single-center pilot trial, 13 anemic and 10 control patients were included. Anemic patients with hemoglobin levels ≤12/13 g/dL in women and men were treated with 500 milligrams (mg) intravenous FCM 12 to 96 hours before elective on-pump cardiac surgery. Patients' blood samples were collected before surgery and at days 0, 1, 3, and 7 after surgery. One sample each was taken of the cardiopulmonary bypass, the autologous red blood cell concentrate generated by cell salvage, and the cell salvage disposal bag. RESULTS: Patients who had received FCM <48 hours before surgery had higher FCM serum levels (median [Q1-Q3], 52.9 [13.0-91.6]) compared to ≥48 hours (2.1 [0.7-5.1] µg/mL, P = .008). Of 500-mg FCM administered <48 hours, 327.37 (257.96-402.48) mg were incorporated compared to administration ≥48 hours with 493.60 (487.78-496.70) mg. After surgery, patients' plasma FCM concentration in the FCM <48 hours group was decreased (-27.1 [-30 to -5.9] µg/mL). Little FCM was found in the cell salvage disposal bag (<48 hours, 4.2 [3.0-25.8] µg/mL, equivalent to 29.0 [19.0-40.7] mg total; equivalent to 5.8% or 1/17th of the 500 mg FCM initially administered), almost none in the autologous red blood cell concentrate (<48 hours, 0.1 [0.0-0.43] µg/mL). CONCLUSIONS: The data generate the hypotheses that nearly all FCM is incorporated into iron stores with administration ≥48 hours before surgery. When FCM is given <48 hours of surgery, the majority is incorporated into iron stores by the time of surgery, although a small amount may be lost during surgical bleeding with limited recovery by cell salvage.

The equatorial Pacific is the largest oceanic source of carbon dioxide to the atmosphere and has been proposed to be a major site of organic carbon export to the deep sea. Study of the chemistry and biology of this area from 170 degrees to 95 degrees W suggests that variability of remote winds in the western Pacific and tropical instability waves are the major factors controlling chemical and biological variability. The reason is that most of the biological production is based on recycled nutrients; only a few of the nutrients transported to the surface by upwelling are taken up by photosynthesis. Biological cycling within the euphotic zone is efficient, and the export of carbon fixed by photosynthesis is small. The fluxes of carbon dioxide to the atmosphere and particulate organic carbon to the deep sea were about 0.3 gigatons per year, and the production of dissolved organic carbon was about three times as large. The data establish El Niño events as the main source of interannual variability.

PURPOSE: Fluorine-18 fluorodeoxyglucose (F-18 FDG) positron emission tomographic (PET) imaging can be a vital tool in the evaluation and preoperative staging of pulmonary neoplasms. Imaging studies on the gamma camera using coincidence PET (co-PET) were introduced recently into clinical practice. This prospective study assessed the efficacy of co-PET for identifying primary tumors, mediastinal lymph node involvement, the presence of distant spread, the effect on disease staging, and influence on disease management. PATIENTS AND METHODS: Sixty consecutive patients with 61 lung tumors were enrolled in the study (36 men, 24 women; age range, 32-87 years; mean age, 67 years). Histopathologic confirmation was obtained in 58 patients (59 tumors). RESULTS: In assessments of a primary lesion to establish its malignant or benign nature, the sensitivity rate of co-PET was 96% (53 of 55 lesions), the specificity rate was 83% (5 of 6 lesions), and the accuracy rate was 95% (58 of 61 lesions). In the co-PET assessment of lymph node involvement in which histopathologic confirmation was obtained (n = 32), the sensitivity rate was 89% (8 of 9 lesions), the specificity rate was 91% (21 of 23 lesions), and the accuracy rate was 91% (29 of 32 lesions). Previously unknown distant metastases were correctly identified in eight patients, but five false-positive lesions were detected in the brain. Disease staging was correctly altered in 20 patients (33%), and disease management plans were changed in 20 patients (33%) based on the co-PET findings. Unnecessary surgery was obviated in six patients (10%). One patient was given the chance for curative treatment based on the findings of the co-PET study. CONCLUSION: The evaluation of patients with lung neoplasms using F-18 FDG coincidence detection appears to be reliable.

In rare cases, extrarenal uptake of technetium-99m diethylenetriamine penta-acetic acid ((99m)Tc-DTPA) has been reported, e.g. in tumours or abscesses. Although in our experience a relatively common occurrence, increased uptake in the left iliac fossa (LIF) and its incidence have not been described. This retrospective study set out to establish the incidence of this phenomenon and to exclude malignancy as the cause. All patients who had a renal (99m)Tc-DTPA scan between 1 January 1996 and 31 December 1997 had their scans reviewed. Presence and grade of increase in iliac fossa activity were determined by consensus agreement of three observers. The list of patients with increased uptake was cross-referenced against the New South Wales (Australia) Cancer Council database to exclude the possibility of a neoplastic lesion in this region. Increased LIF uptake was noted in 41 of the 231 (18%) consecutive patients (18 men, 23 women; mean age 43 years, range 20-83). Among these 41 patients, uptake was severe in 2 (5%), moderate in 12 (29%) and mild in 27 (66%). No patient had increased uptake in the right iliac fossa. Only one patient had a malignant lesion but this was excluded as being the cause of LIF uptake. No other patient developed malignancy (mean follow-up time 4.1 years; range 3.2-5.1). Increase in LIF uptake is a common, benign finding most likely due to activity within the descending colon. It occurs in approximately 18% of the population, and it is important to recognise such uptake in order to avoid misdiagnoses.

RESUMO

The sex behaviour of patients suffering from schizophrenia has been largely overlooked. This study is aimed at describing the pattern of sexual responses and conducts in 113 inpatients with schizophrenia (DSM-III-R). A high rate of sexual dysfunction was found in both males (62.9%) and females (50%). These rates are higher than found in other previous studies. The possible cause factors of sexual dysfunctions in this group of patients and the methodological problems related to this type of study are reviewed.

Patients with end-stage chronic obstructive pulmonary disease (COPD) undergoing lung volume reduction surgery (LVRS) are at high risk of peri-operative cardiac complications, and myocardial perfusion scintigraphy (MPS) is commonly used for risk stratification. This study prospectively assessed the safety of dipyridamole in these patients and compared the incidence of side-effects (particularly dyspnoea) with that in patients undergoing dipyridamole MPS prior to elective non-cardiothoracic surgery. Fifty patients were enrolled: 25 in the LVRS cohort (13 males, 12 females), with a mean age of 65 years and a mean FEV1 of 0.791, and 25 (with no history of asthma or COPD) in the control cohort (14 males, 11 females), with a mean age of 66 years. Fourteen patients (56%) in each group developed side-effects. Dyspnoea was reported by five patients (20%) in the LVRS and two patients (8%) in the control cohort (P=NS). One patient in each cohort developed severe hypotension and bradycardia. Eight (32%) other patients developed minor side-effects in the LVRS cohort compared with 11 (44%) in the control group. All side-effects responded promptly to intravenous aminophylline. In summary, there was a statistically non-significant increase in the incidence of dyspnoea in patients with end-stage COPD and all side-effects responded to aminophylline. Thus, dipyridamole can be used safely in these patients.

Se estudiaron las poblaciones de linfocitos en sangre periferica de 15 pacientes con artritis reumatoide y 15 controles sanos. La fraccion de linfocitos T formadores de rosetas tempranas y los linfocitos B (inmunofluorescencia) fueron semajantes en ambos grupos, mientras que la fraccion de linfocitos T formadores de rosetas tardias mostro una dismunicion significativa en pacientes con artritis reumatoide (p< 0,005). Esto se atribuyo a la actividad clinica o a la terapeutica inmunosupresora que tenian cinco de los enfermos
