BACKGROUND: To understand which genes are really involved in the implantation process, we planned to study the gene basal expression profile during the window of implantation (WOI) of patients who became pregnant in a subsequent ICSI cycle. METHODS: Women attending their first ICSI cycle at ANDROS Day Surgery for severe male factor infertility were included in the study. An endometrial biopsy was performed during the WOI, in one of the last two cycles before the ICSI cycle. Forty-seven selected gene profiles were analyzed using Low Density Array technology. Only biopsies from women who subsequently became pregnant were evaluated, to exclude any bias regarding embryo viability and embryo transfer difficulties. RESULTS: Fifteen patients were included in the analysis as they became pregnant after ICSI procedure. Four of 47 selected genes were excluded from the analysis. Of the 43 genes analyzed, only 6 genes (VEGFA, PLA2G2A, ALPL, LIF, NNMT and STC1) showed a statistically uniform expression among patients who subsequently became pregnant. For all the other genes analyzed there were considerable differences in their expression levels amongst women who subsequently became pregnant. CONCLUSIONS: Our data suggest that very few genes, which change their expression level during the WOI, show a quantitative homogeneous expression in endometrially-receptive patients. In conclusion, in this study only six genes showed a homogeneous expression, and are probably involved in embryo implantation mechanisms.

RESUMO

The Authors refer to their experience on 10 cases of benign cancer of the stomach studied either radiologically or endoscopically before the surgery intervention. Moreover the authors emphasize the importance of the association between the radiological investigation and the fibroscope in the determination of the benign characters of the gastric neoplasm and the possibility given by the endoscope to effect an extemporaneous biopsy in the cases of uncertain interpretation.

RESUMO

Starting from four primitive malignant neoplasias of the duodenum observed during the course of the last two years, the Authors briefly outline the clinical aetiopathological and especially the radiological characteristics of duodenal primitive malignant neoplasias. They stress the importance of the radiological investigation for early diagnosis of such disturbances, as can clearly by deduced from illustration of the radiological pictures concerning the four cases, all male patients aged from 55 to 68 years. A localisation in the duodenal bulb, with ulceration, is to be considered very rare; the other localisations were two at the 2nd and one at the 1st duodenal portion.

BACKGROUND: Our prospective randomized controlled trial was designed to assess whether the use of GnRH antagonists can improve the success rate of controlled ovarian stimulation (COS)/intrauterine insemination (IUI) treatments, via inhibition of the premature LH rise. METHODS: A total of 104 patients were randomly divided, using a randomization list, into two groups: in group A (n = 52), recombinant FSH (rFSH) was given with GnRH antagonist Cetrorelix, and in group B (n = 52), the patients received rFSH alone in a manner similar to that of group A. The primary outcome measure was clinical pregnancy rate per couple. RESULTS: The pregnancy rate per patient was 53.8% in group A and 30.8% in group B (P = 0.017). The rate of premature LH surge was 7% in group A and 35% in group B (P < 0.0001). A premature luteinization was observed in two cycles of 144 in group A (1.4%) and in 16 cycles of 154 in group B (10.4%) (P = 0.001). The mean values of LH and progesterone were significantly lower in patients receiving GnRH antagonist than in those who did not (3.3 +/- 3.3 mIU/ml in group A versus 9.9 +/- 7.9 mIU/ml in group B, P < 0.0001, for LH; 1.3 +/- 1.1 ng/ml versus 2.1 +/- 1.9 ng/ml for group A and B, respectively, P < 0.0001, for progesterone). CONCLUSION: The use of GnRH antagonist in COS/IUI cycles improves pregnancy rate, preventing the premature LH rise and luteinization.

PURPOSE: To present the occurrence of a monozygotic bichorionic twinning after the transfer of a single frozen/thawed embryo has undergone assisted zona hatching (AZH), using a non contact laser. METHODS: A 33-year-old woman who underwent single frozen embryo replacement (FER). Before transfer, the patient was treated with GnRH agonist and with substitutive therapy (estrogens and progesterone). CONCLUSIONS: We are absolutely certain that we are reporting a monozygotic bichorionic pregnancy: a single frozen/thawed embryo was transferred and no concomitant spontaneous conception was possible. Moreover, molecular biology assessment for polymorphisms confirmed clinical findings. The possibility of the occurrence of a monozygotic bichorionic twinning after the transfer of a single frozen/thawed embryo should be borne in mind and its incidence will have to be studied further.

Ultrasonography of the knee joint using small-parts probes was performed on 27 patients with clinical findings suggestive of meniscal cystic degeneration. Surgical or arthroscopic confirmation of sonographic findings was obtained in all cases. Sonography delineated the shape and structure of the meniscal profile and any degenerative changes. In the initial stage (6/27 patients, 22%), minor structural irregularities could be observed. Small, round, transonic cysts were found in 8 patients (30%); these lay mainly within the meniscus and were movable during stress flexion of the leg. In more advanced cases (8/27, 30%), larger cysts sometimes protruded on the skin surface. In 3 patients (12%), the soft tissue swelling was associated with a normal sonographic aspect of the menisci, but sonography could demonstrate an effusion of extra-articular origin. Ultrasound is a simple and effective method for the assessment of degenerative cysts of the menisci. It allows a reliable differential diagnosis between meniscal cysts and other causes of soft tissue swelling, and it is well suited for monitoring the course of the disease.

The prevalence of QT prolongation in a large random sample of Type 1 diabetic patients in Piemonte, Italy and its association with autonomic neuropathy were assessed. Three hundred and seventy-nine Type 1 diabetic patients (age 15-59) with (94, DAN+) and without (280, DAN-) autonomic neuropathy and 118 non-diabetic control subjects participated in the study. QT interval was measured on an ECG recorded at rest and QTc calculated according to Bazett's formula. QTc was greater than 0.440 s in 7.6% (95% CI 2.9-12.3) of control subjects, 25.6% (21.0-30.0) of diabetic patients, 30.8% (21.5-40.1) of DAN+, 23.9% (18.9-28.9) of DAN-. QTc was greater than 0.460 s (mean + 2SD of QTc in control subjects) in 11.7% (8.5-14.9) of diabetic patients, 18.1% (10.3-25.9) of DAN+, 9.6% (6.2-13.0) of DAN-. QT was above the 95% upper limit for the control subjects in the plot of measured QT against RR interval in 21.4% (17.3-25.5) of diabetic patients, 26.6% (17.7-35.5) of DAN+, 19.3% (14.7-23.9) of DAN-. No correlation was found between QT interval and age or disease duration. The prevalence of QT prolongation was higher in diabetic patients than in control subjects and in DAN+ than in DAN-.

The writers report an experimental trial on the mouse, consisting in the induction of osteosarcoma by inoculation of Moloney's murine sarcoma virus (MSV-M) into the proximal metaphysis of the tibia. This experiment was conducted on 90 BALB/c female mice aged twenty-one days and of mean weight 12.8 g. One group of animals was also given immunodepressive drugs. The tumour developed in 100 per cent of cases, producing metastases in some of the animals and resulting in death in three cases. The radiographic changes were initially osteolytic followed later by new bone formation. The histological examinations confirmed that the tumour was of an osteosarcomatous nature. Cells characteristic of rhabdomyosarcoma were observed in the soft tissues surrounding the bone. The skeletal metastases had the characteristics of osteosarcoma, while those in the lymph glands and lungs had those of an undifferentiated sarcoma. The writers also took into consideration a series of previous trial performed at the Institute of Clinical Orthopaedics of the University of Milan, involving a total of 740 mice, in arriving at their final conclusion that MSV-M induces, in the mouse, an osteosarcoma similar to spontaneous osteosarcoma in man, capable of producing osteoid tissue, of giving rise to metastases, and of causing the death of the animal.

The aim of this study was to determine the possibility of the introduction of another parameter--the hypoosmotic swelling test--in the evaluation of the freezing potentially of anonymous donor's semen sample to be utilized in an AID program. 154 donor semen samples were frozen using classic seminal parameters: HOS tests were performed but not utilized as criteria for freezeability. All specimens were thawed and another HOS test was performed. All specimens were divided into two groups on the basis of the pre freezing HOS test value--HOS tests positive (> or = 55%) and HOS test negative (< 55%)--to verify if it's possible to correlate this value to the recovery of a good motility after thawing. A further division was performed: HOS tests highly positive (> or = 70%), HOS positive (60-55%) and HOS tests negative (< 55%) but the authors did not find any statistical difference. As concerns the clinical evaluation, were considered the last 15 pregnancies achieved with 11 samples out of 22 utilized. There was not any statistical difference. The data could seem to confirm the hypothesis that it is not possible to utilize the HOS test as a predictive value of "freezeability" of human semen sample.

High resolution ultrasonography of the hand and wrist was performed on 20 patients with definite or probable rheumatoid arthritis (ARA standard criteria) in its early stage. In all the patients, swelling of the soft tissues of the fingers corresponded to an enlargement of the joint capsule containing a hypoechoic exudate. The rheumatoid nodules appeared as fluid-filled rounded cavities with sharp borders. Rheumatoid tenosynovitis was observed in 18/20 patients. This corresponded to oval or spindle-shaped cavities with a hypoechoic (10/18 cases) or anechoic content (8/18 cases) and with the tendon ribbon inside. Rupture of a tendon was diagnosed in 8/20 cases and it was always confirmed at surgery. Tenosynovitis of the flexor carpi ulnaris at the wrist level was observed in 10/20 patients. Ultrasonography is proposed as an effective first-line approach and as a periodical follow-up survey in early stage rheumatoid arthritis, in combination with standard radiography.

High resolution ultrasonography of the knee was performed on 82 young patients with clinically suspected Osgood-Schlatter disease and on 30 normal subjects; in 45 pathological cases (55%) comparative X-ray films were taken. The ultrasound pictures were equally or more effective than X-ray images in 45/45 cases; their value was particularly marked for soft tissue study. The typical sonographic changes of the ossification center, of the cartilage, and of the surrounding soft tissues are described and classified, both for Osgood-Schlatter and for Sinding-Larsen-Johansson diseases. These signs are based mainly upon cartilage swelling and edema, fragmentation of the ossification center, thickening of the patellar tendon, and bursitis of the infra-patellar bursa. Ultrasonography is proposed as a simple and reliable method for the diagnosis of knee joint osteochondrosis. The ultrasound picture is also suitable for periodical follow-up the course of the disease.

The value of xeroradiography and of high-resolution ultrasonography was compared on minute samples of foreign bodies of various radiographic density (wood, plastic, metallic wires and needles, glass, fish bones). The samples were introduced into pieces of veal meat at different angles with respect to the surface. Xeroradiographic images were taken at 60 kVp, with 32 mAs, and were developed in both positive and negative mode. The echographic examinations were performed using a real-time sector 10 MHz probe placed directly over the specimen, with constant gain rates. The results confirm the value of xeroradiography and demonstrate the effectiveness of high-resolution ultrasonography for the detection of minute foreign bodies. Some materials showed a peculiar echographic pattern, like sand, that corresponded to a frank acoustic shadow, or like metal and glass, showing a comet tail of reverberation artifacts. Wood and fish bones were well outlined, while plastic was less effectively represented. High-resolution ultrasonography is considered to be a first-line technique in searching for many kinds of minute foreign bodies, to be used in combination with xeroradiography in routine clinical examination.
