Amalgapins are susceptible to early fracture during matrix removal and carving. The purpose of this study was to examine the early fracture resistance of amalgapin-retained restorations using a spherical amalgam alloy, an admixed amalgam alloy, a combination of admixed alloy over the spherical alloy, and a recently introduced modified spherical amalgam alloy. Four amalgapin channels with a diameter of 1.4 mm and depth of 2 mm were prepared in cylinders of Macor, a machinable ceramic material. The amalgapins were hand condensed, and the bulk of the restoration was mechanically condensed. In the group using the combination of alloys, 800 mg of spherical alloy was condensed into the amalgapins and over the floor of the preparation. The admixed alloy was then condensed over the spherical alloy to build up the bulk of the restoration. Using an Instron Universal Testing Machine, the restorations were tested to shear failure at an average of 15.8 +/- 1.3 minutes after the initiation of trituration of the amalgam alloy. A metal ring was placed around the restoration and pulled 90 degrees to the long axis to simulate matrix band removal. Data were analyzed using Kruskal-Wallis procedures. The fracture resistance of the spherical alloy group and the spherical/admixed group were significantly higher than admixed or Tytin FC. All fractures occurred in amalgam at the entrance to the amalgapin channel. The combination of spherical and admixed amalgam alloys in a restoration may reduce the potential for early dislodgment while allowing additional time for carving.

RESUMO

Eight own cases of thyroid neoplasias are presented the anamnesis of which shows an irradiation in the head-neck region. After a short survey of the existing literature, the authors discuss the influence of the dose, the relatively long latent time and the different tissular tumor types (carcinomas and sarcomas) in dependence on the age of the patient at the time of irradiation.

A retrospective study was made on the secondary tumor rate of 919 patients treated for mammary carcinoma during the period of 1966 to 1975 at the Knappschafts-Krankenhaus Recklinghausen. Compared to the age and tumor specific incidence, the risk of a secondary mammary carcinoma was 5.5 times higher for those patients suffering from a mammary carcinoma at less than 50 years. Patients with primary mammary carcinoma beyond the age of 50 had no increased incidence of secondary carcinomas compared to the female population without tumor anamnesis. Despite a slightly increased coincidence with cutaneous neoplasias, the rate of secondary malignomas developed in the following years is not essentially increased as compared with normal population.

Two part collectives of patients with carcinomas of the uterus - one consisting of 525 conventionally irradiated patients, the other consisting of 577 patients irradiated with telecobalt - are compared with respect to their five-year survival recovery rate, the frequency of recurrences and metastases and the radiogenic complications. After having introduced the telecobalt therapy - intracavitary radium therapy remaining unchanged - the five-year survival recovery ameliorated by about 10% for carcinomas of the cervix as well as for carcinomas of the body of the uterus. The evaluation according to tumor stages showed that the telecobalt irradiation was particularly advantageous for operated and non-operated patients with a carcinoma of the cervix of stage II as well as for non-operated patients with a carcinoma of the body of the uterus. The authors discuss the possible reasons for the increase of recurrence rate and radiogenic complications observed in patients treated with telecobalt.

RESUMO

A technique of marking non-palpable changes in mammograms for the surgeon is described. The suspected area is injected with contrast medium and patent blue. The radiologist can assess the exact site and the surgeon can recognize it at operation. Radiographs of the specimen removed can make sure whether the suspect area was removed. There were 13 preclinical cancers, 2 cancers in site, 1 sclerosing adenosis, 1 proliferating mastopathy, and 66 fibrous or fibrocytic mastopathies. Among contrast media Urografin 76% appears the best because of its sufficient contrast density and complete reabsorbtion. So far there have been no serious complications
