RESUMO

Patients with damage of the mitral, aortic and tricuspid valves and systolic myocardial dysfunction associated with previous SARS-CoV-2 infection are described. The diagnosis of acquired defect was established in 4 patients based on medical history, electrocardiography, echocardiography, magnetic resonance imaging of the heart, endomyocardial or intraoperative myocardial biopsy, and in one case, autopsy. The study of the myocardium included H&E, Van Gieson staining, immunohistochemical (IHC) study with antibodies to CD3, CD20, CD45, CD68, to the nucleocapsid and Spike proteins of SARS-CoV-2. Previous valve diseases (prolapse, bicuspid aortic valve) served as a background for the development of the defect in 2 patients. In all cases, IHC studies revealed coronavirus proteins, lymphocytic endocarditis and myocarditis, moderate fibrosis, and signs of connective tissue disorganization. High titers of anticardiac antibodies indicated an autoimmune mechanism for carditis. No signs of infective endocarditis or thromboembolic complications were identified in any case. In patients with an unclear nature of valvular heart defects, a previous new coronavirus infection should be identified and taken into account as a possible etiological factor. The simultaneous development of lymphocytic myocarditis significantly increases the risk of surgical intervention on the valves and requires an integrated approach to treatment.

RESUMO

This report presents a clinical case of a 57-year-old female patient who was admitted for dyspnea, productive cough, reduced left ventricular (LV) systolic function, and who had previously undergone thoracocentesis for significant pleural effusion. This case is a unique combination of lymphocytic myocarditis and massive intracardiac and ileo-caval thrombosis. Morphological verification of the diagnosis, that was necessary prior to the administration of immunosuppressive therapy due to the prothrombogenic effect of glucocorticoids, provided a justification for a basis therapy for myocarditis, which significantly improved the patient's condition.

RESUMO

Cardiac myxoma is the most common primary benign cardiac tumor (up to 50% of all primary cardiac neoplasms). The implementation of the modern imaging techniques into the clinical practice, particularly, 2D echocardiography, computed tomography and magnetic resonance tomography (MRI) results to the prompt diagnosis of the myxoma. However, the absence of specific clinical features, insufficient awareness of this condition among the physicians along with a rare prevalence, may lead to a misdiagnosis. This case report is notable for the relatively late diagnosis of a giant left atrial myxoma due to a number of circumstances but with successful surgical treatment.

RESUMO

The paper is related to some aspects of "nurse-patient" interrelations in conditions of rehabilitation of mental patients. The authors studied some factors influencing the evaluation (selection) of nurses by patients with the aid of special questionnaire techniques of nurses and patients in combination with the method of expert assessment. The study covered 141 mental patients and 14 nurses working in shifts. It was possible to distinguish some personality and occupational characteristics of the nurses who were most closely related to the estimation made by the patients. The authors established some relation between the assessment made by the patients with the degree of readiness for social contacts with mental patients by nurses.

RESUMO

Properties of joint immobilization of a proteolytic enzyme (terrilytin, trypsin, collytin, or protease C) and an anti-microbe compound to cellulose copolymer containing carboxyl groups were investigated. It was established that the molecule of anti-microbe substance containing a few base groups enhanced stability of the enzyme immobilized, most likely due to additional fixation of the enzyme macromolecule. Changes in activity of the materials containing both protease and anti-microbe compound were studied upon gamma-sterilization and subsequent prolonged storage. The materials with combined biological action were demonstrated to accelerate markedly wound cleansing and healing.

RESUMO

It is shown in the paper that sensitivity of human organism to the toxic action of toluene is not changed under exposure to the microclimate, producing allowable thermal state, and is increased under exposure to the heating microclimate leading to the development of maximum thermal state. Under toluene exposure in the concentration 300 mg/m3 resistance to the heating microclimate under the conditions of joint exposure to these factors is reduced, which is manifest in an earlier onset of the maximum thermal state in people under observation.

RESUMO

The work was aimed to justify application of gas masks and respirators with autonomous air source fo lower bacterial contamination of inhaled air. The studies also covered possible catch of bacteria by cotton and filters FPP-15-1.5, those composed of antimicrobial materials, containing furagin or copper ions. As the studies proved, for lower bacterial contaminations of inhaled air one can apply autonomous air source apparatus with filters made of Petrianov tissue, antimicrobial tissue (containing furagin or copper ions), as they reduce fungal content of the air. Such filters are self-disinfecting, but do not influence total contamination of the air.

RESUMO

The paper summarizes the results of 10-year activity of the ad hoc commission on "The Problems of Human Protection under Extreme Conditions" of the Interdepartmental Scientific Council on Human Ecology and Environmental Hygiene, Russian Academy of Medical Sciences. It shows the work of the Commission on Planning, Organization, and Coordination of Researches in the Development and Introduction of New Human Protective Agents and Means from Exposures to Chemical, Physical, and Biological Agents under Extreme Conditions. The prospects of the Commission's work have been defined for the coming 5 years, by taking into account the priority lines of development of science, technology, engineering in the Russian Federation.

RESUMO

A group of volunteers was exposed to coaction of carbon dioxide (concentration 300 mg/cu m) and heating microclimate (ambient temperature +50 +/- 2 degrees C, relative humidity 20 +/- 5%), simultaneously they received one of the medicines: placebo, bemitil (0.5 g), bromantane (0.25 g) or bemitil (0.5 g) combined with bromantane (0.25 g). Bromantane (0.25 g) or bemitil (0.5 g) combined with bromantane (0.25 g) were proved to be the most effective method to increase stability of the human body against co-action of carbon dioxide and heating microclimate.

The paper describes medical facilities for saving victims of general cooling. Physiological ways of recovery of the thermal status of individuals exposed to general cooling are substantiated. A physiological rationale is presented for the design of engineering aids to prevent overcooling in persons who do their work in cold. The specifications of electric heating cloth used in the design of these medical rescue facilities are characterized.

RESUMO

Field trials were conducted to evaluate effectiveness of mycoses prophylaxis gained by use of antimicrobial insoles and foot-binding with fungicide agents. The trials involved 100 novice soldiers. Bacteriology and bacterioscopy studies diagnosed 47 soldiers as having subclinical mycosis of feet. Using antimicrobial insoles and foot-binding caused considerably lower occurrence of pathogenic fungi in the skin samples; using antimicrobial insoles with regular foot-binding resulted in no changes of the skin microflora. The authors recommend wide application of antimicrobial stuff with fungicide agents for prophylaxis of feet mycosis.

RESUMO

Stationary dermatologic examination covered 32 sufferers from epidermophytosis of soles, who used 3 types of antimicrobial insoles chosen through laboratory investigations. Clinical trials proved that antimicrobial insoles, if applied during 2 weeks, result in considerably decreased occurrence of causal fungus in the patients' surface skin scarring. The results proved fungicidal and bactericidal activity of insoles including furagin, nitrofurilacroleine, polyhexamethylene guanidine, so such insoles could be recommended as prophylactic measure for mycoses of soles.
