RESUMO

In Georgia, the number of confirmed cases of Coronavirus were 1,85,5289. Among them, 17 132 patients died. Information on risk factors for mortality is insufficient. The purpose of our research is to evaluate clinical features of heavy patients with severe COVID and determine prognostic factors of outcome. Factors associated with critical COVID-19 included older age and certain chronic medical conditions. The clinical material of 250 chronically ill COVID-19 patients admitted to the intensive care unit was retrospectively studied. We divided the patients into two groups. The dead and the survivors. Demographic data, comorbidities, chronic diseases, results of ultrasound, cardiography, computed tomography and laboratory characteristics were studied. In patients with chronic diseases, in the intensive care unit during COVID-19, the relative chance of survival decreases: CRP3 - OR=0.98(95% CI:0.97-0.99Hydrothorax- OR=0.24(95% CI:0.06-0.95); Sepsis/Septic shock - OR=0.07(95% CI:0.01-0.39); WBC - OR=0.86(95% CI:0.74-0.99); Mechanical lung ventilation - OR=0.01(95% CI:0.00-0.05)); increase survival relative chance- pO2 - OR=1.03(95% CI:1.0-1.06). Predictors of mortality in patients with chronic diseases: coagulation characteristics, inflammatory markers, sepsis, and artificial lung ventilation. Risk factors for covid-19 mortality need to be studied to increase pandemic preparedness.

Coronary collateral circulation (CCC) has been shown to have a prognostic role in acute myocardial infarction (MI). We aimed to identify factors associated with CCC development in patients with acute myocardial ischemia. In the present analysis, 673 consecutive patients aged 27 - 94 years (64.7±11.48) with acute coronary syndrome (ACS), who underwent coronary angiography within the first 24 hours after symptom onset were included. Baseline data, including sex, age, cardiovascular risk factors, medication, antecedent angina, prior coronary revascularization, EF%, blood pressure levels were obtained from patient medical records. The study individuals were divided into two groups: patients with Rentrop grade 0 to 1 were classified as the poor collateral group (456 patients), and the patients with grade 2 to 3 - as the good collateral group (217 patients). Prevalence of good collaterals of 32% was found. Odds of good collateral circulation increases with higher eosinophil count - OR=17.36 (95% CI: 3.25-92.86); history of MI (OR=1.76; 95% CI:1.13-2.75); multivessel disease - OR=9.78 (95% CI: 5.65-16.96); culprit vessel stenosis - OR=3.91 (95% CI: 2.35-6.52); presence of angina pectoris > 5 years - OR=5.55 (95% CI:2.66-11.57) and decreases with high N/L- OR=0.37 (95% CI:0.31-0.45) and male gender - OR=0.44 (95% CI:0.29-0.67). High N/L is a predictor of poor collateral circulation, with 68.4 sensitivity and 72.8% specificity (cutoff: 2.73*109). Relative chance of good collateral circulation increases with the higher number of eosinophils, presence of angina pectoris with duration of more than 5 years, history of past myocardial infarction, culprit vessel stenosis, multivessel disease, and reduces if patient is male and has high N/L ratio. Peripheral blood parameters may serve as an additional simple risk assessment tool in ACS patients.

The purpose of the study is to determine the correlation between the oral hygienic condition and the psycho-social factors in the elderly population of Imereti. The research was based on the findings of 374 persons who were older than 50 years of age from the contingent of Kutaisi Regional Dental Center and From the contingent of the boarding house of the Kutaisi elderly and disabled people. The oral hygienic level was determined by the method of GreeneVermilion, and we used the E. Ambjornsen method to evaluate hygienic conditions of removable prostheses. The difference between the groups was determined by F Fischer's criterion; The relationship between the factors was determined by the correlation analysis - the Spearerman's Rank Test. RESUMO

The purpose of the present work is to study the correlation between the risk factors of chronic recurrent aphthous stomatitis. The research was conducted on 62 patients between ages of 40 and 70 years at Tbilisi Hospital for Veterans of War. The analysis was carried out by Spearman's Rank Correlation method using the statistical package SPSS 11.5. We investigated: harmful habits, professional factors, background and accompanying illnesses, pathology of teeth, focal infection, emotional stress, genetic factors. Correlation matrix between the significant risk factors of chronic recurrent aphthous stomatitis is defined. Multiple correlations have the following factors: industrial dust, focal infections, emotional stress, anemia. Correlation diagram of etiological factors of chronic recurrent aphthous stomatitis is helpful for providing professional and expert services.

The purpose of the present work was to study the role of cellular and humoral immunity in patients with aphthous stomatitis. The research was conducted at Tbilisi Hospital for War Veterans. Immunologic parameters of 61 patients aged from 15 to 60 years old were analyzed. The statistical data processing included calculation of average arithmetic values and their standard deviations. Parameters of the immune status of the patients during exacerbation and under treatment were investigated. It was revealed that during exacerbation of stomatitis the quantity of CD3, CD4, cytophagous activity, and NK were diminished. Indices of CD8, CD72, and IgA, were within the norm. Indices of IgM, IgG, IgE, parameter of IL-6, antimicrobial and antitoxic antibody titer were increased. After treatment increase and approach to standard CD3, CD4, NK, and parameters of phagocytosis. IgG, IgM and antitoxic antibody titer decrease and approach to standard. During exacerbation of recurrent aphthous stomatitis the immune status of the human decrease, this is restored after treatment.

Open randomized 2-year trial has been performed to analyze effectiveness of Unguentum of a hesperidin developed by us for the treatment of recurrent aphthous stomatitis. 80 patients with recurrent aphthous stomatitis in the age from 15 till 60 years from the contingent of the polyclinic of the veterans of Tbilisi were studied. After inclusion of the preparation into the treatment the pain and duration of inflammatory process have been decreased; the complications were not observed. In the group of patients getting treatment with hesperidin, frequency of unsatisfactory outcomes decreased significantly (chi(2)=9,8, p < 0,05). Reduction of relative risk of unsatisfactory outcomes was about 0,8 (95% CI: 0,29-0,86), reduction of absolute risk - 0,33 (95% CI: 0,13-0,52). It may be concluded that application of a preparation may be considered as one of the options for the treatment of recurrent aphthous stomatitis.

RESUMO

On the basis of distribution of ABO blood groups among 963 sick children with dysplastic pathology of hip joint and 1402 Georgians from among the total population of the city of Tbilisi there have been obtained significant associations of 0 group with this pathology. The hypothesis about genetic community of these symptoms has been rejected. Phenotypic correlations have demonstrated that 0 group of blood promotes an expression of hereditary predisposition to the hip joint dysplastic pathology. Predisposition to this pathology is increased in case of coincidence of 0 group in the patient's mother and in the patient himself.

On the basis of data of 49 probands' families with dystrophic osseous, cyst, 12 probands' families with giant cell tumor and 55 children from the general population by means of clinical and multidimensional-genetical analysis (cluster, discriminant) have been revealed genetical and environmental factors, contributing to the formation of the osseous cysts in children. There have been developed the directions of the search of the "principal" environmental and genetical factors, determined the qualitative criteria of the factor significance, revealed the structures of the mutual connection of the significant factors and obtained their prognostication value definition.

In the article are discussed the causes of hip joint instability at breech presentation. In the article are presented the results of examination of 116 probands with hip joint dysplasia, aged 5-15 years, and 69 breech presentation born probands, It has been carried out genetic-correlative analysis on the basis of population frequency and frequency among the probands' relatives, differentiated in accordance with the signs discussed. The principal cause of the hip joint dysplastic pathology formation at breech presentation is the mechanical factor.
