OBJECTIVE: Vertical jump height and oral function affect the general muscle condition. This study aimed to evaluate the association between vertical jump height and oral function among healthy older individuals. BASIC RESEARCH DESIGN: Cross-sectional analytic study. PARTICIPANTS: 231 independent older people (mean age, 74.4 ± 5.6 years) who participated in the Kyoto Elders Physical Fitness Measurement Research Project. Individuals with partial or complete edentulousness who did not use a prosthetic device or complained of oral/maxillofacial pain were excluded from the study. INTERVENTIONS: Grip strength was measured using a Smedley Hand Dynamometer. To measure masticatory performance, the participants were instructed to chew a gummy jelly on their habitual chewing side (left or right) for 20 s. Occlusal force, contact area, and pressure were also assessed. MAIN OUTCOME MEASURES: The outcome variable was vertical jump height. The predictor variables were physical status (age, body mass index, and grip strength), oral status (number of present teeth and denture use), and oral function (masticatory performance, occlusal force, occlusal contact area, occlusal pressure, and tongue pressure). These relationships were evaluated with univariate analysis, and then multiple regression analysis was performed with age as the covariate for each male and female participant. RESULTS: Vertical jump height was significantly associated with grip strength in both men and women. Moreover, in women, it was associated with masticatory performance, occlusal force, and occlusal contact area. CONCLUSIONS: Vertical jump height was closely associated with oral function among healthy older women.

This study aimed to analyse occlusal force, area and pressure for individual maxillary and mandibular teeth by assessing their occlusal-supporting abilities. Ninety-nine subjects (49 men and 50 women; average age, 24·7 ± 3·4 years; range, 20-37 years) performed maximal voluntary clenching twice for 3 s in both the intercuspal position and the intercuspal position with sliding movement from edge-to-edge occlusion, with a pressure-sensitive sheet placed between the maxillary and mandibular dental arch. Occlusal force, area and pressure of individual teeth were calculated by colour development in the pressure-sensitive sheet with special analytical equipment and software. Occlusal contact condition of individual teeth was confirmed using the intra-occlusal record. All data were analysed using unpaired Student's t-test, Kruskal-Wallis test and Scheffe's test for multiple comparisons with a significance level of P < 0·05. The occlusal pressure in the intercuspal position with sliding movement from edge-to-edge occlusion was adopted as the representative occlusal-supporting ability for each individual tooth, although there were, in part, statistically significant differences in the effects of laterality, performance and gender. Overall, the occlusal pressure increased gradually from the central incisor, peaked at the canine or first premolar and decreased sharply-and significantly (P < 0·01)-towards the second molar. We conclude that the occlusal pressure of individual teeth can be used as an indicator of occlusal-supporting ability. This is therefore proposed as a possible suitable parameter for epidemiologic research, specifically for verifying the relationship between occlusal-supporting ability and status of residual periodontal ligament support.

We previously reported the expression of hepatocyte growth factor (HGF), a ligand for c-MET, at the level of mRNA and protein in human prostate tissues. The present study was carried out to evaluate the relationship between c-MET expression and cancer cell proliferation or effect of cancer therapy. The expression both in mRNA and protein levels of c-MET proto-oncogene was determined semi-quantitatively by reverse transcription-competitive polymerase chain reaction (RT-competitive PCR) and Western blot analysis for prostate tissues from 32 Japanese male subjects. In addition, tissue localization of c-MET translation product was examined by immunohistochemistry for corresponding specimens. Although there was significantly higher c-MET protein expression in malignant (prostate cancer treated with/without neoadjuvant endocrine therapy) than in non-malignant prostate tissues (normal prostate and benign prostate hyperplasia; BPH), unexpectedly, c-MET mRNA showed high expression in the non-malignant group. Thus, there was no parallelism between mRNA and protein expressions of c-MET. Endocrine therapy did not alter c-MET mRNA and protein expressions in human prostate cancer. Immunohistochemical localization and expression of c-MET protein was found to be intense in cancer cells and weak in epithelia of normal and hyperplastic prostates. Unconcerted expression of mRNA and protein of c-MET, the reason of which is uninterpretable, is supposed to be one of characteristics of human prostate cancer.

Hepatocyte growth factor (HGF) has been revealed to have various functions such as regeneration, cancer invasion and tumor suppression in normal and cancer cells of different organs. To compare HGF expression in non-malignant prostate tissues and prostate cancers pretreated with or without neoadjuvant endocrine therapy (pretreated PC and non-treated PC), the amounts of HGF protein and mRNA were quantitated by Western blot analysis and reverse transcription-competitive polymerase chain reaction. The biologically active HGF (baHGF), which was converted by an HGF activator protein from secretory type proHGF, showed significantly higher expression in pretreated PC than in non-treated PC. The amounts of baHGF in pretreated PC were similar to those in non-malignant prostate tissues. There was no difference in the expression level of full-length HGF mRNA encoding secretory type HGF among the three groups. No variance of tissue content of short variant HGF mRNA encoding a competitive HGF antagonist among the three groups indicated that short variant HGF protein in prostate cancer seemed not to function similarly to a competitive HGF antagonist in benign prostate tissues. These results suggest that the tissue level of baHGF in non-treated and pretreated PC depends on activation of proHGF supplied from distal organs rather than de novo HGF mRNA synthesis in prostate glands. Increased baHGF in pretreated PC is expected to relate to tumor suppression in vivo.

RESUMO

An 86-year-old woman was admitted to our hospital with genitourinary bleeding. Abdominal computerized tomography demonstrated a low density mass on the posterior wall of the bladder. Cystoscopy revealed a nonpapillary bladder tumor. A transurethral cold cup biopsy showed lymphocytic infiltration with no malignant cells. She died 2 years after the initial presentation and autopsy revealed malignant lymphoma, diffuse B cell type, small cell type. A cold cup biopsy of the bladder mucosa sometimes fails to detect primary malignant lymphoma of the bladder because of its submucosal location.

Ninety six rats were divided into 4 groups (normal control group (N = 4), ligation group (N = 20), ligation-removed group (N = 36), and sham surgery group (N = 36)). The neuronal size of the 4-week ligated rats returned to normal 6 weeks after removal of the ligation. Rats ligated for 7, 11 and 20 weeks also exhibited a significant increase in the mean area but they could not make a complete recovery after removal of ligation for 6 weeks. Only the rats in the 1- and 4-week ligation groups could recover their neuronal size. Area densities of smooth muscle: connective tissue (ADsc ratio) was calculated using Elastica van Gieson staining sections of bladder strips. When ligation was removed, the ratio dropped and became nearly equal to the value of the sham surgery group. Only the 7-week group showed a lower ADsc ratio than the sham surgery group after the ligation was removed. These findings suggest that the reversibility of neuronal hypertrophy might become irreversible when the period of partial urethral ligation persisted beyond 7 weeks. Irreversible hypertrophy of major pelvic ganglion (MPG) neurons might be greatly related with the irreversibly enlarged fibrous bladder which could not show the same ADsc ratio as was seen in the sham surgery groups.

Serum prostate specific antigen (PSA) and prostatic acid-phosphatase (PAP) levels in normal controls, and patients with prostate cancer, benign prostate hypertrophy (BPH) and other urological diseases were examined with a newly developed latex turbidimetric immunoassay (LPIA ACE PSA, LPIA ACE PAP, IATRON LABORATORIES, INC., Tokyo, Japan). The advantageous characteristics of this method are small amount (10 microliters) of serum required and short time (about 20 min.) for performing this assay. There was a high linear correlation between LPIA ACE PSA and MARKIT-F PA (r2 = 0.953), between LPIA ACE PSA and TANDEM-E PSA (r2 = 0.881) and between LPIA ACE PAP and ABBOTT-PAP EIA (r2 = 0.946). When the BPH patients (n = 110) were used as negative controls, the cut-off value of PSA was determined to be 4.3 ng/ml. Using this level as the cut-off value, the sensitivity was 78% (42 positive/54 untreated prostate cancer patients), specificity (negative rate in BPH patients) was 95% and efficiency was 89%. In a follow-up study of prostate cancer, the PSA value was elevated above the cut-off value in 68% at the time of clinical progression. These findings suggest that LPIA ACE PSA is a useful tool for serum PSA measurement. The cut-off value of PAP measured with LPIA ACE PAP was 9.0 ng/ml, which was determined by the same method as PSA. The sensitivity, specificity and efficiency ware 39%, 96% and 77%, respectively. These findings indicate that PAP is less useful than PSA in the diagnosis of prostate cancer.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMO

A clinical statistic survey was carried out on the patients, diseases and operations experienced at the ward of the Department of Urology, Fukui Medical School between 1983 and 1993. The numbers of inpatients and operations have been increasing since 1991. The majority of the inpatients are those with neoplastic disorders, and the number is steadily increasing every year. After the development of new endourological technology and ESWL, the mode of operation has dramatically changed during the 10 years, trending toward minimally invasive surgery and improvement of the patient's quality of life.

Four patients with upper urinary tract transitional cell carcinoma were treated with bacillus Calmette-Guerin (BCG) via a percutaneous nephrostomy tube or a retrograde ureteral catheter. A 68-year-old female and an 80-year-old male had carcinoma in situ (CIS) in the left upper urinary tract (cases 1 and 2). A man aged 47 had CIS in the left upper urinary tract, bladder, and prostatic urethra (case 3). CIS in the left upper urinary tract was identified in a woman aged 63 with chronic renal insufficiency (case 4). Two patients (cases 1 and 2) responded to this therapy. In the other two patients nephrectomy was performed due to residual tumor. There were extensive tuberculous granulomas in the kidneys. In one resected kidney (case 4) carcinoma had invaded the renal parenchyma. The reviewed literature showed that BCG perfusion therapy was effective in 71% (27 of 38 renal units) for the upper urinary tract tumors and that there were 5 cases of severe complication, including sepsis in 2, high fever in 2, and ureteral stricture in 1. Based on the fact that the kidney receives a profuse blood supply and that the renal pelvis and ureter have a thin wall, careful management is mandatory to prevent severe adverse effects and insidious tumor progression.

RESUMO

A clinical statistic survey was carried out on the patients and disease experienced at the outpatient clinic of our Department of Urology, between 1983 and 1993. Although the number of newly diagnosed patients was rather constant, the total number of outpatients gradually increased every year, and the average and median age of newly diagnosed patients became higher. The major disease categories were infectious and neoplastic disorders.
