RESUMO

Non-digestible milk oligosaccharides were proposed as receptor decoys for pathogens and as nutrients for beneficial gut commensals like bifidobacteria. Bovine milk contains oligosaccharides, some of which are structurally identical or similar to those found in human milk. In a controlled, randomized double-blinded clinical trial we tested the effect of feeding a formula supplemented with a mixture of bovine milk-derived oligosaccharides (BMOS) generated from whey permeate, containing galacto-oligosaccharides and 3'- and 6'-sialyllactose, and the probiotic Bifidobacterium animalis subsp. lactis (B. lactis) strain CNCM I-3446. Breastfed infants served as reference group. Compared with a non-supplemented control formula, the test formula showed a similar tolerability and supported a similar growth in healthy newborns followed for 12 weeks. The control, but not the test group, differed from the breast-fed reference group by a higher faecal pH and a significantly higher diversity of the faecal microbiota. In the test group the probiotic B. lactis increased by 100-fold in the stool and was detected in all supplemented infants. BMOS stimulated a marked shift to a bifidobacterium-dominated faecal microbiota via increases in endogenous bifidobacteria (B. longum, B. breve, B. bifidum, B. pseudocatenulatum).

From 2005, PACA area has formed think-tank group a priori risk in the transfusional chain. It has enabled to determine each step of the elementary process and evaluate the frequency, the seriousness and critical effect of the errors. Blood sample and conformity are the most critical points and depend on the vigilance identity. In September 2007, the southern blood bank of France has settled 12 nonconformities levels of blood samples. They send the listing of the nonconformities every month. This listing enables the executive staff to determine the errors and, therefore, to solve them. The regional notification of 2007 to 2008 confirms analysis of the think-tank team. Hence, we were able to list the most serious cases. Public and private hospitals have to notify the serious events and will be bound to evaluate professional practices. These acts will be taken into account by the regional-medical contract.

In this report we rate a new, third-generation automated hematology system (Technicon Instruments H-1) that can furnish a full range of values, including erythrocyte parameters and a leukocyte differential count. Particular attention is focused on erythrocyte morphometric parameters, including measurement of cell size and hemoglobin content on a cell-by-cell basis. We assess the usefulness of new parameters derived from these measurements, such as mean corpuscular volume and red blood cell distribution width, which characterize cell size, and mean corpuscular hemoglobin concentration, and hemoglobin distribution width, which characterize cell hemoglobinization in evaluating normal and abnormal subjects. The value of these parameters in classifying anemias is assessed in our patient population that includes those with iron deficiency anemias and thalassemias, as well as other forms of anemia.

Erythrocyte deformability was studied by the filtration technique of Reid & Dormandy using whole blood and washed erythrocytes from insulin-dependent diabetics (IDD) under insulin delivery by an artificial pancreas (AP). The same technique was employed to study deformability in vitro using normal erythrocytes incubated in the presence of insulin. Results of this study show that in IDD the initially poor erythrocyte deformability is improved within hours of insulin administration. Improved deformability was accompanied by increased levels of intra-erythrocyte ATP but without changes in levels of HbG and 23 DPG. Incubation of erythrocytes in medium containing glucose showed that deformability was significantly improved in the presence of insulin. These results indicate that insulin favourably affects erythrocyte deformability in IDD. Before and after 24 hours treatment by AP, platelet aggregation was studied in IDD by the technique of Born using platelet-rich plasma (PRP) and by a modified Breddin technique using PRP, whole blood or whole blood treated by chlorpromazine and mixtures of erythrocytes from IDD with normal PRP. Platelet hyperaggregation was only found in the presence of erythrocytes from untreated diabetics. Chlorpromazine, at a dose (10 mumole) which inhibits haemolysis without inducing platelet hyperaggregation, eliminated the above anomaly. In conclusion, it is conceivable that the insulin-induced correction of poor erythrocyte deformability eliminates excessive fragility of erythrocytes and their haemolysis wit release of ADP, thus avoiding platelet hyperaggregation.

RESUMO

Five months experience with the Hemalog D has enabled the authors to define the mode of integration of the apparatus into a central hematology laboratory of a hospital without a clinical hematology department. The incidence and importance of such factors as the alarms set off by the apparatus, the time-course of blood basophils in patients receiving heparin and the clinical picture in patients for whom the apparatus revealed a peroxidase deficiency are discussed.

Two cases of the development of anti-VIII antibody during the postoperative period are reported. The methods used to demonstrate the antibodies, the different aetiologies, the course and current treatment are reviewed. The cause was found in only one of the two cases.

RESUMO

The daily use of the D Hemalog machine allowed us to recognize thirty-three cases of myeloperoxydase deficiency, on the basis of different "alarms", the most important of which is called LPX (Low Peroxydase). Some patients had a blood disease which was lacking among the other subjects. In no case the enzymatic defect has led to a clinical infection.
