RESUMO

Integrative genetic association methods have shown great promise in post-GWAS (genome-wide association study) analyses, in which one of the most challenging tasks is identifying putative causal genes and uncovering molecular mechanisms of complex traits. Recent studies suggest that prevailing computational approaches, including transcriptome-wide association studies (TWASs) and colocalization analysis, are individually imperfect, but their joint usage can yield robust and powerful inference results. This paper presents INTACT, a computational framework to integrate probabilistic evidence from these distinct types of analyses and implicate putative causal genes. This procedure is flexible and can work with a wide range of existing integrative analysis approaches. It has the unique ability to quantify the uncertainty of implicated genes, enabling rigorous control of false-positive discoveries. Taking advantage of this highly desirable feature, we further propose an efficient algorithm, INTACT-GSE, for gene set enrichment analysis based on the integrated probabilistic evidence. We examine the proposed computational methods and illustrate their improved performance over the existing approaches through simulation studies. We apply the proposed methods to analyze the multi-tissue eQTL data from the GTEx project and eight large-scale complex- and molecular-trait GWAS datasets from multiple consortia and the UK Biobank. Overall, we find that the proposed methods markedly improve the existing putative gene implication methods and are particularly advantageous in evaluating and identifying key gene sets and biological pathways underlying complex traits.

BACKGROUND: Liver cancer is a highly lethal and aggressive form of cancer that poses a significant threat to patient survival. Within this category, liver hepatocellular carcinoma (LIHC) represents the most common subtype of liver cancer. Despite decades of research and treatment, the overall survival rate for LIHC has not significantly improved. Improved models are necessary to differentiate high-risk cases and predict possible treatment options for LIHC patients. Recent studies have identified a set of genes associated with neutrophil extracellular traps (NETs) that may contribute to tumor growth and metastasis; however, their prognostic value in LIHC has yet to be established. This study aims to construct a prognostic signature based on a set of NET-related genes (NRGs) for patients diagnosed with LIHC. METHODS: The transcriptomic data and clinical information concerning LIHC patients were procured from the Cancer Genome Atlas (TCGA) and International Cancer Genome Consortium LIHC (ICLIHC) databases, respectively. To determine the NRG subtypes, the k-means algorithm was employed, along with consensus clustering. The aforementioned analysis aided the construction of a prognostic signature utilizing the last absolute shrinkage and selection operator Cox analysis. To validate the prognostic model, an external dataset, receiver operating characteristic curve, and principal component analysis were utilized. Moreover, the immune microenvironment and the proportion of immune cells between high- and low-risk cases were scrutinized by ESTIMATE and CIBERSORT algorithms. Finally, gene set enrichment analysis was executed to investigate the potential mechanism of NRGs in the pathogenesis and prognosis of LIHC. RESULTS: Two molecular subtypes of LIHC were identified based on the expression patterns of differentially expressed NRGs (DE-NRGs). The two subtypes demonstrated significant differences in survival rates and immune cell expression levels. The study results demonstrated the role of NRGs in antigen presentation, which led to the promotion of tumor immune escape. A risk model was developed and validated with strong overall survival prediction ability. The model, comprising 34 NRGs, showed a strong ability to predict prognosis. CONCLUSION: We built a dependable prognostic signature based on NRGs for LIHC. We identified that NRGs could have a significant interaction in LIHC's immune microenvironment and therapeutic response. This finding offers insight into the molecular mechanisms and targeted therapy for LIHC.

A copper-catalyzed [3 + 2] annulation  reaction of exocyclic enamines/enol ethers with 1,4-benzoquinone esters has been developed, providing facile access to N,O- spiroketals and spiroketals under mild conditions with broad substrate scope (26 examples, 71-94% yields). Gram scale synthesis and chemical transformations demonstrated that this method is potentially useful in the synthesis of natural products and drugs containing a N,O- spiroketal moiety. The chiral N,O-spiroketal could be obtained with 98% ee after recrystallization, when a chiral SaBOX ligand was employed.

Both cis- and trans- tetracyclic spiroindolines are the core of many important biologically active indole alkaloids, but the divergent synthesis of these important motifs is largely hampered by the limited stereoselectivity control. A facile stereoinversion protocol is reported here in Michael addition-initiated tandem Mannich cyclizations for constructing tetracyclic spiroindolines, providing an easy access to two diastereoisomeric cores of monoterpene indole alkaloids with high selectivity. The mechanistic studies including in situ NMR experiments, control experiments, and DFT calculations reveal that the reaction undergoes a unique retro-Mannich/re-Mannich rearrangement including a C-C bond cleavage that is very rare for a saturated six-membered carbocycle. Insights into the stereoinversion process have been uncovered, and the major effects were determined to be the electronic properties of N-protecting groups of the indole with the aid of Lewis acid catalysts. By understanding these insights, the stereoselectivity switching strategy is also smoothly applied from enamine substrates to vinyl ether substrates, which are enriched greatly for the divergent synthesis and stereocontrol of monoterpene indole alkaloids. The current reaction also proves to be very practical and was successfully applied to the gram-scale total synthesis of strychnine and deethylibophyllidine in short routes.

BACKGROUND: Thermal ablation is a minimally invasive and safe treatment for benign thyroid nodules, and the volume reduction rate (VRR) of nodule is a primary clinical efficacy indicator. PURPOSE: To screen factors influencing VRR in benign thyroid nodules after thermal ablation and establish a predictable nomogram. MATERIALS AND METHODS: This retrospective study enrolled 238 patients with benign thyroid nodules who underwent thermal ablation between January 2016 and September 2021. Clinical information and imaging characteristics in routine ultrasound (US) and contrast-enhanced ultrasound (CEUS) were evaluated. Factors influencing the VRR ≥75% were screened using multivariate logistic regression, and a predictable nomogram was established. RESULTS: At the 12-month follow-up, the VRR of nodule was 77.0 ± 20.6% (18.4-100%). Seven factors influencing the VRR ≥75.0% were identified: echogenicity, component, calcification, enhancement degree, enhancement defect, ring enhancement, and energy of ablation. A nomogram was established based on the above factors, and the predictive ability of the model was confirmed by internal validation with 1000 bootstrap repetitions. The area under the receiver-operating characteristic curve (AUC) of the model was 0.926, and the calibration curve and decision curve analysis (DCA) revealed that this model demonstrated predictive ability. CONCLUSION: Seven factors influencing VRR in benign thyroid nodules after thermal ablation were screened out in the present study and used to establish a nomogram to predict the probability of VRR ≥75% at the 12-month follow-up. It would be beneficial to make personalized medical decisions to trigger thermal ablation in patients with benign thyroid nodules.

Background: Isolated pulmonary nodules (SPNs) are small, circular lesions within lung tissue, often challenging to diagnose due to their size and lack of typical imaging features. Timely diagnosis is crucial for treatment decisions. However, the difficulty in qualitative diagnosis necessitates clinical biopsies. Objective: This study aimed to assess the diagnostic accuracy of CT-guided percutaneous lung biopsy for SPNs and identify potential risk factors for malignancy. Methods: We conducted a retrospective analysis of 112 patients with SPNs who underwent CT-guided core needle biopsy (CT-CNB) between June 2020 and June 2022. Histological and cytological results were obtained for all patients, and clinical data and imaging characteristics were compared between benign and malignant SPN groups. Binary logistic regression was used to analyze risk factors for malignancy, and complications were observed. Results: Cytological and histological specimens were successfully obtained for all patients. The cohort consisted of 43 patients with benign SPNs and 69 with malignant SPNs. Among the malignant SPN group, 67 cases were confirmed via CT-CNB and 2 through surgery, resulting in a sensitivity of 97.10% and specificity of 100.00%. The malignant nodules comprised 45 adenocarcinomas, 14 squamous cell carcinomas, 8 metastatic tumors, and 2 small cell carcinomas. Notably, 2 initially diagnosed as malignant cases were found to have chronic inflammation on preoperative biopsy but revealed adenocarcinoma and squamous cell carcinoma post-surgery. The benign nodules encompassed 20 granulomatous inflammation cases, 15 chronic inflammation, 3 fungal granulomas, 2 hamartomas, and 1 fibrous tissue. Cytological smears exhibited a sensitivity of 81.3% and a specificity of 100.0% for malignancy. Significantly, age ≥60, elevated tumor markers, and specific imaging signs (burr, foliation, pleural pull) were identified as risk factors for malignant SPNs using Binary Logistic regression (all P < .05). Conclusions: CT-guided percutaneous lung biopsy demonstrates excellent diagnostic efficacy and safety for distinguishing benign and malignant SPNs.

BACKGROUND: Cardiac cine magnetic resonance (CCMR) imaging plays an important role in the clinical cardiovascular disease (CVD) examination and evaluation. OBJECTIVE: To accurately reconstruct the displacement field and describe the motion of the left ventricular myocardium (LVM), this study proposes and tests a new approach for tracking myocardial motion of the left ventricle based on a biomechanical model. METHODS: CCMR imaging data acquired from 103 patients are enrolled, including two simulated and 101 clinical data. A non-rigid image registration method with a combination of a thin-plate spline function and random sample consensus is used to recover the observed displacement field of LVM. Next, a biomechanical model and a material matrix are introduced to solve the dense displacement field of LVM using a finite element framework. Then, the tracking precision and error of results for the two groups are analyzed. RESULTS: Displacement results of the simulated data show correlation coefficient≥0.876 and mean square error≤0.159, while displacement results of the clinical data show Dice≥0.97 and mean contour distance≤0.464. Additionally, the strain results show correlation coefficient≥0.717. CONCLUSIONS: This study demonstrates that the proposed new method enables to accurately track the motion of the LVM and evaluate strain, which has clinical auxiliary value in the diagnosis of CVD.

A facile method for the construction of the aspidosperma core from indoles functionalized with a nonterminal N-allenamide and dimethyl methylenemalonate is described. Various polysubstituted tetracyclic spiroindolines (27 examples) were afforded in good yields (61-90 %) with >99/1 dr and >99/1 Z/E selectivity under mild conditions. The annulation reaction provides straightforward access to the tetracyclic spiroindoline skeleton with substituents at the C5 position occurring in many natural products. As an application of this reaction, the total synthesis of three important natural products, (-)-deoxoapodine, (-)-kopsifoline D and (±)-melotenine A, was possible in short routes from tryptamine.

The substantial social and economic burden attributable to smoking is well-known, with heavy smokers at higher risk of chronic disease and premature mortality than light smokers and nonsmokers. In aging societies with high rates of male smoking such as in East Asia, smoking is a leading preventable risk factor for extending lives (including work-lives) and healthy aging. However, little is known about whether smoking interventions targeted at heavy smokers relative to light smokers lead to disproportionately larger improvements in life expectancy and prevalence of chronic diseases and how the effects vary across populations. Using a microsimulation model, we examined the health effects of smoking reduction by simulating an elimination of smoking among subgroups of smokers in South Korea, Singapore, and the United States. We found that life expectancy would increase by 0.2 to 1.5 years among light smokers and 2.5 to 3.7 years among heavy smokers. Whereas both interventions led to an increased life expectancy and decreased the prevalence of chronic diseases in all three countries, the life-extension benefits were greatest for those who would otherwise have been heavy smokers. Our findings illustrate how smoking interventions may have significant economic and social benefits, especially for life extension, that vary across countries.

Due to the high spatiotemporal resolution , cardiac cine magnetic resonance imaging (CCMRI) has been widely used to evaluate the cardiac function of cardiovascular diseases such as myocardial ischemia and so on. Segmentation-based motion tracking of left myocardium is very important for comprehensive evaluation of cardiac function in the diagnosis and treatment of cardiovascular diseases. However, it is a challenge to track motion of left myocardium, which is homogeneous and cannot provide effective motion information. In this paper, CCMRI imaging techniques for myocardial motion tracking are introduced firstly. Then approaches for motion tracking of left myocardium based on CCMRI image are described in details and are summarized and prospected at the end, which not only helps beginners to have a quick and comprehensive understanding on this topic, but also provides theoretical reference to related researchers for further optimization of approaches for motion tracking of left myocardium. From the current study, motion tracking approaches for left myocardium based on CCMRI image make comprehensive use of the spatiotemporal motion characteristics of CCMRI image, the motion and structures of myocardium of left ventricle and so on, which can make up for the shortcomings of sparse motion information of CCMRI image. However, it still needs improved constraint framework, verification methods and so on.

RESUMO

A highly enantioselective synthesis of various chiral heterobicyclic molecules including spiroaminals and fused bicyclic acetals has been developed via a chiral copper catalyzed cyclopropanation-rearrangement (CP-RA) approach under mild reaction conditions. Remarkably, the asymmetric CP-RA for exocyclic vinyl substrates without a pro-stereogenic carbon at the ß-position has been realized for the first time and a broad substrate scope with excellent results (33 examples; 34-99 % yields; >95/5 dr and 91-99 % ee) has been achieved. An application of a successive enantioselective CP-RA approach was also described, providing a concise access to complex chiral heteropolycycles.

BACKGROUND: Atezolizumab (a monoclonal antibody against PD-L1), which restores anticancer immunity, improved overall survival in patients with previously treated non-small-cell lung cancer and also showed clinical benefit when combined with chemotherapy as first-line treatment of non-small-cell lung cancer. IMpower130 aimed to assess the efficacy and safety of atezolizumab plus chemotherapy versus chemotherapy alone as first-line therapy for non-squamous non-small-cell lung cancer. METHODS: IMpower130 was a multicentre, randomised, open-label, phase 3 study done in 131 centres across eight countries (the USA, Canada, Belgium, France, Germany, Italy, Spain, and Israel). Eligible patients were aged 18 years or older, and had histologically or cytologically confirmed stage IV non-squamous non-small-cell lung cancer, an Eastern Cooperative Oncology Group performance status of 0 or 1, and received no previous chemotherapy for stage IV disease. Patients were randomly assigned (2:1; permuted block [block size of six] with an interactive voice or web response system) to receive atezolizumab (1200 mg intravenously every 3 weeks) plus chemotherapy (carboplatin [area under the curve 6 mg/mL per min every 3 weeks] plus nab-paclitaxel [100 mg/m2 intravenously every week]) or chemotherapy alone for four or six 21-day cycles followed by maintenance therapy. Stratification factors were sex, baseline liver metastases, and PD-L1 tumour expression. Co-primary endpoints were investigator-assessed progression-free survival and overall survival in the intention-to-treat wild-type (ie, EGFRwt and ALKwt) population. The safety population included patients who received at least one dose of the study drug. This study is registered with ClinicalTrials.gov, number NCT02367781. FINDINGS: Between April 16, 2015, and Feb 13, 2017, 724 patients were randomly assigned and 723 were included in the intention-to-treat population (one patient died before randomisation, but was assigned to a treatment group; this patient was excluded from the intention-to-treat population) of the atezolizumab plus chemotherapy group (483 patients in the intention-to-treat population and 451 patients in the intention-to-treat wild-type population) or the chemotherapy group (240 patients in the intention-to-treat population and 228 patients in the intention-to-treat wild-type population). Median follow-up in the intention-to-treat wild-type population was similar between groups (18·5 months [IQR 15·2-23·6] in the atezolizumab plus chemotherapy group and 19·2 months [15·4-23·0] in the chemotherapy group). In the intention-to-treat wild-type population, there were significant improvements in median overall survival (18·6 months [95% CI 16·0-21·2] in the atezolizumab plus chemotherapy group and 13·9 months [12·0-18·7] in the chemotherapy group; stratified hazard ratio [HR] 0·79 [95% CI 0·64-0·98]; p=0·033) and median progression-free survival (7·0 months [95% CI 6·2-7·3] in the atezolizumab plus chemotherapy group and 5·5 months [4·4-5·9] in the chemotherapy group; stratified HR 0·64 [95% CI 0·54-0·77]; p<0·0001]). The most common grade 3 or worse treatment-related adverse events were neutropenia (152 [32%] of 473 in the atezolizumab plus chemotherapy group vs 65 [28%] of 232 in the chemotherapy group), anaemia (138 [29%] vs 47 [20%]), and decreased neutrophil count (57 [12%] vs 19 [8%]). Treatment-related serious adverse events were reported in 112 (24%) of 473 patients in the atezolizumab plus chemotherapy group and 30 (13%) of 232 patients in the chemotherapy group. Treatment-related (any treatment) deaths occurred in eight (2%) of 473 patients in the atezolizumab plus chemotherapy group and one (<1%) of 232 patients in the chemotherapy group. INTERPRETATION: IMpower130 showed a significant and clinically meaningful improvement in overall survival and a significant improvement in progression-free survival with atezolizumab plus chemotherapy versus chemotherapy as first-line treatment of patients with stage IV non-squamous non-small-cell lung cancer and no ALK or EGFR mutations. No new safety signals were identified. This study supports the benefit of atezolizumab, in combination with platinum-based chemotherapy, as first-line treatment of metastatic non-small-cell lung cancer. FUNDING: F. Hoffmann-La Roche.

OBJECTIVE: Radiogenomics investigates radiographic imaging phenotypes associated with gene expression patterns. This study aims to explore relationships between CT imaging radiomics features and gene expression data in non-small cell lung cancer (NSCLC). METHODS: Eighty-nine NSCLC patients are included in the study. Radiomics features are extracted and selected to quantify the phenotype of tumors on CT-scans. Co-expressed genes are also clustered and the first principal component of the cluster is represented, which is defined as a metagene. Then, statistical analysis was performed to assess association of CT radiomics features with metagenes. In addition, predictive models are built and metagene enrichment are conducted to further evaluate performance of NSCLC radiogenomics statistically and biologically. RESULTS: There are 187 significant pairwise correlations between a CT radiomics feature and a metagene of NSCLC, where eighteen metagenes are annotated with Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) terms. Metagenes are predicted in terms of radiomics features with an accuracy of 41.89% -89.93%. CONCLUSIONS: This study reveals the associations between CT imaging radiomics features and NSCLC co-expressed gene sets. The findings suggest that CT radiomics features can reflect important biological information of NSCLC patients, which may have a significant clinical impact as CT is routinely used in clinical practice, assisting in improving medical decision-support at low cost.

OBJECTIVE: To study the expression of the Ces5a gene in the development of the rat testis. METHODS: Using RT-PCR, Western blot, immunohistochemistry and HE staining, we determined the mRNA transcription level, protein expression and localization of the Ces5a gene in the testes of three litters of rats at different postnatal (PN) days. RESULTS: The expression of Ces5a mRNA was found in the testis tissue of the rats at 2-65 PN days, low at 2-12 days, decreased to the lowest level at 14-16 days (P < 0.05), but significantly increased at 20-35 days (P < 0.05), and elevated to the highest level at 40-65 days (P < 0.05). The expression of the Ces5a protein was also observed in the testis tissue of the rats at 2-65 PN, low at 2-12 days, with no significant change at 14-16 days (P > 0.05), but markedly increased at 20-35 days (P < 0.05), and again with no significant change at 40-65 days (P > 0.05). The Ces5a protein was expressed in the spermatogonia, spermatocytes and round sperm cells. CONCLUSIONS: The Ces5a gene may be involved in the proliferation and meiosis of rat spermatogonia and play a special role in round spermatogenesis and sperm deformation.

RESUMO

A highly diastereoselective copper-catalyzed multicomponent cyclization of exocyclic enol ethers/enamines with methylene malonate and aldehydes has been developed to furnish spiroheterocyclic tetrahydropyrans in high yields with greater than 95:5 d.r. This method is practical, in that 36 examples, including a range of aldehydes and exo-vinyl heterocycles, are presented. By applying the newly developed method, the total synthesis of (+)-broussonetine G and formal synthesis of (+)-broussonetine H were achieved in a concise way.

A selectivity switch in a RhII /carbene-triggered cyclopentannulation with catalytic InCl3 is reported for the first time, affording both diastereomers of the fused spiroindolines and an unusual bridged tetracyclic indoline in high yields with excellent selectivities. Mechanistic studies indicate an intramolecular annulation of the indole with an in situ formed aminocyclopropane. The stepwise thermal conversions from the kinetic spiroindoline to the metastable bridged indoline, and then to the thermodynamic spiroindoline, involving a ring-opening rearrangement of a cyclopentane, is crucial for selectivity control.

Here we report a novel drug delivery system using mesoporous inner-empty upconversion microspheres carrying drugs with ZnO quantum dots (UCMPS@ZnO) acting as a 'door-keeper' for pH-triggered drug release. Compared to other upconversion drug delivery systems, it is smarter, simpler, more efficient and more biocompatible. In particular, the UCMPS@ZnO microspheres show low cytotoxicity, are simple to produce and have a high drug loading rate (15%). Promisingly, a mice treatment experiment demonstrated that the multifunctional nanoparticles have efficient inhibition of tumor growth after a 14-day treatment. This makes UCMPS@DOX-ZnO microspheres a promising theranostic candidate in cancer treatment and clinical trials.

OBJECTIVE: This study aims to establish an experimental mouse model of minimal persistent inflammation (MPI), observe the features of inflammation and hyper-responsiveness of the upper/lower airways, and explore the relationship between inflammation and hyper-responsiveness in the upper/lower airways. METHODS: Sixty-four female BALB/c mice were randomly divided into four groups: allergic rhinitis (AR) group as positive control, MPI group, negative control group and blank control group. Mice were given high and low-concentrated ovalbumin solution after basic and intensive sensitization to establish AR model and MPI model. Nasal mucosa and lung tissues were stained to observe eosinophil infiltration, goblet cell hyperplasia, and expression of intercellular adhesion molecule 1 (ICAM-1). Airway hyper-responsiveness was assessed. Levels of specific immunoglobulin E (sIgE), interleukin (IL)-4 and IL-5 in peripheral blood, nasal lavage fluid (NLF), and bronchoalveolar lavage fluid (BALF) were detected by Enzyme-linked immunosorbent assay. RESULTS: The eosinophil infiltration and expression of ICAM-1 on nasal mucosa and in lung tissues in the AR and MPI groups were significantly elevated compared to control groups. Goblet cells count increased only in the nasal mucosa and not in lung tissues. Eosinophil and neutrophil count of NLF and BALF in the AR and MPI groups increased significantly compared to control groups. Level of IL-4 did not increase significantly, but sIgE and IL-5 did. CONCLUSIONS: Mice in the MPI status exhibits lower airway inflammation and hyper-responsiveness with increase in eosinophil count, goblet cells, ICAM-1, IL-4, and IL-5. These results provide further evidence for the importance of MPI of AR in lower airway diseases.

Despite their powerful antiinflammatory effect, glucocorticoids have shown no significant clinical benefit in respiratory syncytial virus (RSV)-induced bronchiolitis, the reason for which remains unclear. Upon glucocorticoid binding, the cytoplasmic glucocorticoid receptor (GR) translocates to the nucleus with the help of importin 13 (IPO13). Here, we report that RSV infection reduced GR nuclear translocation in nasopharyngeal aspirates from RSV-infected infants, lungs of infected mice, and A549 cells, which coincided with decreased IPO13 expression. This led to repression of GR-induced antiinflammatory genes, such that dexamethasone failed to suppress airway inflammation and airway hyperresponsiveness in the infected mice. The anti-GR effect of RSV was mediated by viral nonstructural protein 1 , which likely functioned by competing with IPO13 for GR binding. Our findings provide a mechanism for the ineffectiveness of glucocorticoids in RSV-related disease and highlight the potential to target the IPO13-GR axis as a treatment for multiple glucocorticoid-related diseases.

RESUMO

A facile and efficient method to produce optically pure benzofuroindolines, especially those without 3-substituents that are susceptible to rearomatization, through [3+2] annulation of indoles with quinones is described. The suitable combination of a BOX ligand CuII hydrate complex and freshly activated molecular sieves functions to give controllably dynamic release of water, which enables the success of this reaction. This reaction can be performed on a gram scale with only 0.5 mol % catalyst loading.
