RESUMO

PIP: This paper examines the disparity and problem of infant and child mortality in 2 rural districts in Kenya: the Siaya District where those under 5 have 4.7 times the probability of dying than in the Nyeri District, a lower mortality area. By using 1980 survey data with multiple regression analysis the key variables that emerged showed that district, education and position of mother in the household were significantly related to child mortality. However, there was still the need for socio-economic data to explain the variance in child mortality rates between the 2 districts. Administrative problems that compound the registration of deaths include the collection of data, the under-reporting of deaths and the different methods of registering causes of deaths. The study determined causes of deaths by: 1) examining the proportions of deaths among infants and children 1-4 that were case or group-related; and 2) calculating age-specific mortality rates by cause. The causes were classified into 11 categories. For infants (39%) and children 1-4 (45%) infectious and parasitic diseases account for the largest part of the difference in death rates; diarrhea is 4th for both age groups; and cardiovascular diseases and "all other and unknown" are 2 or 3 in both age groups. An analysis of measles and malaria are included because of their significance in child morbidity and mortality rates. Measles led the investigation to nutritional deficiencies and the effects of domestic overcrowding on disease in both Nyeri and Siaya, while an analysis of malaria focused on the importance of socioeconomic factors which were found to outweigh the environmental ones in Siaya.^ieng
