RESUMO

Volunteering provides unique benefits to organisations, recipients, and potentially the volunteers themselves. This umbrella review examined the benefits of volunteering and their potential moderators. Eleven databases were searched for systematic reviews on the social, mental, physical, or general health benefits of volunteering, published up to July 2022. AMSTAR 2 was used to assess quality and overlap of included primary studies was calculated. Twenty-eight reviews were included; participants were mainly older adults based in the USA. Although overlap between reviews was low, quality was generally poor. Benefits were found in all three domains, with reduced mortality and increased functioning exerting the largest effects. Older age, reflection, religious volunteering, and altruistic motivations increased benefits most consistently. Referral of social prescribing clients to volunteering is recommended. Limitations include the need to align results to research conducted after the COVID-19 pandemic. (PROSPERO registration number: CRD42022349703). BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare genetic disorder which requires a complex diagnostic workup. Thus, an easy and widely available screening method would be helpful to identify patients who need a further diagnostic workup for PCD. OBJECTIVES: The aim of the study was to develop and validate a computed tomography (CT) score for PCD to facilitate etiological diagnosis in adults with bronchiectasis. METHOD: Chest CTs from 121 adults with bronchiectasis were scored for bronchiectasis morphology, distribution, and associated findings. Patients with and without the etiological diagnosis of PCD (46 and 75, respectively) were compared. Significantly, different imaging findings (p < 0.05) in univariate analysis were considered for multivariate analysis. Distinct findings were used to build the score. Based on this score, receiver operating characteristic (ROC) curve analysis was performed. The score was validated with 2 independent cohorts, another cohort from the same institution with 56 patients (28 with PCD) and an external cohort from another referral center with 172 patients (86 with PCD). RESULTS: The following parameters predicted PCD in adults with bronchiectasis and were included in the score with weighting according to their regression coefficients: 2 points were given for predominance in the middle/lower lobe, 2 points for tree-in-bud pattern, 2 points for atelectasis or prior resection of a middle/lower lobe, and 3 points for absence of emphysema and fibrosis. Situs inversus was only observed in subjects with PCD (Kartagener syndrome) and, thus, was not used in the primary ciliary dyskinesia computed tomography (PCD-CT) score as group comparisons could not be performed. ROC curve analysis revealed an area under the curve (AUC) of 0.90 (95% CI 0.85-0.96). Youden index was the highest at a threshold of >6 with a sensitivity of 83% and a specificity of 83%. In the validation cohorts, ROC curve analysis confirmed the performance of the score with an AUC of 0.83 (95% CI 0.72-0.94) in the first validation cohort and 0.79 (95% CI 0.73-0.86) in the external validation cohort. CONCLUSIONS: The PCD-CT score provides the first validated CT score for PCD and helps physicians in identifying adult bronchiectasis patients who require further diagnostic workup. Key message: The PCD-CT score provides the first validated CT score to assist physicians in identifying adult bronchiectasis patients who require a further diagnostic workup for PCD. It potentially improves earlier recognition of this rare and underdiagnosed disease.

Hydroclimate extremes in North America, Europe, and the Mediterranean are linked to ocean and atmospheric circulation anomalies in the Atlantic, but the limited length of the instrumental record prevents complete identification and characterization of these patterns of covariability especially at decadal to centennial timescales. Here we analyze the coupled patterns of drought variability on either side of the North Atlantic Ocean basin using independent climate field reconstructions spanning the last millennium in order to detect and attribute epochs of coherent basin-wide moisture anomalies to ocean and atmosphere processes. A leading mode of broad-scale moisture variability is characterized by distinct patterns of North Atlantic atmosphere circulation and sea surface temperatures. We infer a negative phase of the North Atlantic Oscillation and colder Atlantic sea surface temperatures in the middle of the 15th century, coincident with weaker solar irradiance and prior to strong volcanic forcing associated with the early Little Ice Age.

A wild-type Baeyer-Villiger monooxygenase was engineered to overcome numerous liabilities in order to mediate a commercial oxidation of pyrmetazole to esomeprazole, using air as the terminal oxidant in an almost exclusively aqueous reaction matrix. The developed enzyme and process compares favorably to the incumbent Kagan inspired chemocatalytic oxidation, as esomeprazole was isolated in 87% yield, in >99% purity, with an enantiomeric excess of >99%.

There is a need for a clear definition of exacerbations used in clinical trials in patients with bronchiectasis. An expert conference was convened to develop a consensus definition of an exacerbation for use in clinical research.A systematic review of exacerbation definitions used in clinical trials from January 2000 until December 2015 and involving adults with bronchiectasis was conducted. A Delphi process followed by a round-table meeting involving bronchiectasis experts was organised to reach a consensus definition. These experts came from Europe (representing the European Multicentre Bronchiectasis Research Collaboration), North America (representing the US Bronchiectasis Research Registry/COPD Foundation), Australasia and South Africa.The definition was unanimously approved by the working group as: a person with bronchiectasis with a deterioration in three or more of the following key symptoms for at least 48 h: cough; sputum volume and/or consistency; sputum purulence; breathlessness and/or exercise tolerance; fatigue and/or malaise; haemoptysis AND a clinician determines that a change in bronchiectasis treatment is required.The working group proposes the use of this consensus-based definition for bronchiectasis exacerbation in future clinical research involving adults with bronchiectasis.

Twospotted spider mite, Tetranychus urticae Koch, is an important pest of peppermint in California, USA. Spider mite feeding on peppermint leaves causes physiological changes in the plant, which coupling with the favorable environmental condition can lead to increased mite infestations. Significant yield loss can occur in absence of pest monitoring and timely management. Understating the within-field spatial distribution of T. urticae is critical for the development of reliable sampling plan. The study reported here aims to characterize the spatial distribution of mite infestation in four commercial peppermint fields in northern California using spatial techniques, variogram and Spatial Analysis by Distance IndicEs (SADIE). Variogram analysis revealed that there was a strong evidence for spatially dependent (aggregated) mite population in 13 of 17 sampling dates and the physical distance of the aggregation reached maximum to 7 m in peppermint fields. Using SADIE, 11 of 17 sampling dates showed aggregated distribution pattern of mite infestation. Combining results from variogram and SADIE analysis, the spatial aggregation of T. urticae was evident in all four fields for all 17 sampling dates evaluated. Comparing spatial association using SADIE, ca. 62% of the total sampling pairs showed a positive association of mite spatial distribution patterns between two consecutive sampling dates, which indicates a strong spatial and temporal stability of mite infestation in peppermint fields. These results are discussed in relation to behavior of spider mite distribution within field, and its implications for improving sampling guidelines that are essential for effective pest monitoring and management.

Seasonal and annual partitioning of water within river floodplains has important implications for ecohydrologic links between the water cycle and tree growth. Climatic and hydrologic shifts alter water distribution between floodplain storage reservoirs (e.g., vadose, phreatic), affecting water availability to tree roots. Water partitioning is also dependent on the physical conditions that control tree rooting depth (e.g., gravel layers that impede root growth), the sources of contributing water, the rate of water drainage, and water residence times within particular storage reservoirs. We employ instrumental climate records alongside oxygen isotopes within tree rings and regional source waters, as well as topographic data and soil depth measurements, to infer the water sources used over several decades by two co-occurring tree species within a riparian floodplain along the Rhône River in France. We find that water partitioning to riparian trees is influenced by annual (wet versus dry years) and seasonal (spring snowmelt versus spring rainfall) fluctuations in climate. This influence depends strongly on local (tree level) conditions including floodplain surface elevation and subsurface gravel layer elevation. The latter represents the upper limit of the phreatic zone and therefore controls access to shallow groundwater. The difference between them, the thickness of the vadose zone, controls total soil moisture retention capacity. These factors thus modulate the climatic influence on tree ring isotopes. Additionally, we identified growth signatures and tree ring isotope changes associated with recent restoration of minimum streamflows in the Rhône, which made new phreatic water sources available to some trees in otherwise dry years. The two-spotted spider mite, Tetranychus urticae Koch, is an economic pest on peppermint [Mentha x piperita (L.), 'Black Mitcham'] grown in California. A sampling plan for T. urticae was developed under Pacific Northwest conditions in the early 1980s and has been used by California growers since approximately 1998. This sampling plan, however, is cumbersome and a poor predictor of T. urticae densities in California. Between June and August, the numbers of immature and adult T. urticae were counted on leaves at three commercial peppermint fields (sites) in 2010 and a single field in 2011. In each of seven locations per site, 45 leaves were sampled, that is, 9 leaves per five stems. Leaf samples were stratified by collecting three leaves from the top, middle, and bottom strata per stem. The on-plant distribution of T. urticae did not significantly differ among the stem strata through the growing season. Binomial and enumerative sampling plans were developed using generic Taylor's power law coefficient values. The best fit of our data for binomial sampling occurred using a tally threshold of T = 0. The optimum number of leaves required for T urticae at the critical density of five mites per leaf was 20 for the binomial and 23 for the enumerative sampling plans, respectively. Sampling models were validated using Resampling for Validation of Sampling Plan Software.

Background: 'Making Every Contact Count' (MECC) is a public health strategy supporting public-facing workers to use opportunities during routine contacts to enable health behaviour change. A mental health hospital in the North East of England is currently implementing a programme to embed MECC across the hospital supporting weight management ('A Weight Off Your Mind'). Bespoke MECC training has been developed to improve staff confidence in discussing physical activity, healthy eating, and related behaviour change with service users. This article describes the protocol for a pragmatic formative process evaluation to inform the implementation plan for MECC and facilitate successful implementation of the bespoke MECC training at scale. Methods/Design: An 18-month, mixed method pragmatic formative process evaluation, including qualitative research, surveys, document review and stakeholder engagement. This project is conducted within a mental health inpatient setting in the North East of England. Programme documents will be reviewed, mapped against MECC national guidelines, Behaviour Change Techniques (BCTs) and intervention functions within the Behaviour Change Wheel. A cross-sectional survey (n = 365) and qualitative semi-structured interviews (n = 30) will be conducted with healthcare practitioners delivering MECC to assess capability, opportunity and motivation. Data collection and fidelity procedures will be examined, including design, training and delivery dimensions of fidelity. Interviews with service users (n = 20) will also be conducted. Discussion: Anticipated outcomes include developing recommendations to overcome barriers to delivery of and access to MECC, including whether to either support the use of the existing MECC protocol or tailor the MECC training programme. The findings are anticipated to improve fidelity of MECC training within mental health inpatient settings as well as provide evidence for MECC training at a national level. We also expect findings to influence strategic plans, policy, and practice specific to MECC and inform best practice in implementing wider brief intervention programmes.

RESUMO

One reason that it is so difficult to build electronic systems for collecting and sharing health information is that their design and implementation requires clear goals and a great deal of collaboration among people from diverse social and occupational worlds. This paper uses empirical examples from two Australian health informatics projects to illustrate the importance of boundary objects and boundary spanning activities in facilitating the high degree of collaboration required for the design and implementation of workable systems.

Making Every Contact Count (MECC) is a national, long-term public health strategy in England. It supports public-facing workers to use opportunities during routine contacts to enable healthy lifestyle changes. This paper reports the findings from an external evaluation of voluntary and community sector (VCS) delivery of MECC in the North East of England, which focused on engaging under-represented client groups. The study aimed to (a) Establish if (and how) MECC had impacted the workforce, including changes to staff knowledge, confidence and behaviour; (b) Identify benefits, challenges and unintended consequences; and (c) Explore outcomes for service users. A multi-stage qualitative design focused on understanding both process and outcomes. The study utilised three data collection methods, including a journey mapping workshop (n = 20), semi-structured interviews with delivery leads, VCS workers and volunteers who had accessed MECC training (n = 11), and focus group discussions with clients (n = 22). The findings illustrated positive early outcomes, including improvements in self-reported staff knowledge and confidence as well as emerging examples of organisational culture shift and individual behaviour change. Alongside this, the data provided a rich picture of barriers and challenges which are examined at different levels-national programme, local programme, VCS sector, partner organisation, worker and client. The research highlights clear successes of the VCS delivery model. However, it is presented as a 'double-edged sword,' in light of associated challenges such as sector-level funding uncertainty and accessibility of MECC resources to diverse client groups. The discussion considers issues related to the measurement and attribution of behaviour change outcomes for brief interventions, as well as fidelity, legacy and long-term sustainability challenges. The recommendations call for system-level analysis and comparison of different MECC implementation models, to improve our understanding of challenges, opportunities and programme reach for behaviour change intervention programmes-particularly in relation to seldom-heard client groups.

Purpose: Nearly one in three US adolescents meet the criteria for anxiety, an issue that has worsened with the COVID-19 pandemic. We developed a video-based slow diaphragmatic breathing stress-reduction curriculum for high school students and evaluated its feasibility, tolerability, and preliminary effectiveness. Methods: This cluster-randomized feasibility pilot compared 5-min slow diaphragmatic breathing for 5 weeks with treatment-as-usual control among four 12th-grade public high school classes. Students individually participated after school during COVID-19-related hybrid teaching, with slow diaphragmatic breathing three times/week and breath science education once/week. Feasibility was based on completion of breathing exercises, breath science education, and preliminary effectiveness assessments, and ease/tolerability was based on qualitative assessments. Preliminary effectiveness was measured with the State-Trait Anxiety Inventory (STAI) and a timed-exhale carbon dioxide tolerance test (CO2TT) of physiological stress response. Descriptive statistics and repeated analysis of variance were performed to quantify and compare outcomes between time periods. Human subjects research approval was granted through Western IRB-Copernicus Group (WCG IRB) [ClinicalTrials.gov, Identifier: NCT05266833.]. Results: Forty-three students consented to participate. Breath practice compliance ranged from 29 to 83% across classes and weeks, and decreased on average over the 5 weeks. Compliance with the breath science videos ranged from 43 to 86%, and that with the weekly STAI-State and CO2TT measures varied from 36 to 86%. Compliance with ease/tolerability assessments ranged from 0 to 60%. Preliminary effectiveness assessments' compliance varied across classes from 83 to 89% during baseline, and 29 to 72% at follow-up. The curriculum was rated as somewhat-to-definitely useful/beneficial, and definitely-to-very easy/tolerable. Students reported enjoying the diaphragmatic breathing, CO2TT, and breath science education; some found the extended exhales challenging and the curriculum and assessments time-consuming. Preliminary effectiveness analyses indicated no significant changes in STAI or CO2TT from baseline to followup or from before to after breathing exercises (p > 0.05 for all). Conclusions: Implementation of this 5-week slow breathing curriculum was feasible and tolerable to this cohort. Compliance, tolerability, and effectiveness may be improved with in-class participation. Future research on simple and accessible slow-breathing exercises is warranted to address today's adolescent stress-management crisis. In this paper, we present a 392-year-long preliminary temperature reconstruction for western Hungary. The reconstructed series is based on five vine- and grain-related historical phenological series from the town of Koszeg. We apply dendrochronological methods for both signal assessment of the phenological series and the resultant temperature reconstruction. As a proof of concept, the present reconstruction explains 57% of the temperature variance of May-July Budapest mean temperatures and is well verified with coefficient of efficiency values in excess of 0.45. The developed temperature reconstruction portrays warm conditions during the late seventeenth and early eighteenth centuries with a period of cooling until the coldest reconstructed period centred around 1815, which was followed by a period of warming until the 1860s. The phenological evidence analysed here represent an important data source from which non-biased estimates of past climate can be derived that may provide information at all possible time-scales.

Emissions from traffic over the past few decades have become a significant source of air pollution. Among the pollutants emitted are nitrogen oxides (NOx), exposure to which can be detrimental to public health. Recent studies have shown that nitrogen (N) stable isotope ratios in tree-rings and foliage express a fingerprint of their major N source, making them appropriate for bio-monitoring purposes. In this study, we have applied this proxy to Aleppo pines (Pinus halepensis) at three distances from one of the busiest roads in Malta, a country known to suffer from intense traffic pollution. Our results showed that N and organic carbon (C) stable isotope ratios in tree-rings do not vary over the period 1980-2018 at any of the investigated sites; however, statistically significant spatial trends were apparent in both tree-rings and foliage. The roadside and transitional sites exhibited more positive δ15N and more negative δ13C values compared to those at a rural control site. This is likely due to the incorporation of 15N-enriched NOx and 13C-depleted CO2 from traffic pollution. Sampled top-soil also exhibited the δ15N trend. Our results constitute the first known application of dendrogeochemistry to atmospheric pollution monitoring in Malta.

Mint oil is a key source of natural flavors with wide industrial applications. Two unbalanced polyploid cultivars named Native (Mentha Spicata L) and Scotch (M. × gracilis Sole) are the main producers of spearmint type oil, which is characterized by high levels of the monoterpenes (-)-carvone and (-)-limonene. These cultivars have been the backbone of spearmint oil production for decades, while breeding and improvement remained largely unexplored, in part, due to sterility in cultivated lines. Here we show that sexual breeding at the diploid level can be leveraged to develop new varieties that produce spearmint type oil, along with the improvement of other important traits. Using field trials and GC-FID oil analysis we characterized plant materials from a public germplasm repository and identified a diploid accession that exhibited 89.5% increase in oil yield, compared to the industry standard, and another that produces spearmint type oil. Spearmint-type oil was present at high frequency in a segregating F2 population (32/160) produced from these two accessions. Field-testing of ten of these F2 lines showed segregation for oil yield and confirmed the production of spearmint-type oil profiles. Two of these lines combined high yield and spearmint-type oil with acceptable analytic and sensory profiles. These results demonstrate that spearmint-type oil can be produced in a diploid background with high yield potential, providing a simpler genetic system for the development of improved spearmint varieties.

The World Health Organization and the United States Centers for Disease Control have recommended universal face masking by the general public to slow the spread of COVID-19. A number of recent studies have evaluated the filtration efficiency and pressure differential (an indicator of breathability) of various, widely available materials that the general public can use to make face masks at home. In this review, we summarize those studies to provide guidance for both the public to select the best materials for face masks and for future researchers to rigorously evaluate and report on mask material testing. Of the tested fabric materials and material combinations with adequate breathability, most single and multilayer combinations had a filtration efficiency of <30%. Most studies evaluating commonly available mask materials did not follow standard methods that would facilitate comparison across studies, and materials were often described with too few details to allow consumers to purchase equivalent materials to make their own masks. To improve the usability of future study results, researchers should use standard methods and report material characteristics in detail.

Tree-ring chronologies underpin the majority of annually-resolved reconstructions of Common Era climate. However, they are derived using different datasets and techniques, the ramifications of which have hitherto been little explored. Here, we report the results of a double-blind experiment that yielded 15 Northern Hemisphere summer temperature reconstructions from a common network of regional tree-ring width datasets. Taken together as an ensemble, the Common Era reconstruction mean correlates with instrumental temperatures from 1794-2016 CE at 0.79 (p < 0.001), reveals summer cooling in the years following large volcanic eruptions, and exhibits strong warming since the 1980s. Differing in their mean, variance, amplitude, sensitivity, and persistence, the ensemble members demonstrate the influence of subjectivity in the reconstruction process. We therefore recommend the routine use of ensemble reconstruction approaches to provide a more consensual picture of past climate variability.

At Jupiter, tail reconnection is thought to be driven by an internal mass loading and release process called the Vasyliunas cycle. Galileo data have shown hundreds of reconnection events occurring in Jupiter's magnetotail. Here we present a survey of reconnection events observed by Juno during its first 16 orbits of Jupiter (July 2016-October 2018). The events are identified using Juno magnetic field data, which facilitates comparison to the Vogt et al. (2010, https://doi.org/10.1029/2009JA015098) survey of reconnection events from Galileo magnetometer data, but we present data from Juno's other particle and fields instruments for context. We searched for field dipolarizations or reversals and found 232 reconnection events in the Juno data, most of which featured an increase in |B θ |, the magnetic field meridional component, by a factor of 3 over background values. We found that most properties of the Juno reconnection events, like their spatial distribution and duration, are comparable to Galileo, including the presence of a ~3-day quasi-periodicity in the recurrence of Juno tail reconnection events and in Juno JEDI, JADE, and Waves data. However, unlike with Galileo we were unable to clearly define a statistical x-line separating planetward and tailward Juno events. A preliminary analysis of plasma velocities during five magnetic field reconnection events showed that the events were accompanied by fast radial flows, confirming our interpretation of these magnetic signatures as reconnection events. We anticipate that a future survey covering other Juno datasets will provide additional insight into the nature of tail reconnection at Jupiter.

RESUMO

The multi-billion NHS Connecting for Health programme in England has completely reconfigured the relationships between the Department of Health, the National Health Service (NHS), primary care computing suppliers and healthcare professionals (including general practitioners). The implications of this reconfiguration are now becoming apparent and have potentially significant effects on the delivery of information and information systems in the health context. This article explores the changes in these relationships by drawing on comparisons with the previous system for procurement of primary care computing systems, which ran for much of the 1990s. The article also comments on characteristics of the CfH procurement/contracting process, the differing responses of suppliers, and the role of the existing installed base as an actor in building a new infrastructure for health records.

RESUMO

This paper reports on the development process of an Electronic Social Care Record (ESCR), which used an adapted approach to Participatory Design (PD). PD is an established range of methodologies and principles for involving users in the design and implementation of systems or products. University researchers worked as part of a wider team including corporate social service stakeholders, a multidisciplinary children's social service team, and a technology provider, to develop a system that supports the integration of information of various teams/disciplines which need to work together to intervene with children who are in need. Using action research and ethnographic techniques, the researchers sought to understand and address the challenges faced by the various stakeholders when designing a system to deliver an ESCR.
