BACKGROUND: The i-STAT (iST) and GEM Premier ChemSTAT (ChmST) are point-of-care systems that measure electrolytes, metabolites, acid-base parameters, and hematocrit on blood. We compared results for creatinine, urea (blood urea nitrogen [BUN]), total carbon dioxide (tCo2), and ionized calcium (ion Ca) on blood by the ChmST and iST to Beckman DxC600 (DxC600) results for creatinine and urea on plasma and GEM Premier 5000 (GEM5K) results for ion Ca and tCo2 on blood. METHODS: 107 heparinized blood specimens were analyzed on the ChmST and iST systems, with plasma tested by DxC600 for creatinine and BUN and blood tested by GEM5K for ion Ca and tCo2. We evaluated the methods primarily by the mean and SD of the biases at clinically relevant concentrations. RESULTS: The correlations of ChmST and iST results on blood for creatinine, BUN, ion Ca, and tCo2 correlated to plasma by the DxC600 and to blood by the GEM5K were r ≥ 0.98 for all analytes except for ion Ca on the iST (r = 0.93). The mean and SD of biases were within clinically and analytically acceptable limits for all methods except for tCo2 on the ChmST, which measures bicarbonate with a bicarbonate-sensing electrode. Also, creatinine and BUN by the ChmST were less affected by icterus or hemolysis than were the DxC600 (icterus) and the iST (hemolysis). CONCLUSIONS: The ChmST and iST results on blood demonstrated strong correlations with each other and with the DxC600 results on plasma. We conclude the ChmST provides reliable results for whole blood creatinine, urea, ion Ca, and tCo2.

Background and aims: GEM Premier ChemSTAT is a new point-of-care system providing rapid creatinine, BUN and tCO2 measurements together with electrolytes, metabolites, hematocrit, pH and pCO2 from a single whole blood specimen in acute care settings such as emergency departments and intensive care units. Accurate measurements of whole blood creatinine can aid in the diagnosis and treatment of renal diseases. Materials and methods: Heparinized whole blood samples from different clinical locations were evaluated on the GEM Premier ChemSTAT and results compared to plasma from the same samples on the Beckman AU5800 or whole blood on the GEM Premier 4000. Precision studies were conducted with whole blood and quality control material. Results: ChemSTAT correlated well with plasma samples on the AU5800 (regression slopes (S): 0.957-1.159, correlation coefficients (r)≥0.952) and with whole blood specimens on the GEM Premier 4000 (S: 0.9646-1.124, r ≥ 0.974). The repeatability was 0.1%-3.1% and QC precision were within lab and manufacturers' specifications. Conclusion: ChemSTAT demonstrated strong correlation to the comparative methods and excellent precision. Combining with its continuous quality management, ChemSTAT is suitable for acute care settings to provide rapid, reliable results, which could minimize time-to-treatment and improve patient outcome.

BACKGROUND: GEM Premier ChemSTAT™ is a point-of-care (POC) system that measures Na+, K+, Ca++, Cl-, glucose, hematocrit, creatinine, blood urea nitrogen (BUN), tCO2, pH, pCO2, and lactate from a single whole blood specimen, providing rapid results in POC settings such as the emergency department (ED). Accurate measurements of creatinine in whole blood and reporting of estimated glomerular filtration rate (eGFR) can minimize adverse effects of contrast-induced nephropathy. METHODS: Heparinized whole blood specimens from the ED were analyzed on the ChemSTAT by POC staff. Method comparison was performed against the cobas Integra c501 for creatinine, BUN, and tCO2, and against the GEM Premier 4000 for all other analytes. Precision was conducted with whole blood specimens assayed in triplicate over 6 days. Creatinine results from whole blood and plasma were used for eGFR, by isotope dilution mass spectrometry-traceable Modification of Diet in Renal Disease and Chronic Kidney Disease Epidemiology Collaboration equations, and eGFR concordance was assessed. RESULTS: Creatinine, BUN, and tCO2 correlated well with plasma samples on the cobas, and all other analytes correlated well with whole blood specimens on the GEM Premier 4000 across the tested sample ranges. The regression slope was 0.951 to 1.047, along with a correlation coefficient (r) of ≥0.982 for all analytes. The pooled within-sample precision was 0% to 2.5% for all analytes. CONCLUSIONS: ChemSTAT demonstrated a strong correlation with the comparative methods and excellent precision. The system's analytical performance and continuous quality management make it suitable for use in the ED to provide rapid reliable test results, which could minimize the time to treatment and improve ED efficiency.
