RESUMO

This review compared the effects of eccentric versus concentric exercise training in healthy people and people with metabolic disease. A systematic search on Cochrane Central Register of Controlled Trials, MEDLINE, Embase, CINAHL, SPORTDiscus, Web of Science, SCOPUS and PubMed was conducted in February 2022. Randomised controlled trials conducted on sedentary healthy adults or those with an existing metabolic disease that compared eccentric versus concentric exercise training interventions of four weeks or longer that involved multiple joints and large muscle groups (e.g., walking, whole-body resistance training) were included in the review. The primary outcome was glucose handling, measured as HbA1c, HOMA, fasting glucose or insulin. Measures of cardiovascular health, muscle strength, and functional physical fitness were secondary outcomes. Nineteen trials involving 618 people were included. Results of meta-analyses showed that eccentric exercise had no benefit to glucose handling (HbA1c level; SMD - 0.99; 95% CI, -2.96 to 0.98; n = 74; P = 0.32) but resulted in significant increases in overall muscle strength (SMD 0.70; 95% CI 0.25 to 1.15; n = 224; P = 0.003) and decreases in blood pressure (Systolic Blood Pressure; MD -6.84; 95% CI, -9.84 to -3.84; n = 47, P = 0.00001, and Diastolic Blood Pressure; MD -6.39; 95% CI -9.62 to -3.15; n = 47, P = 0.0001). Eccentric exercise is effective for improving strength and some markers of cardiovascular health compared to traditional exercise modalities. Additional high-quality studies are necessary to validate these results. (PROSPERO registration: CRD42021232167).

The purpose of this study was to compare the effects of velocity-based strength training (VBT) and percentage-based strength training (PBT) on absolute strength, explosive strength, speed, and agility, as well as markers of muscle damage after 6 weeks of exercise programs. The study included 30 young female individuals, divided into three groups of 10 participants: VBT, PBT, and control group. The main findings indicated that the VBT group and PBT group showed significant improvement in 1RM squat exercise (Δ% 27.87 and Δ% 8.98, respectively) and 1RM bench press (Δ% 14.47 and Δ% 8.65, respectively), but a greater enhancement was observed in the VBT group. In addition, VBT induced substantial changes in SJ (Δ% 14.32) and CMJ height (Δ% 7.69), while PBT had an improvement only in the SJ test (Δ% 6.72). The improvement noted in the VBT group could be attributed to its ability to tailor training intensity according to the speed of movement execution. This approach allows athletes to perform each repetition as fast as possible, thus maintaining an optimal intensity for explosive strength development. The capacity of VBT to adapt training intensity based on the speed of movement execution may be the key factor contributing to these results. Therefore, coaches and athletes should consider implementing VBT as a valuable tool to optimize strength and power development. In conclusion, VBT induced greater improvement in the 1RM squat, 1RM bench press, SJ, and CMJ compared to the group that performed the traditional strength training modality. Therefore, VBT is considered a more effective training tool regarding the development of absolute and explosive strength in young women.

OBJECTIVE: The objective of this meta-analysis is to evaluate and compare the effectiveness of different mind-body therapies in reducing the symptoms of schizophrenia. METHODS: A systematic search was performed using databases such as PubMed, Embase, Cochrane Library, Web of Science, and Scopus. Randomized controlled trials that assessed the effects of mind-body therapies on patients with schizophrenia were included. The search covered the period between the inception of each database and November 17th, 2022. The methodological quality of the trials was assessed using the Cochrane risk of bias tool. A network meta-analysis was conducted to compare the effects of various mind-body therapies, including Yoga, Mindfulness, Tai Chi, Baduanjin, and Yijinjing. RESULTS: The analysis included 22 randomized controlled trials involving a total of 2064 subjects. The network meta-analysis revealed that Yoga and Mindfulness interventions were more effective than other mind-body therapies in reducing the symptoms of schizophrenia. Specifically, Yoga improved PANSS-positive symptom scores (SUCRA: 74.8 %) and PANSS-negative symptom scores (SUCRA: 80.4 %), whereas Mindfulness improved PANSS-positive symptom scores (SUCRA: 85.6 %). CONCLUSION: The findings of this study indicate that Yoga may be a promising intervention for the treatment of schizophrenia. However, the small sample size and the low quality of the included studies have limited the generalizability of our findings Therefore, this study must be understood with caution, and further investigation is warranted when more relevant studies emerge.

The aging population warrants the increase of mild cognitive impairment (MCI) prevalence, a condition that could progress to dementia. Efforts have been made to improve the MCI and prevent its progression, including the introduction of Tai Chi, a Chinese traditional exercise. The aim of this systematic review and meta-analysis was to evaluate the efficacy of Tai Chi in attenuating MCI among the elderly population. Records investigating the effect of Tai Chi exercise intervention on cognitive function among elderly patients were searched systematically from PubMed, ScienceDirect, Google Scholar, and Europe PMC as of April 13, 2023. The risk of bias (RoB 2.0) quality assessment was employed in the quality appraisal of the studies included. Review Manager 5.4.1 was used for data extraction and meta-analysis, where the standard mean difference (SMD) and 95% confidence interval (95%CI) were computed. Eight randomized control trials with a total of 1379 participants were included in this meta-analysis. Six trials assessed Montreal Cognitive Assessment scores, where its pooled analysis suggested that Tai Chi was as effective as conventional exercise (SMD=0.15, 95%CI: -0.11 to 0.40, p=0.26). However, pooled analysis of the Mini-Mental Status Examination suggested that Tai Chi intervention more effectively improved cognitive function and reduced the rate of cognitive impairment in elderly patients (SMD=0.36, 95%CI: 0.18 to 0.54, p<0.01) as compared to the control group. This systematic review and meta-analysis suggest that, in some extent, Tai Chi is efficacious in improving cognitive function and slowing down the rate of cognitive impairment among elderly patients.

To improve and even reverse sarcopenia in elderly people, this study developed a self-determined sequence exercise program consisting of strength training exercise, Yijinjing exercise (a traditional Chinese exercise), and hybrid strength training with Yijinjing exercise. Ninety-four community-dwelling older adults screened for sarcopenia using the Asian Working Group for Sarcopenia criteria were randomly assigned to 24 weeks of a control group (CG, n = 30), self-determined sequence exercise program group (SDSG, n = 34) or strength training group (STG, n = 30). The study examined the effects of three interventions on participantsL3 skeletal muscle fat density, L3 skeletal muscle fat area, L3 skeletal muscle density, L3 skeletal muscle area, muscle fat infiltration, relative skeletal muscle mass index, and grip strength using a repeated-measures ANOVA to evaluate the experimental data. To evaluate the real effect of this model in reversing sarcopenia after the intervention, nine classification models were trained. Significant interaction effects were observed with grip strength, RSMI, L3 SMD, and L3 SMA. At the 24th week, participants' grip strength, L3 SMFA, L3 SMA, and RSMI were improved significantly in the SDSG and STG. The SDSG achieved significantly greater RSMI and grip strength than the STG and CG after the intervention. The self-determined sequence exercise program exhibited better performance than the single type of exercise modality in reversing sarcopenia and improving older adults' skeletal muscle area. Consequently, the stacking model is feasible to make a prediction as to whether or not sarcopenia may be reversed in older adults.

Tai Chi, a practice that combines elements of both exercise and mindfulness, offers a wide range of health benefits. The body of evidence concerning the impact of Tai Chi on diabetes has recently been growing. This editorial aims to provide a concise summary of the current state of evidence for Tai Chi's effects on individuals with type 2 diabetes (T2D). The review includes 3 randomized controlled trials (RCTs) and 5 systematic reviews and meta-analyses, all of which investigate the effectiveness of Tai Chi on various health outcomes in individuals with T2D. Tai Chi demonstrates a significant effect to enhance glycemic control, lower blood pressure, improve serum lipid profiles, reduce insulin resistance, positively influence obesity-related indices, and improve overall quality of life in individuals with T2D. However, it is noteworthy that recent RCTs have reported inconsistent findings regarding the effects of Tai Chi on glycemic control and insulin resistance. The author also delves into potential mechanisms by which Tai Chi may exert its influence on the human body. Finally, the editorial highlights the critical issues that warrant further exploration in the future.

Objective: Although Traditional Chinese Yijinjing Qigong Exercise (YJJQE) as mind-body intervention is popularly used among adults to ameliorate depressive symptoms in China, no randomized controlled trials (RCTs) are available to evaluate the effects of YJJQE in patients with poststroke depression (PSD). This study aims to explore the clinical efficacy and the neurological and psychiatric mechanism in brain network functional connectivity underlying electroencephalography (EEG). Materials and methods: A total of 60 patients, diagnosed with mild PSD, were randomly (1:1) assigned to YJJQE group (n = 30) and control group of routine segmental rehabilitation training group (n = 30) for a 60-min exercise session once a day for 3 weeks. All outcome measures were collected at baseline and 3-weeks ending intervention. The primary outcome was the 24-item Hamilton Depression Scale (HAMD-24) score, evaluation at more time points for 1 month of follow-up. The secondary outcomes were EEG data in four frequency domains (δ, θ, α, and ß), global efficiency (GE), local efficiency (LE), GE/LE curve [areas under the curve (AUC)], Phase Lag Index (PLI), (HAMD-24) Score and EEG correlation analysis. Results: All patients showed no significant differences in baseline data. After 3 weeks and 1 month of follow-up, the YJJQE group demonstrated significant decreasing changes compared to the control group on the HAMD-24 scores (p < 0.001). Furthermore, the YJJQE group also showed a significant reduction in θ wave, and an increase in both GE and LE. Compared to the control group, the YJJQE Qigong group showed significantly greater functional connectivity in the δ, θ, and ß frequency bands in the brain network of the degree of phase synchronization (p < 0.001). HAMD-24 Score and EEG correlation analysis negative correlation in the Qigong group θ wave (p < 0.001). Conclusion: Our findings demonstrated that YJJQE is estimated to effectively alleviate the depressed mood of patients with PSD by promoting the efficiency in information transmission of network functional connectivity and its integration ability in different brain regions. Therefore, the YJJQE would be useful as a non-pharmacological treatment to prevent PSD. Knee osteoarthritis is a chronic degenerative disease. Cartilage and subchondral bone degeneration, as well as synovitis, are the main pathological changes associated with knee osteoarthritis. Mechanical overload, inflammation, metabolic factors, hormonal changes, and aging play a vital role in aggravating the progression of knee osteoarthritis. The main treatments for knee osteoarthritis include pharmacotherapy, physiotherapy, and surgery. However, pharmacotherapy has many side effects, and surgery is only suitable for patients with end-stage knee osteoarthritis. Exercise training, as a complementary and adjunctive physiotherapy, can prevent cartilage degeneration, inhibit inflammation, and prevent loss of the subchondral bone and metaphyseal bone trabeculae. Increasing evidence indicates that exercise training can improve pain, stiffness, joint dysfunction, and muscle weakness in patients with knee osteoarthritis. There are several exercise trainings options for the treatment of knee osteoarthritis, including aerobic exercise, strength training, neuromuscular exercise, balance training, proprioception training, aquatic exercise, and traditional exercise. For Knee osteoarthritis (KOA) experimental animals, those exercise trainings can reduce inflammation, delay cartilage and bone degeneration, change tendon, and muscle structure. In this review, we summarize the main symptoms of knee osteoarthritis, the mechanisms of exercise training, and the therapeutic effects of different exercise training methods on patients with knee osteoarthritis. We hope this review will allow patients in different situations to receive appropriate exercise therapy for knee osteoarthritis, and provide a reference for further research and clinical application of exercise training for knee osteoarthritis.

BACKGROUND AND PURPOSE: Therapeutic exercises are considered effective treatments for ankylosing spondylitis(AS). Current study aimed to evaluate efficacy and safety of Baduanjin qigong, a traditional Chinese exercise, for treatment of AS in a pilot RCT setting. MATERIALS AND METHODS: A total of 60 patients were randomly assigned, at a 1:1 ratio, to receive a 12-week Baduanjin qigong training(exercise group) or maintain their current lifestyle(no-treatment group). As primary outcomes, Bath Ankylosing Spondylitis Disease Activity Index(BASDAI) and other AS symptoms were assessed at baseline and end of treatment period. RESULTS: A total of 46 patients completed the study. At the end of treatment period, although total BASDAI scores were not statistically different, reduced scores were observed in the exercise group, compared to no-treatment group, with respect to fatigue(P = 0.03), intensity(P = 0.04) and duration(P = 0.01) of morning stiffness; exercise group also exhibited higher patient global assessment scores(P = 0.04). CONCLUSION: Baduanjin qigong exercise appeared to improve AS symptoms.
