RESUMO

AIMS AND OBJECTIVES: The aim of this study is to enhance the understanding of the core elements and influencing factors on the community-based epilepsy nurse's role and responsibilities. BACKGROUND: Internationally, epilepsy nurse specialists play a key role in providing person-centred care and management of epilepsy but there is a gap in understanding of their role in the community. DESIGN: A national three-stage, mixed-method study was conducted. METHODS: One-on-one, in-depth semi-structured qualitative interviews were conducted online with 12 community-based epilepsy nurses (Stage 1); retrospective analysis of data collected from the National Epilepsy Line, a nurse-led community helpline (Stage 2); and focus group conducted with four epilepsy nurses, to delve further into emerging findings (Stage 3). A thematic analysis was conducted in Stages 1 and 3, and a descriptive statistical analysis of Stage 2 data. Consolidated Criteria for Reporting Qualitative studies checklist was followed for reporting. RESULTS: Three key themes emerged: (1) The epilepsy nurse career trajectory highlighted a lack of standardised qualifications, competencies, and career opportunities. (2) The key components of the epilepsy nurse role explored role diversity, responsibilities, and models of practice in the management of living with epilepsy, and experiences navigating complex fragmented systems and practices. (3) Shifting work practices detailed the adapting work practices, impacted by changing service demands, including COVID-19 pandemic experiences, role boundaries, funding, and resource availability. CONCLUSION: Community epilepsy nurses play a pivotal role in providing holistic, person-centred epilepsy management They contribute to identifying and addressing service gaps through innovating and implementing change in service design and delivery. RELEVANCE TO CLINICAL PRACTICE: Epilepsy nurses' person-centred approach to epilepsy management is influenced by the limited investment in epilepsy-specific integrated care initiatives, and their perceived value is impacted by the lack of national standardisation of their role and scope of practice. NO PATIENT OR PUBLIC CONTRIBUTION: Only epilepsy nurses' perspectives were sought.

RESUMO

Many young adults in countries with advanced economies make linear career transitions from school to work. However, there is a great variability in youth's postsecondary education and early work experiences and more work is needed to identify the career trajectories of modern youth. Using longitudinal data on 1747 young adults from the National Longitudinal Survey of Youth-1997, the current study identifies work and school pathways commonly seen in the transition to adulthood and investigates multiple levels of adolescent precursors. Using latent class analysis, we identified four work and school pathways centered on educational attainment. Whereas parental education and future educational expectations in adolescence significantly shaped the career pathways, local labor market conditions played a limited role in predicting these pathways.

Career development frameworks help professionals better understand career decision-making, but the genetic counseling field lacks a comprehensive framework to describe career development. The purpose of this qualitative study was to explore commonalities across participants' career trajectories and identify factors which influence decision-making throughout genetic counseling careers. Using purposive sampling, 17 genetic counselors with advanced skills were interviewed about their career trajectories, factors which influenced career decisions, and the process and outcomes of those decisions. Content analysis was both inductive and deductive, employing triangulation techniques to enhance analytic rigor. Results highlighted common experiences and critical processes of interviewees' career trajectories and contributed to the development of the Genetic Counselor Career Trajectory Framework (GCCTF), which depicts an iterative process of considering change in one's career. Each iteration is prompted by predisposing influences (past experiences, personal attributes, and contextual factors), characterized by self-assessment and flexible planning, and completed when a decision about making a change is reached. Multiple iterations collectively create evolution of a career trajectory. The GCCTF adds to existing theories of career development by emphasizing dynamic processes of considering change and applies established concepts to a specialized healthcare profession. Individual genetic counselors can utilize the GCCTF to expand awareness of factors influencing a specific career decision and gain insight into experiences of change across their careers.

A scientist may publish tens or hundreds of papers over a career, but these contributions are not evenly spaced in time. Sixty years of studies on career productivity patterns in a variety of fields suggest an intuitive and universal pattern: Productivity tends to rise rapidly to an early peak and then gradually declines. Here, we test the universality of this conventional narrative by analyzing the structures of individual faculty productivity time series, constructed from over 200,000 publications and matched with hiring data for 2,453 tenure-track faculty in all 205 PhD-granting computer science departments in the United States and Canada. Unlike prior studies, which considered only some faculty or some institutions, or lacked common career reference points, here we combine a large bibliographic dataset with comprehensive information on career transitions that covers an entire field of study. We show that the conventional narrative confidently describes only one-fifth of faculty, regardless of department prestige or researcher gender, and the remaining four-fifths of faculty exhibit a rich diversity of productivity patterns. To explain this diversity, we introduce a simple model of productivity trajectories and explore correlations between its parameters and researcher covariates, showing that departmental prestige predicts overall individual productivity and the timing of the transition from first- to last-author publications. These results demonstrate the unpredictability of productivity over time and open the door for new efforts to understand how environmental and individual factors shape scientific productivity.

Students of nursing enter their programmes of study with preconceived ideas of what a career in their chosen profession will entail. The literature suggests that images from the media and past experiences contribute to these perceptions. Although it is positive images of the profession that will usually attract an individual to a career in nursing, often more negative perceptions will direct students away from potentially rewarding areas of specialization. This paper describes career projections of nursing students enrolled in the first year of four preservice nursing programmes at the rural campus of one Australian university. Part of a larger study, the data reported here indicate that most respondents intend to practice in the areas of midwifery, paediatrics and emergency nursing. Oncology, community nursing, aged care and mental health nursing all ranked poorly across three rounds of surveys. These findings have implications for practicing nurses and nurse educators who seek to dispel inaccurate images of these important specializations.

Taking a gap year to obtain an additional degree or for research is becoming increasingly popular among medical students pursuing dermatology to bolster residency application competitiveness. The purpose of this study was to determine whether doing so influences future academic achievement and career trajectory. A list of dermatologists who achieved board certification in 2010 was obtained from the American Board of Dermatology. Information regarding additional post-graduate degrees, research years, fellowship specialization, practice setting, publication number, and H-index were obtained using publicly available data. Additional degrees were associated with higher research productivity, higher H-index, and practicing at teaching hospitals while taking a research year was only associated with a higher H-index. This data can be used to further inform medical students wishing to achieve high levels of research productivity and careers at teaching institutions and residency programs wishing to recruit such applicants.

Background: Contemporary research has shown that only a small proportion of high achieving young athletes continue to become successful senior athletes. However, there is a lack of empirical literature tracking the success of senior male and female athletes who were considered high achieving as a youth. Hypothesis: Athletes of both sexes who are successful in youth categories (U18 and U20) are more likely to be successful senior athletes. Conclusion: Data from 67,600 athletes were collated from the tilastopaja.org platform. The inclusion criteria for both genders were determined by top-100 ranking in the U18 and U20 age groups and progression to the top-100 as a senior athlete. Only 23.5% of successful track and field athletes (ranked in top-100) at U18 became a successful senior athlete, while 35.4% were from the U20 group. Girls ranked in the top-100 U18 and U20 categories are significantly more likely to be ranked in the top-100 as a senior when compared to boys. Although, being ranked in the top-50 at U18 and U20 significantly increases the probability of becoming a successful senior athlete when compared with less successful athletes at these age groups (p < 0.001). Notably, the majority (68.5%) of the most successful senior athletes were not ranked in the top-100 when in the U18 or U20 age groups. Only a small group of track and field athletes that are successful at U18 and U20 become successful at senior level. The most successful track and field youth athletes are significantly more likely to succeed as a senior athlete than their less successful peers, while girls are more likely to be successful than boys.

We present norms for faculty citation counts based on 811 faculty members at 30 PhD-granting psychology departments in the United States across the range of the National Research Council rankings. The metrics were highly skewed, with most scientists having a low to moderate number of citations of their work and a few scientists having extremely high numbers. However, the median per-year citation count was 149, showing widespread scientific contributions across scholars. Some individuals in lower ranked departments are more highly cited than the average scholar in higher ranked departments, with enormous variation in citation counts in both the low- and high-ranking departments. Citation counts overall have risen in recent years, and the citations of early-career scholars are increasing at a faster rate than their senior colleagues did at the same point in their careers. We found that citation counts at the beginning of scientists' careers substantially predict lifetime citation success. Young scholars' citation counts are associated with obtaining positions at higher ranked universities. Finally, we found no significant differences for subfields of psychology. In sum, although a few highly productive scientists have a very large influence, trends reveal that contributions to psychological science are growing over time, widespread, and not limited to a few stars and elite departments.

Despite extensive evidence for a positive association between socio-economic status (SES) and health, some studies have shown that this well-established pattern of health inequality is reversed in Japan due to individuals of high SES working in stressful workplace environments. High-SES workers in Japan generally belong to a lifetime employment system (LES) in large companies. Thus, in this study, individuals who had been working for a single company for several decades at the time of a 2005 survey (LES workers) were compared with other workers by logistic regression and ordinary least squares regression. These analyses showed that LES workers had 36% more household savings. However, despite their relatively high income, the LES workers were more likely to develop diabetes (odds ratio 1.134: 95% CI 1.022-1.259) and hyperlipidemia (odds ratio 1.184: 95% CI 1.079-1.300). Among women, LES workers were at higher risk of developing cancer (odds ratio 1.570: 95% CI 1.174-2.098). In addition, these effects were consistently found in subsequent surveys between 2006 and 2015, suggesting that the LES had long-term adverse effects on health. These results taken together show that career trajectory is an important determinant of health inequality in the elderly population.

The Association of Pathology Chairs Senior Fellows Group provided reflections on activities that have kept them engaged and inspired after stepping down as chair. They offered advice to current chairs who were considering leaving their positions and also to individuals contemplating becoming pathology chairs. A majority (35/41) responded: 60% maintained teaching/mentoring activities; 43% engaged in hobbies; 40% took other administrative positions including deans, medical center chief executive officers, and residency program directors; 31% continued research; 28% wrote books; 20% performed community service; 14% led professional organizations; 14% developed specialized programs; 11% engaged in clinical service; and 11% performed entrepreneurial activities. Most individuals had several of these activities. One-third indicated that those considering becoming chair should be able to place faculty and department needs before their own. One-fourth emphasized the need to know why one wants to become chair, the need to develop clear goals, and the need to know what one wants to accomplish as chair before applying for and accepting the position. More than half (57%) indicated that before stepping down as chair, one should have a clear plan and/or professional goals that can be served by stepping down. Some even suggested that this be in place before applying for the chair. Almost two-thirds (63%) indicated they had no regrets stepping down as chair. These findings may be valuable to those contemplating stepping down from or stepping into any department chair position or other academic leadership role.

OBJECTIVES: To assess the impact of a 1-week training seminar jointly developed and conducted by the World Health Organization and the University Institute of Social and Preventive Medicine of Lausanne targeting senior policy-makers in low- and middle-income countries on public health aspects of noncommunicable diseases (NCDs). METHODS: A short qualitative questionnaire was emailed to all participants to one of the nine seminars organized between 2010 and 2015. RESULTS: From the 195 participants from 96 different countries, 122 (63%) completed the questionnaire. Among them, 87% reported that the seminar made a positive contribution to their professional development and 48% said it helped strengthening their national NCD program. All respondents remained directly or indirectly involved in NCD work. A frequent suggestion was that similar seminars are developed in their region or country. CONCLUSIONS: The evaluation strongly suggests that this short-term seminar had positive impact on both participants' personal development and the organization they worked for. There is a demand for organizing similar seminars at regional/country levels to support NCD prevention and control programs.
