OBJECTIVE: We aimed at the identification of genetic alterations that may functionally substitute for CTNNB1 mutation in ß-catenin-activated hepatocellular adenomas (HCAs) and hepatocellular carcinoma (HCC). DESIGN: Large cohorts of HCA (n=185) and HCC (n=468) were classified using immunohistochemistry. The mutational status of the CTNNB1 gene was determined in ß-catenin-activated HCA (b-HCA) and HCC with at least moderate nuclear CTNNB1 accumulation. Ultra-deep sequencing was used to characterise CTNNB1wild-type and ß-catenin-activated HCA and HCC. Expression profiling of HCA subtypes was performed. RESULTS: A roof plate-specific spondin 2 (RSPO2) gene rearrangement resulting from a 46.4 kb microdeletion on chromosome 8q23.1 was detected as a new morphomolecular driver of ß-catenin-activated HCA. RSPO2 fusion positive HCA displayed upregulation of RSPO2 protein, nuclear accumulation of ß-catenin and transcriptional activation of ß-catenin-target genes indicating activation of Wingless-Type MMTV Integration Site Family (WNT) signalling. Architectural and cytological atypia as well as interstitial invasion indicated malignant transformation in one of the RSPO2 rearranged b-HCAs. The RSPO2 gene rearrangement was also observed in three ß-catenin-activated HCCs developing in context of chronic liver disease. Mutations of the human telomerase reverse transcriptase promoter-known to drive malignant transformation of CTNNB1-mutated HCA-seem to be dispensable for RSPO2 rearranged HCA and HCC. CONCLUSION: The RSPO2 gene rearrangement leads to oncogenic activation of the WNT signalling pathway in HCA and HCC, represents an alternative mechanism for the development of b-HCA and may drive malignant transformation without additional TERT promoter mutation.

Microplastics (MPs) content of the digestive tract of two commercial fish from the northern shores of the Oman Sea were investigated. The MPs were characterized by optical microscopy, fluorescent microscopy, and SEM-EDX for their number, shape, size, and color. Polymer composition was analyzes using micro-Raman spectroscopy (RMS). MPs were recovered in all fish samples (100 %), with an average of 43.16 ± 8.23 items/individual in Otolithes ruber, and 29.9 ± 2.73 items/individual in Acanthopagrus latus. The predominant shape of MPs in both fishes was fiber (46 %) with black, transparent, and white colors. The majority of MPs were <1000 µm (75 %), and half of the MPs were smaller than 300 µm in size. Their synthetic nature was confirmed by Nile Red staining and determination of the elemental composition of selected items. Polypropylene (PP) and polyethylene (PE) were the dominant plastic polymers in the fish digestive tracts. This study reveals abundance distribution of MPs in digestive tract of commercial marine fish. High number of ingested MPs can alarm the accumulation of MPs in the northern of Oman Sea ecosystem with anthropogenic activities and raises issues in public health.

The distribution of trace elements in sediments and five commercially important fish species of the Oman Sea were assessed using an atomic absorption spectrometer to determine ecological and human health risk assessment. The ranges of trace elements concentration in sediments were: Cr: 5.42-68.94, Zn: 5.22-18.11, Cu: 1.44-4.87, As: 18.07-79.96, Cd: 0.10-1.27, Ni: 43.0-65.34 and Pb: 8.92-30.53 mg/kg dry weight. Cr, Zn, Cu, Cd and Pb were below the ERL (effects range low) and TEL (threshold effect level) values at all the sampling stations with rarely biological effects. Whereas, As and Ni were higher than ERL and TEL with occasionally biological effects. The patterns of trace elements bioaccumulation in the tissues of all species were: liver> skin> gill> muscle. A significant positive correlation suggests that trace elements accumulation in fish is determined by trace elements in sediments. Estimated Daily Intake (EDI) was significantly lower than the tolerable daily intake (TDI) value. Target Hazard quotient (THQ), total target hazard quotient (TTHQ) and target carcinogenic risk (TR) values indicated no hazard risk from fish consumption. Hence, the consumption of concerned species to people in the Oman Sea is safe.

Nowadays, as technology grows, human life formation changes every second. These changes sometimes create harmful habits and affect our lifestyle, which unfortunately bring various diseases and disorders. ADHD (Attention Deficit Hyperactivity Disorder) & SPD (Sensory Processing Disorder) are common disorders in children. By giving children the right treatment at the younger ages, we can help them overcome their disabilities and not face any symptoms in their adulthood. The multi-sensory balance board is designed and developed to help medics diagnose the disorders in patients at early stages and monitor the treatment progress. The balance board uses multiple sensors to detect common human senses, visual, auditory, balance, and tactile sensing. Due to the safety issues for children during the COVID-19 pandemic, the balance board has been clinically tested for adults with stroke disorder. The research has concluded that it provides a fast low-cost with reliable results to diagnose the disorders in patients.

BACKGROUND: Smoking hookah is considered a health problem among the adolescents, which currently has a high prevalence. The present study aimed to determine the reasons for tendency toward smoking hookah. METHODS: Participants of this qualitative study were 24 adolescents from Tehran selected using purposeful sampling method in 2017. Data gathered through individual semi-structured interviews and two focus groups. Using directed content analysis approach 423 primary codes were categorized into 28 subcategories and 9 categories. RESULTS: Most of the participants were 16 to 18 yr old (64%), male (58%) and had high school degree (58%) and 75% of them were student. In Theory of Planned Behavior, attitude toward smoking hookah contained categories of replacement for cigarette, the pleasurableness of hookah, easy access and spending the leisure times. Subjective norms contained the categories of family, friends and society. The construct of behavioral control contained the categories of personal and social factors. CONCLUSION: Our Results could be helpful in policy-making and planning toward improving the awareness and changing the attitude and performance of the adolescents toward smoking hookah.

BACKGROUND: Self-medication is a serious health problem that leads to an increased per capita consumption of medications, drug resistance, lack of optimal treatment, drug poisoning, and other unwanted complications. This study was conducted to compare self-medication in pregnant and nonpregnant women presenting to Shahid Akbar Abadi Teaching Hospital in Tehran, Iran. MATERIALS AND METHODS: To conduct this cross-sectional study, 210 pregnant women and 210 nonpregnant women aged 15-45 years presenting to Shahid Akbar Abadi Teaching Hospital, Tehran, Iran, were selected through random sampling. Data were collected through interviews using a semi-structured questionnaire. The Chi-square test, t-test, and logistic regression model were used to analyze the data. RESULTS: The prevalence of self-medication was 34.8% in the pregnant and 77.1% in the nonpregnant women. The age group in which the most frequent instances of self-medication were observed (53.4%) was the 21-30 age group in the pregnant women and the 31-40 age group (44.4%) in the nonpregnant women, suggesting a statistically significant intergroup difference in terms of age (P = 0.0001). Medicinal plants were the most common medications used by the pregnant women (19.6%) and synthetic medications were the most common used by the nonpregnant women (38.1%). The reasons for using medications without a prescription included believing in the illness being mild (22.8%), not having health insurance (9%), easy access in the pregnant women, a previous history of the illness, and easy access in the nonpregnant women. CONCLUSIONS: As medicinal plants are the most common medications used by pregnant women and since assessing the risk of herbal substances is difficult, pregnant women should be advised against the arbitrary use of these substances.

BACKGROUND: Changes in health programs in Iran have led to an increase in administrative costs. One cost-saving option available to hospital administrators is to outsource administrative services. OBJECTIVE: This study aimed to explore the attitudes of hospital staff towards outsourcing health information management services in advance of a decision being taken, to assist healthcare organisations to assess the potential benefits and challenges of outsourcing such services. METHOD: Six hundred and four clinical and allied health employees in two hospitals in Iran, who had had prior experience with outsourcing hospital services, responded to a survey designed to measure staff attitudes towards outsourcing health information management services, based on their perceptions of potential costs and benefits for the organisation and their own employment prospects. A 16-item attitude scale, developed by the researchers, was used in the study and demographic data were also collected. RESULTS: Summary statistics showed that approximately one third of the sample (34.53%) had a negative view of outsourcing, one third (35.16%) had a positive view, and 30.31% were neutral. An exploratory factor analysis of items on the attitude scale identified three underlying constructs, labelled: data security and management; workplace environment; and staff and customer satisfaction. One item (concern about the impact of outsourcing on staffing levels) did not load on any of the factors. A separate analysis of this single item showed a significant relationship between the sex of participants and their views on the impact of outsourcing on the number of hospital staff employed (p<0.05). CONCLUSION: While results of this study indicated that staff in hospitals surveyed held both positive and negative views of outsourcing, a large number of staff in these hospitals (a third of the total sample) had reservations about the skills of outside providers to securely manage hospital data, and did not consider that outsourcing health information management services would be positive for the organisation, their working environment or for staff and patient satisfaction. These findings have important implications for healthcare organisations planning to outsource health information services. Further research that focuses on communication skills of senior managers and their ability to provide team leadership is needed, as is research into the impact of geographical location and current market forces that impact on outsourcing tasks.

OBJECTIVE: In human beings, the balance control is a result of an integration of physical and psychological factors and people show different reactions when feel anxious or face stability threatened conditions such as standing on an unstable surface. University students, particularly the female ones, are a group of the society who are prone to face anxieties. The current study focused on the effects of anxiety on balance index in young female university students. METHODS: A group of 15 female students with a high anxiety score (higher than 42 in Shpielberger Questionnaire) and a group of 15 female students with a low anxiety score (lower than 42 in Shpielberger Questionnaire) were recruited for this study. Using a Biodex stabilometer (Biodex System, USA), the antero-posterior (AP), medio-latral (ML) and the overall dynamic stability index (SI) of the subjects were recorded and compared. RESULTS: The results revealed a significant difference between the two groups. Subjects with a high anxiety score showed a stability index higher than those in the low anxiety group (p<0.005), which simply indicate significantly less stability in this group. CONCLUSION: This study showed that subjects with higher anxiety scores were less stable compared to those subjects with lower anxiety scores.
