RESUMEN

What are the harsh consequences of racism and our country's slavery past in the life of every black woman today? How does the current federal government reinforce the reproduction of structural sexism and racism during a pandemic? Hunger, domestic violence, femicide, informal work, unemployment, and political under-representation are issues that are urgently presented by the need for an anti-racist social transformation. Black women remain vulnerable to social inequalities. And in the pandemic, these inequalities are not only related to its management (contamination, death, and vaccination), but also its economic and social consequences.

Despite causing high morbidity, IgG4-related disease (IgG4-RD) and its gastroenterological manifestations lack better and greater theoretical contributions. Therefore, the objective of this work was to evaluate the clinical-epidemiological, diagnostic and treatment aspects of the gastrointestinal manifestations of this disease. A systematic review was carried out using the PubMed, Scopus and Embase databases between January 2012 and March 2023 with the following descriptors: "Immunoglobulin G4-Related Disease" (MeSH) AND #2 "Gastrointestinal Tract" (MeSH). Our data collection grouped a total of 3607 patients from mostly epidemiological cohort studies and cross-sectional follow-ups. In the subgroup analysis, IgG4-RD was associated with male gender, with an estimated prevalence between 54% and 80%. In our findings, the prevalence by topography was presented in the following ranges: lacrimal glands (17-57%); salivary glands (28-72%); pancreas (19-60%); biliary tract (5-40%); retroperitoneal cavity (9-43%). Longitudinal studies are needed to better map the natural history of the gastrointestinal manifestations of IgG4-RD and enable the formulation of individualized treatments.

RESUMEN

The growing rate of chronic diseases among children and adolescents, especially diabetes mellitus, calls for integrated health care of the individual in his or her social environment. The objective of this study was to understand the health care experiences and feelings of adolescents and their mothers about having diabetes and the treatment. Semi-structured interviews were performed with eight adolescents and seven mothers at a public healthcare service in the city of Barbalha, in the state of Ceará, Brazil. Analysis revealed the following categories: feelings expressed when the disease was diagnosed; living with the disease and its psychosocial implications; and changes in one's lifestyle. The adolescents' difficulties raise concerns about behaviors and adaptations to the new way of being and self-care. There is a need for care that integrates physical and psychosocial dimensions to improve the care that is delivered to adolescents and their families.

RESUMEN

O estudo descreve a experiência de uso da telemedicina no processo de ensino-aprendizagem em Pediatria. Trata-se de relato de experiência da prática de telemedicina em Pediatria do curso de graduação em Medicina da Universidade Federal do Ceará Campus Cariri. A telemedicina em Pediatria foi desenvolvida pela Rede Universitária de Teleconferência em parceria com instituições federais de ensino médico. O projeto desenvolveu-se mediante programação semestral com sessões de temas relevantes em Pediatria. Entre as etapas de sua realização, destacaram-se os debates, desenvolvimento de grupo focal e avaliação dos conteúdos abordados na teleconferência. As sessões ocorreram na universidade, com participação de professores, estudantes de Medicina do terceiro ano, internos e residentes em Pediatria. Os resultados evidenciaram a teleconferência como recurso para a consolidação de metodologias ativas do processo de ensino-aprendizagem, com protagonismo dos estudantes em sua formação acadêmica, como um instrumento importante na integração ensino-serviço e como tecnologia inovadora para a problematização pedagógica de práticas clínicas. Conclui-se que a telemedicina representa uma possibilidade ampliada de construção do conhecimento e aponta-se a necessidade de maior investimento nesta tecnologia.


This study reports on the experience of using telemedicine to teach pediatrics as part of the undergraduate course in Medicine at the Federal University of Ceará, Cariri Campus. Telemedicine in pediatrics was developed by the Teleconference University Network in partnership with other federal medical schools. The project was developed through six-monthly scheduling of sessions focused on relevant pediatric topics. The project was developed through debates, focus groups and teleconference content evaluation. The sessions were held at the university, with the participation of teachers, third-year medical students, pediatric internists and residents. The results revealed that teleconference could be used as a resource to consolidate active learning methodologies, with a student-centered approach to academic training, as well as an important tool in service-learning integration and innovative technology for pedagogical critical questioning of medical practice. It can be concluded that telemedicine represents a great opportunity for knowledge construction and shows the need for more investment in technology.

O aumento de doenças crônicas entre crianças e adolescentes, especialmente a Diabetes mellitus, requer conhecimentos que integrem os cuidados à saúde e a integração do indivíduo ao seu meio social. O objetivo foi compreender as experiências e os sentimentos de adolescentes e de suas mães sobre a condição de ser diabético, o tratamento e os cuidados à saúde. Realizamos entrevista semiestruturada com oito adolescentes e sete mães em um serviço público de Barbalha - Ceará - Brasil. A análise dos discursos resultou nas categorias: Sentimentos expressos diante da descoberta da doença; A convivência com a doença e as implicações psicossociais; Mudanças no estilo de vida. As dificuldades dos adolescentes induzem à reflexões sobre comportamentos e adaptações ao novo modo de ser e ao autocuidado. Há necessidade de um cuidado integrando as dimensões físicas e psicossociais de modo a melhorar a assistência ao adolescente e à sua família.

El aumento de enfermedades crónicas entre niños y adolescentes, especialmente la Diabetes mellitus, requiere conocimientos que integren los cuidados a la salud y la integración del individuo a su medio social. El objetivo fue comprender las experiencias y los sentimientos de adolescentes y de sus madres sobre la condición de ser diabético, el tratamiento y los cuidados de la salud. Realizamos una entrevista semiestructurada con ocho adolescentes y siete madres en un servicio público de Barbalha - Ceará - Brasil. El análisis de los discursos resultó en las categorías: Sentimientos expresados delante de la descubierta de la enfermedad; La convivencia con la enfermedad y las implicaciones psicosociales; y, Cambios en el estilo de vida. Las dificultades de los adolescentes inducen a la reflexión sobre comportamientos y adaptación al nuevo modo de ser y al auto cuidado. Hay necesidad de ofrecer un cuidado integrando las dimensiones físicas y psicosociales de modo a mejorar la asistencia al adolescente y a su familia.
