RESUMEN

The concentration of 25-hydroxyvitamin D was measured in serum of 90 healthy and 123 sick infants. The mean concentration of 25-OH-D in the group of healthy infants given intermittent prophylactic doses of vitamin D3 was 44.0 microgram/l and was nearly three times higher than the average concentration in healthy adults not given any vitamin D substitution. In the 123 clinically treated infants the average 25-OH-D concentration was 86.2 microgram/l, and 59.4% of infants had values over 40 microgram/l. In the subgroup of infants admitted to the hospital because of diarrhea, lower values were found than the average ones of clinically-treated infants. Two children with clinical rickets had very low 25-OH-D concentrations. The 25-OH-D half life in serum of infants varied from 19 to 27 days and single dose--300,000 IU (7.5 mg) of vitamin D3 was found to assure a sufficient 25-OH-D concentration in serum for as many as 190 days. It is concluded that high intermittent oral doses of vitamin D3 give an effective protection against rickets but at the cost of several times higher 25-OH-D concentration in serum and at the risk of hypervitaminosis D in some healthy infants.

RESUMEN

The author points to the need of further training gynecologists in subspecialties resulting from the progress in this basic branch of medicine. It is particularly important to organize further training in gynecological oncology, reproductive endocrinology, maternal-fetal medicine (perinatology) and gynaecological urology. Accepting training in subspecialties, preparing programs and tests of skills and knowledge will give better results in specialist therapy of patients.

RESUMEN

PIP: 30 women, aged 22-37 years, were inserted with the Lippes Loop. 10 of these women were subjected to clinical observations and histological examinations 6 months after insertion. These served as a control to the other 20 users, who were subjected to the same examinations 2 years later. The groups who used the device for the longer period were observed to have clinically longer and more heavy menstruation than the control group. The most frequent histological aberration noted in these women was a lengthening of the proliferative phase and lateness of the secretory phase of the mucus membrane in the menstrual cycle. In our work we evaluated according to Kupperman Index the effectiveness of treating menopausal signs and symptoms during the therapy with estrogen--progestagen preparation Klimonorm firm Jenapharm. During 6 months observation increase in blood pressure was not observed, neither an increase in body weight in a group of 116 women was not observed. Among these, mean Kupperman index was decreased from 28.38 to 11.6 after 3 months and up to 5.47 after 6 months therapy with Klimonorm. These patients estimated this agent as very good and good in 93.21% tolerated as very good and good in 91.43% and 91.59% of these patients planned to continue treatment with Klimonorm.

Now triphasic contraceptive preparations are used widely. Some news low dose formulations are available on the market, among them Trisiston. The study was conducted to confirm clinically acceptance and effectiveness of Trisiston. There were 437 volunteers mostly multiparous, with a mean age of +/- 25 years who participated in a multi center study. They were taking pills for a 3 to 12 cycles. Cumulatively 4026 cycles were observed. The investigation was conducted according special protocol. At the admission a complete medical history was obtained and a general physical and gynecological examination was performed. All subject were controlled every 3rd month included laboratory tests (blood levels of Hb, WBC, RBC, ESR, carbohydrate metabolism, clotting factors, bilirubin, creatinine, protein, glucose, albumin, LDL, HDL). There was no case of pregnancy. Pearl index = 0. The Trisiston was well tolerated by most of women (80%). Some effects like headache, vertigo, mastalgia, spotting and breakthrough bleeding, loss of libido, premenstrual syndrome were observed in 20% patients, soma of our patients (20%) discontinued contraceptive.

This paper estimates the excretory kidneys activity in women with pregnancy complicated by EPH-gestosis and in women with a physiological pregnancy by denoting albuminuria, beta-2-microglobulinuria and beta-2-microglobulin in blood serum. It was claimed that the excretion of albumins with urine increases considerably in women with a pregnancy complicated by EPH-gestosis before the delivery in comparison with women with a pregnancy of a physiological course. The increase of albumins excretion with urine in a labour in two groups of tested pregnant women was also noticed. However, the increase in excretion of albumins with urine during delivery was much bigger in the group of women with pregnancy complicated by EPH-gestosis in comparison to the group of women with a physiological course of pregnancy. The albumins size before delivery as well as during the next 48 hours after the delivery did not differ statistically in both groups of tested women. Beta-2-microglobulin concentration in blood serum was bigger in the group of pregnant women with EPH-gestosis than women with a physiological pregnancy during the whole period of research. Bigger excretion of beta-2-microglobulin with urine was noticed among women with pregnancy complicated by EPH-gestosis before delivery, during delivery and during the next 48 hours after the delivery in relation to the group of women with a physiological pregnancy.

Hemodynamic changes related to pregnancy in women with Congenital Heart Disease (ConHD) or Acquired Valvular Heart Disease (AVHD) require special medical care during pregnancy and especially immediately prior to, and after delivery. Following a five year retrospective study (1978-1982), the authors monitored prospectively over a period of 12 years (1983-1994), 378 pregnancies in 252 women with ConHD and AVHD. Based on the results of the initial clinical examination and monthly follow-up visits, a numerical risk score was assigned for each patient. The study showed good conformity between the predictive risk score values and the patient's condition in the course of pregnancy and delivery. Thus, this method represents a novel and potentially very useful clinical tool for management of pregnancy in patients with ConHD and AVHD.
