Telomere length is associated with chronic diseases and in younger populations, may represent a biomarker of disease susceptibility. As growing evidence suggests that environmental factors, including metals, may impact telomere length, we investigated the association between 17 metals measured in toenail samples and leukocyte relative telomere length (RTL), among 472 five- to seven-year-old children enrolled in the Bangladesh Environmental Research in Children's Health (BiRCH) cohort. In single exposure linear regression models, a doubling of arsenic (As) and mercury (Hg) (µg/g) were associated with a -0.21 (95%CI: -0.032, -0.010; p=0.0005) and -0.017 (95%CI: -0.029, -0.004; p=0.006) difference in RTL, respectively. In Bayesian Kernel Machine Regression (BKMR) mixture models, the overall metal mixture was inversely associated with RTL (P-for-trend <0.001). Negative associations with RTL were observed with both log2-As and log2-Hg, while an inverted U-shaped association was observed for log2-zinc (Zn) with RTL. We found little evidence of interaction among metals. Sex-stratification identified stronger associations of the overall mixture and log2-As with RTL among females, compared to males. Our study suggests that As and Hg may independently influence RTL in mid-childhood. Further studies are needed to investigate potential long-term impacts of metal-associated telomere shortening in childhood on health outcomes in adult life.

BACKGROUND: Major Depressive Disorder (MDD) is a common mental ailment and is the primary reason for disability. It manifests a severe impact on moods, thoughts, and physical health. At present, this disorder has become a concern in the field of public health. Alteration of neurochemicals is thought to be involved in the pathogenesis of many psychiatric disorders. Therefore, we aimed to evaluate serum IL-3 and lipocalin-2 in MDD patients and healthy controls (HCs). METHOD: We included a total of 376 participants in this study. Among them, 196 were MDD patients, and 180 were age-sex-matched HCs. MDD patients were recruited from the Psychiatry Department of Bangabandhu Sheikh Mujib Medical University (BSMMU), but the controls were from different parts of Dhaka. All study participants were evaluated by a psychiatrist using the DSM-5 criteria. To assess the severity of the depression, we used the Hamilton depression (Ham-D) rating scale. Serum IL-3 and lipocalin-2 levels were measured using commercially available enzyme-linked immune-sorbent assay kits (ELISA kits). RESULTS: According to this study, we observed elevated serum levels of IL-3 (1,024.73 ± 29.84 pg/mL) and reduced levels of serum lipocalin-2 (29.019 ± 2.073 ng/mL) in MDD patients compared to HCs (911.11 ± 20.55 pg/mL and 48.065 ± 3.583 ng/mL, respectively). No associations between serum levels of IL-3 and lipocalin-2 and depression severity were observed in patients. CONCLUSIONS: According to the present findings, alterations of serum IL-3 and lipocalin might be associated with the pathogenesis of MDD. These results support that altered serum neurochemicals can serve as early risk assessment markers for depression. Further interventional studies are recommended for a better understanding of the role of IL-3 and lipocalin-2 in the pathophysiology of depression.

OBJECTIVE: Interleukin-17A (IL-17A) genetic polymorphisms are associated with multiple cancer types, including colorectal cancer (CRC). However, no previous study was performed in the Bangladeshi population to evaluate the association. Our study aimed to find the association between two IL-17A variants (rs10484879 C/A and rs3748067 G/A) and susceptibility of CRC. METHODS AND MATERIALS: This retrospective case-control study comprised 292 CRC patients and 288 age, sex, and BMI matched healthy volunteers. Genotyping of both variants was done by the tetra-primer ARMS-PCR method, and the results were analyzed by the SPSS software package (version-25.0). RESULTS: Logistic regression analysis indicated that in case of IL-17A rs10484879 polymorphism, AC and AA genotype carriers showed 2.44- and 3.27-times significantly increased risk for CRC development (OR = 2.44, P = .0008 and OR = 3.27, P = .0133, individually). A significant association was also observed for AC + AA genotype (OR = 2.58, P = .0001). Again, over-dominant and allelic model revealed statistically significant link to CRC risk (OR = 2.13, P = .0035 and OR = 2.22, P = .001). For rs3748067 polymorphism, AG and AA genotype carriers showed 2.30- and 2.45-times enhanced risk for CRC (OR = 2.30, P = .005 and OR = 2.45, P = .031). A statistically significant association was also observed for AG + AA genotype (OR = 2.35, P = .001), over-dominant model (OR = 2.05, P = .014), and allelic model (OR = 2.11, P = .0004). CONCLUSION: This study highlights that IL-17A rs10484879 and rs3748067 polymorphisms may be associated with CRC development. However, further functional research with larger samples may reveal more statistically significant outcomes.

Cardiovascular disease (CVD) is the leading cause of morbidity and mortality worldwide, with â¼80% of CVD-related deaths occurring in low- and middle-income countries. Growing evidence suggests that chronic arsenic exposure may contribute to CVD through its effect on endothelial function in adults. However, few studies have examined the influence of arsenic exposure on cardiovascular health in children and adolescents. To examine arsenic's relation to preclinical markers of endothelial dysfunction, we enrolled 200 adolescent children (ages 15-19 years; median 17) of adult participants in the Health Effects of Arsenic Longitudinal Study (HEALS), in Araihazar, Bangladesh. Participants' arsenic exposure was determined by recall of lifetime well usage for drinking water. As part of HEALS, wells were color-coded to indicate arsenic level (<10 µg/L, 10-50 µg/L, >50 µg/L). Endothelial function was measured by recording fingertip arterial pulsatile volume change and reactive hyperemia index (RHI) score, an independent CVD risk factor, was calculated from these measurements. In linear regression models adjusted for participant's sex, age, education, maternal education, land ownership and body weight, individuals who reported always drinking water from wells with >50 µg/L arsenic had a 11.75% lower level of RHI (95% CI: -21.26, -1.09, p = 0.03), as compared to participants who drank exclusively from wells with ≤50 µg/L arsenic. Sex-stratified analyses suggest that these associations were stronger in female participants. As compared to individuals who drank exclusively from wells with ≤50 µg/L arsenic, the use of wells with >50 µg/L arsenic was associated with 14.36% lower RHI (95% CI: -25.69, -1.29, p = 0.03) in females, as compared to 5.35% lower RHI (95% CI: -22.28, 15.37, p = 0.58) in males for the same comparison. Our results suggest that chronic arsenic exposure may be related to endothelial dysfunction in adolescents, especially among females. Further work is needed to confirm these findings and examine whether these changes may increase risk of later adverse cardiovascular health events.

This paper addresses coverage loss and rapid energy depletion issues for wireless livestock sensor networks by proposing a UAV-based energy-efficient reconfigurable routing (UBER) scheme for smart wireless livestock sensor networking applications. This routing scheme relies on a dynamic residual energy thresholding strategy, robust cluster-to-UAV link formation, and UAV-assisted network coverage and recovery mechanism. The performance of UBER was evaluated using low, normal and high UAV altitude scenarios. Performance metrics employed for this analysis are network stability (NST), load balancing ratio (LBR), and topology fluctuation effect ratio (TFER). Obtained results demonstrated that operating with a UAV altitude of 230 m yields gains of 31.58%, 61.67%, and 75.57% for NST, LBR, and TFER, respectively. A comparative performance evaluation of UBER was carried out with respect to hybrid heterogeneous routing (HYBRID) and mobile sink using directional virtual coordinate routing (MS-DVCR). The performance indicators employed for this comparative analysis are energy consumption (ENC), network coverage (COV), received packets (RPK), SN failures detected (SNFD), route failures detected (RFD), routing overhead (ROH), and end-to-end delay (ETE). With regard to the best-obtained results, UBER recorded performance gains of 46.48%, 47.33%, 15.68%, 19.78%, 46.44%, 29.38%, and 58.56% over HYBRID and MS-DVCR in terms of ENC, COV, RPK, SNFD, RFD, ROH, and ETE, respectively. The results obtained demonstrated that the UBER scheme is highly efficient with competitive performance against the benchmarked CBR schemes.

We assessed whether personal exposure to household air pollution [PM2.5 and black carbon (BC)] is associated with lung functions (FEV1, FVC, and their ratio) in non-smoking adults in rural Bangladesh. We measured personal exposure to PM2.5 using gravimetric analysis of PM2.5 mass and BC by reflectance measurement between April 2016 and June 2019. The average 24-hour PM2.5 and BC concentration was 141.0µgm-3 and 13.8µgm-3 for females, and 91.7 µgm-3 and 10.1 µgm-3 for males, respectively. A 1 µgm-3 increase in PM2.5 resulted in a 0.02 ml reduction in FEV1, 0.43 ml reduction in FVC, and 0.004% reduction in FEV1/FVC. We also found a similar inverse relationship between BC and lung functions (9.6 ml decrease in FEV1 and 18.5 ml decrease in FVC per 1µgm-3 increase in BC). A higher proportion of non-smoking biomass fuel users (50.1% of the females and 46.7% of the males) had restrictive patterns of lung function abnormalities, which need further exploration.

In this paper, a new optimization algorithm called motion-encoded electric charged particles optimization (ECPO-ME) is developed to find moving targets using unmanned aerial vehicles (UAV). The algorithm is based on the combination of the ECPO (i.e., the base algorithm) with the ME mechanism. This study is directly applicable to a real-world scenario, for instance the movement of a misplaced animal can be detected and subsequently its location can be transmitted to its caretaker. Using Bayesian theory, finding the location of a moving target is formulated as an optimization problem wherein the objective function is to maximize the probability of detecting the target. In the proposed ECPO-ME algorithm, the search trajectory is encoded as a series of UAV motion paths. These paths evolve in each iteration of the ECPO-ME algorithm. The performance of the algorithm is tested for six different scenarios with different characteristics. A statistical analysis is carried out to compare the results obtained from ECPO-ME with other well-known metaheuristics, widely used for benchmarking studies. The results found show that the ECPO-ME has great potential in finding moving targets, since it outperforms the base algorithm (i.e., ECPO) by as much as 2.16%, 5.26%, 7.17%, 14.72%, 0.79% and 3.38% for the investigated scenarios, respectively.

Colorectal cancer is the fourth most common type of malignancy worldwide that may develop due to the accumulation of several genetic variations. Different single nucleotide polymorphisms of SMAD1 gene are assumed to be linked with increased colorectal cancer risk. The current case-control study was conducted to verify the association of genetic polymorphisms of SMAD1 (rs11100883 and rs7661162) with colorectal cancer in the Bangladeshi population. This study was performed on 275 colorectal cancer patients and 300 healthy volunteers using polymerase chain reaction-restriction fragment length polymorphism method. The odds ratios were adjusted for age and sex with logistic regression analysis. In case of SMAD1 rs11100883 polymorphism, GA heterozygous genotype, GA + AA (dominant model), and minor allele "A" were significantly associated with colorectal cancer (adjusted odds ratio = 1.55, 95% confidence interval = 1.09-2.20, p = 0.014; adjusted odds ratio = 1.59, 95% confidence interval = 1.13-2.23, p = 0.008; and odds ratio = 1.35, 95% confidence interval = 1.06-1.73, p = 0.015, respectively) and the significance exists after the Bonferroni correction. Again, single nucleotide polymorphism rs7661162 showed significant association with an elevated colorectal cancer risk for AG heterozygous genotype, AG + GG (dominant model), AG versus AA + GG (overdominant model), and minor allele "G" (adjusted odds ratio = 1.78, 95% confidence interval = 1.24-2.56, p = 0.002; adjusted odds ratio = 1.68, 95% confidence interval = 1.18-2.39, p = 0.004; adjusted odds ratio = 1.76, 95% confidence interval = 1.23-2.53, p = 0.002; and odds ratio = 1.47, 95% confidence interval = 1.08-2.00, p = 0.014, respectively) and significance withstands after the Bonferroni correction. No significant age and gender differences between cases and controls were observed. In silico, gene expression analysis showed that the SMAD1 mRNA level was downregulated in the colon and rectal cancer tissues compared to healthy tissues. In conclusion, our findings indicate that SMAD1 rs11100883 and rs7661162 polymorphisms are responsible for increasing the susceptibility of colorectal cancer development in the Bangladeshi population.

BACKGROUND: Chronic arsenic exposure is a public health concern in many parts of the world, with elevated concentrations in groundwater posing a threat to millions of people. Arsenic is associated with various cancers and an array of chronic diseases; however, the relationship with adverse pregnancy outcomes and child mortality is less established. OBJECTIVES: We evaluated associations between individual-level prenatal arsenic exposure with adverse pregnancy outcomes and child mortality in a pregnancy study among 498 women nested in a larger population-based cohort in rural Bangladesh. METHODS: Creatinine-adjusted urinary total arsenic concentration, a comprehensive measure of exposure from water, food, and air sources, reflective of the prenatal period was available for participants. Self-reported pregnancy outcomes (livebirth, stillbirth, spontaneous/elective abortion) were ascertained. Generalized estimating equations, accounting for multiple pregnancies of participants, were used to estimate odds ratios and 95% confidence intervals in relation to adverse pregnancy outcomes. Vital status of livebirths was subsequently ascertained through November 2015. Cox proportional hazards models were used to estimate hazard ratios and 95% confidence intervals in relation to child mortality. RESULTS: We observed a significant association between prenatal arsenic exposure and the risk of stillbirth (greater than median; adjusted OR = 2.50; 95% CI = 1.04, 6.01). We also observed elevated risk of child mortality (greater than median; adjusted HR = 1.92; 95% CI = 0.78, 4.68) in relation to prenatal arsenic exposure. CONCLUSIONS: Prospective studies should continue to evaluate prenatal and early life health effects of arsenic exposure and arsenic remediation strategies for women of child-bearing age.

Salt enhances the taste as well as the nutritional value of food. Besides, several reports are available on the incidence and epidemiology of various illnesses in relation to salt intake. Excessive salt consumption has been found to be linked with high blood pressure, renal disease, and other cardiovascular disorders due to the result of vascular inflammation. Nevertheless, studies aimed at elucidating the molecular processes that produce vascular inflammation have yet to reach their conclusions. This article emphasizes the significance of investigating the mechanisms underlying both acute and chronic vascular inflammation induced by salt. It also explores the logical inferences behind cellular oxidative stress and the role of endothelial dysfunction as the potential initiator of the inflammatory segments that remain poorly understood. It is therefore hypothesized that salt is one of the causes of chronic vascular inflammation such as atherosclerosis. The hypothesis's secrets, when revealed, can help assure cardiovascular health by proactive efforts and the development of appropriate preventative measures, in combination with medication, dietary and lifestyle adjustments.

Background: Despite being preventable, cervical cancer remains a leading cause of mortality among Bangladeshi women. This article addresses the trends in Bangladesh's response to the World Health Organization's (WHO) request for the eradication of cervical cancer within the nation. Discussion: When it comes to cervical cancer, healthcare institutions need to be concerned in terms of protocols for diagnosis and treatment, staff education, and available resources. More than a quarter of all female cancers in Bangladesh are caused by cervical cancer, which can be prevented through better healthcare infrastructure, earlier diagnosis, more qualified healthcare professionals, improved urban and rural hospital infrastructure, community-based clinics, expanded affordable vaccinations, school-based delivery systems, adoption of single-dose vaccine schedules, raising awareness, and compiling a registry of previously affected results. WHO applauds Bangladesh's Ministry of Health and Family Welfare for its efforts to develop the National Strategy for cervical cancer prevention and control, which will guide and strengthen the country's activities to prevent and treat cervical cancer. Conclusion: The endeavor to eradicate this global disease burden should not be limited to Bangladesh; all nations should participate collectively to prevent the malignancy from returning and threatening human civilization.

Exposure to inorganic arsenic (As) is recognized as a risk factor for non-melanoma skin cancer (NMSC). We followed up with 7000 adults for 6 years who were exposed to As. During follow-up, 2.2% of the males and 1.3% of the females developed basal cell carcinoma (BCC), while 0.4% of the male and 0.2% of the female participants developed squamous cell carcinoma (SCC). Using a panel of more than 400 cancer-related genes, we detected somatic mutations (SMs) in the first 32 NMSC samples (BCC = 26 and SCC = 6) by comparing paired (tissue-blood) samples from the same individual and then comparing them to the SM in healthy skin tissue from 16 participants. We identified (a) a list of NMSC-associated SMs, (b) SMs present in both NMSC and healthy skin, and (c) SMs found only in healthy skin. We also demonstrate that the presence of non-synonymous SMs in the top mutated genes (like PTCH1, NOTCH1, SYNE1, PKHD1 in BCC and TP53 in SCC) significantly affects the magnitude of differential expressions of major genes and gene pathways (basal cell carcinoma pathways, NOTCH signaling, IL-17 signaling, p53 signaling, Wnt signaling pathway). These findings may help select groups of patients for targeted therapy, like hedgehog signaling inhibitors, IL17 inhibitors, etc., in the future.

BACKGROUND: Major depressive disorder (MDD) is a common and debilitating mental illness characterized by persistent feelings of sadness, hopelessness, and a lack of interest in daily activities. The objective of this study was to investigate whether levels of macrophage inflammatory protein-1ß (MIP-1ß) and macrophage chemoattractant protein-2 (MCP-2) in the blood were associated with the pathophysiology and development of MDD compared to healthy controls (HCs). METHODS: This case-control study was conducted involving 50 MDD patients and 38 HCs. We performed a comprehensive assessment to match age, sex, BMI, and socio-demographic profile between the groups. The study excluded participants with chronic infection, inflammatory diseases, coexisting psychiatric disorder, history of liver and kidney diseases, and individuals who are under antipsychotic medications. A professional psychiatrist diagnosed MDD patients and evaluated HCs based on the Diagnostic and Statistical Manual of Mental Disorders-5 (DSM-5) criteria. The severity of depression was assessed using the Hamilton Depression (Ham-D) rating scale. Commercially available enzyme-linked immunosorbent assay (ELISA) kits were used to quantify the serum MIP-1ß and MCP-2 levels. RESULTS: The results indicated elevated serum MIP-1ß levels (207.73±24.24 pg/ml) in MDD patients compared to HCs (58.77±9.14 pg/ml). This difference in concentration is positively correlated with severity of disease symptoms (r = 0.451; p<0.001). Similarly, the levels of MCP-2 were found to be elevated in patients compared to controls (143.61±19.92 vs. 56.84±4.02 pg/ml; p = 0.003), with a positive correlation with the Ham-D scores (r = 0.373; p = 0.004). CONCLUSION: According to this study, elevated levels of MIP-1ß and MCP-2 may be associated with the pathophysiology and development of MDD. These increased serum MIP-1ß and MCP-2 levels could be used as risk assessment tools for MDD. The present findings urge further research and the development of therapeutic and diagnostic approaches for depression.

This paper is a follow-up to a recent work by the authors on recoverable UAV-based energy-efficient reconfigurable routing (RUBER) scheme for addressing sensor node and route failure issues in smart wireless livestock sensor networks. Time complexity and processing cost issues connected to the RUBER scheme are consequently treated in this article by proffering a time-aware UAV-based energy-efficient reconfigurable routing (TUBER) scheme. TUBER scheme employs a synchronized clustering-with-backup strategy, a minimum-hop neighborhood recovery mechanism, and a redundancy minimization technique. Comparative network performance of TUBER was investigated and evaluated with respect to RUBER and UAV-based energy-efficient reconfigurable routing (UBER) schemes. The metrics adopted for this comparative performance analysis are Cluster Survival Ratio (CSR), Network Stability (NST), Energy Dissipation Ratio (EDR), Network Coverage (COV), Packet Delivery Ratio (PDR), Fault Tolerance Index (FTI), Load Balancing Ratio (LBR), Routing Overhead (ROH), Average Routing Delay (ARD), Failure Detection Ratio (FDR), and Failure Recovery Ratio (FRR). With reference to best-obtained values, TUBER demonstrated improvements of 36.25%, 24.81%, 34.53%, 15.65%, 38.32%, 61.07%, 31.66%, 63.20%, 68.96%, 66.19%, and 78.63% over RUBER and UBER in terms of CSR, NST, EDR, COV, PDR, FTI, LBR, ROH, ARD, FDR, and FRR, respectively. These experimental results confirmed the relative effectiveness of TUBER against the compared routing schemes.

BACKGROUND: Readymade garment workers globally experience distinctive vulnerabilities at the workplace. The situation is worse in many developing countries. However, there is a lack of scientific evidence about the health and safety of garment workers in the workplace. OBJECTIVE: The objective of this study was to examine the patterns of illness and injury of garment workers, factory level health safety policies and programs, and views and perception of management staff at factories in Bangladesh. METHODS: This mixed-method study was conducted among garment workers and management level staff between January 2018 and April 2019. We extracted 4000 health records of the workers who attended primary health care centers of the factories and conducted 11 key informant interviews using qualitative method from different management-level staff. RESULTS: Half of the workers (51.0%) were working in the sewing section following 12.8% in quality control and 12.3% in the laundry section. A review of the medical records showed that peptic ulcer diseases (PUDs) (19.2%), fever (11.7%), loose motion (10.3%), and headache (9.4%). Needle injury was uncommon for taking medical care. Occupational safety and health management, safe drinking water, access to maternity leave and other facilities were better in large and medium factories compared with those in small factories. CONCLUSIONS: Although the readymade garments sector is one of the largest sources of foreign currency revenue earnings in Bangladesh, occupational health and safety issues of workers remain a big concern. Thus, support from the government needs more focus on the health and safety of workers.

The coronavirus disease 2019 (COVID-19) pandemic has resulted in significant global mortality and morbidity affecting millions of lives. As healthcare authorities worldwide are still paying substantial attention to COVID-19, other diseases continue to cause more deaths than COVID-19. The decreasing number of COVID-19 cases and deaths indicates that the pandemic is close to the end. For effective pandemic management, healthcare facilities worldwide have established COVID-19 units and testing facilities, instituting infection prevention and control measures, and employing telehealth services. Healthcare professionals have identified some promising treatments for COVID-19; also, mass vaccinations have improved patient outcomes. Instead of COVID-19 as a pandemic, it is time to pay more attention to other diseases to lessen their impact on public health. Therefore, the World Health Organization (WHO) has declared the end of the pandemic phase of COVID-19 considering the current COVID-19 situation and our preparedness, past pandemic experience, and long pandemic impact on social and economic life on May 5, 2023. In this article, we briefly discussed the present challenges due to COVID-19, necessary precautions, and future directions to return to life as before COVID-19.

Marburg virus disease (MVD) caused by the Marburg virus has a high mortality rate. Rousettus aegyptiacus fruit bats act as the natural reservoir host of the virus. But it can also potentially be transmitted from person to person through direct contact with body secretions. The recent outbreaks have already killed seven people out of nine confirmed cases in Equatorial Guinea and five patients out of eight confirmed cases in Tanzania. In the recent past, Ghana reported three MVD cases and two associated deaths in 2022. Specific treatments or vaccines are unavailable for MVD, and supportive care is the primary treatment option. The history of MVD outbreaks and the current scenario show its potential to become an emerging threat to global public health. The recent outbreaks in Tanzania and Equatorial Guinea have already caused a high fatality rate. The absence of effective treatment and vaccines raises concerns about the potential to cause widespread harm. Besides, its capacity for human-to-human transmission and potential to cross the country's border could result in a multicountry outbreak. Therefore, we recommend intensive surveillance of MVD, preventative measures, and early detection to limit the spread of the disease and prevent another pandemic.

Background and Aims: The monkeypox (mpox) virus was endemic to some African countries that have recently spread worldwide creating multicountry outbreaks outside of Africa. This most current mpox outbreak is caused by a variety of factors. However, there are some conflicting ideas and pejorative claims about this. Therefore, we conducted this narrative review to highlight the associations of mpox infection with sexual orientations and encounters and to break the misconception about mpox. Methods: To find pertinent information and data, we searched Google, Google Scholar, and PubMed. The results of the study were supported by a large number of studies that demonstrated a connection between the mpox and sexual orientation. To gather information, we read the pertinent articles. Results: According to the available documents, we have seen that there are normally two ways of monkeypox transmission. However, the LGBTQ and MSM communities are disproportionately affected by mpox infection. Numerous research have only focused on the sexual orientation of victims. According to a study, there were 54,709 laboratory-confirmed cases of mpox where the majority of these cases were in men who had sex with men and reported recent sexual activity with one or more partners. According to a study from Italy, the mpoxvirus was recently discovered in a sample of human semen from cases of confirmed mpox. However, this finding has not yet been confirmed because there is a chance that the specimen was contaminated by another means. Conclusion: The present study suggests that there might have a prominent association between epidemiology of mpox infection and sexual orientations patients. Therefore, the general public should be made aware of mpox infections by healthcare authorities. They should let the people know about the facts about mpox. Only comprehensive planning and preventive measures will be able to halt this epidemic infection's pandemic progression.
