RESUMEN

The prevalence of infectious esophagitis in immunocompetent people is low. The main etiological agent is HSV11,2, affecting mainly young men4 with intense odynophagia and dysphagia. It is unknown whether SARS-CoV-2 infection or its vaccine are predisposing factors. The objective is to determine the clinical, diagnostic, endoscopic and therapeutic characteristics of patients diagnosed with viral esophagitis since the beginning of the pandemic.

Latanoprost (LAT) has been shown to have a hypertrichotic effect, which makes it a promising candidate for alopecia treatments. For the first time, LAT has been encapsulated in nanotransfersomes in order to increase its efficacy. Ex vivo skin biodistribution was studied by confocal laser microscopy both in human scalp and pig skin. Results showed that nanotransfersomes increase the penetration of two different fluorochromes, with similar patterns in both species, compared with fluorochrome solutions containing no nanotransfersomes. Nanotransfersomes were stable under accelerated conditions (40 °C/75% RH) and long-term conditions (25 °C/60% RH) for up to 1 year, with no differences in vesicle size and polydispersity when LAT was loaded. Nanotransfersomes increased the LAT cell proliferation effect in HaCaT cell via MAPK signaling pathway. Collectively, our results demonstrate LAT-nanotransfersomes formulation could be a promising therapy for hair growth disorders.

Tissue engineering for spinal cord injury (SCI) remains a complex and challenging task. Biomaterial scaffolds have been suggested as a potential solution for supporting cell survival and differentiation at the injury site. However, different biomaterials display multiple properties that significantly impact neural tissue at a cellular level. Here, we evaluated the behavior of different cell lines seeded on chitosan (CHI), poly (ε-caprolactone) (PCL), and poly (L-lactic acid) (PLLA) scaffolds. We demonstrated that the surface properties of a material play a crucial role in cell morphology and differentiation. While the direct contact of a polymer with the cells did not cause cytotoxicity or inhibit the spread of neural progenitor cells derived from neurospheres (NPCdn), neonatal rat spinal cord cells (SCC) and NPCdn only attached and matured on PCL and PLLA surfaces. Scanning electron microscopy and computational analysis suggested that cells attached to the material's surface emerged into distinct morphological populations. Flow cytometry revealed a higher differentiation of neural progenitor cells derived from human induced pluripotent stem cells (hiPSC-NPC) into glial cells on all biomaterials. Immunofluorescence assays demonstrated that PCL and PLLA guided neuronal differentiation and network development in SCC. Our data emphasize the importance of selecting appropriate biomaterials for tissue engineering in SCI treatment.

RESUMEN

Transcatheter arterial chemoembolization (TACE) is currently one of the standards of treatment for unresectable hepatocellular carcinoma in the intermediate stage. It is a minimally invasive procedure whose adverse outcomes are well documented. Among those considered uncommon, we find skin outcomes. We report a 73-year-old man who, after undergoing TACE, develops a necrotic retiform purpura due to occlusion of the microcirculation of the cutaneous and subcutaneous tissue caused by migration of doxorubicin spheres. This is an infrequent complication, which presents with pain in the affected area. Its management is based on prevention, which is the reason why awareness of this condition is so important.

BACKGROUND: Since May 2022, a new outbreak of monkeypox has been reported in several countries, including Spain. The clinical and epidemiological characteristics of the cases in this outbreak may differ from those in earlier reports. OBJECTIVES: To document the clinical and epidemiological characteristics of cases of monkeypox in the current outbreak. METHODS: We conducted a prospective cross-sectional study in multiple medical facilities in Spain to describe the cases of monkeypox in the 2022 outbreak. RESULTS: In total, 185 patients were included. Most cases started with primarily localized homogeneous papules, not pustules, in the probable area of inoculation, which could be cutaneous or mucous, including single lesions. Generalized small pustules appeared later in some of them. Heterogeneous lesions occurred during this generalized phase. All patients had systemic symptoms. Less common lesions included mucosal ulcers (including pharyngeal ulcers and proctitis) and monkeypox whitlows. Four patients were hospitalized, none died. Smallpox vaccination and well-controlled HIV disease were not associated with markers of severity. Contact during sex is the most likely mechanism of transmission. In this outbreak, cases have been described in men who have sex with men and are strongly associated with high-risk sexual behaviours. Seventy-six per cent of the patients had other sexually transmitted diseases upon screening. CONCLUSIONS: The clinical findings in this outbreak differ from previous findings and highly suggest contact transmission and initiation at the entry site. The characterization of the epidemiology of this outbreak has implications for control. What is already known about this topic? Monkeypox eruption is described as consisting of pustules. The roles of HIV and previous smallpox vaccination in the prognosis are unknown. The transmission route was initially described as respiratory droplets and was later suggested to be via sexual contact. What does this study add? Initial lesions at the probable inoculation area were homogeneous and papular (pseudopustules). Generalized small pustules appeared later in some of them. Heterogeneous lesions occurred during this generalized phase. All patients had systemic symptoms. Less common signs included mucosal ulcers (including pharyngeal ulcers and proctitis) and monkeypox whitlows. Well-controlled HIV and previous smallpox vaccination were not associated with severity. No patient died. The data support the hypothesis of transmission via contact during sex. Although this might change, the outbreak is currently limited mostly to men who have sex with men, with high-risk factors for sexually transmitted diseases.

The number of people with cardiovascular diseases is a leading public health problem. The Ministry of Health of Mexico and the local representative office of the Pan American Health Organization have joined forces to implement the "HEARTS: technical package for cardiovascular disease management in primary health care." The objective of this article is to describe the implementation design of the "Healthy-lifestyle counselling" module, which is part of the HEARTS technical package. To this end, an interdisciplinary and inter-institutional group was formed to discuss implementation, the topics to be addressed in each brief intervention, and follow-up and monitoring. In conclusion, working with the advisory group from various health disciplines contributed to a broad vision of the person-centered approach, and helped with the implementation design of the module. The objective is to achieve active communication between patients and health professionals, helping to generate interventions that result in informed patients who are active in their health care, and health teams that enable patients to obtain better health outcomes.


O número de pessoas com doenças cardiovasculares é um dos principais problemas de saúde pública. A Secretaria de Saúde do México e a Representação da Organização Pan-Americana da Saúde no país uniram esforços para a implementação da iniciativa "HEARTS: pacote técnico para o manejo das doenças cardiovasculares na atenção primária à saúde". O objetivo deste artigo é descrever o delineamento da implementação do módulo "Hábitos e estilos de vida saudáveis: aconselhamento para pacientes", que faz parte do pacote técnico HEARTS. Para tanto, formou-se um grupo interdisciplinar e interinstitucional no qual foram discutidos o objetivo da implementação e os temas a serem abordados em cada intervenção breve, bem como seu acompanhamento e monitoramento. Em conclusão, o trabalho com o grupo assessor composto por várias disciplinas da área da saúde contribuiu para se ter uma visão ampla da abordagem da pessoa, bem como para o delineamento da implementação do módulo. Espera-se alcançar uma comunicação ativa entre paciente e profissional de saúde, contribuindo para gerar intervenções que gerem pacientes informados e ativos no cuidado de sua saúde, bem como equipes de saúde com mais ferramentas que lhes permitam obter melhores resultados de saúde.

The behavioral and physiological mechanisms of thermoregulation and the morphological traits of lizards result in a particular range of body temperatures, which influence performance and ultimately fitness. We studied the thermal biology and locomotor performance of the lizard Liolaemus wiegmannii from the coastal dunes in the southeastern Pampas of Argentina. During the austral summer, we examined the link between thermoregulation and optimal locomotor performance. Liolaemus wiegmannii faced a stressful environment due to high risk of overheating; despite this, the species was able to achieve field body temperatures (Mean Tb ± SD = 35.58 ± 2.86 °C) than expected by chance (i.e., the null model) and suitable for sustaining its physiological performance. Locomotion in this species was thermally-sensitive, with lizards showing high-performance bouts at a relatively wide range of body temperatures (30-38 °C). Lizards exhibited a mean maximum running speed of 1.30 m/s at 37.3 °C (i.e., optimal temperature for locomotion) which was within the set point range of preferred temperature (Tset = 35.4-37.5 °C). Therefore, we found a correspondence between thermal optimum and preferred temperature. Our findings suggest that L. wiegmannii, like other lizard species with a broad distribution, is capable of performing well across a wide range of temperatures despite the spatiotemporal thermal fluctuations of the environment.

Primary intestinal lymphangiectasia is a rare disorder associated with protein-losing enteropathy. The main manifestations are those resulting from hypoalbuminemia. Diagnosis requires the typical endoscopic image of intestinal lymphangiectasia and increased 24-hour fecal alpha-1-antitrypsin clearance. Treatment is basically dietary.

AIMS: To evaluate the results of a hepatitis B and C screening program in hospitalized COVID-19 patients. METHOD: Transversal prospective study conducted in two Spanish hospitals. Patients admitted from March 1st to December 31st 2020 with a diagnosis of COVID-19 were tested for markers of hepatitis B (HBsAg, anti-HBc) and C (anti-HCV, HCV RNA) infection. RESULTS: In this period, 4662 patients with COVID-19 were admitted to our centers: 56.3% were male, median age was 76 (0-104) years. Data regarding HBV infection was available in 2915 (62.5%) patients; 253 (8.75%) were anti-HBc+ and 11 (0.38%) HBsAg+. From these, 4 patients did not have a previous diagnosis of hepatitis B, 7 received corticosteroids and one received prophylaxis. There was one HBV reactivation. Anti-HCV was available in 2895 (62%) patients; 24 (0.83%) were positive. From these, 13 patients had a previous hepatitis C diagnosis: 10 patients had been treated with SVR, one achieved spontaneous cure and 2 did not receive treatment. From the 11 previously unknown anti-VHC+patients, 10 had a negative HCV RNA. Overall, only 3 (0.10%) patients tested RNA HCV+. However, none received HCV treatment (2 older than 90 years with comorbidities, 1 died from COVID-19). CONCLUSION: Screening of hepatitis C infection in hospitalized COVID-19 patients seems less useful than expected. The low prevalence of active infection after antiviral treatments and the high age of our population limit the detection of potential candidates for treatment. HBV screening should be aimed to prevent reactivation under immunosuppressive treatments.

The multiple myeloma (MM) landscape has changed in the last few years, but most patients eventually relapse because current treatment modalities do not target clonogenic stem cells, which are drug-resistant and can self-renew. We hypothesized that side population (SP) cells represent myeloma clonogenic stem cells and, searching for new treatment strategies, analyzed the anti-myeloma activity of natural killer (NK) cells against clonogenic cells. Activated and expanded NK cells (NKAE) products were obtained by co-culturing NK cells from MM patients with K562-mb15-41BBL cell line and characterized by flow cytometry. Functional experiments against MM cells were performed by Eu-TDA release assays and methylcellulose clonogenic assays. Side population was detected by Dye Cycle Violet labeling and then characterized by flow cytometry and RNA-Seq. Self-renewal capacity was tested by clonogenic assays. Sorting of both kind of cells was performed for time-lapse microscopy experiments. SP cells exhibited self-renewal potential and overexpressed genes involved in stem cell metabolism. NK cells from MM patients exhibited dysregulation and had lower anti-tumor potential against clonogenic cells than healthy donors' NK cells. Patients' NK cells were activated and expanded. These cells recovered cytotoxic activity and could specifically destroy clonogenic myeloma cells. They also had a highly cytotoxic phenotype expressing NKG2D receptor. Blocking NKG2D receptor decreased NK cell activity against clonogenic myeloma cells, and activated NK cells were able to destroy SP cells, which expressed NKG2D ligands. SP cells could represent the stem cell compartment in MM. This is the first report describing NK cell activity against myeloma clonogenic cells.

We performed phylogenomic analysis of severe acute respiratory syndrome coronavirus-2 from 88 infected individuals across different regions of Colombia. Eleven different lineages were detected, suggesting multiple introduction events. Pangolin lineages B.1 and B.1.5 were the most frequent, with B.1 being associated with prior travel to high-risk areas.

Microbial communities in surface waters used for recreational purposes are indicators of contamination and risk of contact with human pathogens. Hence, monitoring microbial communities in recreational waters is important for potential public health threats to humans. Such monitoring is rare in Colombia, even in its capital, Bogotá, the most populous city in the country. This city encompasses metropolitan and linear parks with recreational water bodies that are used frequently by the public, and the presence of pathogens can compromise the health of the citizens. Therefore, we examined the bacterial, and eukaryotic communities in urban recreational lakes (URL) in four metropolitan parks in Bogotá, Colombia. Samples from four metropolitan parks (Los Novios, Simon Bolivar, El Tunal, and Timiza) and one stream contaminated with sewage from a linear park (El Virrey) were collected. We used amplicon next-generation sequencing of the 16S-rRNA gene and 18S-rRNA gene to characterize microbial communities followed by bioinformatics analyses. In addition, general water quality parameters-pH, hardness, acidity, alkalinity, dissolved oxygen, and nitrites-were recorded using a commercial kit. Genera of pathogens, including Legionella, Pseudomonas, Mycobacterium, Candida, and Naegleria, were found in lake waters. The stream El Virrey was, however, the only surface water that showed an abundance of fecal bacteria, often associated with low oxygen concentrations. All water bodies showed a predominance of fungal phyla, except for the lake at Timiza. This lake showed the highest pH, and its ecological dynamics are likely different from other water bodies. Likewise, some URLs displayed a greater abundance of cyanobacteria, including toxin-producing species. Algal genera associated with eutrophication were predominant among primary producing microorganisms. This study shows for the first time the description of the bacterial and eukaryotic communities of some URLs and a stream in Bogotá. The URLs and the stream harbored various pathogens that might pose a risk to the citizen's health.

BACKGROUND: There is limited and controverting evidence looking at possible associations of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA copies and patient variables in large cohorts of symptomatic and asymptomatic patients. METHODS: We studied 2275 symptomatic and asymptomatic patients from Colombia with coronavirus disease 2019 (COVID-19) and analyzed the associations between RT-PCR cycle threshold (Ct) value with gender, age, comorbidities, symptomatology, and disease severity. RESULTS: 15.4 % of the samples (n = 428) reported at least one comorbidity. There were 2011 symptomatic cases (72.4 %), being the most common reported symptom cough (57.2 %, n = 1586). Respiratory distress was present in 21.4 % of patients (n = 595), and 435 patients (15.6 %) required hospital admission. We observed that patients with no prior medical history harbored higher RNA copies than patients with comorbidities (p = 0.02). No significant differences in RNA copies were observed between symptomatic and asymptomatic patients (p = 0.82). Strong correlations were detected between Ct values and the presence of odynophagia (p = 0.03), diarrhea (p = 0.04), and headache (p = 0.0008). An inverse association was found between RNA copy number and markers of disease severity, namely, respiratory distress (P < 0.0001) and hospitalization requirement (P < 0.0001). CONCLUSIONS: SARS-CoV-2 RT-PCR cycle thresholds reveal strong associations with a prior medical history, specific symptomatology, and disease severity markers. Further research controlling potential confounding variables needs to be conducted to evaluate the nature and usefulness of these associations in managing COVID-19 patients.

BACKGROUND: Psoriasis, a chronic inflammatory disease affecting 2-3% of the population, is characterised by epidermal hyperplasia, a sustained pro-inflammatory immune response and is primarily a T-cell driven disease. Previous work determined that Connexin26 is upregulated in psoriatic tissue. This study extends these findings. METHODS: Biopsies spanning psoriatic plaque (PP) and non-involved tissue (PN) were compared to normal controls (NN). RNA was isolated and subject to real-time PCR to determine gene expression profiles, including GJB2/CX26, GJB6/CX30 and GJA1/CX43. Protein expression was assessed by immunohistochemistry. Keratinocytes and fibroblasts were isolated and used in 3D organotypic models. The pro-inflammatory status of fibroblasts and 3D cultures was assessed via ELISA and RnD cytokine arrays in the presence or absence of the connexin channel blocker Gap27. RESULTS: Connexin26 expression is dramatically enhanced at both transcriptional and translational level in PP and PN tissue compared to NN (>100x). In contrast, CX43 gene expression is not affected, but the protein is post-translationally modified and accumulates in psoriatic tissue. Fibroblasts isolated from psoriatic patients had a higher inflammatory index than normal fibroblasts and drove normal keratinocytes to adopt a "psoriatic phenotype" in a 3D-organotypic model. Exposure of normal fibroblasts to the pro-inflammatory mediator peptidoglycan, isolated from Staphylococcus aureus enhanced cytokine release, an event protected by Gap27. CONCLUSION: dysregulation of the connexin26:43 expression profile in psoriatic tissue contributes to an imbalance of cellular events. Inhibition of connexin signalling reduces pro-inflammatory events and may hold therapeutic benefit.

Thermoregulation in ectotherms may be modulated by climatic variability across geographic gradients. Environmental temperature varies along latitudinal clines resulting in heterogeneous thermal resource availability, which generally induces ectotherms to use compensatory mechanisms to thermoregulate. Lizards can accommodate to ambient temperature changes through a combination of adaptive evolution and behavioral and physiological plasticity. We studied the thermal ecology of the endangered endemic lizard Liolaemus multimaculatus at six different sites distributed from the northern to southern areas of the distribution (700 km) in the Atlantic dune barriers of Argentina, and even including the borders areas of the distribution range. Environmental temperatures and relative humidity showed a strong contrast between northern and southern limits of the distribution range. The northern localities had operative temperatures (Te) above the range of preferred temperatures (Tset), instead, the southern localities had large proportion of Tes within the Tset. Although these different climatic conditions may constrain the thermal biology of L. multimaculatus, individuals from all localities maintained relatively similar field body temperatures (XTb = 34.07 ± 3.02 °C), suggesting that this parameter is conservative. Thermal preference partially reflected latitudinal temperature gradient, since lizards from the two southernmost localities showed the lowest Tsel and Tset. Thermoregulatory efficiency differed among localities, since E values in the northern localities (E = 0.53-0.69) showed less variability than those of southern localities (E = 0.14-0.67). Although L. multimaculatus employed a strategy of having a conservative Tb and being able to acclimatize the thermal preference to copes with latitudinal changes in the thermal environment, other local factors, such as ecological interactions, may also impose limitations to thermoregulation and this may interfered in the interpretation of results at wider spatial scale.

Dysregulation of Connexin (CX) expression and function is associated with a range of chronic inflammatory conditions including psoriasis and nonhealing wounds. To mimic a proinflammatory environment, HaCaT cells, a model human keratinocyte cell line, were challenged with 10 µg/ml peptidoglycan (PGN) isolated from Staphylococcus aureus for 15 min to 24 hr in the presence or absence of CX blockers and/or following CX26, CX43, PANX1 and TLR2 small interfering RNA (siRNA) knockdown (KD). Expression levels of IL-6, IL-8, CX26, CX43, PANX1, TLR2 and Ki67 were assessed by quantitative real-time polymerase chain reaction, western blot analysis and/or immunocytochemistry. Nuclear factor kappa ß (NF-κß) was blocked with BAY 11-7082, CX-channel function was determined by adenosine 5'-triphosphate (ATP) release assays. Enzyme-linked immunosorbent assay monitored IL6 release following PGN challenge in the presence or absence of siRNA or blockers of CX or purinergic signalling. Exposure to PGN induced IL-6, IL-8, CX26 and TLR2 gene expression but it did not influence CX43, PANX1 or Ki67 messenger RNA expression levels. CX43 protein levels were reduced following 24 hr PGN exposure. PGN-induced CX26 and IL-6 expression were also aborted by TLR2-KD and inhibition of NF-κß. ATP and IL-6 release were stimulated following 15 min and 1-24 hr challenge with PGN, respectively. Release of both agents was inhibited by coincubation with CX-channel blockers, CX26-, CX43- and TLR2-KD. The IL-6 response was also reduced by purinergic blockers. CX-signalling plays a role in the innate immune response in the epidermis. PGN is detected by TLR2, which via NF-κß, directly activates CX26 and IL-6 expression. CX43 and CX26 maintain proinflammatory signalling by permitting ATP release, however, PANX1 does not participate.

Small lizards can accommodate to constraints imposed by temporal changes in ambient temperature through a combination of adaptive evolution and behavioral and physiological plasticity. Thermal physiology plasticity may compensate for climate variation and favor performance while minimizing behavioral costs in sub-optimal conditions. The Tandilia's lizard, Liolaemus tandiliensis, occurs in an isolated mountain range of the Argentinean temperate Pampas. In this study, we compared the thermal biology of L. tandiliensis between late spring (December) and mid-summer (February). The habitats' thermal quality was lower in late spring than in mid-summer. The lizard's field-body temperature (Tb) was 2-3 °C higher than the operative temperature (Te). Overall, the mean preferred temperature (Tsel) was 37.4 °C [preferred range (Tset): 36.2-38.7 °C], and was similar to other Liolaemus species. The Tset and Tsel of females in late spring were 1.8 °C lower than in mid-summer. In the case of males, the Tsel did not vary among seasons, while the Tset had a difference of 2.5 °C between seasons. Adults were moderate thermoregulators, but females were more efficient only in late spring (Emales = 0.69; Efemales = 0.58), compared to mid-summer (Emales = 0.68; Efemales = 0.50). Juveniles did not show temporal differences in temperature preferences and had a relatively higher efficiency in late spring (E = 0.38) compared to mid-summer (E = 0.28). An increased proportion of juveniles and adults shifted their Tb near to the Tset in late spring respect to mid-summer. The adults also matched their preferred temperatures to their current body temperature. These results suggest that seasonal shifts in the thermoregulatory parameters of L. tandiliensis may improve their thermoregulatory efficiency. Although temporal variation in ambient temperatures might influence the thermal biology of the studied lizards, other factors such as changes in the reproductive status may have also interfered.

RESUMEN

The intensity of the infrared radiation emitted by objects is mainly a function of their temperature. In infrared thermography, this feature is used for multiple purposes: as a health indicator in medical applications, as a sign of malfunction in mechanical and electrical maintenance or as an indicator of heat loss in buildings. This paper presents a review of infrared thermography especially focused on two applications: temperature measurement and non-destructive testing, two of the main fields where infrared thermography-based sensors are used. A general introduction to infrared thermography and the common procedures for temperature measurement and non-destructive testing are presented. Furthermore, developments in these fields and recent advances are reviewed.
