RESUMEN

This study was carried out to assess the occurrence of food hypersensitivity (type I) in adults. The diagnosis of food allergy, based on suggestive history and positive skin prick test and/or RAST, was established in 30 (0.98%) of 3034 patients over 14 years of age who were attended between June 1992 and March 1993. The foods implicated were as follows: fruit (46.8%), dried fruits (19.1%), seafood (14.9%), vegetables (12.8%), fish (4.2%) and chicken eggs (2.1%). This study confirmed the low occurrence of food allergy in adulthood. The differences found with regard to the sensitizing foods (cow's milk and eggs were infrequent; fruit, seafood and vegetables were common) may be attributed to dietary habits in our geographical region.

RESUMEN

We present nine patients with delayed allergic reactions to beta-lactams. Four of them developed reactions to ampicillin; the diagnostic studies suggested the involvement of a type IV hypersensitivity mechanism to an antigenic determinant in the side-chain structure, with intradermal and patch tests positive to ampicillin but good tolerance to benzylpenicillin. Three patients showed delayed hypersensitivity reactions to benzylpenicillin (positive delayed skin tests and/or challenges). Arthus-type reactions were seen in two other patients. Specific IgE of uncertain relationship to benzylpenicillin and ampicillin was later detected in four cases.

RESUMEN

Reproducible, exercise-induced anaphylactic reactions occur in some patients only after certain foods have been eaten before exercise. We describe a patient in whom hazelnuts were a triggering factor for exercise-induced anaphylaxis. Skin tests and RAST were positive for nuts and grass and weed pollen. The exercise challenge test after hazelnut ingestion was positive. Food hypersensitivity should be investigated in all cases of exercise-induced anaphylaxis.

RESUMEN

A case of an 85 year old female patient who during an infectious episode developed multiple autoimmune markers, autoantibodies, hypergammaglobulinemia, bicytopenia, hemolytic anemia, and cryoglobulinemia is described. All the anomalies disappeared during the following four weeks and during this period the patient presented no clinical manifestations. The hypothesis is that the infectious stimulus provoked a markedly exaggerated response. We highlight the rareness of this out of proportion transitory and silent immune response, emphasizing the greater cell sensibility to different stimuli in older patients.
