BACKGROUND: Birthing people in the United States face numerous challenges when accessing adequate prenatal care (PNC), with transportation being a significant obstacle. Nevertheless, previous studies that relied solely on the distance to the nearest provider cannot differentiate the effects of travel burden on provider selection and care utilization. These may exaggerate the degree of inequality in access and fail to capture perceived travel burden. This study investigated whether travel distances to the initially visited provider, to the predominant PNC provider, and perceived travel burden (measured by the travel disadvantage index (TDI)) are associated with PNC utilization. METHODS: A retrospective cohort of people with live births were identified from South Carolina Medicaid claims files in 2015-2018. Travel distances were calculated using Google Maps. The estimated TDI was derived from local pilot survey data. PNC utilization was measured by PNC initiation and frequency. Repeated measure logistic regression test was utilized for categorical variables and one-way repeated measures ANOVA for continuous variables. Unadjusted and adjusted ordinal logistic regressions with repeated measure were utilized to examine the association of travel burdens with PNC usage. RESULTS: For 25,801 pregnancies among those continuously enrolled in Medicaid, birthing people traveled an average of 24.9 and 24.2 miles to their initial and predominant provider, respectively, with an average TDI of -11.4 (SD, 8.5). Of these pregnancies, 60% initiated PNC in the first trimester, with an average of 8 total visits. Compared to the specialties of initial providers, predominant providers were more likely to be OBGYN-related specialists (81.6% vs. 87.9%, p < .001) and midwives (3.5% vs. 4.3%, p < .001). Multiple regression analysis revealed that every doubling of travel distance was associated with less likelihood to initiate timely PNC (OR: 0.95, p < .001) and a lower visit frequency (OR: 0.85, p < .001), and every doubling of TDI was associated with less likelihood to initiate timely PNC (OR: 0.94, p = .04). CONCLUSIONS: Findings suggest that the association between travel burden and PNC utilization was statistically significant but of limited practical significance.

BACKGROUND: Most emergency department (ED) patients arrive by their own transport and, for various reasons, may not choose the nearest ED. How far patients travel for ED treatment may reflect both patients' access to care and severity of illness. In this study, we aimed to examine the travel distance and travel time between a patient's home and ED they visited and investigate how these distances/times vary by patient and hospital characteristics. METHODS: We randomly sampled and collected data from 14,812 patients discharged to the community (DTC) between January and March 2016 from 50 hospital-based EDs nationwide. We geocoded and calculated the distance and travel time between patient and hospital-based ED addresses, examined the travel distances/ times between patients' home and the ED they visited, and used mixed-effects regression models to investigate how these distances/times vary by patient and hospital characteristics. RESULTS: Patients travelled an average of 8.0 (SD = 10.9) miles and 17.3 (SD = 18.0) driving minutes to the ED. Patients travelled significantly farther to avoid EDs in lower performing hospitals (p < 0.01) and in the West (p < 0.05) and Midwest (p < 0.05). Patients travelled farther when visiting EDs in rural areas. Younger patients travelled farther than older patients. CONCLUSIONS: Understanding how far patients are willing to travel is indicative of whether patient populations have adequate access to ED services. By showing that patients travel farther to avoid a low-performing hospital, we provide evidence that DTC patients likely do exercise some choice among EDs, indicating some market incentives for higher-quality care, even for some ED admissions. Understanding these issues will help policymakers better define access to ED care and assist in directing quality improvement efforts. To our knowledge, our study is the most comprehensive nationwide characterization of patient travel for ED treatment to date.

Seeds play a major role in plant species persistence and expansion, and therefore they are essential when modeling species dynamics. However, homogeneity in seed traits is generally assumed, underestimating intraspecific trait variability across the geographic space, which might bias species success models. The aim of this study was to evaluate the existence and consequences of interpopulation variability in seed traits of the invasive species Carpobrotus edulis at different geographical scales. We measured seed production, morphology, vigour and longevity of nine populations of C. edulis along the Catalan coast (NE Spain) from three differentiated zones with a human presence gradient. Geographic distances between populations were contrasted against individual and multivariate trait distances to explore trait variation along the territory, evaluating the role of bioclimatic variables and human density of the different zones. The analysis revealed high interpopulation variability that was not explained by geographic distance, as regardless of the little distance between some populations (< 0.5 km), significant differences were found in several seed traits. Seed production, germination, and persistence traits showed the strongest spatial variability up to 6000% of percent trait variability between populations, leading to differentiated C. edulis soil seed bank dynamics at small distances, which may demand differentiated strategies for a cost-effective species management. Seed trait variability was influenced by human density but also bioclimatic conditions, suggesting a potential impact of increased anthropogenic pressure and climate shifts. Geographic interpopulation trait variation should be included in ecological models and will be important for assessing species responses to environmental heterogeneity and change.

Competing claims exist about how the geographic distance between parents and their adult children has changed historically. A classic modernisation hypothesis is that people currently live further away from their parents than in the past. Others have argued for stability and the remaining importance of local family ties, in spite of a long-term decline in co-residence of adult children and parents. The current paper uses a novel design that relies on reports by grandchildren to study long-term changes in intergenerational proximity in the Netherlands. The analyses show that there has been a clear and continuous decline in intergenerational proximity between the 1940s and the 1990s. Mediation analyses show that educational expansion and urbanisation are the main reasons why proximity declined. No evidence is found for the role of secularisation and increasing international migration. Proximity to parents declined somewhat more strongly for women than for men.

During the transition to adulthood, emerging adults may experience new geographic distance separating them from their siblings. Digital leisure is common among emerging adults and may serve as a tool for maintaining relationship quality across geographic distance. Using qualitative data, we created individual digital leisure diversity scores for four categories of digital leisure identified from a constructed taxonomy. The sample included 185 college-attending emerging adults (M age = 20.1, 64.3% male). Analysis of variance indicated dyadic gender differences in social media and gaming. Digital leisure diversity was associated with greater levels of affective and cognitive sibling relationship quality, regardless of sibling dyad gender. The association between digital leisure diversity and cognitive relationship quality was significant for participants who lived closer to their sibling. During the transition to adulthood when new geographic separation is common, digital leisure shows potential as a compensatory activity that may help siblings maintain relationship quality.

The Monteiro is a feral pig found in the Brazilian Pantanal ecosystem. The goal of this research is to generate data and knolewdge related to animal populations wich can be used for management and development of an in vitro conservation program for animal resourses at Pantanal ecosystem. The present study evaluated animals sampled from 10 distinct locations within the region, using 19 microsatellite markers (N = 189) and the control region of mitochondrial DNA (mtDNA) (N = 392). Low genetic differences were found between populations with the microsatellite data. The FST range was between 0.009 and 0.063 (p-value < 0.05). The Mantel test corroborated with previous results, as low correlations between genetic and geographic distances were observed (r2 = 0.2309, p = 0.06). Bayesian analysis for genetic structure identification placed the Monteiro pigs into three main clusters (MOB, Pop 1 and all others Pantanal populations). Most of the Monteiro pigs share a single European haplotype as seen by mtDNA analyses. This haplotype is not exclusive, as it is shared with other swine populations (commercial and other locally adapted breeds). Monteiro populations from different geographic locations within Pantanal are not isolated and can be considered as a large unique population. Since animals roam freely to seek food and water, or even due to seasonal flooding of their habitat, the Monteiro populations presented absence of major genetic structure and evidence of high gene flow. These results can be used to create a management plan and in situ and ex situ conservation program for conservation and use of the Monteiro breed in the Pantanal ecosystem.

OBJECTIVES: Crown and root traits, like those in the Arizona State University Dental Anthropology System (ASUDAS), are seemingly useful as genetic proxies. However, recent studies report mixed results concerning their heritability, and ability to assess variation to the level of genomic data. The aim is to test further if such traits can approximate genetic relatedness, among continental and global samples. MATERIALS AND METHODS: First, for 12 African populations, Mantel correlations were calculated between mean measure of divergence (MMD) distances from up to 36 ASUDAS traits, and FST distances from >350,000 single nucleotide polymorphisms (SNPs) among matched dental and genetic samples. Second, among 32 global samples, MMD and FST distances were again compared. Correlations were also calculated between them and inter-sample geographic distances to further evaluate correspondence. RESULTS: A close ASUDAS/SNP association, based on MMD and F ST correlations, is evident, with r m -values between .72 globally and .84 in Africa. The same is true concerning their association with geographic distances, from .68 for a 36-trait African MMD to .77 for F ST globally; one exception is F ST and African geographic distances, r m = 0.49. Partial MMD/F ST correlations controlling for geographic distances are strong for Africa (.78) and moderate globally (.4). DISCUSSION: Relative to prior studies, MMD/F ST correlations imply greater dental and genetic correspondence; for studies allowing direct comparison, the present correlations are markedly stronger. The implication is that ASUDAS traits are reliable proxies for genetic data-a positive conclusion, meaning they can be used with or instead of genomic markers when the latter are unavailable.

Both environmental and geographic factors interact to structure the metacommunities in river networks, but the importance of these factors is difficult to distinguish. We used six aquatic taxonomic groups to test the relationship between environmental and geographic factors and their effect on species turnover patterns in an agriculturally dominated river (Chaohu Lake Basin, China). The relationships between three dissimilarity indices and geographic distance were assessed using the Mantel test while considering the differences in environmental factors between sites. Then, we employed a variation partitioning method to distinguish the isolated and combined effects of environmental and geographic distance on species turnover. There were significant relationships between environmental distance and species turnover in all groups. All organisms except periphytic diatoms were significantly correlated with two geographic (Euclidean and network) distances when the Chao dissimilarity index was considered. The results suggest that the strength of the correlations changed with environmental and geographic distances and with the aquatic community. The communities displayed more complex relationships with the distance measures when different dissimilarity (Jaccard, Chao, and Bray-Curtis dissimilarity) indices were considered. Nevertheless, aquatic communities are strongly influenced by both environmental and geographic distance, and the former has a stronger effect than the latter.

Admixture can break up divergent genetic architectures between populations, resulting in phenotypic novelty and generating raw material for environmental selection. The contribution of admixture to progeny trait variation and fitness varies based on the degree of genetic isolation between the parental populations, for which most studies have used geographic distance as a proxy. A novel approach is to estimate optimal crossing distance using the adaptive genetic distance between mates estimated from loci that contribute directly to local adaptation. Here, we aim to understand the effect of admixture on disrupting local adaptation of ecotypes of Arabidopsis thaliana separated along gradients of geographic, background, and locally adaptive genetic distances. We created experimental F1 hybrids between ecotypes that vary in geographic distance and used SNP data to estimate background (putatively neutral) and adaptive genetic distance. Hybrids were grown under controlled conditions, and fitness, growth, and phenology traits were measured. The different traits measured showed a clear effect of adaptive genetic distance, but not geographic distance. The earliest bolting hybrids were intermediate in the adaptive genetic distance between their parents, and also had higher biomass and fitness in terms of fruit and seed production. Our results suggest that disruption of locally adaptive genomic loci decreases the performance of offspring between distantly related parents, but that crosses between very closely related parents also reduce performance, likely through the expression of deleterious recessive alleles. We conclude that during admixture, selection may have to balance the consequences of disrupting local adaption while also avoiding inbreeding depression.

Quercus liaotungensis is a major tree species in deciduous broad-leaved forests in northern China. In this study, we investigated ectomycorrhizal (ECM) communities associated with Q. liaotungensis from five typical habitats across northern China. We used internal transcribed spacer-polymerase chain reaction and DNA sequencing to identify ECM fungi, and we detected 220 operational taxonomic units. In general, at the regional scale, the dominant ECM lineages were /tomentella-thelephora, /cenococcum, /russula-lactarius, and /inocybe. Analysis of variance demonstrated significant differences in alpha diversity among these ECM communities, and the ECM fungal richness was positively correlated with elevation and soil organic matter. Analysis of similarity and a nonmetric multidimensional scaling analysis revealed that there were significant differences in community composition, and the geographical distance was correlated with the ECM fungal communities. Among the environmental factors we studied, soil parameters and climate factors were the primary direct driving factors of the ECM fungal communities. Our study primarily advances our understanding of environmental factors affecting ECM fungal communities at regional scale.

Early HIV diagnosis enables prompt treatment initiation, thereby contributing to decreased morbidity, mortality, and transmission. We aimed to describe the association between distance from residence to testing sites and HIV disease stage at diagnosis. Using HIV surveillance data, we identified all new HIV diagnoses made at publicly funded testing sites in central North Carolina during 2005-2013. Early-stage HIV was defined as acute HIV (antibody-negative test with a positive HIV RNA) or recent HIV (normalized optical density <0.8 on the BED assay for non-AIDS cases); remaining diagnoses were considered post-early-stage HIV. Street distance between residence at diagnosis and (1) the closest testing site and (2) the diagnosis site was dichotomized at 5 miles. We fit log-binomial models using generalized estimating equations to estimate prevalence ratios (PR) and robust 95% confidence intervals (CI) for post-early-stage diagnoses by distance. Models were adjusted for race/ethnicity and testing period. Most of the 3028 new diagnoses were black (N = 2144; 70.8%), men who have sex with men (N = 1685; 55.7%), and post-early-stage HIV diagnoses (N = 2010; 66.4%). Overall, 1145 (37.8%) cases traveled <5 miles for a diagnosis. Among cases traveling ≥5 miles for a diagnosis, 1273 (67.6%) lived <5 miles from a different site. Residing ≥5 miles from a testing site was not associated with post-early-stage HIV (adjusted PR, 95% CI: 0.98, 0.92-1.04), but traveling ≥5 miles for a diagnosis was associated with higher post-early HIV prevalence (1.07, 1.02-1.13). Most of the elevated prevalence observed in cases traveling ≥5 miles for a diagnosis occurred among those living <5 miles from a different site (1.09, 1.03-1.16). Modest increases in post-early-stage HIV diagnosis were apparent among persons living near a site, but choosing to travel longer distances to test. Understanding reasons for increased travel distances could improve accessibility and acceptability of HIV services and increase early diagnosis rates.

To test the hypothesis that different drivers shape the diversity and biogeography of the total and active bacterial community, we examined the bacterial community composition along two transects, one from the inner Pearl River estuary to the open waters of the South China Sea (SCS) and the other from the Luzon Strait to the SCS basin, using 454 pyrosequencing of the 16S rRNA and 16S rRNA gene (V1-3 regions) and thereby characterizing the active and total bacterial community, respectively. The diversity and biogeographic patterns differed substantially between the active and total bacterial communities. Although the composition of both the total and active bacterial community was strongly correlated with environmental factors and weakly correlated with geographic distance, the active bacterial community displayed higher environmental sensitivity than the total community and particularly a greater distance effect largely caused by the active assemblage from deep waters. The 16S rRNA vs. rDNA relationships indicated that the active bacteria were low in relative abundance in the SCS. This might be due to a high competition between active bacterial taxa as indicated by our community network models. Based on these analyses, we speculate that high competition could cause some dispersal limitation of the active bacterial community resulting in a distinct distance-decay relationship. Altogether, our results indicated that the biogeographic distribution of bacteria in the SCS is the result of both environmental control and distance decay.

Bean leaf beetle, Cerotoma trifurcata (Forster) (Coleoptera: Chrysomelidae), is a common pest of soybean in the Midwest United States. However, there are currently no reports on the genetic variability of C. trifurcata. This study examined 15-30 individuals from 25 sample locations to estimate genetic variability and gene flow within and among C. trifurcata from across the Mid- west. Amplified fragment length polymorphism generated 175 markers for analyses. Results from analysis of molecular variance (AMOVA) indicated that the majority of genetic variation was from within samples; only a small amount of the total variation was attributed to the variation among the samples. The GST for the entire C. trifurcata population indicated that the majority of genetic variation was found within the samples, further supporting the AMOVA results. The estimated average gene flow among the C. trifurcata samples was 1.83. The Mantel test revealed no indication of correlation between geographical and genetic distance for all the C. trifurcata samples. These findings show that C. trifurcata in the Midwest are genetically heterogeneous and part of a large, interbreeding population.

Beta-diversity reflects the spatial changes in community species composition which helps to understand how communities are assembled and biodiversity is formed and maintained. Larch (Larix) forests, which are coniferous forests widely distributed in the mountainous and plateau areas in North and Southwest China, are critical for maintaining the environmental conditions and species diversity. Few studies of larch forests have examined the beta-diversity and its constituent components (species turnover and nestedness-resultant components). Here, we used 483 larch forest plots to determine the total beta-diversity and its components in different life forms (i.e., tree, shrub, and herb) of larch forests in China and to evaluate the main drivers that underlie this beta-diversity. We found that total beta-diversity of larch forests was mainly dependent on the species turnover component. In all life forms, total beta-diversity and the species turnover component increased with increasing geographic, elevational, current climatic, and paleoclimatic distances. In contrast, the nestedness-resultant component decreased across these same distances. Geographic and environmental factors explained 20%-25% of total beta-diversity, 18%-27% of species turnover component, and 4%-16% of nestedness-resultant component. Larch forest types significantly affected total beta-diversity and species turnover component. Taken together, our results indicate that life forms affect beta-diversity patterns of larch forests in China, and that beta-diversity is driven by both niche differentiation and dispersal limitation. Our findings help to greatly understand the mechanisms of community assemblies of larch forests in China.

Understanding the adaptability of small populations in the face of environmental change is a central problem in evolutionary biology. Solving this problem is challenging because neutral evolutionary processes that operate on historical and contemporary timescales can override the effects of selection in small populations. We assessed the effects of isolation by colonization (IBC), isolation by dispersal limitation (IBDL) as reflected by a pattern of isolation by distance (IBD), and isolation by adaptation (IBA) and the roles of genetic drift and gene flow on patterns of genetic differentiation among 19 cave-dwelling populations of Icelandic Arctic charr (Salvelinus alpinus). We detected evidence of IBC based on the genetic affinity of nearby cave populations and the genetic relationships between the cave populations and the presumed ancestral population in the lake. A pattern of IBD was evident regardless of whether high-level genetic structuring (IBC) was taken into account. Genetic signatures of bottlenecks and lower genetic diversity in smaller populations indicate the effect of drift. Estimates of gene flow and fish movement suggest that gene flow is limited to nearby populations. In contrast, we found little evidence of IBA as patterns of local ecological and phenotypic variation showed little association with genetic differentiation among populations. Thus, patterns of genetic variation in these small populations likely reflect localized gene flow and genetic drift superimposed onto a larger-scale structure that is largely a result of colonization history. Our simultaneous assessment of the effects of neutral and adaptive processes in a tractable and replicated system has yielded novel insights into the evolution of small populations on both historical and contemporary timescales and over a smaller spatial scale than is typically studied.

Exploring the response of the diversity of phytoplankton species and functional groups to environmental variables is extremely important in maintaining biodiversity in aquatic ecosystems. Although there were more taxonomic units at the species level than at the functional group level, it remained unclear whether species diversity was more sensitive than functional group diversity to environmental variables. In this study, taxonomic composition and alpha-beta diversity of phytoplankton were investigated in 23 subtropical reservoirs located in the Han River Basin in South China during wet and dry seasons. Structural Equation Modelling (SEM) and Generalized Dissimilarity Modelling (GDM) were employed to validate the response of phytoplankton species and functional group alpha-beta diversities to environmental variables. The results indicated that the community compositions of phytoplankton in eutrophic reservoirs were similar between wet and dry seasons, while there were distinct differences for community composition in oligotrophic-mesotrophic reservoirs between the two seasons. Across all reservoirs, there were no significant differences in alpha and beta diversities of species and functional groups between wet and dry seasons. The SEM and GDM results revealed that total phosphorus was the primary driving factor influencing alpha and beta diversities of species and functional groups in the 23 reservoirs. Meanwhile, the non-linear results of species beta diversity were stronger than the non-linear results of functional group beta diversity, indicating that phytoplankton species exhibited a higher explanatory power in responding to environmental changes compared to that of functional groups. Compared to that of species beta diversity, the response of functional group beta diversity to environmental variables was significantly lower in the dry season. These research findings lead to re-evaluating the common practice relating to the use of phytoplankton functional groups to assess environmental conditions, which may overlook the explanatory power of subtle changes at the species level, especially during periods of habitat diversification in the dry season.

Online medical consultation platforms enable patients to seek health advice from physicians across geographic regions. In this study, we analyze patterns of online consultation between patients and physicians. We examine the joint effects of regional medical resource disparity, geographic distance, and cultural differences between patients and physicians on patients' decisions about which physicians they consult online. Using a unique dataset of city-to-city tuples based on 813,684 online consultation records and combining it with region-level data from multiple external sources, we find that while regional medical resource disparity drives patients from medically disadvantaged regions to seek online consultations with physicians from medically advantaged regions, geographic distance and cultural differences tend to constrain these consultations. We also find that cultural differences can amplify the impact of regional medical resource disparity, whereas geographic distance may lessen this effect. Further, we discover that the constraining effect of geographic distance is partly due to the online-to-offline nature of online medical consultations. Moreover, additional analyses suggest that physicians' online reputation and information about physicians' participation on the platform can help alleviate the negative effects of geographic distance and cultural differences. These findings hold significant implications for the allocation of medical resources and the formulation of healthcare policies.

This study investigated the geographic and pairwise distances of nine Chinese local Cashmere goat populations through the analysis of 20 microsatellite DNA markers. Fluorescence PCR was used to identify the markers, which were selected based on their significance as identified by the Food and Agriculture Organization of the United Nations (FAO) and the International Society for Animal Genetics (ISAG). In total, 206 alleles were detected; the average allele number was 10.30; the polymorphism information content of loci ranged from 0.5213 to 0.7582; the number of effective alleles ranged from 4.0484 to 4.6178; the observed heterozygosity was from 0.5023 to 0.5602 for the practical sample; the expected heterozygosity ranged from 0.5783 to 0.6464; and Allelic richness ranged from 4.7551 to 8.0693. These results indicated that Chinese Cashmere goat populations exhibited rich genetic diversity. Further, the Wright's F-statistics of subpopulation within total (FST) was 0.1184; the genetic differentiation coefficient (GST) was 0.0940; and the average gene flow (Nm) was 2.0415. All pairwise FST values among the populations were highly significant (p<0.01 or p<0.001), suggesting that the populations studied should all be considered to be separate breeds. Finally, the clustering analysis divided the Chinese Cashmere goat populations into at least four clusters, with the Hexi and Yashan goat populations alone in one cluster. These results have provided useful, practical, and important information for the future of Chinese Cashmere goat breeding.

RESUMEN

PURPOSE: The purpose of this scoping review was to document the state of the literature regarding the challenges experienced by rural informal caregivers of older adults in the United States. METHOD: We reviewed peer-reviewed academic articles published through December 1, 2021, based on Arksey and O'Malley's framework. RESULTS: The initial search resulted in 1,255 articles, of which 12 studies were included for the final review. Thematic content analysis was utilized to identify emerging themes of challenges experienced by rural informal caregivers of older adults. The identified challenges include a lack of knowledge regarding resources, financial difficulties, health-related challenges, and barriers related to geographic distance. DISCUSSION AND CONCLUSION: The implications of these challenges are used to shape recommendations for social work, service planning, and policy changes that can improve caregiving experiences for rural families.

Genetic differentiation between populations is determined by various factors, including gene flow, selection, mutation, and genetic drift. Among these, gene flow is known to counter genetic differentiation. The genus Eranthis, an early flowering perennial herb, can serve as a good model to study genetic differentiation and gene flow due to its easily detectable population characteristics and known reproductive strategies, which can be associated with gene flow patterns. Eranthis populations are typically small and geographically separated from the others. Moreover, previous studies and our own observations suggest that seed and pollen dispersal between Eranthis populations is highly unlikely and therefore, currently, gene flow may not be probable in this genus. Based on these premises, we hypothesized that the genetic differentiation between the Eranthis populations would be significant, and that the genetic differentiation would not sensitively reflect geographic distance in the absence of gene flow. To test these hypotheses, genetic differentiation, genetic distance, isolation by distance, historical gene flow, and bottlenecks were analyzed in four species of this genus. Genetic differentiation was significantly high, and in many cases, extremely high. Moreover, genetic differentiation and geographic distance were positively correlated in most cases. We provide possible explanations for these observations. First, we suggest that the combination of the marker type used in our study (chloroplast microsatellites), genetic drift, and possibly selection might have resulted in the extremely high genetic differentiation observed herein. Additionally, we provide the possibility that genetic distance reflects geographic distance through historical gene flow, or adaptation in the absence of historical gene flow. Nevertheless, our explanations can be more rigorously examined and further refined through additional observations and various population genetic analyses. In particular, we suggest that other accessible populations of the genus Eranthis should be included in future studies to better characterize the intriguing population dynamics of this genus.
