RESUMO

PURPOSE: To share our experience about partial resection (nephron sparing) in localized (T1aN0M0) renal hypernephroid cancer as organ preserved operation. Opportunities and longterm results. MATERIAL AND METHOD: For last 10 years we have done 89 partial resections in patients with localized renal hypernehroid cancer. In all cases we have done lymphatic dissection of relevant lymph region of the affected side. RESULTS AND DISCUSSING: All operated patients had five years of survival. We observed developing contralateral hypernefroid carcinoma in one patient that necessary performing a partial resection in this place. In two patients we performed a partial resection of a solitarykidney. The results of surgery are very good when we stage properly cancer process that allows organ preservation.

RESUMO

AIM: To introduce our experience, opportunities, complications and conclusions with our colegues about first twelve cases of laparoscopic transperitonealis radical nephrectomy. MATERIAL AND METHODS: For the period from 16.04.08 to 16.09.08 we made twelve laparoscopic transperitoneal radical nephrectomies. Seven males and five females age 48 to 81 have been operated. RESULTS: In two of these cases we made revision for bleeding. The other cases were without complications. CONCLUSION: The laparoscopic transperitoneal radical nephrectomy is the most perfect technic for the moment--because of the low bleeding and short postoperative period in the hospital it is a good method of choice.

RESUMO

An assessment is made of the results of plastic closure of the chest in cases presenting mediastinitis. The analysis covers nineteen patients with a history of mediastinitis following open-heart surgery, treated by the same method of plastic reconstruction of the chest. The technique of plastic closure of the chest using a rotation muscle flap (pectoralis and right rectus abdominis muscles) is described.

RESUMO

Aorto-caval fistula is a rare complication of infrarenal abdominal aortic aneurysm /AAA/, and it occurs when the aneurysm ruptures into inferior vena cava. Literature indicates that such a communication is present in less than 2% of the elective AAA resections and in 3%-6.97% of the cases of AAA ruptures. From 1986 and 2000 in the University Hospital "St. Ekaterina" 207 elective AAA cases and 43 ruptured AAA cases underwent surgery. In 2 cases (4.65% of the ruptured AAA) we encountered aorto-caval communication. We report a patient with infrarenal AAA having significant oedema of lower extremities bilaterally, treated in another hospital for iron deficiency anemia and vegetative disorders. Patient presented for consultation and abdominal Echo showed AAA. The angiogram performed demonstrated aorto-caval fistula. At surgery bifurcated graft was implanted successfully. This is the first case of diagnosed prior to surgery aorto-caval fistula followed by successful surgery reported in Bulgaria.
