This paper presents the results of a study of the characteristics of phase synchronization between electrocardiography(ECG) and electroencephalography (EEG) signals during night sleep. Polysomnographic recordings of eight generally healthy subjects and eight patients with obstructive sleep apnea syndrome were selected as experimental data. A feature of this study was the introduction of an instantaneous phase for EEG and ECG signals using a continuous wavelet transform at the heart rate frequency using the concept of time scale synchronization, which eliminated the emergence of asynchronous areas of behavior associated with the "leaving" of the fundamental frequency of the cardiovascular system. Instantaneous phase differences were examined for various pairs of EEG and ECG signals during night sleep, and it was shown that in all cases the phase difference exhibited intermittency. Laminar areas of behavior are intervals of phase synchronization, i.e., phase capture. Turbulent intervals are phase jumps of 2π. Statistical studies of the observed intermittent behavior were carried out, namely, distributions of the duration of laminar sections of behavior were estimated. For all pairs of channels, the duration of laminar phases obeyed an exponential law. Based on the analysis of the movement of the phase trajectory on a rotating plane at the moment of detection of the turbulent phase, it was established that in this case the eyelet intermittency was observed. There was no connection between the statistical characteristics of laminar phase distributions for intermittent behavior and the characteristics of night breathing disorders (apnea syndrome). It was found that changes in statistical characteristics in the phase synchronization of EEG and ECG signals were correlated with blood pressure at the time of signal recording in the subjects, which is an interesting effect that requires further research.

The present paper reports on the synthesis and characterization of a new chemosensor for fluoride ions, a hydrazone derived from pyridoxal 5'-phosphate and benzothiazole. The structure of the chemosensor was confirmed using 1H and 13C NMR, FT-IR and mass spectroscopy. The conformational diversity of the chemosensor influencing the sensor activity was studied by the quantum chemistry methods on the B3LYP/6-311++G(d, p) (H, C, N, O, P, S) level, and the optimal structure of the chemosensor was chosen. The selective capability of detecting F- in the aqueous solution, which also contains Cl-, Br-, I-, NCS-, ClO4-, HSO4-, and NO3- was demonstrated. The detection limit (LOD) for fluoride ions was 0.22 µM as determined by the 3σ method. The turn-on effect in the presence of fluoride ions is based on the deprotonation of the chemosensor and its subsequent aggregation in DMSO. In addition, the chemosensor was used for the detection and estimation of F- in real samples using fluorescence spectroscopy.

In our work, we compare EEG time-frequency features for two types of K-complexes detected in volunteers performing the monotonous psychomotor test with their eyes closed. Type I K-complexes preceded spontaneous awakenings, while after type II K-complexes, subjects continued to sleep at least for 10 s after. The total number of K-complexes in the group of 18 volunteers was 646, of which of which type I K-complexes was 150 and type II K-complexes was 496. Time-frequency analysis was performed using continuous wavelet transform. EEG wavelet spectral power was averaged upon several brain zones for each of the classical frequency ranges (slow wave, δ, θ, α, ß1, ß2, γ bands). The low-frequency oscillatory activity ( δ-band) preceding type I K-complexes was asymmetrical and most prominent in the left hemisphere. Statistically significant differences were obtained by averaging over the left and right hemispheres, as well as projections of the motor area of the brain, p<0.05. The maximal differences between the types I and II of K-complexes were demonstrated in δ-, θ-bands in the occipital and posterior temporal regions. The high amplitude of the motor cortex projection response in ß2-band, [20;30] Hz, related to the sensory-motor modality of task in monotonous psychomotor test. The δ-oscillatory activity preceding type I K-complexes was asymmetrical and most prominent in the left hemisphere may be due to the important role of the left hemisphere in spontaneous awakening from sleep during monotonous work, which is an interesting issue for future research.

OBJECTIVE: To assess the effect on the duration and course of the nasal mucosa function recovery after anterior nasal packing during surgery involving the nasal septum and inferior nasal conchae combined with concomitant therapy with the herbal medicinal product Sinupret extract (Bionorica SE, Germany). MATERIAL AND METHODS: The study included 25 males aged 18-50 (the main group), permanent residents of the Novosibirsk region diagnosed with deviated nasal septum at least 2 years ago. All patients underwent rhinoseptoplasty with nasal packing with gauze tampons for 24 hours at the clinical base of the Otorhinolaryngology Department of the Novosibirsk State Medical University.Patients of the main group received Sinupret extract 1 tablet BID for 10 days before the planned rhinoseptoplasty and 7 days after the rhinoseptoplasty. Patients in control group 1 (n=16) received Sinupret 2 coated tablets TID for 10 days before the surgery and 7 days after the surgery. Patients in control group 2 (n=16) did not receive secretory therapy in the preoperative and postoperative periods of rhinoseptoplasty. RESULTS: The herbal medicinal product improved nasal breathing. All studied parameters demonstrated the benefit of the treatment including Sinupret extract (p<0.05). On day 5 in patients of the main study group, nasal breathing was normal, and nasal crusting was reduced due to activation of the mucous membrane secretory function. Nasal mucosa ciliary clearance was better in patients in the main group than in patients in the control groups at all stages of the study. Sinupret extract for 10 days before and 5 days after the surgery helps reduce nasal crusting and restore nasal breathing by day 2 and normalizes ciliary clearance by day 5 after nasal packing. CONCLUSION: Due to the wide use of nasal packing with gauze tampons, otorhinolaryngologists commonly administer herbal medicinal products as a means of drug protection of the nasal mucous membrane.

We report the first measurement of the average of the electron-proton and positron-proton elastic scattering cross sections. This lepton charge-averaged cross section is insensitive to the leading effects of hard two-photon exchange, giving more robust access to the proton's electromagnetic form factors. The cross section was extracted from data taken by the OLYMPUS experiment at DESY, in which alternating stored electron and positron beams were scattered from a windowless gaseous hydrogen target. Elastic scattering events were identified from the coincident detection of the scattered lepton and recoil proton in a large-acceptance toroidal spectrometer. The luminosity was determined from the rates of Møller, Bhabha, and elastic scattering in forward electromagnetic calorimeters. The data provide some selectivity between existing form factor global fits and will provide valuable constraints to future fits.

RESUMO

The study of coordinated behavior between different systems of the human body provides useful information on the functioning of the body. The peculiarities of interaction and coordinated dynamics of the heart rate and respiration are of particular interest. We investigated the coherence of the processes of respiration and autonomic control of the heart rate for people of different ages in the awake state, in sleep with rapid eye movement, and in deep sleep. Our analysis revealed a monotonic decrease in the coherence of these processes with increasing age. This can be explained by age-related changes in the system of autonomic control of circulation. For all age groups, we found a qualitatively similar dynamics of the coherence between the studied processes during a transition from the awake state to sleep.

Uterine leiomyoma (UL) is the most common benign tumor in women of reproductive age. Gene therapy using suicidal genes appears to be a promising approach for UL treatment. One of key factors for success of gene therapy is the right choice of genetic construct carrier. A promising group of non-viral carriers for cell delivery of expression vectors is cationic Cys-flanked peptides which form tight complexes with DNA due to electrostatic interactions and the presence of interpeptide disulfide bonds. The paper reports a comparative study of the physico-chemical, toxic, and transfectional properties of the DNA-peptide complexes obtained by matrix polymerization or oxidative polycondensation of Cys-flanked peptides using the chain growth terminator 2-amino ethanethiol. We have demonstrated the therapeutic effect of the delivery of the pPTK-1 plasmid carrying the herpes simplex virus type 1 (HSV-1) thymidine kinase gene into PANC-1, and HEK-293T cell culture as well as into primary UL cells. It has been shown that the carriers obtained by oxidative polycondensation transform primary UL cells more efficiently than those produced by matrix polymerization. Treatment with ganciclovir resulted in the death of up to 40% of UL cells transfected with the pPTK-1 plasmid. The perspectives of use of the polyR6 carrier produced by oxidative polycondensation as a tool for the development of modular peptide carriers for the purposes of UL gene therapy were discussed.

PURPOSE: to determine the characteristics of the surgeon training curve in the development of percutaneous nephrolithotripsy (PN). MATERIALS AND METHODS: The first 125 PNs performed by one team of surgeons were investigated. In chronological order, five groups of 25 operations were formed. The assessment was carried out according to the following parameters: the duration of the operation; the number of perioperative complications; stone free rate (SFR). RESULTS: With the advanced training of the surgical team, the duration of the operation tended to decrease; after 50 PN the time decreased from 164+/-12 to 113+/-11 minutes. The duration of the next 50 PN conducted in the third and fourth groups remained unchanged - 105+/-10 and 103+/-12 minutes. In the fifth group, a decrease in time to 99+/-9 minutes had no significant differences with the third and fourth groups. The frequency of complications tended to decrease up to the 75th operation, followed by a plateau. The structure was dominated by complications related, according to the modified Clavien-Dindo classification, to Grade I and Grade II. With the accumulation of surgeons experience, there was a tendency towards an increase in SFR: in the 1st group - 72%, in 2 - 84%, in 3 and 4 - 88%, in 5 - 92% (p<0,05). CONCLUSIONS: in order to achieve the average indicators of the duration and effectiveness of PN, the surgical team must have experience of at least 50 such operations. Due to the long training process, it is advisable to use PN in organization with an estimated number of such operations of at least 30 per year.

The origin of complex irregular dynamics in a cardiovascular system is still being actively debated. Some hypotheses suggest the crucial role of stochastic modulation of cardiovascular parameters, while others argue for the importance of cardiac pacemakers' chaotic deterministic dynamics. In the present study, we estimate the largest Lyapunov exponent and the correlation dimension for the 4-h experimental interbeat intervals and the chaotic signals generated by the mathematical model of the cardiovascular system. We study the complexity of the mathematical model for such cases as the autonomic blockade, the exclusion of all the stochastic components, and the absence of variability of respiration. The obtained results suggest that the complexity of the heart rate variability is largely due to the chaotic dynamics in the loops of autonomic control of circulation.

We studied antidepressant and antiparkinsonian properties of N-(5-hydroxynicotinoyl)-Lglutamic acid calcium salt (Ampasse) in rodents. It was found that Ampasse in a dose of 30 mg/kg exhibited antidepressant activity in the forced swimming test in mice and in a dose of 0.1 mg/kg maximally alleviates the symptoms of parkinsonian syndrome induced by systemic administration of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine in C57Bl/6 mice, and haloperidol-induced catalepsy in rats.

AIM: To study the impact of rehabilitation measures on the dynamics of pain syndrome and on the state of postural muscle balance in the early postoperative period in patients with degenerative-dystrophic diseases of the lumbar spine who have undergone decompressive-stabilizing interventions. MATERIAL AND METHODS: This paper comparatively analyzed the level of pain syndrome in the lumbar spine and lower extremities, the stabilometric indicators characterizing the amplitude of center-of-pressure oscillations in patients who had undergone decompressive-stabilizing operations at 7±2 days after surgical treatment for degenerative-dystrophic diseases of the lumbar spine. Two groups of 60 people in each were formed: rehabilitation measures were implemented, starting on day 2 of an early postoperative period (the duration of the latter was 7±2 days after surgery); the basic complex was supplemented with stabilometric training in Group 1 (a study group); only the basic complex was used in Group 2 (a comparison group). RESULTS: In the study group, the frequency of rehabilitation outcomes with an achieved excellent result depending on the level of pain syndrome in the lower extremities and spine on a visual analogue scale was 62.5 and 88.9%, respectively; which significantly statistically exceeds the proportion of patients with the same outcome in the comparison group (37.5 and 11.1%; p<0.01 in both cases). There was a statistically significant more pronounced decrease in the level of pain syndrome in the lumbar spine (p=0.0001) and lower limbs (p=0.003) in the patients of the study group in the early postoperative period. Intergroup comparison revealed a statistically significant decrease in all the indicators monitored in the study, which characterize the amplitude of the center-of-pressure oscillations. Moreover, the value of the oscillation area parameter between the patient groups differed by 1.76 and 1.83 times during Romberg's test with the eyes open or closed, respectively (p=0.0001). CONCLUSION: The findings suggest that the comprehensive treatment supplemented with stabilometric training in the early postoperative period improves the state of postural muscle balance. There was a statistically significant predominance of compliance with normative values in patients of the study group during Romberg's test with the eyes open or closed (p=0.007 and p=0.00002, respectively), which contributes to a more marked decrease in the level of pain syndrome in the lumbar spine (p=0.001) and lower extremities (p=0.003).

Silicon is more than the dominant material in the conventional microelectronics industry: it also has potential as a host material for emerging quantum information technologies. Standard fabrication techniques already allow the isolation of single electron spins in silicon transistor-like devices. Although this is also possible in other materials, silicon-based systems have the advantage of interacting more weakly with nuclear spins. Reducing such interactions is important for the control of spin quantum bits because nuclear fluctuations limit quantum phase coherence, as seen in recent experiments in GaAs-based quantum dots. Advances in reducing nuclear decoherence effects by means of complex control still result in coherence times much shorter than those seen in experiments on large ensembles of impurity-bound electrons in bulk silicon crystals. Here we report coherent control of electron spins in two coupled quantum dots in an undoped Si/SiGe heterostructure and show that this system has a nuclei-induced dephasing time of 360 nanoseconds, which is an increase by nearly two orders of magnitude over similar measurements in GaAs-based quantum dots. The degree of phase coherence observed, combined with fast, gated electrical initialization, read-out and control, should motivate future development of silicon-based quantum information processors.

RESUMO

We study the synchronization of infra-slow oscillations in human scalp electroencephalogram signal with the respiratory signal. For the cases of paced respiration with a fixed frequency and linearly increasing frequency, we reveal the phase and frequency locking of infra-slow oscillations of brain potentials by respiration. It is shown that for different brain areas, the infra-slow oscillations and respiration can exhibit synchronous regimes of different orders.

The study examined the effect of calcium salt of N-(5-hydroxynicotinoil)-L-glutamic acid (Ampasse preparation) on neuronal activity in hippocampal CA1 area evoked by stimulation of Schaffer collaterals at a rate of 1 Hz (30 impulses during 30 sec) in the surviving hippocampal slices of Wistar rats. The records of 1st and 30th orthodromic population spikes showed that Ampasse in concentrations of 500 µM, 1, 2, and 10 mM facilitated the synaptic transmission in Schaffer collaterals - hippocampal CA1 pyramidal neurons axis; the maximum effect was observed at 2 mM Ampasse. When used in a concentration of 10 mM, Ampasse provoked epileptiform activity, which could be prevented by MK-801, a specific noncompetitive antagonist of the NMDA-receptor complex.

The aim of the study was to evaluate the influence of chronopharmacotherapy on the indices of the structural and functional state of the left ventricular myocardium in patients with arterial hypertension (AH), type 2 diabetes mellitus and heart failure with preserved ejection fraction (HFpEF) considering of salt-sensitivity. MATERIALS AND METHODS: Included 130 patients with uncontrolled AH, type 2 diabetes and HFpEF (81 women and 49 men), median age - 59 (38-72) years. Patients were divided into 2 groups: saltsensitive (group 1) and salt-resistant (group 2), and then randomized to 2 subgroups depending on the treatment option: morning ramipril and indapamide retard, in the evening amlodipine (subgroup 1A and 2A) or in the evening ramipril, in the morning indapamide-retard and amlodipine (subgroup 1B and 2B). Initially and after 24 weeks of antihypertensive therapy, an echocardiographic study was carried out with an assessment of the main indicators of the structural and functional state of the left ventricular (LV) myocardium, a 6-minute walk test for determining the functional state of patients. RESULTS: After 24 weeks in all subgroups there was a reliable positive dynamics of echocardiographic indicators. In the group of salt-sensitive patients, the reception of the angiotensin converting enzyme inhibitor (ACE inhibitor) in the evening and thiazide diuretic (TD) + calcium antagonists (AC) in the morning provided a signifcantly more signifcant reduction in the majority of LV myocardial remodeling parameters compared with the administration of ACE inhibitor + TD in the morning and AK in the evening. In the case of salt-resistant patients, comparable positive changes in echocardiographic indicators were noted against the background of both dosing regimens during the day. Against the background of the treatment, irrespective of the salt sensitivity and dosing regimen, an unreliable and comparable increase in the distance of the 6-minute walk test was recorded during the day. CONCLUSION: In salt-sensitive patients with AH, type 2 diabetes and HFpEF, the use of ACE inhibitor before bedtime provided signifcantly more signifcant regression of the parameters of the structural and functional state of LV myocardium compared with the morning reception. In the group of salt-resistant patients, the efcacy of both dosing regimens during the day was comparable.

Acute myocardial infarction (MI), the most severe complication of coronary artery disease, develops in 2-4% of patients with various malignancies. PURPOSE: to explore the specific properties of ST-segment elevation myocardial infarction (STEMI) course in patients with cancer and its effect on short-term outcome. MATERIALS AND METHODS: We included in this study 45 patients with STEMI and history of cancer hospitalized in the period from 01.01.2015 to 01.01.2017 (group I; 58% men, mean age 69.07±11.60 years). In the comparison group (group II) we selected 90 age and gender matched persons (58% men, mean age 68.16±11.75 years) from patients with STEMI without oncological diseases. RESULTS: There were no differences between groups in main risk factors of cardiovascular diseases, except greater proportion of patients with damage of peripheral arteries in group 1 (22.2% compared with 5.6% in group 2, p=0.025). There were also no differences between groups in clinical characteristics at admission. However values of the following parameters were significantly lower in group I: concentration of hemoglobin (115.56±23.07 vs. 133.70±16.45 g/l in group 2, р<0.001), red blood cell count (3.95±0.66x1012/l vs. 5.57±0.72x1012/l in group 2, р<0.001), platelets (93±0.97x109/l vs 186±18.3x109/l in group 2, p<0.001), total cholesterol (4.12±2.17 vs. 6.24±2.56 mmol/L in group 2, р<0.001). There were differences in the frequency of use of antiplatelet drugs: at prehospital stage acetylsalicylic acid was given to 48.9% and 77.8% of patients in groups 1 and 2, respectively (p=0.044); clopidogrel and ticagrelor were also significantly more rarely used in group 1. There was no difference in use of thrombolysis and percutaneous coronary interventions (4.4 and 53.3% vs 5.6 and 56.6% in groups 1 and 2, respectively). In group 2 greater portion of patients was given statins (68.9 vs. 77.8%, p=0.021) and angiotensin-converting enzyme inhibitors (55.6 vs. 82.2%, p=0.008). Inhospital mortality was the same in both groups (4.4%). Occurrence of complications (bleeding, pulmonary edema, cardiogenic shock), and frequency of use of intra-aortic balloon counterpulsation, temporary cardiac pacing were similar. However in group 1 greater portion of patients had ventricular tachyarrhythmias (15.6 vs 2.2% in group 2, р=0.007); the presence of cancer was the only independent predictor of their emergence in multiple logistic regression (OR 8.11 [1.11-40.83], p=0.011). CONCLUSIONS: Rates of application of myocardial reperfusion in STEMI patients with and without history of cancer were the same. Despite similar hospital mortality revealed peculiarities of STEMI course could affect long-term outcomes. Confirmation of this hypothesis requires planned prospective studies.

RESUMO

Achieving a high quality of life for patients is a paradigm of modern medicine. The doctrine of the quality of life, originated in the 60s of the twentieth century, is currently actively developing in all fields of medicine, including urology. This article reviews the history, terminology, and methods of studying the quality of life. Along with the general questionnaires, disease specific quality of life instruments are gradually emerging in the standards of examination and follow-up of urological patients.

Pathological processes in the craniovertebral region (CVR) are usually accompanied by dislocation complications leading to gross neurological disorders. One of the diseases that affect the CVR and lead to atlanto-axial dislocation (AAD) is rheumatoid arthritis. Errors in the diagnosis of rheumatoid disease and in the choice of a treatment approach may cause adverse outcomes. OBJECTIVE: To define the approach for surgical treatment of AAD associated with rheumatoid disease of the CVR, with allowance for the rigidity of deformity. MATERIAL AND METHODS: Five patients with rheumatoid AAD, 4 females and 1 male, aged 54 to 73 years underwent surgery. All dislocations were anterior ones. Three patients had mobile pannus-associated dislocation. In 2 cases, AAD was rigid and combined with odontoid invagination into the foramen magnum (FM). RESULTS: In all mobile AAD cases, decompression of the brainstem and restoration of the normal anatomical relationships in the CVR were achieved by dislocation correction and atlanto-axial fusion performed from the posterior approach, avoiding transoral interventions. In this case, spontaneous resorption of the pannus occurred within several months after surgery. In the postoperative period, all patients achieved significant regression of pain and neurological disorders. Complications in the form of wound infection developed in 1 case. CONCLUSION: A decision algorithm for choosing a surgical treatment option was based on the degree of deformity stability. Mobile AADs serve as an indication for indirect decompression using instrumental correction of dislocation and atlantoaxial fixation from the posterior approach. In the case of fixed AAD, posterior fixation is complemented by anterior decompression via the transoral approach.

Minimally invasive neurosurgery has significantly evolved over the last several decades. This is due to the improvement of neurodiagnostics, microneurosurgical techniques, neuroanesthesiology and intraoperative imaging. Aim - authors present the results of the use of minimally invasive surgery (MIS) in cerebral aneurysms. In the period 2013-2017, we used 4 minimally invasive approaches in 240 patients with 246 aneurysms. The choice of surgical approach was determined after a thorough evaluation of the intracranial structures and aneurysms. We retrospectively evaluated outcomes, postoperative complications and cosmetic result after MIS. On average, the catamnesis was 6-12 months. Mortality, disability or serious permanent approach-related complications in our group have not been noted. All patients had expected periorbital edema, which was not regarded as a complication. Postoperative cosmetic result was assessed by patients as excellent. Minimally invasive surgery with adequate selection of patients is a safe and effective alternative to traditional approaches in cerebral aneurysm surgery. Rapid recovery of patients and a reduction in the duration of inpatient treatment with the possibility of fast social and labor adaptation after complex neurosurgical interventions is a priority necessity today.
