RESUMO

The present study strived to gain a more profound understanding of the distinctions in development between swimmers who are considered to be on track to the elite level at late junior age (males aged 16; females aged 15) compared to those who are not. In this effort, swimmers were followed during their pubertal years (males aged 13-15; females aged 12-14), which marks a period when performance development aligns with maturation. Longitudinal data of 90 talented sprint and middle-distance swimmers on season best times (SBT) and underlying performance characteristics (anthropometrics, maximal swimming velocity, stroke index [SI] and countermovement jump [CMJ]) were collected over three swimming seasons. Based on their SBT at late junior age (males aged 16; females aged 15), swimmers were classified as high-performing late juniors or lower-performing late juniors. Retrospectively studying these swimmers, we found that all but two high-performing late juniors were already on track to the elite level at early junior age (males aged 13; females aged 12), evidenced with faster SBT throughout puberty compared to their lower-performing peers (p < 0.05). Independent sample t-tests revealed that high-performing late juniors significantly outscored their lower-performing peers when they were early juniors on maximal swimming velocity (males aged 13-15 and females aged 12-14), SI (males aged 13 and 14; females aged 12), CMJ (females aged 14) and height (females aged 13 and 14, p < 0.05). Additionally, multilevel models showed faster rates of development for high-performing late juniors on maximal swimming velocity (males and females) and SI (males) compared to lower-performing peers throughout puberty (p < 0.05). Higher initial levels of SBT and underlying performance characteristics at early junior age as well as the faster rates of development on SBT, maximal swimming velocity and SI (males only) during the pubertal years, may be crucial factors in maintaining the trajectory towards the elite level after puberty.

The VAR has introduced a new element to football. Before, a decision to award a penalty kick could not be reversed. The player taking the penalty could take it relatively quickly after the referee called the foul. After the introduction of the VAR, every penalty kick decision is replayed by the VAR and also quite often reviewed on-field. As a consequence, time between the initial decision by the referee and the actual penalty kick has increased substantially. We examined the influence of the time-interval duration between the foul and the actual penalty kick on the conversion probability using a logit model. Also, we assessed the consequences of a VAR intervention, if any. We used data on all 2888 penalties awarded in top leagues in England, Spain, Germany, Italy, the Netherlands, and the UEFA Champions League, over the course of five seasons (2015/2016-2019/2020). We found that the duration between the foul and the moment the penalty is taken does not impact the conversion probability. However, whether or not the VAR intervenes with the referee's decision has a negative effect on the conversion probability of penalty kicks. Football teams and coaches should incorporate this element of uncertainty in training.

Covid-19 and measures to contain spreading the disease have led to changed physical activity behavior. This study aims to investigate the relationship between socioeconomic status (SES) and changes in the amount of moderate to vigorous physical activity (MVPA) during the Covid-19 crisis. Using the Dutch Lifelines Covid-19 cohort study (n = 17,749), the amount of MVPA was measured at 15 time-points between March and December 2020, and compared with the amount before the Covid19 pandemic. For SES, the population was stratified in three education and income levels. Logistic regression models were used to estimate the odds ratio (OR) and confidence interval (CI) of altered MVPA for low and high SES groups, with the middle SES category as the reference group. A clear socioeconomic gradient in changes in MVPA behavior was observed. Low educated individuals had significantly higher odds (OR = 1.14; CI: 1.03-1.27) of decreasing MVPA, while the high educated had significantly lower odds of decreased MVPA (OR = 0.84, CI: 0.79-0.90). Both low education (OR = 0.87; CI: 0.77-0.98) and low income (OR = 0.85; CI 0.78-0.92) had significantly lower odds to increase MVPA, while high education (OR = 1.21, CI: 1.12-1.30) and high income (OR = 1.17; CI: 1.07-1.28) had significantly higher odds to increase MVPA. Most findings were consistent over the full research period. Socioeconomic inequalities in MVPA have increased during the Covid-19 pandemic, even when Covid-19 containment measures were relaxed. Our findings suggest that future public health policies need to increase efforts to improve physical activity behavior with an even larger focus on low SES groups.

This study examined the association of sport participation with health outcomes and whether this relation differs between body mass index (BMI)-level subpopulations. Research outcomes for sport participation were compared with other types of leisure-time physical activity (PA). We used the Cox proportional hazards regression models to assess the associations of sport participation, and four other PA types (cycling, gardening, doing odd jobs, and walking), with the risk of prediabetes, type 2 diabetes mellitus (T2DM), and all-cause mortality in 97,212 individuals (58.4% women; mean age: 46.5 years) in the Dutch LifeLines cohort. Outcomes were stratified by three BMI levels: healthy weight (BMI: 18.5-24.9 kg/m2 ), overweight (BMI: 25.0-29.9 kg/m2 ), and obesity (BMI: 30.0 kg/m2 or above). Sport participation was associated with lower health risks, but only significantly so for prediabetes (HR = 0.86, 95% CI: 0.81-0.92). For healthy weight persons, sport participation was associated with the largest risk reductions, with significantly lower risks of prediabetes (HR = 0.78, 95% CI: 0.68-0.90) and all-cause mortality (HR = 0.79, 95% CI 0.65-0.96). Other PA types were not associated with significantly lower health risks, with the exception of cycling, for which significantly lower health risks for persons with overweight were found. Our findings show that sport participation is associated with lower health risks, especially prediabetes, but the effect varies between BMI levels, with the strongest link for persons with a healthy weight. Sport participation, together with cycling, is likely to be more effective in reducing health risks than other types of PA.

INTRODUCTION: There is a strong socioeconomic gradient in health care costs. However, little is known about the role of lifestyle factors in the association between health care costs and socioeconomic status (SES). This study investigates variation in the association between lifestyle indicators and health care costs between and within neighborhoods with similar SES. METHOD: Using 2016 whole-population data for all 790 neighborhoods of the Netherlands, we estimated the association between neighborhood average health care cost performance (i.e., health care costs adjusted for population age and gender) and neighborhood socioeconomic status (NSES) and four lifestyle indicators - smoking, alcohol consumption, exercise and sport club membership. Additionally, using regression analysis, we explored the multivariate relationship between average health care cost performance, NSES and lifestyle indicators. RESULTS: Neighborhoods with proportionally fewer smokers and more sport club members had significantly lower average health care costs. Remarkably, neighborhoods with more people who complied with the recommended maximum alcohol consumption had significantly higher health care costs. These findings were consistent within and between neighborhoods with different SES levels. Neighborhoods with more compliance with exercise guidelines had lower health care costs. However, this relationship was inconsistent across different NSES levels, with the largest cost reductions found in the most deprived neighborhoods. CONCLUSION: Our findings suggest that prevention policies aimed at reducing the number of smokers and increasing sport club membership may reduce health care costs across all NSES groups, while increasing compliance with physical exercise norms may be effective mainly in low SES neighborhoods.

BACKGROUND: The present study investigated longitudinally the performance development of a multigenerational sample of competitive swimmers. The aim of the study was to provide unique insight into the junior toward senior performance development of those few who reached top-elite level. Season Best Times (SBT) of 100 m freestyle performance of international swimmers, (1.305 males, aged 12-26 and 1.841 females, aged 12-24) competing in at least five seasons between 1993 and 2018, were corrected for the prevailing world record (WR). Swim performance was defined as a relative measure: relative Season Best Time=(SBT/WR) × 100. Based on rSBT, four performance groups were defined: top-elite, elite, sub-elite, and high-competitive. RESULTS: Univariate analyses of variance showed that male top-elite swimmers outperformed high-competitive swimmers from the age of 12, sub-elite swimmers from the age of 14 and elite swimmers from the age of 18 while female top-elite swimmers outperformed high-competitive and sub-elite swimmers from the age of 12 and elite swimmers from the age of 14 (P < .05). Frequency analysis showed that male top-elite swimmers for the first time achieved top-elite level between the 17 and 24 years old (mean age of 21) while female top-elite swimmers started to perform at top-elite level between the 14 and 24 years old (mean age of 18). CONCLUSION: Male and female top-elite swimmers are characterized by a high-performance level from 12 years on and progressively outperform swimmers from similar age. However, this goes together with a large variety in the individual pathways toward top-elite level within and between sexes.

Objectives. To identify disparities in several types of insured health care costs in the Netherlands across neighborhoods with different socioeconomic statuses and to assess the room for improvement. Methods. We used 2015 Dutch whole-population registry data to estimate the age- and gender-specific cost structure by neighborhood for total, specialist, pharmaceutical, and mental health care. Classifying neighborhoods by the quintile of their neighborhood socioeconomic status (NSES), we determined differences in observed and expected health care costs for several scenarios of NSES improvement. Results. From low to high NSES, we found a clear downward gradient in health care costs. Total health care costs would drop by 7.3% if each neighborhood's cost structure was equal to that of the most affluent neighborhoods. The potential for cost reduction appeared highest for females, for age groups between 40 and 60 years, and for pharmaceutical care. Conclusions. Low NSES is associated with relatively high health care costs, and represents considerable potential for cost savings in health care. Public Health Implications. Our research suggests that policies aimed at improving the socioeconomic determinants of health locally may be pivotal in containing health care costs.

Sports performance benchmarks useful to select and guide future elites are limited in literature. The present study introduces a method to enable comparison between sports performance of different generations and creates performance benchmarks for the future elites in speed skating. 1500 m Season Best Times (SBT) of Dutch skaters (1043 females, 1812 males, age 13-26 years), who competed in at least six seasons between 1993 and 2013, were corrected for the prevailing world record (WR): rSBT=(SBT/WR)*100%. Regression analyses showed that the calendar year affected SBT (p<0.01), but not rSBT (p>0.05). Based on rSBT, performance groups were defined: elite (rSBT<110%), sub-elite (110%125%). Benchmarks were based on the slowest rSBT per age of the elite group. Of the total skaters performing within the elite benchmarks, the elite performance group represented <20% up to age 16 and <50% up to age 21. An out of sample group (n=299) confirmed the usability of the benchmarks. So, by correcting time-trial performance for the prevailing WR, elite performance benchmarks can be made based on multiple generations of elite skaters. The benchmarks can be used to select and guide future elite skaters from age 13-26 years.

Little is known about the role of sport participation in socioeconomic health inequalities. We studied the association between different aspects of sport participation with all-cause mortality, type 2 diabetes mellitus (T2DM) and obesity, including inequalities between socioeconomic subpopulations. Using the Dutch Lifelines cohort study (n = 84,230), we assessed the associations of sport participation, as well as the amount, intensity, type and number of sports, with all-cause mortality, T2DM and obesity in individuals. We studied the effect of sport participation on health outcomes within and between educational categories. Outcomes were compared with moderate to vigorous physical activity (MVPA). Sport participation was significantly associated with lower mortality (HR = 0.81), T2DM (HR = 0.70), and obesity (HR = 0.77). No significant additional effects of the amount or intensity of sport participation were found, while participating in teams sport was associated with significantly lower mortality (HR = 0.53) compared with other types of sport. These effects were similar among educational categories. Sport participation explained between 11% (T2DM and obesity) and 22% (mortality) of health inequalities between educational categories. This was more than twice the effect size of MVPA. The sensitivity analysis with net income as the socioeconomic indicator showed similar results. Our results suggest that to reduce socioeconomic differences in health, public health policies should focus on increasing sport participation in groups with a low socioeconomic status, rather than increasing the amount or intensity of sport participation, or MVPA in general.

The present study strived to gain a more profound understanding of the distinctions in development between swimmers who are considered to be on track to the senior elite level compared to those who are not. Longitudinal data of 29 talented sprint and middle-distance swimmers (12 males; 17 females) on season best performances (season best times) and underlying performance characteristics (anthropometrics, starts, turns, maximal swimming velocity, stroke index [SI, an indirect measure of swimming efficiency] and lower body power) were collected over four swimming seasons (median of n = 3 seasons per swimmer). Based on their season best performance at early senior age (males aged 18-19; females aged 17-18), some swimmers were considered to be on track to reach the elite level (referred to as high-performing seniors; 6 males and 10 females), whereas others were not (referred to as lower-performing seniors; 6 males and 7 females). Retrospectively studying these swimmers (males and females separately), we found that all high-performing seniors were already on track to the elite level at late junior age (males aged 17; females aged 16), evidenced with faster season best performances throughout their transition compared to their lower-performing peers (p < 0.05). Independent sample t-tests revealed that high-performing seniors significantly outscored their lower-performing peers on maximal swimming velocity (males and females), starts and turns (males), SI (females) and lower body power (females) at late junior age (p < 0.05). Additionally, multilevel models showed faster rates of development for high-performing seniors on turns and maximal swimming velocity (males), and SI (females) compared to lower-performing peers during the junior-to-senior transition (p < 0.05). Particularly, the higher initial levels of swim performance and underlying characteristics at late junior age as well as the ability to keep progressing on season best performances (males and females), turns and maximal swimming velocity (males), and SI (females) during the junior-to-senior transition, may be crucial factors in the attainment of swimming expertise.

PURPOSE: This study aimed to use a large-scale longitudinal design to investigate the development of the distribution of effort (e.g., pacing) in adolescent swimmers, specifically disentangling the effects of age and experience and differentiating between performance levels in adulthood. METHODS: Season best times and 50-m split times of 100- and 200-m freestyle swimmers from five continents were gathered between 2000 and 2021. Included swimmers competed in a minimum of three seasons between 12 and 24 yr old (5.3 ± 1.9 seasons) and were categorized by performance level in adulthood (elite, sub-elite, high-competitive; 100-m: n = 3498 (47% female); 200-m: n = 2230 (56% female)). Multilevel models in which repeated measures (level 1) were nested within individual swimmers (level 2) were estimated to test the effects of age, race experience, and adult performance level on the percentage of total race time spent in each 50-m section ( P < 0.05). RESULTS: In the 100-m, male swimmers develop a relatively faster first 50-m when becoming older. This behavior also distinguishes elite from high-competitive swimmers. No such effects were found for female swimmers. Conversely, more experienced male and female swimmers exhibit a slower initial 50-m. With age and race experience, swimmers develop a more even velocity distribution in the 200-m. Adolescent swimmers reaching the elite level adopt a more even behavior compared with high-competitive. This differentiation occurs at a younger age in female (>13 yr) compared with male (>16 yr) swimmers. CONCLUSIONS: Pacing behavior development throughout adolescence is driven by age-related factors besides race experience. Swimmers attaining a higher performance level during adulthood exhibit a pacing behavior that better fits the task demands during adolescence. Monitoring and individually optimizing the pacing behavior of young swimmers is an important step toward elite performance.

Home advantage is a pervasive phenomenon in sport. It has been established in team sports such as basketball, baseball, American football, and European soccer. Attention to home advantage in individual sports has so far been limited. The aim of this study was to examine home advantage in professional tennis. Match-level data are used to measure home advantage. The test used is based on logit models, and consistent specification is addressed explicitly. Depending on the interpretation of home advantage, restrictions on the specification of the model need to be imposed. We find that although significant home advantage exists for men, the performance of women tennis players appears to be unaffected by home advantage.

Objective: The main goal of the present study was to investigate the interim performance progression (IPP) of talented swimmers. Part of this group ultimately made it to the top (referred to as elite swimmers) whereas others did not make it to the top (referred to as high-competitive swimmers). Rather than investigating performance progression based solely on season best performances, we included the first swim performance of the season in the metrics of IPP. Knowledge about the IPP of talented swimmers from and toward their season best performances relative to the first swim performance of the season will enhance our understanding of changes in season best performances during the talent trajectory and provide valuable insights for talent development and selection processes in competitive swimming. Methods: Fifteen thousand nine hundred and forty four swim performances (first swim performances of the season and season best performances) between 1993 and 2019 of 3,199 talented swimmers (of whom 556 reached elite level and 2,643 reached high-competitive level) were collected from Swimrankings and related to the prevailing world record of the corresponding sex. The pattern of IPP was represented by two phases: phase A and phase B. Phase A reflected the performance progression between the previous season best performance and the first swim performance of the current season (PPA) and phase B reflected the performance progression between the first swim performance of the current season and the season best performance of the current season (PPB). Depending on the normality check, we used independent sample t-tests or Mann Whitney tests to establish significant differences in PPA and PPB between elite and high competitive swimmers per age category per sex (p < 0.05). Results: Without denying individual differences, male elite swimmers improved more during phase B from age 15 till 24 compared to high-competitive swimmers (20.5% vs. 13.1%, respectively, p < 0.05). Female elite swimmers improved more during phase B from age 13 till 23 compared to high-competitive swimmers (21.1% vs. 14.6%, respectively, p < 0.05). Except for age 14 in males, there were no significant differences between performance groups in PPA. Conclusion: Talented swimmers who ultimately made it to the top (elite swimmers) are characterized with different patterns of IPP compared to talented swimmers who did not make it to the top (high-competitive swimmers). After puberty, elite and high-competitive swimmers performed in general ~1% slower at the start of their season compared to their previous season best performance (PPA). However, elite swimmers improved more in the period between their first swim performance of the season and their season best performance (PPB) from age 13 (females) and age 15 (males) onwards.

Home advantage is a well-documented phenomenon in many sports. Home advantage has been shown to exist for team sports (soccer, hockey, football, baseball, basketball) and for countries organizing sports tournaments like the Olympics and World Cup Soccer. There is also some evidence for home advantage in some individual sports, but there is a much more limited literature. This paper addresses the issue of home advantage in speed skating. From a methodological point of view, it is difficult to identify home advantage, because skaters vary in their abilities and the conditions of tournaments vary. There is a small but significant home advantage using a generalized linear mixed model, with random effects for skaters and fixed effects for skating rinks and seasons. Even though the home advantage effect exists, it is very small when compared to variation in skating times due to differences of rinks and individual abilities.

BACKGROUND: Grouping of patients based on a predominant dyspeptic symptom is frequently employed in management strategies for dyspepsia. Such subdivision, however, suggests that dyspeptic symptom patterns are constant over time. OBJECTIVE: To investigate the behavior of symptoms over time and to study the effects of diagnostic procedures and treatment on the pattern and severity of dyspeptic symptoms. METHODS: Patients with persistent dyspeptic symptoms completed a validated questionnaire at regular time intervals as part of a clinical trial in primary care. Based on predominant symptoms, patients were classified into ulcer-like dyspepsia, reflux-like dyspepsia, dysmotility-like dyspepsia, and unspecific dyspepsia according to the Rome II criteria. RESULTS: Questionnaires were returned at baseline, 1, 3, and 6 months by 185, 172, 169, and 170 patients, respectively. At baseline, 35% of patients reported predominantly reflux-like dyspepsia, 34% had ulcer-like dyspepsia, 16% had dysmotility-like dyspepsia, and in 15% symptoms were not specific. During the 6-month follow-up period, only 35% of patients kept the same predominant symptom. Symptom (in)stability was not dependent on diagnostic procedures or on therapy with proton pump inhibitors, H2-receptor antagonists, prokinetics, or antacids. CONCLUSION: In the majority of dyspeptic patients, symptoms change continuously as time goes on. Symptom instability is not influenced by diagnostic procedures or therapy. Thus, there is little sense in symptom-based management of dyspepsia in primary care.
