BACKGROUND: Poor disinfection is the main cause of blood contamination, so its elimination is key to limiting the entry of bacteria into the collection system. With the advancement of antiseptic technology, antiseptics with sterile, disposable applicators are now available. AIM: To evaluate in situ two antiseptics (with and without applicators) for blood banks and to demonstrate in vitro antiseptic activity on bacterial biofilms of importance in transfusion medicine. METHODS: Antiseptic A (2% sterile solution of chlorhexidine gluconate/70% isopropyl alcohol provided with applicator) and bulk antiseptic B (10% povidone-iodine) were evaluated. The deferred blood donor arms were subjected to disinfection with antiseptics A and B and the contralateral arms were cultured to determine the baseline bacterial load (control). Antiseptic activity was assessed by ANOVA and logaritmic reduction values (LRV) and percentage reduction values (PRV) were calculated. Finally, the in vitro activity of antiseptic A was analyzed by confocal laser scanning microscopy (CLSM) on biofilm models. RESULTS: Prior to disinfection tests, commensal and clinically important bacteria were identified; antiseptic A showed post-disinfection bacterial growth rates of zero compared to controls (p < 0.0001). The frequency of bacterial growth with antiseptic B was 74%. A significant difference was identified between both antiseptics, where antiseptic A showed higher activity (p < 0.5468). LRV and PRV were 0.6-2.5/100% and 0.3-1.7/66.7-99.7% for antiseptics A and B, respectively. Through CLSM, disinfectant A (without applicator) showed lower in vitro antiseptic activity on the tested biofilms at the exposure times recommended by the manufacturer. CONCLUSIONS: Sterile solution of chlorhexidine gluconate/isopropyl alcohol with applicator showed advantages disinfection in deferred blood donors over povidone-iodine.

Depletion of veins for dialysis access is a challenging life threatening situation for patients in need of haemodialysis. The utilisation of intracardiac catheter is a rare procedure with scarce reported experience. We describe the case of a 68-year-old male that contributes to the limited knowledge of performing a life-saving intracardiac catheter placement for emergency haemodialysis in a patient without immediate alternative renal replacement therapy available. We also retrospectively analyse the experience reported so far and summarise complications and outcomes. In our case, the patient was able to pursue haemodialysis after intracardiac catheter placement without any complications. Two weeks later, the patient successfully received a kidney transplant from a deceased donor and has a serum creatinine of 1.7 mg/dL after 2 years of follow-up. There are only four reported cases of kidney transplantation after the procedure, including our own. Intracardiac catheter is an emerging option that could be considered in certain patients as the last resort. Further investigation with regards to patient candidacy and procedure security are necessary.

Acinetobacter baumannii is a relevant bacterium due to its high-resistance profile. It is well known that antimicrobial resistance is primarily linked to mutations and the acquisition of external genomic material, such as plasmids or phages, to which the Clustered Regularly Interspaced Short Palindromic Repeats associated with Cas proteins, or CRISPR-Cas, system is related. It is known that the system can influence the acquisition of foreign genetic material and play a role in various physiological pathways. In this study, we conducted an in-silico analysis using 91 fully assembled genomes of clinical strains obtained from the NCBI database. Among the analyzed genomes, the I-F1 subtype of the CRISPR-Cas system was detected showcasing variations in architecture and phylogeny. Using bioinformatic tools, we determined the presence, distribution, and specific characteristics of the CRISPR-Cas system. We found a possible association of the system with resistance genes but not with virulence determinants. Analysis of the system's components, including spacer sequences, suggests its potential role in protecting against phage infections, highlighting its protective function.

BACKGROUND: There is scarce evidence on the fourth dose of severe acute respiratory syndrome coronavirus 2 vaccines in chronic kidney disease (CKD) patients. We evaluated the humoral response and effectivity of the fourth dose in the CKD spectrum: non-dialysis CKD (ND-CKD), haemodialysis (HD), peritoneal dialysis (PD) and kidney transplant (KT) recipients. METHODS: This is a prespecified analysis of the prospective, observational, multicentric SENCOVAC study. In patients with CKD who had received a complete initial vaccination and one or two boosters and had anti-Spike antibody determinations 6 and 12 months after the initial vaccination, we analysed factors associated with persistent negative humoral response and higher anti-Spike antibody titres as well as the efficacy of vaccination on coronavirus disease 2019 (COVID-19) severity. RESULTS: Of 2186 patients (18% KT, 8% PD, 69% HD and 5% ND-CKD), 30% had received a fourth dose. The fourth dose increased anti-Spike antibody titres in HD (P = .001) and ND-CKD (P = .014) patients and seroconverted 72% of previously negative patients. Higher anti-Spike antibody titres at 12 months were independently associated with repeated exposure to antigen (fourth dose, previous breakthrough infections), previous anti-Spike antibody titres and not being a KT recipient. Breakthrough COVID-19 was registered in 137 (6%) patients, 5% of whom required admission. Admitted patients had prior titres <620 UI/ml and median values were lower (P = .020) than in non-admitted patients. CONCLUSIONS: A fourth vaccine dose increased anti-Spike antibody titres or seroconverted many CKD patients, but those with the highest need for a vaccine booster (i.e. those with lower pre-booster antibody titres or KT recipients) derived the least benefit in terms of antibody titres. Admission for breakthrough COVID-19 was associated with low anti-Spike antibody titres.

PURPOSE: To report on results of pars plana vitrectomy with ILM peeling in patients with rhegmatogenous retinal detachment (RRD) and concomitant macular hole (MH) and to assess for preoperative associated conditions related to this type of RRD. METHODS: Patients undergoing surgical repair for RRD between 2014 and 2021 were reviewed, and subjects with concomitant, non-causal, macular hole were identified. We studied post-operative macular status, retinal reattachment rate and visual acuity. RESULTS: Over 532 eyes operated on for RRD, 11 (2.06%) had a concurrent non-causal macular hole. Preoperative PVR B or superior was recorded in 86 eyes (16.6%) of the entire cohort and in 6 eyes (54.54 %) with RRD and concomitant MH (p=0.00001). Severe hypotony with choroidal detachment was present in 15 eyes (2.81%) of the entire cohort and in 3 eyes (27.27%) with RRD and concomitant MH (p=<0.00001). CONCLUSIONS: RRD with concomitant MH is an infrequent association. Retinal reattachment and anatomical hole closure can be achieved in most of cases but despite this fact, functional recovery is usually not good. Preoperative PVR is a more frequent finding in this group of patients, as well as severe hypotony with choroidal detachment.

The incidence of antibiotics and transcriptional regulation of ARGs in isolated bacteria from wastewater needs to be explored. By HPLC, in samples of untreated wastewater, ampicillin (49.74 ± 5.70 µg/mL), chloramphenicol (0.60 ± 0.03 µg/mL), tylosin (72.95 ± 2.03 µg/mL), and oxytetracycline (0.22 ± 0.01 µg/mL) was determined. Through metagenomic analysis identified 58 bacterial species belonging to 9 phyla and at least 14 species have shown resistance to a variety of antibiotics. Twenty-two bacterial isolates were proved to be resistant to fifteen antibiotics of new generation and used in medical research to combat infectious diseases. Fourteen strains were shown to harbor plasmids in size ranges of 2-5 Kb, 6-10 Kb and plasmids with size greater than 10 Kb. By quantitative PCR it was possible to identify genes sul, qnr, cat1, aadA1, and sat-1 gene were shown to be present in gDNA samples from treated and untreated samples of wastewater and by relative expression analysis, differential expression of cat1, ermB, act, and tetA genes was demonstrated in strains that showed identity with Escherichia coli, Bacteroides fragilis, and Salmonella thyphi, and that were stressed with different concentrations of antibiotics. The presence of ARGs in untreated water samples, as well as in bacterial isolates, was indicative that in these habitats there are microorganisms that can resist ß-lactams, aminoglycosides, tetracyclines, sulfonamides, and quinolones.

A 12-day experiment was conducted to investigate the effects of Artemia enrichment with two experimental microalgal emulsions (formulated with selected fatty acid contents) on Penaeus vannamei postlarvae. For this purpose, 405,000 postlarvae (stage 1) were obtained from a commercial hatchery in Santa Elena, Ecuador, and distributed into nine fiberglass tanks. Postlarvae were fed for 12 days with three experimental diets (three tanks per treatment): treatment A (Artemia enriched with experimental microalgal emulsion A and dry diet), treatment B (Artemia enriched with experimental microalgal emulsion B and dry diet) and nonenriched Artemia (Artemia without enrichment and dry diet). At the end of the experiment, length (mm), coefficient of variation of population sizes, number of postlarvae in a gram of weight (PL-gram), biochemical composition, fatty acid profile, hepatopancreas perimeter, and histopathological hepatopancreas status of P. vannamei postlarvae (stage 12) were analyzed. To evaluate the status of the hepatopancreas, a categorization range (1-5) was created with different histological parameters such as number of B cells, vesicles around them, healthy tubules, and degradation tissues. Growth traits did not present differences between treatments; total length was 10.17 mm, 10.83 mm, and 10.27 mm for treatment A, treatment B, and nonenriched treatment, respectively, and PL-gram was 141.00, 162.00, and 142.33 for treatment A, treatment B, and nonenriched treatment, respectively. Biochemical composition of postlarvae (lipids, ash, and protein content) did not present differences between the three treatments. Significant differences were observed in the content of three essential fatty acids (DHA, DPA, and ARA) in Penaeus vannamei postlarvae fed with Artemia enriched with experimental emulsions. Thus, DHA content was significantly superior in animals fed with Artemia enriched with treatments A and B (9.80 ± 0.71% and 9.75 ± 0.44%, respectively) than in animals fed with unenriched Artemia (5.78 ± 0.68) (P < 0.05). Concerning arachidonic acid (ARA), treatments A and B showed 3.31 ± 0.20% and 3.19 ± 0.09%, respectively, higher than postlarvae fed with unenriched Artemia, 2.73 ± 0.04% (P < 0.05). Regarding DPA content, treatments A and B reported higher values of MA and MB (0.81 ± 0.06% and 0.86 ± 0.08%, respectively), than unenriched Artemia (0.43 ± 0.02%) (P < 0.05). Interestingly, the increase in DHA, DPA, and ARA contents in postlarvae coincided with the increase in hepatopancreas perimeter. In addition, a large number of B cells, a large number of healthy tubules, increased dilatation of the central tube, and a lower percentage of deteriorated tissue were observed in the hepatopancreas when postlarvae were fed with enriched Artemia.

This paper presents the implementation of a measurement system that uses a four microphone array and a data-driven algorithm to estimate depth of cut during end milling operations. The audible range acoustic emission signals captured with the microphones are combined using a spectral subtraction and a blind source separation algorithm to reduce the impact of noise and reverberation. Afterwards, a set of features are extracted from these signals which are finally fed into a nonlinear regression algorithm assisted by machine learning techniques for the contactless monitoring of the milling process. The main advantages of this algorithm lie in relatively simple implementation and good accuracy in its results, which reduce the variance of the current noncontact monitoring systems. To validate this method, the results have been compared with the values obtained with a precision dynamometer and a geometric model algorithm obtaining a mean error of 1% while maintaining an STD below 0.2 mm.

Acinetobacter baumannii is a Gram-negative bacillus that causes multiple infections that can become severe, mainly in hospitalized patients. Its high ability to persist on abiotic surfaces and to resist stressors, together with its high genomic plasticity, make it a remarkable pathogen. Currently, the isolation of strains with high antimicrobial resistance profiles has gained relevance, which complicates patient treatment and prognosis. This resistance capacity is generated by various mechanisms, including the modification of the target site where antimicrobial action is directed. This mechanism is mainly generated by genetic mutations and contributes to resistance against a wide variety of antimicrobials, such as ß-lactams, macrolides, fluoroquinolones, aminoglycosides, among others, including polymyxin resistance, which includes colistin, a rescue antimicrobial used in the treatment of multidrug-resistant strains of A. baumannii and other Gram-negative bacteria. Therefore, the aim of this review is to provide a detailed and up-to-date description of antimicrobial resistance mediated by the target site modification in A. baumannii, as well as to detail the therapeutic options available to fight infections caused by this bacterium.

Class D ß-lactamases OXA-232 and OXA-48 hydrolyze penicillin, cephalosporins and carbapenems, limiting the pharmacological therapeutics in bacteraemia. OXA producer microorganisms are considered a great emergent threat, especially in nosocomial environments. To determine the resistance profile and genomic characterization of two isolates initially identified as potential carbapenemase-producer Klebsiella oxytoca in a third level hospital. Automated platform BD Phoenix-100 System was used to identify and to biochemically characterize both isolates. Furthermore, the resistance profile was determined through CLSI methods and the whole genome sequences were obtained using Next-Generation Sequencing. Resistance genes were analyzed, and the virtual fingerprinting was determined to corroborate the similarity with related bacteria. Both strains correspond to Raoultella ornithinolytica carrying OXA 232 and OXA-48 genes, confirming the class D ß-lactamases assay results. Here, we present the genetic and phenotypic analysis of multidrug resistance R. ornithinolytica, representing the first report in Mexico.

Continuous authentication systems have been proposed as a promising solution to authenticate users in smartphones in a non-intrusive way. However, current systems have important weaknesses related to the amount of data or time needed to build precise user profiles, together with high rates of false alerts. Voice is a powerful dimension for identifying subjects but its suitability and importance have not been deeply analyzed regarding its inclusion in continuous authentication systems. This work presents the S3 platform, an artificial intelligence-enabled continuous authentication system that combines data from sensors, applications statistics and voice to authenticate users in smartphones. Experiments have tested the relevance of each kind of data, explored different strategies to combine them, and determined how many days of training are needed to obtain good enough profiles. Results showed that voice is much more relevant than sensors and applications statistics when building a precise authenticating system, and the combination of individual models was the best strategy. Finally, the S3 platform reached a good performance with only five days of use available for training the users' profiles. As an additional contribution, a dataset with 21 volunteers interacting freely with their smartphones for more than sixty days has been created and made available to the community.

Resting tremor in Parkinson's disease (PD) is one of the most distinctive motor symptoms. Appropriate symptom monitoring can help to improve management and medical treatments and improve the patients' quality of life. Currently, tremor is evaluated by physical examinations during clinical appointments; however, this method could be subjective and does not represent the full spectrum of the symptom in the patients' daily lives. In recent years, sensor-based systems have been used to obtain objective information about the disease. However, most of these systems require the use of multiple devices, which makes it difficult to use them in an ambulatory setting. This paper presents a novel approach to evaluate the amplitude and constancy of resting tremor using triaxial accelerometers from consumer smartwatches and multitask classification models. These approaches are used to develop a system for an automated and accurate symptom assessment without interfering with the patients' daily lives. Results show a high agreement between the amplitude and constancy measurements obtained from the smartwatch in comparison with those obtained in a clinical assessment. This indicates that consumer smartwatches in combination with multitask convolutional neural networks are suitable for providing accurate and relevant information about tremor in patients in the early stages of the disease, which can contribute to the improvement of PD clinical evaluation, early detection of the disease, and continuous monitoring.

Solar damage due to ultraviolet radiation (UVR) is implicated in the development of two proliferative lesions of the ocular surface: pterygium and pinguecula. Pterygium and pinguecula specimens were collected, along with adjacent healthy conjunctiva specimens. RNA was extracted and sequenced. Pairwise comparisons were made of differentially expressed genes (DEGs). Computational methods were used for analysis. Transcripts from 18,630 genes were identified. Comparison of two subgroups of pterygium specimens uncovered evidence of genomic instability associated with inflammation and the immune response; these changes were also observed in pinguecula, but to a lesser extent. Among the top DEGs were four genes encoding tumor suppressors that were downregulated in pterygium: C10orf90, RARRES1, DMBT1 and SCGB3A1; C10orf90 and RARRES1 were also downregulated in pinguecula. Ingenuity Pathway Analysis overwhelmingly linked DEGs to cancer for both lesions; however, both lesions are clearly still benign, as evidenced by the expression of other genes indicating their well-differentiated and non-invasive character. Pathways for epithelial cell proliferation were identified that distinguish the two lesions, as well as genes encoding specific pathway components. Upregulated DEGs common to both lesions, including KRT9 and TRPV3, provide a further insight into pathophysiology. Our findings suggest that pterygium and pinguecula, while benign lesions, are both on the pathological pathway towards neoplastic transformation.

The increase in the use of antimicrobials such as colistin for the treatment of infectious diseases has led to the appearance of Aeromonas strains resistant to this drug. However, resistance to colistin not only occurs in the clinical area but has also been determined in Aeromonas isolates from the environment or animals, which has been determined by the detection of mcr genes that confer a resistance mechanism to colistin. The variants mcr-1, mcr-3, and mcr-5 have been detected in the genus Aeromonas in animal, environmental, and human fluids samples. In this article, an overview of the resistance to colistin in Aeromonas is shown, as well as the generalities of this molecule and the recommended methods to determine colistin resistance to be used in some of the genus Aeromonas.

Presently, large cities have significant problems with noise pollution due to human activity. Transportation, economic activities, and leisure activities have an important impact on noise pollution. Acoustic noise monitoring must be done with equipment of high quality. Thus, long-term noise monitoring is a high-cost activity for administrations. For this reason, new alternative technological solutions are being used to reduce the costs of measurement instruments. This article presents a design for a versatile electronic device to measure outdoor noise. This device has been designed according to the technical standards for this type of instrument, which impose strict requirements on both the design and the quality of the device's measurements. This instrument has been designed under the original equipment manufacturer (OEM) concept, so the microphone-electronics set can be used as a sensor that can be connected to any microprocessor-based device, and therefore can be easily attached to a monitoring network. To validate the instrument's design, the device has been tested following the regulations of the calibration laboratories for sound level meters (SLM). These tests allowed us to evaluate the behavior of the electronics and the microphone, obtaining different results for these two elements. The results show that the electronics and algorithms implemented fully fit within the requirements of type 1 noise measurement instruments. However, the use of an electret microphone reduces the technical features of the designed instrument, which can only fully fit the requirements of type 2 noise measurement instruments. This situation shows that the microphone is a key element in this kind of instrument and an important element in the overall price. To test the instrument's quality and show how it can be used for monitoring noise in smart wireless acoustic sensor networks, the designed equipment was connected to a commercial microprocessor board and inserted into the infrastructure of an existing outdoor monitoring network. This allowed us to deploy a low-cost sub-network in the city of Málaga (Spain) to analyze the noise of conflict areas due to high levels of leisure noise. The results obtained with this equipment are also shown. It has been verified that this equipment meets the similar requirements to those obtained for type 2 instruments for measuring outdoor noise. The designed equipment is a two-channel instrument, that simultaneously measures, in real time, 86 sound noise parameters for each channel, such as the equivalent continuous sound level (Leq) (with Z, C, and A frequency weighting), the peak level (with Z, C, and A frequency weighting), the maximum and minimum levels (with Z, C, and A frequency weighting), and the impulse, fast, and slow time weighting; seven percentiles (1%, 5%, 10%, 50%, 90%, 95%, and 99%); as well as continuous equivalent sound pressure levels in the one-third octave and octave frequency bands.

Freezing of gait (FOG) is one of the most incapacitating motor symptoms in Parkinson's disease (PD). The occurrence of FOG reduces the patients' quality of live and leads to falls. FOG assessment has usually been made through questionnaires, however, this method can be subjective and could not provide an accurate representation of the severity of this symptom. The use of sensor-based systems can provide accurate and objective information to track the symptoms' evolution to optimize PD management and treatments. Several authors have proposed specific methods based on wearables and the analysis of inertial signals to detect FOG in laboratory conditions, however, its performance is usually lower when being used at patients' homes. This study presents a new approach based on a recurrent neural network (RNN) and a single waist-worn triaxial accelerometer to enhance the FOG detection performance to be used in real home-environments. Also, several machine and deep learning approaches for FOG detection are evaluated using a leave-one-subject-out (LOSO) cross-validation. Results show that modeling spectral information of adjacent windows through an RNN can bring a significant improvement in the performance of FOG detection without increasing the length of the analysis window (required to using it as a cue-system).

Iron molecule is of great importance in the synthesis of hemoglobin which is essential for oxygen transport. Iron levels are quantified by accurately high sensitivity tests, such as serum ferritin (SF). However, common studies to quantify SF are long and strenuous (~ 5 h), for example enzyme-linked immunosorbent assay (ELISA). In this paper, blood serum samples were analyzed by Raman spectroscopy (RS), and a computational analysis of spectra is proposed to detect differences in SF as an alternative procedure. Serum samples were obtained from 22 patients, 9 who were clinically diagnosed with anemia and 13 controls. Patients with anemia had low levels of SF (< 30 ng/ml), and a control group had levels between 30 and 500 ng/ml. The spectra obtained were conditioned with a baseline correction and smoothing, then evaluated by principal component analysis (PCA), and a predictive model was estimated by lineal discrimination analysis (LDA). The results showed a clear differentiation of the study groups by PCA, also 99.69% sensitivity and 100% specificity by LDA. This study suggest that Raman spectroscopy is a fast (~ 5 min) and a powerful tool capable to qualitative differentiate ferritin concentrations.

To reveal mycovirus diversity, we conducted a search of as-yet-unexplored Mediterranean isolates of the phytopathogenic ascomycete Rosellinia necatrix for virus infections. Of seventy-nine, eleven fungal isolates tested RNA virus-positive, with many showing coinfections, indicating a virus incidence of 14%, which is slightly lower than that (approximately 20%) previously reported for extensive surveys of over 1000 Japanese R. necatrix isolates. All viral sequences were fully or partially characterized by Sanger and next-generation sequencing. These sequences appear to represent isolates of various new species spanning at least 6 established or previously proposed families such as Partiti-, Hypo-, Megabirna-, Yado-kari-, Fusagra- and Fusarividae, as well as a newly proposed family, Megatotiviridae. This observation greatly expands the diversity of R. necatrix viruses, because no hypo-, fusagra- or megatotiviruses were previously reported from R. necatrix. The sequence analyses showed a rare horizontal gene transfer event of the 2A-like protease domain between a dsRNA (phlegivirus) and a positive-sense, single-stranded RNA virus (hypovirus). Moreover, many of the newly detected viruses showed the closest relation to viruses reported from fungi other than R. necatrix, such as Fusarium spp., which are sympatric to R. necatrix. These combined results imply horizontal virus transfer between these soil-inhabitant fungi.

Wearable technology has had a significant growth in the last years; this is particularly true of smartwatches, due to their potential advantages and ease of use. These smart devices integrate sensors that can be potentially used within industrial settings and for several applications, such as safety, monitoring, and the identification of occupational risks. The accelerometer is one of the main sensors integrated into these devices. However, several studies have identified that sensors integrated into smart devices may present inaccuracies during data acquisition, which may influence the performance of their potential applications. This article presents an analysis from the metrological point of view to characterize the amplitude and frequency response of the integrated accelerometers in three currently available commercial smartwatches, and it also includes an analysis of the uncertainties associated with these measurements by adapting the procedures described in several International Organization for Standardization (ISO) standards. The results show that despite the technical limitations produced by the factory configuration, these devices can be used in various applications related to occupational risk assessment. Opportunities for improvement have also been identified, which will allow us to take advantage of this technology in several innovative applications within industrial settings and, in particular, for occupational health purposes.

PURPOSE: To provide data of the incidence and management of common urological malignancies in renal transplant recipients. MATERIALS AND METHODS: We conducted a retrospective analysis of a prospective database from August 1967 to August 2015. A descriptive analysis of the sample was performed. RESULTS: Among 1256 consecutive RTR a total of 88 patients developed malignancies (7%). There were 18 genitourinary tumors in the 16 patients (20.45% of all malignant neoplasms), incidence of 1.27%. The most common neoplasm encounter was renal cancer (38.8%), followed by urothelial carcinoma (33.3%). Median follow up of transplantation was 197 months (R, 36-336). Mean time from RT to cancer diagnosis 89±70 months (R, 12-276). CsA and AZA was the most common immunosuppression regimen in 68.75%. Mean follow-up after diagnosis was 103±72 months (R 10-215). Recurrence free survival rate of 100%. Overall survival of 89.5% of the sample; there were two non-related cancer deaths during follow up. CONCLUSIONS: The incidence of neoplasms in RTR was lower than in other series, with favorable functional and oncologic results after treatment. This suggests that actions to reduce the risk of these malignancies as well as a strict follow-up are mandatory for an early detection and treatment.
