RESUMO

The article presents data on non-alcoholic fatty liver disease and chronic viral hepatitis C in combination with opisthorchosis invasion. With the system approach considers the specific features of the clinical, laboratory and functional data in patients with combined pathology. Observed frequency of the pain, asthenic and allergic cholestatic syndromes, the latter as part of the triad Paltsev. The high efficiency of the placenta hydrolisat - laennec, as means of pathogenetic therapy.

AIM: To study the specific features of blood lipid composition in combat veterans (CVs) with posttraumatic stress disorder (PTSD). SUBJECTS AND METHODS: One hundred and sixty-one CVs were examined; their mean age was 42.8 +/- 0.7 years. All the patients underwent a clinical and neurological examination using additional studies. I.O. Kotenev's trauma stress questionnaire was used. The mean intensity of PTSD was 65.5 +/- 0.95 scores. Fasting biochemical blood tests were carried out using clinical chemical analyzers. RESULTS: Dyslipidemias characterized mainly by elevated levels of total cholesterol and low-density lipoproteins are identified in the majority of present-day CVs. Higher triglyceride levels and lower high-density lipoproteins are encountered 2 and 1.5 times more frequently, respectively. A tendency has been found towards an inverse correlation between the degree of PTSD and the levels of total cholesterol and low-density lipoproteins. CONCLUSION: Neuroendocrine changes accompanying PTSD and leading to intensified adrenergic influences are an important trigger of accelerated hyperlipidemia development in CVs. Age-related changes and metabolic hepatic disorders associated with alcohol abuse and consequences of prior infectious diseases have become to play an important role in the pathogenesis of dyslipidemias in patients over 40-45 years of age.

RESUMO

The clinical laboratory investigations of patients with chronic viral hepatitises combined with chronic opisthorchosis are presented. Among the examined, 60 patients lived in Western Siberia and 50 patients in the North. More severe course of combined diseases was marked in the Northerners. Prevalence of such syndromes as diskynetic, cholestasis, hepatomegalies, and disorders of microbiocenosis of gastrointestinal tract has proved this fact. Features of treatment of combined diseases have been shown, stage therapy has been developed.

OBJECTIVES: The purpose of the work was to reveal some pathophysiological regularities of digestive diseases (DD) development, depending on a period of living in the North. STUDY DESIGN: A total of 1,014 workers of industrial enterprises in Mirny-city (Yakutia) have been screened according to the WHO\ CINDI program. DD have been revealed in 321 patients. Dynamics of the clinical manifestations of DD has been analyzed depending on a period of living in the North with time intervals 0-5 years, 5-9 years, 10-29 years, 30 years and more. METHODS: There have been defined the content of cortisol, insulin, total cholesterol, triglycerides, alpha-cholesterol, neutrophils' activity on lysosomecation test with their percent distribution in blood serum. RESULTS AND CONCLUSION: It is stated that at digestive diseases, the hormonal-metabolic indices depend on a period of living in the North. The phase dynamics of changes of lipid, cortisol, insulin of cationic protein in blood neutrophils has been revealed. The data presented give more deep information about formation mechanisms of DD pathology.

RESUMO

The effect of the immunity state on the clinical course of stomach ulcer was studied. The study involved 50 patients (38 men and 12 women being 17-67 years old) with stomach ulcer (n = 14) and duodenal ulcer (n = 36) in the acute condition and 25 healthy blood donors. Twenty-seven patients underwent an immunology study. It was revealed that the evident increase of circulatory lymphocyte apoptosis (> 10), which is associated with the depression of their proliferative activity in vitro culture, is recorded with 40.7% of patients (11/27). It was also revealed that a high level of circulatory lymphocyte apoptosis and apoptosis-associated dysfunctions are found more frequently in people with long-term and complicated clinical course.

A clinical and endoscopic examination and morphological study of gastrobiopsies were conducted in 62 patients with Helicobacter (H.) pylori-negative and -positive chronic atrophic gastritis (CAG) to study the manifestations of the atrophic process in the mucous coat of the stomach (MCS) and stages of its progression in terms of pre-cancerous changes (second stage). The MCS atrophy is characterized by considerable frequency of concomitant endocrinopathies, connective tissue dysplasia, systemic lesions of the mucous coats of the gastrointestinal tract, as well as morphologically diverse disorders of proliferation and differentiation. Thus, the MCS atrophy appears as a result of homeostasis dysregulation at various levels. CAG being a particular manifestation of the atrophy phenomenon characterized by polyetiology and complexity of pathogenesis cannot be reduced to one causal factor (HP), and therefore it deserves special attention and needs further study.

The data characterizing tolerance and efficiency of autologous bone marrow cells in the treatment of patients with cirrhosis of the liver are presented. Injection of autologous bone marrow cells was not associated with the development of adverse reactions. Cell therapy of patients with compensated cirrhosis arrested asthenic syndrome, reduced cytolysis, increased the level of serum albumin and platelet count. Ultrasonic examination revealed reduction of portal hypertension (the area of the spleen and the portal vein lumen decreased). In patients with decompensated cirrhosis, a positive response presenting as reduction of the disease severity (by 1.9 points) was observed in 48.6% cases. Positive shifts in these patients were associated with a decrease of ALT and AST levels, reduction of laboratory signs of cirrhosis, increase in platelet count, and reduction of the asthenic syndrome. Hence, therapy with autologous bone marrow cells is safe and, according to preliminary results, can be regarded as a new approach to the treatment of patients with cirrhosis of the liver.
