A new method for plasty of the parotid duct with its appreciable dilatation is proposed. A semioval flap is cut from the wall of a dilated duct and sutured to the buccal mucosa. A proper opening of the parotid duct is thus created and injuries to facial nerve branches during surgery are prevented.

A rare case, heterotopia of the salivary gland in a female patient with repeated recurrences of radicular cysts of the mandible, is described. Histologic examination of removed material revealed dystopic acinar tissue in the subepithelial layer of the radicular cyst. A hypothesis on the possible effect of acinar heterotopic tissue on recurrences of the cystic process is offered.

A diagnostic error occurred in a case with one of the forms of chronic sialadenitis, the so-called Küttner's [correction of Kuttmer's] "tumor". This disease resembles a tumor by its clinical course, and this may become the cause of an unjustified surgical intervention. The etiology of the disease is unknown; possible enlargement of the submandibular salivary glands as their substitute hypertrophy in diabetes mellitus is discussed.

A case of Hodgkin's disease with local involvement of the submandibular area is described. Profound examinations found that the tumor did not originate from the salivary gland. Anatomic examination of the resected tumor revealed Hodgkin's diseases. The authors emphasize the necessity of differential diagnosis between Hodgkin's disease and chronic sialadenitis.

A new method for the treatment of chronic sialodochitis and sialolithiasis is suggested for cases with essential dilatation of the duct in the intraglandular compartment of the parotid gland: the external wall of dilated duct is dissected and the two resultant parts are twisted inside and sutured to the internal wall of the dilated duct; the parotid duct is ligated at the site of its exit from the gland.

The authors have analyzed wound microflora composition and characteristics in 422 patients with maxillofacial phlegmons and abscesses, treated over a period of 25 years (1963-1988). They have examined the microbiologic structure in these conditions in various years. Their findings evidence a gradual reduction of the incidence of Staphylococcus aureus and S. epidermidis and a significant elevation of the detection rate of gamma-hemolytic streptococcus (63.9 percent).

The efficacy of local use of antiseptic solutions (chlorhexidine bigluconate, chlorfillipt and furaciline) on clinical presentation and microflora of purulent wounds was studied in 236 patients with maxillofacial abscesses and phlegmons. The wound microflora was represented by a wide spectrum of opportunistic agents. A pronounced therapeutic effect was achieved in patients with staphylococcal infection. Marked resistance to the drugs was noted in alpha-hemolytic streptococcus.

The authors analyze the results of diagnosis and treatment in 1062 patients with diffuse inflammatory diseases of the maxillofacial area and of complications of these diseases, such as mediastinitis, thrombosis of the cavernous sinus of the dura mater, meningoencephalitis. Improvement of the program of diagnosis and treatment helped improve the results: mortality from sepsis reduced from 50 to 26%. Approaches to prevention of progressive purulent infection are outlined.

Examinations including tasks presented as tests were carried out in 319 fourth-year students of dental surgery. This method for assessing the knowledge and skills of students proved to be highly objective.
