RESUMO

BACKGROUND: Accurate information in provider directories are vital in health care including health information exchange, health benefits exchange, quality reporting, and in the reimbursement and delivery of care. Maintaining provider directory data and keeping it up to date is challenging. The objective of this study is to determine the feasibility of using natural language processing (NLP) techniques to combine disparate resources and acquire accurate information on health providers. METHODS: Publically available state licensure lists in Connecticut were obtained along with National Plan and Provider Enumeration System (NPPES) public use files. Connecticut licensure lists textual information of each health professional who is licensed to practice within the state. A NLP-based system was developed based on healthcare provider taxonomy code, location, name and address information to identify textual data within the state and federal records. Qualitative and quantitative evaluation were performed, and the recall and precision were calculated. RESULTS: We identified nurse midwives, nurse practitioners, and dentists in the State of Connecticut. The recall and precision were 0.95 and 0.93 respectively. Using the system, we were able to accurately acquire 6849 of the 7177 records of health provider directory information. CONCLUSIONS: The authors demonstrated that the NLP- based approach was effective at acquiring health provider information. Furthermore, the NLP-based system can always be applied to update information further reducing processing burdens as data changes.

RESUMO

With an increase in the legalization of recreational marijuana across the USA, advertising for marijuana products is more widespread, especially on the Internet where such practices pose a regulatory challenge. In this study, we examined the content of marijuana advertising on Weedmaps, a popular website that markets marijuana retailers online. A total of 146 recreational marijuana retailers in Colorado and Washington were examined on Weedmaps. We studied the age verification practices made in retailers' own websites, the presence of health claims they made about marijuana on Weedmaps, and the characteristics of followers of Weedmaps on social media sites. Many retailers had no security measure to determine age (41 % in Colorado, 35 % in Washington). Approximately 61 % of retailers in Colorado and 44 % in Washington made health claims about the benefits of marijuana, including anxiety reduction, treatment of depression, insomnia, and pain/inflammation. Inferred demographic characteristics of followers of Weedmaps on Twitter and Instagram revealed that over 60 % were male and nearly 70 % or more were age 20-29 years old, yet some (15-18 %) were under the age of 20. Our findings indicate that marijuana retailers have a visible presence on the Internet. Potential customers might be enticed by retailers who tout health claims about marijuana use. It may also be appealing for a younger demographic to overlook age restrictions and engage with marijuana retailers via social media. As a whole, our findings can help to guide future policy making on the issue of marijuana-related advertising.

RESUMO

OBJECTIVES: The goal of the HealthMap Vaccine Finder is to provide a free, comprehensive, online service where users can search for locations that offer immunizations. In this article, we describe the data and systems underlying the HealthMap Vaccine Finder (HVF) and summarize the project's first year of operations. METHODS: We collected data on vaccination services from a variety of providers for 2012-2013. Data are used to populate an online, public, searchable map. RESULTS: In its first year, HVF collected information from 1256 providers representing 46 381 locations. The public Web site received 625 124 visits during the 2012-2013 influenza vaccination season. CONCLUSIONS: HVF is a unique tool that connects the public to vaccine providers in their communities. During the 2012-2013 influenza season, HVF experienced significant usage and was able to respond to user feedback with new features.

RESUMO

INTRODUCTION: Early access to Emergency Medical Services determines survival in out-of-hospital cardiac arrest. However, a significant proportion of adults do not know the emergency phone number (EN) and no intervention has been proposed to improve it. Therefore, we aimed to assess prospectively the effectiveness of single advice from a physician on knowledge of the EN in adult population. MATERIALS AND METHODS: The study was conducted among participants of "Health, Alcohol and Psychosocial Factors in Eastern Europe" study. A total of 942 persons (aged 48-82 years) randomly selected from an urban population registry were interviewed and then instructed about the correct EN (the intervention group). After 12 months knowledge of the EN was assessed in the intervention group (n=716) and in matched control group (n=435). RESULTS: The correct EN was given by 498 (69.6%) participants at baseline and in 550 (76.8%) participants 12 months afterwards (p<0.001). At follow-up the knowledge of EN was higher by in intervention group than in controls (76.8% vs 70.6%, p=0.02). Factors associated with better educational effect were male sex (OR 1.49; 95% CI 1.04-2.1) and secondary or higher level of education (OR 1.44; 95% CI 1.08-1.91). CONCLUSIONS: We concluded that a single instruction about the EN from a physician increases its long-term knowledge and should be offered during medical visits.

RESUMO

PURPOSE: Until 1996, there were only 7 masters-level programs in health professions education (HPE); currently, there are 76 such programs. The purpose of this article is to provide information and perspectives about the available masters programs in HPE worldwide, with a focus on their mission, objectives, content, instructional strategies, format, duration, and cost, as well as the similarities and differences among them. METHOD: A literature and web search was conducted to develop a complete list of programs that offer a masters degree in HPE or closely related areas. RESULTS: Forty three percent [43% (33)] of these programs are in Europe, 20% (15) in North America, 17% (13) in Asia, 7% (5) in Latin America, 5% (4) in the Middle East, 5% (4) in Australia, and 3% (2) in Africa. The mission of these masters programs is to prepare leaders in HPE or sometimes narrowly focusing on clinical or dental education. The content is addressed in core courses and electives, generally grouped under the following headings: curriculum development, instructional methods/teaching and learning, assessment strategies, program evaluation, research design, and sometimes leadership and management. Medical schools sponsor approximately two-thirds of these programs, and the average duration for completion is 2 years. The vast majority of these programs are offered in English. CONCLUSION: The commonalities among these programs include focus, content, and educational requirements. The variations are mostly in organization and structure. There is a need to establish criteria and mechanisms for evaluation of these programs. The geographic maldistribution of these programs is a major concern.

RESUMO

Elder mistreatment is unrecognized, hidden, and underreported. Studies show that health professionals, nurses among them, are largely unaware of the various forms of elder mistreatment that take place and of the proper course of action to pursue when mistreatment is suspected. In this article, we describe elder mistreatment policies, examining them in the light of the United States national elder abuse policies with a focus on important provisions of the Elder Justice Act. Next, we review the various types of abuse, the identification of abuse, and the nurses role in addressing elder mistreatment. We present an example of a case of physical abuse and conclude by discussing nurses' advocacy role and issuing a call to action, challenging nurses to '"act now" to protect the well being of elders.

RESUMO

Over 1.4 million new cases of cancer are diagnosed each year, and many of these patients will, by necessity, be treated in private practice, including dental practice. Dental professionals play a key role in helping patients understand that good oral care can prevent or reduce oral complications. Treatment of oral cancers and other malignancies cause oral sequelae that can compromise patients' quality of life and dictate reduction or discontinuation of optimal therapeutic regimens, which in turn reduces the odds of long-term survival. This can be prevented or better managed if dental and medical health care providers work together. The purpose of this article is to identify the cancer centers associated with dental clinics and the dental practitioners in the state of Texas, including maxillofacial prosthodontists, with training and/or a special interest in providing oral care to cancer patients. To be included on the list, which will be available on the Dental Oncology Education Program (DOEP) Web site (doep.org), please contact Grady Basler at the DOEP office (grady@doep.org), or the Department of Public Health Sciences (214-828-8350).

RESUMO

Internet websites offering androgenic anabolic steroids (AAS) were identified and available products were examined. Keywords for the website search were: "anabolic steroids," "anabolic steroids buy," "anabolic steroid purchase." The first 10 websites offering AAS in the first 10 pages of results were considered. At least two AAS-containing products per website were selected. Thirty AAS-selling websites were identified, mainly located in the United States (46.7%) and Europe (30%). Most websites sold other anabolic/ergogenic products (clenbuterol, 76.7%; GH/IGF, 60.0%; thyroid hormones, 46.7%; erythropoietin, 30.0%; insulin, 20.0%) or products for AAS-related adverse effects (mainly: estrogen antagonists, 63.3%; products for erectile dysfunction, 56.7%; 5α-reductase inhibitors, 33.3%; anti-acne products, 33.3%). AAS were sold as medicines (69.6%) or as dietary supplements (30.4%). AAS in medicines were mainly: nandronole (20.4%), methandrostenolone (18.4%), and testosterone (12.2%). Dietary supplements contained mainly DHEA and included several fake compounds. Manufacturers were declared for 97.9% of medicines and 66.7% of dietary supplements; however, several manufacturers were not found on the Internet. Described benefits were usually few adverse effects and no estrogenicity. Toxicity was seldom reported and presented as mild. Recommended doses were two-fourfold higher than current medical recommendations. In conclusion, misleading information and deceiving practices were common findings on AAS-selling websites, indicating their deleterious potential for public health.

RESUMO

BACKGROUND: Judo therapy is a well established Japanese co-medical profession specializing in outpatient manual treatment of fractures and sprains. Recently, the number of judo therapists has been rapidly increasing as a result of proliferation judo therapy academies. This study examines whether such rapid increases have improved geographical distribution of judo therapy facilities in Japan. METHODS: The number of judo therapy facilities and the population in each municipality were obtained from the Web yellow pages and from Japanese census data for 2004, 2006, and 2008, respectively. Lorenz curves and Gini indices were calculated to demonstrate distributions of judo therapy facilities per 100,000 people. A bootstrapped method was used to identify statistical significances of differences in Gini indices. RESULTS: In all municipalities, the mean numbers of judo therapy facilities per 100,000 people were 15.3 in 2004, 15.8 in 2006, and 17.6 in 2008. The Gini indices for judo therapy facilities nationally were 0.273 in 2004, 0.264 in 2006, and 0.264 in 2008. The numbers of judo therapy facilities increased significantly between 2006 and 2008 (p < 0.05) but the indices did not change significantly in the same period. The Gini indices for local towns and villages remained unchanged and were consistently higher (p < 0.05) than those in urban areas throughout the study periods. CONCLUSION: Our results suggest that recent increases in the number of judo therapy facilities have not necessarily led to greater equality in their geographic distribution in terms of Gini indices.

RESUMO

The way research is conducted has changed over time, from simple experiments to computer modeling and simulation, from individuals working in isolated laboratories to global networks of researchers collaborating on a single topic. Often, this new paradigm results in the generation of staggering amounts of data. The intensive use of data and the existence of networks of researchers characterize e-Science. The role of libraries and librarians in e-Science has been a topic of interest for some time now. This column looks at tools, resources, and projects that demonstrate successful collaborations between libraries and researchers in e-Science.

RESUMO

Lesbian, gay, bisexual, and transgender (LGBT) persons, while widely diverse in many ways, share health disparities related to the stigma and discrimination they experience, including disproportionate rates of psychiatric disorders, substance abuse, and suicide. Lesbians, gay men, bisexuals, and the transgender communities have additional health concerns and disparities unique to each population. This paper highlights the national recognition of these health issues and disparities and presents web-based information resources about them and their mitigation.

RESUMO

With improvements in care, there has been exponential growth in the population of adult congenital heart disease (ACHD) patients. We sought to assess the availability of specialized ACHD care in the United States. We analyzed the Adult Congenital Heart Association's ACHD clinic directory for information on patient volume, provider training, and other characteristics. The information is self-reported and unverified. The ACHD directory included 72 programs in the United States. Across programs, the majority of patients (33%) seen were between 21 and 30 years old. Program directors had between 2 and 50 years (median 15) of ACHD experience and had dedicated between 10 and 100% (median 30%) of their clinical time to ACHD. There were 2,800 ACHD operations performed per year, ranging from 0 to 230 (median 28) per program. There were between 0 and 5 cardiac surgeons (median 2) involved per program. Each surgeon averaged 20 ACHD operations per year. The growing ACHD population is a largely underserved group. Few programs in the United States provide specialized care, and this care is variably conducted within pediatric and/or adult facilities. These data should serve as a stimulus to improving accessibility of services for this vulnerable population.
