RESUMO

Photobiomodulation with low-level laser therapy (PBM-LLLT) has been introduced as a new tool to relieve nipple pain and repair nipple damage in breastfeeding women; however, evidence is needed to assess its effectiveness. The aim was to evaluate the effect of a single application of PBM-LLLT for breastfeeding women with nipple pain and damage; side effects were also collected. We conducted a randomized double-blinded controlled trial with women with nipple damage who were exclusively breastfeeding and rooming-in at Amparo Maternal maternity service, São Paulo, Brazil (May 2016 to May 2017). Women were randomly assigned into laser (n = 40) or control group (n = 40). Intervention was a single irradiation (660 nm, 100 mW, 2 J, 66.66 J/cm2, 3.3 W/cm2, 20 s of irradiation, punctual, and continuous mode) applied directly. Women reported pain levels at recruitment (before and immediately after irradiation), 6 and 24 h after the treatment. Pain level during a breastfeed was assessed using the Visual Analogue Scale (0 to 10). The primary outcome was the level of nipple pain immediately after the laser irradiation. Data were analyzed using hierarchical model and Wald test. At baseline, pain levels were similar (mean of 7.4 in laser group and 7.1 in control group). Women's perception of pain reduced approximately one point in both groups. Thirty-one percent of participants in the laser group (11/36) reported secondary effects, such as tingling (10/36) and pricking (2/36). The laser protocol of a single application was not effective in reducing pain in women with damaged nipples. Tingling sensation may be experienced by women receiving laser treatment for nipple damage.

OBJECTIVE: To analyze the association between the surgical characteristics of breast implants, time elapsed since surgery, access route, implant placement and implanted volume and variables related to breastfeeding, type, first 'milk let-down', breast engorgement, pain, lesion, milk production and use of galactagogues. METHOD: A prospective cohort carried out during the hospital stay (12 to 72 hours after delivery), home care (5thto 7thday after delivery) and telephone contact (between the 30thand 32ndday postpartum) of 115 postpartum women with breast implants between 2015 and 2017. RESULTS: The first evaluation identified more frequent use of oral galactagogues (p=0.029) by puerperal women with prepectoral implants, and of oxytocin spray by those with implants up to 270 ml (p=0.040). The second evaluation showed a higher pain score among those with prepectoral implants (p=0.046). Around the 30thday postpartum, the presence of nipple lesion (p=0.021), pain (p=0.025) and a higher pain score (p=0.039) was more frequent among those with mammoplasty performed less than 10 years ago. CONCLUSION: The presence of pain and a higher pain score, the occurrence of lesion and the use of oral and nasal galactagogues were associated with implant placement, implant size and time elapsed since surgery.

OBJECTIVES: To identify breastfeeding assessment tools, their application in clinical practice, and their validation and cross-cultural adaptation. METHOD: This is an integrative review of literature obtained from six databases and an online library, conducted from August 2014 to December 2015, without a temporal delimitation. RESULTS: We identified 19 assessment tools, of which 12 were validated and five were cross-culturally adapted. In terms of adaptation, the tools were used to assess the risk of early weaning (BAPT) and the perception/behaviour of mothers during nursing (BSES-SF and IIFAS). CONCLUSIONS: The identification of the available instruments and their indications for breastfeeding assessments can help health workers choose the ideal instrument, and qualify maternal and child care.

Pain accruing from nipple lesions caused by inadequate latching by the baby is a common complaint among breastfeeding women and an important obstacle to successful breastfeeding. Nipple pain occurs during the first days after delivery and is considered one of the main causes for early weaning. To investigate the efficacy of low-level laser therapy as a treatment for nipple pain due to breastfeeding. A triple-blind, randomized, clinical study. A university-affiliated hospital in São Paulo, Brazil. Fifty-nine women with nipple lesions at the time of their admission. Thirty women (intervention group) received three sessions of laser therapy (InGaAIP laser, 660 nanometer, 40 milliwatts of power, 5 Joules per square centimeter of energy density for 5 seconds each, total energy = 0.6 Joules) in the region of the nipples at three different points in time (0 hour, 24 hours, and 48 hours after diagnosis of nipple lesion). Twenty-nine women with similar clinical conditions were randomly assigned to the control group. Self-reported pain was recorded before and after laser therapy using a visual analogue scale. The intervention group experienced a decrease of 2.0 centimeters in intensity of pain (p = .016) 24 hours after the first intervention and also presented lower levels of pain compared with the control group. Low-level laser therapy was considered effective for treating nipple lesions in breastfeeding women with pain, providing relief and prolonging exclusive breastfeeding. More clinical trials with different laser dosimetry and parameters are necessary to optimize laser therapy protocols for breastfeeding women.

OBJECTIVE: To analyze the temperature curve of raw or pasteurized human milk exposed to different heating methods. METHOD: Experiments with volumes of 5 ml to 100 ml of human milk were carried out between 2016 and 2021 and analyzed according to the exposure time by different heating methods. Descriptive statistics included the calculation of means, medians, minimum and maximum values, measures of dispersion and standard deviation. RESULTS: The thermal curve made it possible to identify the heating of human milk close to body temperature when subjected to a water bath and microwaves. Milk exposed to room temperature (21°C) was unable to reach this temperature. When heated in a water bath at 40°C, smaller volumes reached body temperature between 3 and 5 minutes, while in a microwave at 50% power, practically all volumes reached temperature. CONCLUSION: The temperature curves of raw or pasteurized human milk were constructed, and it was possible to verify its behavior using different heating methods for administering the food in a neonatal intensive care unit, considering the volume, type and time of heating and temperature.

The objective of this research was to identify the pattern of exclusive breastfeeding (EBF) in the first 6 months of infants born in a Baby-Friendly Hospital and the factors that contribute to early weaning. This was a prospective cohort study with 261 mothers and children. The data were analyzed via the construction of a Kaplan-Meier survival curve, and the log-rank test was used for the univariate analysis. A multivariate analysis was performed using the Cox proportional-hazards regression model. During the 6 months, the percentage of mothers who practiced EBF for 30, 90, 120, 150 and 180 days was 75%, 52%, 33%, 19% and 5.7%, respectively. In the multivariate analysis, the variables that conferred a risk for early weaning were the hospital and the occurrence of a follow-up visit due to mammary complication, improper positioning and the association of both of these factors. The Baby-Friendly Hospital Initiative favored EBF.

RESUMO

The hospitalization of a newborn separates mother and child at a time when their contact is essential for developing the mother's role. Therefore, mothers tend to feel incapable of meeting their child's needs, and face difficulties in dealing with their personal feelings and the demands of their family. The objective of this study was to identify the defining characteristics of the nursing diagnosis Conflict in performing the role of mother in studies addressing the experience of being a mother in the neonatal unit. This is an integrative literature review, which utilized 15 qualitative studies published between 2004 and 2009, in journals indexed in the Latin American and Caribbean Health Sciences Literature and on the Medical Literature and Retrieval System Online. This diagnosis is a nursing phenomenon to be studied by neonatal nurses so they are able to recognize and propose interventions to meet the mothers' needs, considering that nine out of ten defining characteristics were identified in the mothers' statements.

OBJECTIVES: to construct and validate a classification of nipple and areola complex lesions resulting from breastfeeding, according to content and appearance. METHODS: this is a methodological study, developed in four stages: operational definition, instrument construction, content and appearance validation. The Delphi technique and a Likert-type scale were used to validate content and appearance, through the participation of ten and sixteen judges, respectively. For analysis, Content Validity Index and Kappa Coefficient were applied. RESULTS: Content Validity Index obtained an overall value of 0.93 and, for appearance, 0.94. Kappa values ranged between 0.46 and 1. The high rates of agreement among judges demonstrated the quality of the proposed content validity. CONCLUSIONS: the Nipple and Areola Complex Lesions Classification Instrument developed obtained acceptable values of its indexes, proving to be valid in terms of content and appearance.

Our aim was to explore the feeding practices of indigenous children younger than two years old, living in Cruzeiro do Sul and Mâncio Lima, in Acre, Brazil. A cross-sectional study was conducted with indigenous children of the Katukina (Cruzeiro do Sul), Nukini, Nawa and Poyanawa (Mâncio Lima) ethnicities in October 2013. We used a structured questionnaire on the food consumption of the day before the collection, based on the food indicators proposed by the Ministry of Health. A total of 94 children (50% Katukina, 27% Poyanawa, 13% Nukini, and 10% Nawa) were studied. Among children under 6 months of age, although the majority was breastfed, 42.1% drank water, 15.8%, savory food, and 11.1%, local cuisine. In the age groups of 6 to 12 months and 13 to 23 months, a high consumption of ultra-processed foods was identified (52.6% and 28.6% respectively), while the consumption of complementary food was insufficient (33% of fruits and 25% of savory food between 6 and 13 months, and 41.2% of fruits and 19.6% of savory food between 13 and 23 months). The feeding profile of these indigenous children was inadequate for their age, revealing a concerning situation for this population.


Objetivou-se explorar a prática alimentar das crianças indígenas de zero a dois anos, residentes nos municípios de Cruzeiro do Sul e Mâncio Lima no Acre. Realizou-se estudo transversal com crianças indígenas das etnias Katukina (Cruzeiro do Sul), Nukini, Nawa e Poyanawa (Mâncio Lima) em outubro de 2013. Utilizou-se um questionário estruturado sobre o consumo alimentar do dia anterior à coleta, baseado nos indicadores alimentares propostos pelo Ministério da Saúde. Estudou-se 94 crianças (50% da etnia Katukina, 27% Poyanawa, 13% Nukini e 10% da Nawa). As crianças menores de 6 meses de idade, apesar da maioria receber leite materno, 42,1% delas consumiam água, 15,8% comida de sal e 11,1% preparados típicos da região. Nas faixas de idade de 6 a 12 meses e 13 a 23 meses identificou-se um alto consumo de alimentos ultraprocessados (52,6% e 28,6% respectivamente) enquanto que o consumo de alimentos complementares relevou-se insuficiente (33% de frutas e 25% de comida de sal na faixa de 6 a 13 meses e 41,2% de frutas e 19,6% de comida de sal na faixa de 13 a 23 meses). O perfil alimentar das crianças indígenas mostrou-se inadequado para a idade, refletindo uma situação preocupante na saúde infantil desta população.

The aim of the study was to identify the breast feeding position and holding variables related to nipple trauma. This case-control study assessed the onset of nipple trauma among women hospitalized at a University Hospital in the city of São Paulo, in 2004 and 2005. Subjects were puerperae diagnosed with unilateral or bilateral nipple trauma. Data analysis was performed using chi-square, Student's t, and odds ratio tests (CI = 95%) and correspondence analysis. Participants were 146 puerperal women and their newborns, being 73 cases and 73 controls. Statistically significant position and holding variables for causing lesions were the following: newborns with their necks bent/contorted, chin away from the breast and lip-related defect (turned inward). Trauma prevention at the beginning of breast feeding is crucial for continuing this practice. Following adequate positioning is decisive for establishing effective and prolonged breast feeding.

Resumo Objetivo Analisar a suficiência de leite humano nas Unidades de Terapia Intensiva Neonatais da Região Metropolitana de São Paulo, segundo o quantitativo de prematuros, demanda estimada de leite humano e consumo efetivo. Métodos Estudo transversal em Postos de Coleta e Bancos de Leite Humano cadastrados na Rede Paulista de Bancos de Leite Humano em 2017. Utilizou-se dados de estatísticas vitais censitárias de 2017 para identificar a distribuição dos prematuros nas Redes Regionais de Atenção à Saúde. Resultados Participaram do estudo 100% dos Postos de Coleta e 84,6% dos Bancos de Leite Humano de São Paulo. Identificou-se uma insuficiência na distribuição do leite humano pasteurizado, entre a demanda estimada e o consumo efetivo dos prematuros. No período do estudo, apenas as regiões Centro e Sudeste atendiam 100% da cobertura de leite humano, seguidas da região do grande ABC com 44,62%. Conclusão A maioria dos Postos de coleta e Bancos de Leite humano da região metropolitana de São Paulo, não atendem todas as crianças prematuras. O gerenciamento destas unidades pode contribuir para estimular a captação de doadoras e a otimização do leite humano coletado.

Se utilizaron datos de estadísticas vitales censitarias de 2017 para identificar la distribución de los prematuros en las Redes Regionales de Atención en Salud. Resultados Participaron en el estudio el 100 % de los Puntos de Recolección y el 84,6 % de los Bancos de Leche Humana de São Paulo. Se identificó una insuficiencia en la distribución de leche humana pasteurizada entre la demanda estimada y el consumo efectivo de los prematuros. En el período del estudio, solo las regiones Centro y Sudeste atendían el 100 % de la cobertura de leche humana, seguidas por la región del Gran ABC con el 44,62 %. Conclusión La mayoría de los Puntos de Recolección y Bancos de Leche Humana de la región metropolitana de São Paulo no atiende a todos los bebés prematuros. La administración de estas unidades puede contribuir para estimular la captación de donantes y la optimización de la leche recolectada.


Abstract Objective To analyze human milk sufficiency in Neonatal Intensive Care Units in the Metropolitan Region of São Paulo according to the number of premature babies, estimated demand for human milk and actual consumption. Methods This is a cross-sectional study, carried out at Human Milk Collection Stations and Human Milk Banks registered with the São Paulo Network of Human Milk Banks in 2017. Data from 2017 census vital statistics were used to identify premature baby distribution in Regional Health Care Networks. Results 100% of Human Milk Collection Stations and 84.6% of Human Milk Banks in São Paulo participated in the study. Pasteurized human milk distribution insufficiency was identified between estimated demand and actual consumption by premature babies. During the study period, only the Center and Southeast regions met 100% of human milk coverage, followed by the Greater ABC region, with 44.62%. Conclusion Most Human Milk Collection Stations and Human Milk Banks in the metropolitan region of São Paulo do not assist all premature children. The management of these units can contribute to encouraging donor recruitment and collected human milk optimization.

OBJECTIVE: To identify the profile and assess the self-esteem of postpartum women, to characterize their babies and partners, and to verify the association of these characteristics with the occurrence of intimate partner violence (IPV). METHOD: Cross-sectional study with 207 postpartum women assisted in a public clinic. RESULTS: There was no statistic correlation among the sociodemographic variables, personal and obstetric characteristics with the occurrence of IPV. The postpartum women who showed low self-esteem presented a higher exposure risk to IPV (p<0.01; OR=2.01 and CI 95% [1.40-2.87]). The mothers of the babies that were born with low weight (less than 2,500 g) had almost twice the chances of suffering violence (p<0.05; OR=1.74 and CI 95% [1.00-3.03]). The women whose partners did not consume alcohol presented a lower probability to be exposed to IPV (p<0.05; OR=0.182 and CI 95% [0.03-0.93]). CONCLUSION: Women's low self-esteem, babies' inappropriate weight and consumption of alcohol by partners were correlated to the occurrence of IPV.

OBJECTIVE: To identify the main recommendations found in systematic reviews regarding exclusive breastfeeding protective factors. DATA SOURCE: Integrative review based on the guiding question: What evidence is found in literature regarding the protective factors of exclusive breastfeeding during the intrahospital period? A search was conducted in the Cochrane Library, PubMed/MEDLINE and LILACS database using the keyword "Breast Feeding" and the word "Breastfeeding". Systematic reviews published from 2007 to 2016 that answered the guiding question were included in the study, whereas systematic reviews that analyzed breastfeeding of preterm infants and breastfeeding of children with orofacial malformation were excluded. The sample included eight systematic reviews. DATA SYNTHESIS: The recommendations related to the protective factors for exclusive in-hospital breastfeeding found in the systematic reviews were: early skin-to-skin contact, rooming-in care, intervention for treating painful nipples during breastfeeding, restriction of infant supplementation, baby-led breastfeeding and educational interventions and support for mothers during hospital stay. The proposed measures included the six practices presented as protective factors. CONCLUSIONS: The review enabled the identification of evidence to support the recommended measures from delivery room to hospital discharge, with the aim of encouraging breastfeeding and preventing intrahospital weaning.


OBJETIVO: Identificar as principais recomendações encontradas em revisões sistemáticas relacionadas aos fatores de proteção do aleitamento materno exclusivo intra-hospitalar. FONTE DE DADOS: Revisão integrativa partindo da questão norteadora: Quais são as evidências identificadas na literatura relacionadas aos fatores de proteção ao aleitamento materno exclusivo no período intra-hospitalar? Foi realizada uma busca nas bases de dados Cochrane Library, PubMed/MEDLINE e LILACS por meio do descritor Breast Feeding e da palavra Breastfeeding. Os critérios de inclusão foram revisões sistemáticas publicadas entre 2007 e 2016 que respondiam à questão norteadora; e os critérios de exclusão, revisões sistemáticas que analisaram o aleitamento de crianças prematuras e com má formação orofacial. A amostra foi composta por oito revisões sistemáticas. SÍNTESE DOS DADOS: As recomendações relacionadas aos fatores de proteção ao aleitamento materno exclusivo intra-hospitalar encontradas nas revisões sistemáticas foram: o contato pele a pele precoce; a permanência da criança em alojamento conjunto; a intervenção na dor mamilar durante a amamentação; a restrição do uso de suplementação para lactentes; o aleitamento materno sob livre demanda; e as intervenções educativas por meio de suporte individual e/ou em grupos durante a internação. O conjunto de medidas proposto incluiu todas as seis práticas apresentadas. CONCLUSÕES: A revisão possibilitou a identificação de evidências que embasaram as medidas propostas com a finalidade de incentivar o aleitamento materno exclusivo intra-hospitalar, iniciado na sala de parto e seguido pela internação até a alta hospitalar.

RESUMEN Objetivo: Analizar la curva de temperatura de la leche humana cruda o pasteurizada expuesta a diferentes métodos de calentamiento. Método: Se realizaron experimentos con volúmenes de 5 ml a 100 ml de leche humana entre 2016 y 2021 y se analizaron en función del tiempo de exposición mediante diferentes métodos de calentamiento. La estadística descriptiva incluyó el cálculo de medias, medianas, valores mínimos y máximos, medidas de dispersión y desviación estándar. Resultados: La curva térmica permitió identificar el calentamiento de la leche humana próximo a la temperatura corporal cuando se sometió a baño maría y microondas. La leche expuesta a temperatura ambiente (21°C) fue incapaz de alcanzar esta temperatura. Cuando se calentó en un baño de agua a 40°C, los volúmenes más pequeños alcanzaron la temperatura corporal entre 3 y 5 minutos, mientras que en un microondas al 50% de potencia, prácticamente todos los volúmenes alcanzaron la temperatura. Conclusión: Se construyeron las curvas de temperatura de la leche humana cruda o pasteurizada y se pudo comprobar su comportamiento utilizando diferentes métodos de calentamiento para administrar el alimento en una unidad de cuidados intensivos neonatales, teniendo en cuenta el volumen, el tipo y el tiempo de calentamiento y la temperatura.


RESUMO Objetivo: Analisar a curva de temperatura do leite humano cru ou pasteurizado exposto a diferentes métodos de aquecimento. Método: Experimentos com volumes de 5 ml a 100 ml de leite humano foram realizados entre 2016 e 2021 e analisados segundo o tempo de exposição por diferentes métodos de aquecimento. A estatística descritiva incluiu o cálculo das médias, medianas, valores mínimos e máximos, medidas de dispersão e desvio padrão. Resultados: A curva térmica permitiu identificar o aquecimento do leite humano próximo da temperatura corporal quando submetidos a banho-maria e micro-ondas. O leite exposto à temperatura ambiente (21°C) não foi capaz de atingir tal temperatura. No aquecimento em banho-maria a 40°C, volumes menores alcançaram a temperatura corporal entre 3 e 5 minutos, enquanto em micro-ondas na potência de 50%, praticamente todos os volumes alcançaram essa temperatura. Conclusão: As curvas de temperatura do leite humano cru ou pasteurizado foram construídas, sendo possível verificar o seu comportamento mediante diferentes métodos de aquecimento para administração do alimento em unidade de terapia intensiva neonatal, considerando o volume, tipo e tempo de aquecimento e temperatura.

ABSTRACT Objectives: to construct and validate a classification of nipple and areola complex lesions resulting from breastfeeding, according to content and appearance. Methods: this is a methodological study, developed in four stages: operational definition, instrument construction, content and appearance validation. The Delphi technique and a Likert-type scale were used to validate content and appearance, through the participation of ten and sixteen judges, respectively. For analysis, Content Validity Index and Kappa Coefficient were applied. Results: Content Validity Index obtained an overall value of 0.93 and, for appearance, 0.94. Kappa values ranged between 0.46 and 1. The high rates of agreement among judges demonstrated the quality of the proposed content validity. Conclusions: the Nipple and Areola Complex Lesions Classification Instrument developed obtained acceptable values of its indexes, proving to be valid in terms of content and appearance.


RESUMEN Objetivos: construir y validar una clasificación de las lesiones pezón-areolares resultantes de la lactancia materna según su contenido y apariencia. Métodos: estudio metodológico, desarrollado en cuatro etapas: definición operativa, construcción del instrumento, validación de contenido y apariencia. Para validar contenido y apariencia se utilizó la técnica Delphi y la escala tipo Likert, mediante la participación de diez y dieciséis jueces, respectivamente. Para el análisis se aplicó el Índice de Validez de Contenido y el Coeficiente Kappa. Resultados: el Índice de Validez de Contenido obtuvo un valor global de 0,93 y, por apariencia, 0,94. Los valores de Kappa oscilaron entre 0,46 y 1. Las altas tasas de acuerdo entre los jueces demostraron la calidad de la validez del contenido propuesto. Conclusiones: el Instrumento de Clasificación de Lesiones Pezón-Areolares desarrollado obtuvo valores aceptables para sus índices, demostrando ser válido en términos de contenido y apariencia.

Utilizaram-se a técnica Delphi e a escala tipo Likert para a validação de conteúdo e de aparência, por meio da participação de dez e dezesseis juízes, respectivamente. Para análise, aplicou-se o Índice de Validade de Conteúdo e o Coeficiente Kappa. Resultados: o Índice de Validade de Conteúdo obteve valor global de 0,93 e, de aparência, 0,94. Os valores de Kappa variaram entre 0,46 e 1. As altas taxas de concordância entre os juízes demonstraram a qualidade da validade de conteúdo proposta. Resumo Objetivo Analisar a duração do aleitamento materno e os fatores associados ao desmame total de crianças de seis a 23 meses e 29 dias de idade residentes no município de Cruzeiro do Sul, na Amazônia Ocidental Brasileira. Métodos Estudo transversal, realizado durante a Campanha Nacional de Multivacinação em 2016 e Contra a Influenza em 2017. A amostra foi calculada por conglomerados. A coleta de dados foi efetuada com as mães ou os responsáveis de 679 crianças que compareceram às campanhas de vacinação e responderam a um questionário. Utilizou-se a análise de sobrevivência de Kaplan-Meier e, para os fatores associados ao desmame total, a regressão de Cox. Para todos os testes estatísticos, foi considerado um nível de significância de 5%. Resultados O aleitamento materno foi praticado por 65,3% das crianças, cuja média de idade foi de 13,7 meses (DP± 4,9 meses). O tempo médio de desmame total foi de 16,7 meses (IC95%: 16,06 - 17,36) e a mediana de 22 meses, sendo a probabilidade de tempo de aleitamento materno até dois anos em 49,7%. Os fatores associados ao desmame total foram o tempo da experiência anterior em amamentação menor que seis meses, não praticar o aleitamento materno na primeira hora de vida, uso de chupeta e mamadeira. Conclusão A duração do aleitamento materno foi abaixo do recomendado. Os fatores associados ao desmame total de crianças entre 6 e 23 meses estão relacionados à experiência materna prévia, ao início precoce da prática de amamentação e ao uso de bicos artificiais.

Métodos Estudio transversal, realizado durante la Campaña Nacional de Multivacunación en 2016 y Contra la Influenza en el 2017. La muestra fue calculada por conglomerados. La recopilación de datos se realizó con las madres o los responsables de 679 niños que asistieron a las campañas de vacunación y respondieron un cuestionario. Se utilizó el análisis de supervivencia de Kaplan-Meier y, para los factores asociados al destete total, la regresión de Cox. Para todas las pruebas estadísticas se consideró un nivel de significación del 5 %. Resultados La lactancia materna se practicó en el 65,3 % de los niños, cuyo promedio de edad fue de 13,7 meses (DP± 4,9 meses). El tiempo promedio de destete total fue de 16,7 meses (IC95 %: 16,06 - 17,36) y la mediana de 22 meses, con una probabilidad de tiempo de lactancia materna hasta los dos años del 49,7 %. Los factores asociados al destete total fueron el tiempo de una experiencia anterior de lactancia inferior a seis meses, no practicar la lactancia materna en la primera hora de vida, uso de chupete y de mamadera. Conclusión La duración de la lactancia materna estuvo por debajo de lo recomendado. Los factores asociados al destete total de niños entre 6 y 23 meses están relacionados a experiencias maternas previas, al inicio precoz de la práctica de la lactancia y al uso de tetinas artificiales.


Abstract Objective To analyze the duration of breastfeeding and the factors associated with total weaning of infants aged six to 23 months and 29 days living in the municipality of Cruzeiro do Sul, in the Brazilian Western Amazon. Methods A cross-sectional study conducted during the National Multi-Vaccination Campaign in 2016 and Against Influenza in 2017. The sample was estimated by clusters. The data was collected with the mothers or guardians of 679 infants who attended vaccination campaigns and answered a questionnaire. Kaplan-Meier survival analysis was used, and Cox regression was used for the factors associated with total weaning. A 5% significance level was considered for all statistical tests. Results Breastfeeding was practiced by 65.3% of the infants, whose mean age was 13.7 months (SD± 4.9 months). The mean total weaning time was 16.7 months (95% CI: 16.06 - 17.36) and the median of 22 months, which is the probability of duration of breastfeeding up to two years old in 49.7%. The factors associated with total weaning were previous breastfeeding experience for less than six months, not breastfeeding in the first hour of life, pacifier use and bottle-feeding. Conclusion The duration of breastfeeding was below the recommended. The factors associated with total weaning of infants between six and 23 months are related to previous maternal experience, early initiation of breastfeeding and the use of artificial nipples.

OBJECTIVE: to identify the relationship between posttraumatic stress disorder, trait and state anxiety, and intimate partner violence during pregnancy. METHOD: observational, cross-sectional study developed with 358 pregnant women. The Posttraumatic Stress Disorder Checklist - Civilian Version was used, as well as the State-Trait Anxiety Inventory and an adapted version of the instrument used in the World Health Organization Multi-country Study on Women's Health and Domestic Violence. RESULTS: after adjusting to the multiple logistic regression model, intimate partner violence, occurred during pregnancy, was associated with the indication of posttraumatic stress disorder. The adjusted multiple linear regression models showed that the victims of violence, in the current pregnancy, had higher symptom scores of trait and state anxiety than non-victims. CONCLUSION: recognizing the intimate partner violence as a clinically relevant and identifiable risk factor for the occurrence of anxiety disorders during pregnancy can be a first step in the prevention thereof.

ABSTRACT Objectives: to identify the main quality management interventions used by professionals working at Human Milk Banks. Methods: a scope review conducted at PubMed, VHL, Scopus, SciELO, CAPES thesis and Google Scholar databases. Primary studies were included that address quality improvement strategies to improve Milk Bank processes in Portuguese, English and Spanish. Results: search totaled 192 scientific studies, 17 of which met the objective of the study and inclusion criteria. The main quality management interventions used in the Human Milk Bank addressed tools for continuous improvement. Six were aimed at improving processes, one to prevent errors and failures and two to achieve continuous monitoring. Final Considerations: the tools used by professionals working in Human Milk Banks have demonstrated effectiveness in managing the quality of services.


RESUMEN Objetivos: identificar las principales intervenciones de gestión de la calidad utilizadas por los profesionales que trabajan en los Bancos de Leche Humana. Métodos: revisión del alcance realizada en las bases de datos de tesis PubMed, BVS, Scopus, SciELO, CAPES y Google Scholar. Se incluyeron estudios primarios que abordan estrategias de mejora de la calidad para mejorar los procesos del Banco de Leche en portugués, inglés y español. Resultados: la búsqueda totalizó 192 producciones científicas, de las cuales 17 cumplieron el objetivo del estudio y los criterios de inclusión. Las principales intervenciones de gestión de la calidad utilizadas en Bancos de Leche Humanas abordaron herramientas para la mejora continua, con seis destinadas a mejorar los procesos, una para evitar errores y fallas y dos para lograr un monitoreo continuo. Consideraciones finales: las herramientas utilizadas por los profesionales que trabajan en los Bancos de Leche Humana han demostrado su eficacia en la gestión de la calidad de los servicios.

Foram incluídos estudos primários que abordam as estratégias de melhoria da qualidade para aperfeiçoar os processos nos Bancos de Leite nos idiomas português, inglês e espanhol. Resultados: a busca totalizou 192 produções científicas, sendo que 17 atenderam ao objetivo do estudo e critérios de inclusão. As principais intervenções de gestão da qualidade utilizadas em Bancos de Leite Humano abordaram ferramentas de melhoria contínua, sendo seis voltadas para aperfeiçoar os processos, uma para prevenir erros e falhas e duas para alcançar o monitoramento contínuo. Considerações Finais: as ferramentas usadas pelos profissionais que atuam em Bancos de Leite Humano demonstraram efetividade na gestão da qualidade dos serviços.

Cross-sectional descriptive study conducted in the neonatal unit of a public teaching hospital in the state of São Paulo, Brazil, which aimed to determine the perceptions of mothers about their newborns hospitalized children. The sample consisted of 100 women questioned, through the Neonatal Perception Inventory Broussard, about how much trouble was expected to be presented by babies of the general unit, on behaviors such as crying; feeding; regurgitate or vomit; evacuate; sleep and have a routine. Then, the same questions were repeated about their own babies. Ninety mothers considered their children with fewer difficulties than other babies at the unit. Younger women and mothers of infants with higher weights tended to consider their children with more difficulty. The Inventory is easy to apply and may be useful in the evaluation of mother-child interaction, although its result cannot be considered in isolation.

OBJECTIVE: To verify if mothers of newborns hospitalized recognize the defining characteristics of "parental role conflict" as representative of that experience. METHODS: A cross-sectional and descriptive study, developed in a neonatal unit of a public teaching hospital in the state of São Paulo. The sample consisted of 100 women who assigned scores of 1 to 5 to the defining characteristics of the diagnosis, where 1 meant "not at all characteristic" and 5 meant "completely characteristic of what I am experiencing." RESULT: Of the total sample, 96 women self-identified with the diagnosis. The most prevalent defining characteristics were: "anxiety," "mother expresses concern(s) in relation to changes in maternal role"; "verbalizes feelings of frustration," "reports concern about family" and "fear". Women who were with their children less often during hospitalization had a higher number of defining characteristics. CONCLUSION: There was a high prevalence of the defining characteristics of the studied diagnosis, suggesting the relevance of the topic and the need for further studies to be developed in the neonatal unit.
