RESUMO

Lactobacillus reuteri INIA P579 was used for the production and purification of reuterin. The purity of reuterin was assessed by high resolution electrospray ionization mass spectrometry (HRESIMS) and nuclear magnetic resonance (NMR) spectroscopy. After purification, reuterin concentration obtained was 1.3 M. The inhibitory activity using Escherichia coli K12 as indicator strain was estimated to be 510 AU/ml. Survival curves in tryptic soy broth revealed that reuterin required to inhibit the growth of three Listeria monocytogenes strains was in the range of 2-4 AU/ml. Purified reuterin (10 AU/g) significantly reduced the growth of L. monocytogenes in cold-smoked salmon kept under moderate or strong temperature abuse conditions. After 15 d at 8 °C, cold-smoked salmon with added reuterin exhibited L. monocytogenes counts 2.0 log CFU/g lower than control smoked salmon with no reuterin added. At 30 °C, reuterin also controlled the growth of the pathogen, with counts 1.4 and 0.9 log CFU/g lower than those observed in control smoked salmon after 24 and 48 h, respectively. The addition of purified reuterin might be used as a hurdle technology to improve the safety and extend the shelf-life of lightly preserved seafood products such as cold-smoked salmon.

RESUMO

Differentiated thyroid cancer (DTC) is the most common endocrine cancer and its outcome is usually favorable. Its basic treatment is well codified, but its monitoring is much less. The value of thyroglobulin (Tg) is one of the main elements for monitoring DTC, while the use of iodine scintigraphy is becoming less recommended. In this case report, we discuss a clinical situation where a patient presented differentiated thyroid metastatic lesions confirmed by biopsy, uptaking radioactive iodine, with undetectable levels of Tg (in the absence of autoantibodies). We discuss the various hypotheses explaining this clinical situation, the potential advantages of performing periodic iodine scintigraphy in some intermediate and high-risk patients and report the documented clinical benefit of radioiodine therapy.

Subacute sclerosing panencephalitis is an uncommon progressive neurological disorder caused by a persistent defective measles virus, typically affecting children. We describe a case of fulminant subacute sclerosing panencephalitis in a 25-year-old male. Brain tissue biopsy showed histologic evidence of encephalitis with eosinophilic intranuclear inclusion bodies (Cowdry Type A and B), intracytoplasmic inclusion bodies, perivascular lymphoplasmacytic infiltration and gliosis. Immunohistochemical studies were positive using an anti-measles antibody. Reverse transcriptase-PCR detected measles virus RNA and phylogenetic analysis indicated a C2 genotype. The rare adult-onset form is often atypical and difficult to diagnose and should be included in the differential diagnosis of subacute "unexplained" neurological diseases and uncommon infectious disorders.

For many years, the retrolabyrinthine approach has been limited to functional surgery of the cerebellopontine angle (CPA). As a result of the increased surgical exposure, particularly the opening of the internal auditory meatus (IAM), the widened retrolabyrinthine technique permits tumor excision from both the CPA and the IAM, regardless of the histological nature of the tumor. The authors have treated 60 acoustic neuromas of varying sizes via this approach (6% intrameatal tumors; 30% > 25 mm in diameter). The postoperative mortality rate was 0%. The risk of fistula formation was 3.3%, and 3.3% of the patients suffered from postoperative meningitis. The results for facial nerve function were equivalent to those obtained previously via a widened translabyrinthine approach and those in a series treated via a suboccipital approach (80% with Grades I and II, 15% with Grade III, and 5% with Grades V and VI). One patient (1.7%) required a secondary hypoglossal-facial nerve anastomosis and had attained a Grade IV result 6 months later. Postoperatively 21.7% of these patients maintained socially useful hearing and 20% had mediocre hearing. Socially useful hearing was preserved in 50% of a subgroup of 20 patients who had both good preoperative hearing and a tumor that involved less than half of the IAM regardless of its volume. Additionally, 15% had mediocre hearing that could be improved with hearing aids. Because of its efficacy in preserving hearing, the authors favor the retrolabyrinthine over the occipital approach, with the latter being considered less subtle and more aggressive.

RESUMO

A brain tumour in a newborn is rare. The incidence of congenital tumours is 0.34 per one million births. We report a case of a huge right hemispheric tumour operated upon successfully in a child 5 days old, who had a total tumoural excision and an uneventful outcome with 5 months follow up. This tumour appears to be the first meningeal haemangiopericytoma described in a newborn. Adequate treatment and histological findings are discussed. meningeal haemangiopericytomas are now considered as originating in pericytes, in the light of recent immunopathological results, and are to be separated from true meningiomas.

RESUMO

One hundred and ninety-two cases of supratentorial astrocytic tumors are classified in 4 groups according to the presence or absence in the pathological material of simple morphological criteria: abnormal cellular density, nuclear pleomorphism, neovascularization, necrosis. Each one of these criteria is strongly correlated with prognosis. Nevertheless only a simple classification in low and high grade lesions has a definite predictive value. A multivariate analysis utilizing Cox's hazard function confronts these histological findings with a number of clinical and etiological possible factors of prognosis. Age and performance status at the time of diagnosis are the best predictors of survival time. The clinical use of a predictive model derived from Cox's function analysis is discussed.
