PURPOSE: The aim of this study was to compare the visual outcomes and subjective visual quality between bilateral implantation of an extended depth of focus intraocular lens, J&J Vision Tecnis Symfony® ZXR00 (Group A) and bilateral implantation of a diffractive trifocal intraocular lens, Alcon Acrysof IQ PanOptix® TNFT00 (Group B). METHODS: This prospective, nonrandomized, comparative study of consecutive cases assessed 52 eyes of 26 patients operated on by the same surgeon (WTH) and binocularly implanted with multifocal intraocular lenses between May 2016 and July 2018. Binocular visual acuity for far, intermediate and near was tested in all cases. Ophthalmological evaluation included the measurement of binocular uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected near visual acuity (UNVA) at 40 cm, uncorrected intermediate visual acuity (UIVA) at 70 cm, monocular visual defocus curve and the quality of life (QoL) questionnaire, National Eye Institute Visual Functioning Questionnaire - 25 ( NEI-VFQ 25). RESULTS: Postoperative UDVA was 0.00 and 0.09 logMAR (P<0.001), UIVA was 0.20 and 0.39 logMAR (P<0.001) and UNVA was 0.16 and -0.01 logMAR (P<0.001) in groups A and B, respectively; postoperative CDVA was -0.05 and 0.06 logMAR (P<0.001) in groups A and B, respectively. CONCLUSION: Both groups reported good subjective quality of vision regarding long, intermediate and short distances. Group A had a better performance for binocular UDVA, UIVA at 70 cm and CDVA, while regarding the monocular defocus curve, Group A outperformed Group B for long distances. Furthermore, Group B surpassed it in the short to very short distances, between the range of ≥2.00 D to 5.00 D of vergence. While Group A had a better performance regarding the vergences between 0.00 and 1.00 D (P<0.05) and at the vergence of +2.50 D (P=0.007). Group B outran Group A for UNVA at 40 cm.

PURPOSE: To assess the stability and reliability of femtosecond laser-assisted cataract surgery (FLACS) incisions design and dimensions using anterior segment optical coherence tomography (AS-OCT) imaging. SETTING: Renato Ambrósio Ophthalmologic Study Center from Hospital Oftalmológico de Brasília, Brasília, Brazil. DESIGN: Prospective nonrandomized controlled case series. METHODS: Eyes undergoing FLACS with triplanar main temporal clear corneal incision (CCI) were evaluated at the end of the case. Eyes that required any incision hydration, surgical complications or lacked follow-up were excluded. The AS-OCT was performed after femtosecond delivery; at the end of the case; at 1 day and at 30 days after surgery. Data of pachymetry, endothelial and epithelial gaps, Descemet detachment and CCI architecture were compared. RESULTS: Eleven eyes from 11 patients completed follow-up. Corneal thickness was statistically different between after femtosecond delivery and the end of the case (P-value <0.05), but without difference compared to 30 days evaluation. There was an increase of Descemet detachments (P-value <0.05) and endothelial gaps (P-value =0.0133) at the end of the case compared to post-femtosecond delivery. As for the architecture of the CCI, significant difference was found between the parameters of entry angle and exit angle measured with AS-OCT and the programmed. CONCLUSION: The AS-OCT was capable of visualizing changes in the cornea at the CCI. Despite the stress caused by manipulation, results indicated good stability of incision and reproducibility of tunnel length. SYNOPSIS: Difference of corneal thickness at the CCI between after femtosecond and after phacoemulsification measurements (P-value <0.05), with increase of endothelial gaps (P-value =0.0133) and Descemet detachments (P-value <0.05).

PURPOSE: To compare intraoperative parameters between venturi and peristaltic pump in WhiteStar Signature® phacoemulsification machine using the bevel-down technique. SETTING: Hospital Oftalmológico de Brasília, Brasília, Federal District, Brazil. DESIGN: Prospective, comparative, patient-masked study. METHODS: Three hundred eyes were randomly assigned to have a phacoemulsification procedure with WhiteStar Signature® using either peristaltic (n=150 eyes) or venturi (n=150 eyes) pumps by a single surgeon (WTH). Elliptical ultrasound setting and prefracture (prechop or preslice) techniques were used in all cases. Cataract nucleus density was graded using lens opacities classification system III and Pentacam Nucleus Staging classification. Clinical measurements included preoperative- and postoperative- corrected visual acuity, preoperative and 2-month postoperative endothelial cell counts, and preoperative and 1-day postoperative central corneal thickness. Intraoperative measurements at the end of the case were phaco time, fluid used, total case time, and Efx energy. RESULTS: There were no statistically significant differences between groups regarding age, cataract density, and phaco time (P>0.05). Intraoperatively, we observed significantly less ultrasound energy (P=0.011), case time (P=0.0001), and balanced saline solution (P=0.001) usage in the venturi group. Clinically, both fluidic settings can provide similar clinical outcomes and visual recovery, regarding corrected distance visual acuity, endothelial cell count, and central corneal thickness. CONCLUSION: Our data show that to minimize fluid use, case time, and energy with the prefracture technique, the venturi pump was the most efficient system and was statistically superior to peristaltic pump.

PURPOSE: To compare visual outcomes and performance between bilateral implantation of a diffractive trifocal intraocular lens (IOL) Acrysof®PanOptix® TFNT00 and blended implantation of two different near add power bifocal IOLs: Acrysof® Restor® SV25T0 in dominant eye and Acrysof® Restor® SN6AD1 in the nondominant eye. METHODS: This prospective, nonrandomized, consecutive and comparative study assessed 20 patients (40 eyes) who had bilateral cataract surgery performed using the IOLs described. Patients were divided into groups, bilateral trifocal implant and blended implant. Evaluation included measurement of binocular uncorrected and corrected distance visual acuity at 4 m (UDVA, CDVA) and uncorrected intermediate (60 cm) and near (at 40 cm) visual acuity; contrast sensitivity (CS) and visual defocus curve. RESULTS: Postoperative CDVA comparison showed no statistical significance between groups. UDVA was significantly better in the trifocal groups. Under photopic conditions, the trifocal group had better CS in higher frequencies with and without glare. The binocular defocus curve demonstrated a trifocal behavior in both groups, with the bilateral trifocal group exhibiting better performance for intermediate vision. CONCLUSION: Both lens combinations were able to provide good near, intermediate and distance vision, with the trifocal group showing significantly better performance at intermediate distances and better CS under photopic conditions.

Binocular visual acuity was tested in all cases. Ophthalmological evaluation included the measurement of uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected near visual acuity (UNVA), and uncorrected intermediate visual acuity (UIVA), with the analysis of contrast sensitivity (CS), and visual defocus curve. RESULTS: Postoperative UDVA was 0.01 and -0.096 logMAR (p<0.01) in groups A and B, respectively; postoperative CDVA was -0.07 and -0.16 logMAR (p<0.01) in groups A and B, respectively; UIVA was 0.14 and 0.20 logMAR (p<0.01) in groups A and B, respectively; UNVA was -0.03 and 0.11 logMAR (p<0.01) in groups A and B, respectively. Under photopic conditions group B had better CS at low frequencies with and without glare. CONCLUSION: Both groups promoted good quality of vision for long, intermediate, and short distances. Group B exhibited a better performance for very short distances and for intermediate and long distances ≥-1.50 D of vergence. Group A exhibited a better performance for UIVA at 60 cm and for UNVA at 40 cm.

PURPOSE: The purpose of this study was to compare intraoperative parameters between femtosecond laser-assisted cataract surgery (FLACS) and conventional phacoemulsification using Centurion® Vision System with Active Fluidics. SETTING: This study was performed at Brasília Ophthalmologic Hospital, Brasília, Federal District, Brazil. PATIENTS AND METHODS: This was a prospective randomized comparative study. Patients with the diagnosis of cataract and surgical extraction programmed were divided into two groups: conventional phacoemulsification and FLACS. Intraoperative data were collected and submitted for statistical analysis. RESULTS: A total of 400 eyes were enrolled, 200 in each group. There were no surgical complications. Groups were statistically equivalent in age and nucleus density. Cumulative dissipated energy and torsional time were significantly reduced in the FLACS group. Conventional surgery had less fluid usage, total case time and aspiration (ASP) time. CONCLUSION: FLACS with Active Fluidics System can reduce the ultrasound energy use during cataract surgery, in spite of increasing case time, fluid usage and ASP time.

METHODS: This prospective, non-randomized comparative study included the assessment of 74 eyes in 37 patients referred for cataract surgery and candidates for multifocal intraocular lens implants. Exclusion criteria included existence of any other eye disease, previous eye surgery, high axial myopia, preoperative corneal astigmatism of >1.00 cylindrical diopter (D), and intraoperative or postoperative complications. Ophthalmological evaluation included the measurement of uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), distance-corrected near visual acuity (DCNVA), and distance-corrected intermediate visual acuity (DCIVA), with analysis of contrast sensitivity (CS), wavefront, and visual defocus curve. RESULTS: Postoperative UDVA was 0.09 and 0.08 logMAR in the SN6AD1 and ZMB00 groups, respectively (p=0.868); postoperative CDVA was 0.04 and 0.02 logMAR in the SN6AD1 and ZMB00 groups, respectively (p=0.68); DCIVA was 0.17 and 0.54 logMAR in the SN6AD1 and ZMB00 groups, respectively (p=0.000); and DCNVA was 0.04 and 0.09 logMAR in the SN6AD1 and ZMB00 groups, respectively (p=0.001). In both cases, there was an improvement in the spherical equivalent and UDVA (p<0.05). Under photopic conditions, the SN6AD1 group had better CS at low frequencies without glare (p=0.04); however, the ZMB00 group achieved better sensitivity at high frequencies with glare (p=0.003). The SN6AD1 and ZMB00 lenses exhibited similar behavior for intermediate vision, according to the defocus curve; however, the ZMB00 group showed a shorter reading distance than the SN6AD1 group. There were no significant differences regarding aberrometry between the two groups. CONCLUSION: Both lenses promoted better quality of vision for both long and short distances and exhibited a similar behavior for intermediate vision. The SN6AD1 and ZMB00 groups showed better results for CS under photopic conditions at low and high spatial frequencies, respectively.

RESUMO Objetivo: Comparar o desempenho óptico e resultado visual entre duas lentes multifocais difrativas: AMO Tecnis® ZMB00 e AcrySof® ReSTOR® SN6AD1. Métodos: O estudo prospectivo, comparativo não randomizado incluiu avaliação de 74 olhos em 37 pacientes com indicação de facectomia e candidatos a implante de lente multifocal. Critérios de exclusão foram: presença de outras doenças oculares; cirurgia ocular prévia; alta miopia axial; astigmatismo cornenano maior que 1,00 D cilíndrica; complicações intraoperatórias ou pós-operatórias. A avaliação oftalmológica contou com medida da acuidade visual para longe corrigida (CDVA) e não corrigida (UDVA), intermediária (DCIVA) e curta distância (DCNVA) com melhor correção óptica para longe; teste de sensibilidade ao contraste; análise de frente de onda e curva visual de Defocus. Resultados: A UDVA foi de 0,09 logMAR para o grupo SN6AD1 e 0,08 logMAR para o grupo ZMB00; com correção foi de 0,04 logMAR para SN6AD1 e 0,02 para o grupo ZMB00 (p>0,05). O grupo SN6AD1 obteve valor de 0,04 logMAR e o grupo ZMB00 de 0,09 logMAR para DCNVA (p<0,05) e DCIVA de 0,17 logMAR para SN6AD1 e 0,54 logMAR para ZMB00 (p=0,000). Houve melhora do equivalente esférico e da UDVA em ambos os grupos (p<0,05). Em condições fotópicas, o grupo SN6AD1 teve melhor sensibilidade ao contraste em baixas frequências sem ofuscamento (p<0,05), contudo grupo ZMB00 obteve melhor sensibilidade em altas frequências (p<0,05) com ofuscamento. As lentes SN6AD1 e ZMB00 obtiveram comportamento semelhantes para visão intermediária na curva de Defocus, porém, o grupo ZMB00 mostrou menor distância de leitura que o grupo SN6AD1. Não houve diferença com significância estatística relacionada à aberrometria na comparação dos dois grupos. Conclusão: As duas lentes promoveram melhor qualidade de visão para longe e perto e comportamento semelhante para visão intermediária. O grupo ZMB00 exibiu melhores resultados para sensibilidade ao contraste em condições fotópicas em alta frequência espacial e SN6AD1 em baixa frequência espacial.

RESUMO

Este trabalho tem por objetivo relatar um caso de sínquise cintilante de câmara anterior em lactente sem causa elucidada. O paciente apresentou desvio no olhar desde o nascimento. Antecedentes pessoais e familiares sem anormalidades. Ao exame em OE: Esotropia constante, nistagmo horizontal, leucocoria e microftalmia, já em OD: sem anormalidades. Ultrassonografia (USG) em OE: redução do comprimento axial, retina aplicada e vítreo hiperecogênico. Aventaram-se hipóteses de persistência de vítreo primário hiperplásico, catarata congênita e retinoblastoma, e solicitou-se tomografia de crânio e cavidade orbitária (TC). Paciente retornou após 6 meses trazendo resultado da TC cuja única alteração evidenciada era OE com áreas hiperatenuantes e sem sinais de calcificação. Ao exame em OD: reflexo pupilar direto positivo e consensual negativo e em OE: hiperemia conjuntival, análise de reflexo pupilar, inviabilizada por presença de sínquises cintilantes de coloração ocre na câmara anterior, que não estava presente no exame inicial. Solicitou-se nova USG, evidenciando: ecos puntiformes na cavidade vítrea, sugerindo hemorragia, espessamento de hialoide posterior, retina aplicada e coroide com espessura aumentada. A sínquise cintilante ou “colesterolosis bulbi” é um processo degenerativo comumente secundário a trauma, inflamação ou hemorragia intraocular. Cursa com deposição de cristais de colesterol (provenientes do cristalino em degeneração ou do próprio vítreo) na cavidade vítrea, espaço sub-retiniano e, mais raramente, na câmara anterior. Sugerese que o fenômeno decorra de traumas, catarata de longa duração, hifema, glaucoma secundário ou descolamento de retina e mais raramente, de uveítes, neoplasias ou vasculopatias. Até o presente, não há relato na literatura de sínquise cintilante de câmara anterior envolvendo um lactente.


The propose of this article is presenting a case report of Synchisis scintillans of the anterior chamber in an infant patient without any elucidation. The patient’s initial complaint was “strabismus since birth”. There was not found any other personal or family abnormalities. On examination: OS: Esotropia maintained, horizontal nystagmus, microphthalmia and leukocoria. OD: no abnormalities. Ultrasonography (USG) OS: reduction of the axial length, retinal applied and hyperechoic vitreous. Raised hypotheses were persistence of hyperplastic primary vitreous, congenital cataract and retinoblastoma, and there was also requested cranial and orbital cavity tomography (TC). The patient returned after six months with TC showing as sole evidenced hyperattenuating areas without signs of calcification on OS. On examination: OD showing direct pupillary reflex positive and consensual pupillary reflex negative. OS: conjunctival hyperemia and analysis of pupillary reflex frustrated by the presence of sparkling colored ocher in the anterior chamber. We asked for a new USG, which showed: punctate echoes in the vitreous cavity, suggesting hemorrhage; thickening of the posterior hyaloids; choroid and retina were attached, both with increased thickness. The synchisis scintillans or “colesterolosis bulbi” is a degenerative process commonly secondary to trauma, inflammation or intraocular hemorrhage. Evolves with deposition of cholesterol crystals (from degeneration of the lens or vitreous itself) in the vitreous cavity, subretinal space and rarely, in the anterior chamber. There are evidences that the phenomenon could arise from severe trauma, long-term cataract, hyphema, glaucoma or retinal detachment and, even more rarely, uveitis, neoplasias or vascular disorders. Until this case, there was no report of Synchisis scintillans in the anterior chamber involving an infant.

Meningocele da bainha do nervo óptico é uma condição extremamente rara, com poucos casos relatados na literatura. Exames de imagem revelam alargamento tubular-cístico do nervo óptico, com espessamento do mesmo. Os sintomas são muitas vezes relacionados com o comprometimento do nervo, ocasionando diminuição de lenta a acelerada da acuidade visual. O tratamento cirúrgico precoce por meio da descompressão da bainha do nervo óptico pode proporcionar melhora da função visual. Apresenta-se um caso de paciente com as características clínicas e radiológicas desta condição patológica rara. Paciente masculino, atendido no serviço com queixa de proptose do olho direito (OD) desde nascimento, com progressão nos últimos meses associada à dor. Melhor acuidade visual corrigida de conta dedos a 50 cm do OD. Olho esquerdo sem anormalidades. Ressonância Magnética de OD demonstrou formação expansiva cística de limites definidos em situação intraconal em órbita direita, em íntima relação com nervo óptico, determinando compressão, deformidade e deslocamento anterior do bulbo ocular, além de apresentar sinal semelhante ao líquor em todas as sequências obtidas. Suscitou-se hipótese diagnóstica de meningocele da bainha do nervo óptico direito e o paciente foi encaminhado para cirurgia descompressiva.


Meningocele of the optic nerve sheath is an extremely rare condition with a few cases reported in literature. Image studies reveal tubularcystic enlargement of the optic nerve although with the same thickness. Symptoms are often related to the involvement of the optic nerve, leading from slow to accelerated decreasing of the visual acuity. The early surgical treatment is the decompression of the optic nerve sheath, which it could provide improvement of visual function. We are presenting a case report of a patient who showed clinical and radiological signs of this rare pathological condition. Male patient attended at service complaining of proptosis of right eye (OD) since birth, in progress during the last months associated to stabbing pain. Best corrected visual acuity (BCVA) of OD was movements at 50 cm far; OS showed no abnormalities. Nuclear Magnetic Resonance of the OD showed expansive formation with cystic aspect, defined boundaries, located in an intraconal situation on the right orbit cavity and in a closing anatomical relationship to the optic nerve, inducing compression, deformity and anterior displacement of this eye besides presenting signal similar to spine liquor in all sequences obtained. The first hypothesis was meningocele of right optic nerve sheath. Then, patient was referred for surgical decompression.

Objetivo: Medir e comparar o tamanho e forma de capsulotomias realizadas com laser de femtossegundo com os de capsulorrexes curvilíneas contínuas (CCC) realizadas com auxilio guiado por imagem digital e avaliar o resultado refracional. Métodos: Durante cirurgia de catarata, 40 olhos de 40 pacientes tiveram a capsulotomia realizada com auxílio do laser de femtossegundo e seus resultados foram comparados com os de 40 olhos de 40 outros pacientes que tiveram a capsulorrexe guiada por sistema de imagem digital. Os parâmetros de circularidade, forma e overlap foram medidos usando o Adobe Photoshop (Adobe Systems Inc.) e os resultados refracionais pós-operatórios foram avaliados em ambos os grupos. Resultados: Os diâmetros, tamanho e forma de alta precisão e previsibilidade foram atingidos com laser de femtossegundo e houve diferença estatística entre os grupos. Quando comparado o equivalente esférico entre os grupos, não houve diferença estatística. Conclusão: As capsulotomias realizadas pelo laser de femtossegundo possuem circularidade programada, diâmetro pretendido e valores de desvio padrão médios, indicando resultados reprodutíveis mais elevados. No entanto, CCC realizada por um cirurgião experiente com auxílio guiado de imagem digital, com configurações apropriadas, fornece resultados semelhantes e sugere que diferentes técnicas são igualmente eficazes. .


Purpose: To measure and compare size and shape parameters of femtosecond laser capsulotomy with manually continuous curvilinear digital guided capsulorhexis (CCC) and their refractive outcomes. Methods: Laser capsulotomies in 40 eyes of 40 patients were performed using LenSx femtosecond laser device (Alcon, Forthworth, US) and its results were compared with the CCC digital guided carried out in 40 eyes of 40 patients using the Callisto Eye digital image system (Zeiss, Germany). Capsulorhexis circularity, shape and capsule overlap were measured using Adobe Photoshop (Adobe Systems Inc.) and postoperative refraction outcomes were evaluated in both groups. Results: Highly accurate and predictable capsulotomy diameter, size and shape were achieved with femtosecond laser capsulotomy compared with capsulorhexis and showed statistical difference between groups. Spherical equivalent comparison between groups showed no statistical difference. Conclusions: Femtosecond laser anterior capsulotomy with programed circularity had the intended diameter with average standard deviation values, indicating higher reproducible outcomes. Capsulorhexis performed by an experienced surgeon with auxiliary image guide and appropriate settings provides similar results our results suggest that different techniques are equally effective. .

RESUMO

OBJETIVO: Este trabalho tem por objetivo avaliar a incidência dos fatores de risco em uma amostra da população, avaliada durante uma campanha de prevenção da cegueira pelo glaucoma, bem como analisar os dados epidemiológicos sobre a ocorrência de casos suspeitos de glaucoma na cidade de João Pessoa, Paraíba. MÉTODOS: Após prévia divulgação, pessoas com idade ≥ 40 anos, compareceram voluntariamente ao Hospital Universitário Lauro Wanderley, João Pessoa, Paraíba. Realizou-se anamnese, medida da pressão intraocular (PIO) com tonômetro de aplanação de Goldmann e biomicroscopia de fundo. Considerados suspeitos aqueles com PIO ≥ 21 mmHg ou disco óptico com relação escavação sobre disco (E/D) ≥ 0,5 x 0,5. Os suspeitos foram subdivididos por idade, sexo e tipo de alteração e divididos em: Grupo I de 40 e 49 anos; Grupo II de 50 e 59 anos; Grupo III ≥ 60 anos; P+ se apenas PIO Analysis of risk factors and epidemiology of blindness prevention campaign by glaucoma in João Pessoa, Paraíba 21 mmHg; E/D+ se apenas E/D ≥ 0,5x0,5; e P/E/D+ quando ambos alterados. RESULTADO: No total de 244 pacientes, 29,50% foram considerados suspeitos (48,61% homens (H) e 51,38% mulheres(M), sendo 41,66% pertencentes ao Grupo I; 23,61% ao Grupo II e 34,72% ao Grupo III. Com relação ao tipo de alteração: 70,83% eram E/D+; 15,27% eram P+ e 13,88% P/E/D+. Foi ainda realizada uma análise detalhada entre homens e mulheres. CONCLUSÃO: Apesar da irreversibilidade das alterações conseqüentes ao glaucoma, o diagnóstico precoce e o tratamento adequado, associado à conscientização da população, são capazes de prevenir cegueira, comprovando a importância da realização periódica dessas campanhas.


OBJECTIVE: This study aims evaluating the incidence of risk factors in a population sample, assessed during campaign of prevention of blindness by glaucoma, and analyzing epidemiological data incidence of glaucoma in the city of João Pessoa (PB). METHODS: After prior disclosure, individuals aged≥ 40 years, attended voluntarily at Lauro Wanderley University Hospital, Joao Pessoa (PB) for examination, anamnesis, intraocular pressure (IOP) measured by applanation tonometer of Goldmann, and direct fundoscopy. Those who were considered suspect showed IOP ≥ 21 mmHg or/and a cup to disc ratio (CDR) ≥ 0,5. The cases suspects were separated by age, sex and abnormal signs. We referred to Group I individuals from 40 to 49 years old, Group II from 50 to 59 years old, Group III ≥ 60 years, "P+" for IOP ≥ 21 mmHg, "E/ D+" for CDR ≥ 0.5 and "P/E/D+" for individuals with all the showing signs. RESULTS: Within a total of 244 patients, 29.50% were considered suspects (48.61% male, 51.38% female, and 41.66% of group I, 23.61% of Group II and 34.72% of Group III). Regarding the type of sign: 70.83% were E/D+, 15.27% were P+ and 13.88% were P/ E/D+. There was also carried out a detailed analysis of men and women. CONCLUSION: Despite the irreversibility of the modifications caused by glaucoma, early diagnosis and proper treatment associated with raising awareness of population are preventive actions to inhibit blindness, confirming the relevance of conducting periodic preventive campaigns.
